Abstract

Elevated blood glucose levels in type II diabetes mellitus (DM) can cause serious organ damage. In Indonesia, the prevalence of DM is increasing, with many patients not receiving adequate treatment. Non-pharmacological therapies, such as laughter therapy, are considered effective in reducing blood glucose levels and stress, especially in elderly patients with type II DM. Research Objective: To describe the outcomes of implementing laughter therapy nursing care in type II DM patients with the nursing problem of blood glucose instability in the working area of Citra Medika Health Center, Lubuklinggau, in 2024. Research Method: A case study method involving two individuals with a medical diagnosis of type II DM, utilizing family nursing care, and implementing laughter therapy over four sessions in one week, with a duration of 20-30 minutes each session. Research Results: Subject I experienced a decrease in random blood glucose levels from 189 mg/dl on the first day to 83 mg/dl on the fourth day. Subject II experienced a decrease from 237 mg/dl on the first day to 116 mg/dl on the fourth day. The difference in reduction between the two subjects was influenced by factors such as family support, the use of hyperglycemia medication, and diet control. Conclusion: The application of laughter therapy can reduce blood glucose levels in patients with type II DM. Further research should include the control of independent repeated actions, the use of diabetes medication, and a larger number of respondents. Suggestions: Laughter therapy can be used as a complementary therapy in the management of type II DM in older adults.

Abstract

Leprosy, caused by Mycobacterium leprae, can lead to severe inflammatory reactions, known as type 2 reactions or erythema nodosum leprosum (ENL). Zinc, a crucial micronutrient for immune function, may influence the incidence of these reactions. This study investigates the relationship between serum zinc levels and the occurrence of type 2 reactions in leprosy patients in East Nusa Tenggara, Indonesia. Objective: To assess whether low serum zinc levels are associated with an increased risk of type 2 reactions in leprosy patients in East Nusa Tenggara.Method: A prospective cohort study was conducted involving leprosy patients at health centers and hospitals in East Nusa Tenggara. Serum zinc levels were measured using atomic absorption spectrophotometry. Patients were monitored for the development of type 2 reactions over a 12-month period. Data were analyzed using descriptive, bivariate, and multivariate statistical methods to explore associations between zinc deficiency and type 2 reactions. Results: The study found a significant association between low serum zinc levels and an increased risk of type 2 reactions. Patients with deficient zinc levels were more likely to experience severe inflammatory responses compared to those with normal zinc levels. Multivariate analysis confirmed that zinc deficiency was an independent risk factor for the occurrence of type 2 reactions. Conclusions: Zinc deficiency is linked to an increased risk of type 2 reactions in leprosy patients. Addressing zinc deficiency through supplementation could be an effective strategy to reduce the incidence and severity of these reactions. Incorporating zinc status assessments into leprosy management programs may improve patient outcomes and help control disease complications.

Abstract

Gout is a disease caused by the accumulation of monosodium grate crystals in the human body. The cause of the high incidence of gout is a lack of knowledge about preventing gout, so efforts can be made with Health Education to increase understanding. This research aims to determine the effect of health education about a low-purine diet delivered through video media on the level of knowledge among gout sufferers. This quantitative research employs a quasi-experimental study design, specifically a "one-group pretest-posttest design." A total of 33 respondents were selected to assess their level of knowledge using a questionnaire obtained through accidental sampling. Data collection involved distributing questionnaires to respondents (pretest), where the questionnaire had undergone a validity test with a result of r- calculated > r-table (0.632) and a reliability test with a Cronbach's alpha value of 0.978 therefore, the questionnaire was declared valid and reliable, after which the researchers provided health education regarding a low-purine diet. The questionnaire was then distributed again to the respondents (posttest). The data analysis utilized the Wilcoxon Signed Ranks Test. The Wilcoxon Ranks test produced statistically significant values (sign. = 0.00 < 0.05). The findings of this study indicate that health education about a low-purine diet delivered through video media influences the level of knowledge among gout sufferers.

Abstract

Stunting is chronic malnutrition which is based on an index of body length compared to age (PB/U) or height compared to age (TB/U) with a limit (z-score) of less than -2 SD (Standard Deviation). Stunting problems need to be detected early, so that the interventions given can reduce long-term effects. Detection and management of stunting is carried out through empowerment efforts. Family empowerment as an effective step to reduce stunting cases through prevention and management efforts. The role of parents is very important in preventing stunting through detection and management. Anthropometric-based instruments that are easy to use are needed for the detection and management of stunting. Objective to analyze the variety of health promotion strategies to prevent stunting. The research design is a Systematic Literature Review of articles or journals published in 2018-2023, accessed on Google Scholar, PubMed and Scopus with the Prisma Flow technique. Research with different designs ranging from descriptive studies to quasi-experimental and participatory studies shows that an integrated and family-based approach can significantly improve knowledge, skills and awareness about stunting prevention. Programs involving education on exclusive breastfeeding, healthy food processing for toddlers, and maternal reproductive health have proven effective in reducing the prevalence of stunting. Health promotion strategies found in the literature review were mostly conceptual rather than operational and did not show when these strategies successfully reduced stunting.

Abstract

Cervical cancer patients stage IIIB receive their first chemotherapy treatment. One of the effects of chemotherapy is that it can cause stress. Stress, as a psychological effect, worsens many diseases and pathological conditions and weakens the immune system. This can reduce the cytotoxic activity of T lymphocyte cells and thus increase cell proliferation. The combination of murottal and dhikr therapy can be considered a comprehensive approach to dealing with stress. After listening to murottal, a person may feel more connected to the verses of the Qur'an. Dhikr therapy, as a form of repetition and reflection on the greatness of Allah, can be the next step in strengthening spiritual bonds and relieving stress. Purpose: This study aims to determine the stress level of cervical cancer patients in the first chemotherapy program with murottal and dhikr therapy at Dr. Moewardi Hospital. Method: This type of research is quantitative research with the research design used is Quasy Experiment one group pre test-post test design. The total population of the study was 52 patients. Abstract

Leprosy is still a health problem in endemic countries, one of which is Indonesia. It is caused by the lack of information about leprosy, which affects the self-care management of leprosy sufferers to increase the risk of leprosy severity. Leprosy is a chronic granular infection caused by the obligate intracellular organism M. leprae. These germs initially attack the peripheral nervous system, then the skin, mucosa, respiratory tract, retinal system, eyes, retina, bones, and tissues. This study aims to determine the relationship of information on self-care management in leprosy patients in the Sumenep Regency. This type of research was an observational analysis using a cross-sectional study approach. The population was 32 respondents, with a sample of 30 respondents. The sampling technique used simple random sampling. Data collection used a questionnaire, and data analysis used Rho Spearman with a significance level of p<0.005. The analysis result obtained significance, or the value of information availability (0.000) was much more standard significant than 0.05 or (p < a), then H0 was rejected, and H1 was accepted, which means that there is a relationship between information and self-care management in leprosy patients. Information can affect self-care management in leprosy patients. Self-care management has an important role in preventing the severity and transmission of leprosy. With enough information, leprosy patients can carry out good self-care- management and reduce the impact of leprosy effectively.

Abstract

Posterior cruciate ligament (PCL) avulsion fractures are rare but significant injuries that can lead to knee instability and degeneration if untreated. Treatment options range from conservative management to surgical fixation. The objective of this study is to evaluate and compare the outcomes of different treatment modalities for posterior cruciate ligament (PCL) avulsion fractures, with a focus on surgical interventions, specifically open surgery and arthroscopy. Methods: A systematic literature search was conducted using the MEDLINE, EMBASE, and Cochrane Library databases from August 17, 2003 to August 17, 2023. Studies included involved patients with PCL avulsion fractures treated via arthroscopic or open fixation. Data was extracted on demographics, surgical approaches, outcomes (Lysholm scores), and complications. The Coleman methodology score was applied to assess study quality by scoring factors like study size, design, follow-up, and outcome measures, and only studies scoring ≥60 were included for data analysis. Results: Out of 1,470 studies, 12 met the inclusion criteria. Lysholm scores ranged from 85 to 100 for open approaches and 80 to 100 for arthroscopic approaches. Arthroscopic treatment showed fewer complications, such as infections or hardware irritation, while both approaches demonstrated favorable outcomes. Postoperative limitations in motion were observed in both groups, with a slightly higher incidence in the arthroscopic cohort. Conclusions: Both arthroscopic and open surgical approaches effectively treat PCL avulsion fractures, with arthroscopy showing a slight advantage in postoperative outcomes and fewer complications. However, open surgery with a small open procedure also offers satisfactory results, particularly for complex fractures.
