Abstract

Chemotherapy is a cancer therapy that can prevent the development and spread of malignant cells, but causes various side effects (loss of appetite, nausea, vomiting, mucositis, changes in taste, pain, fatigue, dry mouth, diarrhea, constipation. These side effects result in discomfort for children with cancer, in more severe conditions it can worsen the child's nutritional status due to inadequate nutritional intake. Nursing care using the Kolcaba comfort approach can be applied to overcome children's discomfort cancer who experience nutritional imbalance problems. The method used is a case study. The comfort needs in the five management cases are assessed based on the physical, psychospiritual, environmental and sociocultural contexts. The comfort interventions provided include standards of comfort, coaching, and comfort food for the soul . Nursing evaluation of the problem of nutritional imbalance which is a physical aspect of the patient, namely, the comfort type for the five children is ease comfort and three of the five children also have transcendence type comfort for other aspects. Kolcaba's Comfort Theory can be applied to pediatric cancer patients who experience nutritional disorders and provides opportunities for nurses to develop innovative interventions to improve the quality of nursing care.

Abstract

The Integrated Referral Information System (SISRUTE) is an application designed and implemented nationally since 2016 which helps the referral process effectively and efficiently by providing referral information for individual health services. SISRUTE has been used by Community Health Centers as First Level Health Facilities (FKTP) and Hospitals (Advanced Care Health Facilities). This research aims to analyze the factors that influence the implementation of SISRUTE in Indonesia, both at the FKTP and FKRTL levels in Indonesia. The research method used is a systematic review, complemented by secondary data search using the PRISMA method to analyze the influencing factors in the process of implementing the integrated referral system in Indonesia. The results obtained show that the application of SISRUTE is influenced by several factors including data quality, human resources and organization, facilities, budget allocation, policies, system quality, communication and reporting and evaluation monitoring. In general, the most influential factors are data quality, infrastructure, policies and standard operational procedures, data management processes and human resources both in terms of training or appointing a team specifically for handling SISRUTE. There is a need for commitment from the leadership of FKTP and FKTRL to be able to encourage improvements in infrastructure both in terms of hardware, software and internet networks as well as technical assurance regarding human resources who are responsible for operating the system.

Abstract

Coronavirus Disease (COVID-19) infection can cause complications in the respiratory system including oxygen saturation, this condition can affect the condition of post-surgery patients. In patients who will undergo surgery, optimal oxygen saturation is very important in maintaining hemodynamic stability and success in anesthesia and patient recovery. The purpose of this study was to determine the impact of a history of COVID-19 infection on oxygen saturation in elective surgical patients. The design of this study was an observational analytical study with a cross-sectional approach. The sample size was 76 people consisting of 38 groups with a history of COVID-19 and 38 groups with a history of COVID-19. The sampling method used a purposive sampling technique with respondent criteria that had been determined according to the inclusion and exclusion criteria. The results of the Independent Sample t-test showed a significant difference in decreased oxygen saturation in patients with a history of COVID-19 compared to patients without a history of COVID-19 with the results (p <α = 0.05). It can be concluded that a history of COVID-19 infection has an impact on decreased oxygen saturation in elective surgical patients. These findings suggest the importance of careful preoperative evaluation and more intensive oxygenation monitoring in patients with a history of COVID-19 infection to minimize the risk of intraoperative and postoperative complications.

Abstract

TB-HIV patients are expected to routinely undergo OAT and ARV treatment simultaneously, this causes the side effects of drugs in patients to be double, resulting in many patients who are not compliant in undergoing treatment. Non-compliance will result in treatment resistance. The purpose of this study was to determine the relationship between self-motivation in adherence to taking OAT and ARV drugs and side effects in TB-HIV patients at Dr. M. Djamil Padang General Hospital. The study design was observational with a cross-sectional approach. The number of samples was 60 people, the sampling technique used Total Sampling. With the respondent criteria that have been determined according to the inclusion and exclusion criteria. The test results showed that patient motivation has a p value of 0.000, drug side effects (0.000), It can be concluded that services for TB-HIV patients must provide education to patients and their families about the need to have medication adherence. With this education, patients will have good adherence so that the success rate of treatment is higher.

Abstract

Hemorrhoids are a condition in which the veins in the rectum and anus become swollen and inflamed, causing discomfort and bleeding. If left untreated, hemorrhoids will protrude permanently (grade IV hemorrhoids) and the only therapy for this condition is hemorrhoidectomy. However, excision after hemorrhoidectomy surgery often causes extreme pain. Based on data from the Ministry of Health in 2018, it is estimated that in 2030 there will be 21.3 million people in Indonesia suffering from hemorrhoids. The purpose of this study was to determine the application of autonomous sensory meridian response (ASMR) to reduce pain intensity after hemorrhoid surgery. This type of research is descriptive with a case study approach model. The sample was 3 respondents after hemorrhoidectomy surgery who experienced severe pain. Data is collected by means of observation and measurement. Analysis is carried out in the form of diagnosis and nursing care. In respondents I, II, III before ASMR therapy, the pain scale felt after hemorrhoid surgery was a scale of 8, with severe pain intensity. Then after being given ASMR therapy for 10 minutes, in respondents I-II, the pain intensity felt was severe in the range (7-8) decreased to moderate pain in the range (4-6). However, in respondent III, the pain intensity was felt to be stable, namely the severe pain scale, there was no decrease in pain intensity. The results of this study can be concluded that ASMR relaxation has significant benefits in reducing post-hemorrhoid surgery pain if it is carried out on people with an open attitude towards ASMR therapy, enjoying the videos provided, full concentration and carried out in accordance with the SOP (Standard Operating Procedure).

Abstract

Menstruation is a natural cycle in women characterized by the release of blood through the vagina. Menstrual pain or dysmenorrhea often causes a throbbing sensation, cramps in the lower abdomen, and pain that can spread to the back and inner thighs. Deep breathing relaxation techniques are an effective method for reducing pain by relieving muscle tension, reducing anxiety, and preventing the spread of pain stimuli. This study aims to analyze the effectiveness of deep breathing relaxation techniques in reducing menstrual pain in women with dysmenorrhea. The research method uses a descriptive case study approach involving two clients who were given deep breathing relaxation techniques as an intervention. The research subjects were 80 female adolescents in the Meilia Medika Health Vocational School area. Sampling was done using purposive sampling technique and only 2 participants were taken. Data was analyzed descriptively. The results showed that in the first client, before the relaxation technique was performed, the pain scale felt was 6/10. After the application of the relaxation technique, the pain decreased to 3/10 and on the second to fifth day it dropped to 1/10. On the sixth and seventh days, the pain was no longer felt. The second client initially experienced pain on a scale of 9/10, which decreased to 5/10 after the application of the relaxation technique. On the second to fourth day, the pain scale decreased to 2/10, and on the sixth to seventh day the pain was no longer felt. The conclusion of this study is that deep breathing relaxation techniques are proven to be effective in reducing the scale of menstrual pain if done correctly according to the Standard Operating Procedure (SOP). Both clients stated that this method helped reduce menstrual pain significantly.
