Abstract

Neurology is a field of medicine that focuses on the human brain. However, the elderly experience a natural aging process that causes changes in the brain and nervous system, increasing the risk of developing neurological diseases. One of them is dementia or alzheimer's disease, Parkinson's disease, seizures, stroke, sleep disorders, and others. The purpose of this study was to analyze the effect of lemongrass extract on reducing neurological diseases in the elderly. The research method was a systematic literature review on theses, articles, and books published in the range of 2018-2024. Data were collected from three databases (Google scholar, pubmed, and garuda portal) using the keywords "Lemongrass extract" and "Neurological diseases". The results obtained in this study indicate that lemongrass extract has an effect on reducing neurological diseases in the elderly, which can reduce dementia or alzheimer's disease, parkinson's disease, seizures, stroke, sleep disorders. So by using or consuming lemongrass drinks, lemongrass aromatherapy, lemongrass oil for foot massage, lemongrass decoction compresses, lemongrass extract drugs, lemongrass water foot soaks, lemongrass water for bathing can reduce neurological diseases in the elderly.

Abstract

Controlling drug inventory in hospitals plays an important role in ensuring the continuation of safe, timely, and efficient health services. However, problems such as overstock, out-of-stock, and inefficiency of the distribution process are often the main obstacles that still occur frequently. Lean Six Sigma in drug inventory control in hospitals is an approach that combines Lean and Six Sigma principles to improve efficiency, reduce waste, and decrease variation in the drug management process. The implementation of Lean Six Sigma aims to overcome problems such as stock out, overstock, and inefficient drug purchases, thereby improving the quality of pharmaceutical services in hospitals. Methods: This literature study used a combination of keywords "six", "sigma", lean", "inventory", "pharmacy", "hospital" and "Indonesia" in three databases ProQuest, ScienceDirect and Google Scholar. With the guidance of PRISMA, the selection of research articles that meet the inclusion and exclusion requirements is carried out. Results: Using the PRISMA guide, 6703 articles were found from the combined keywords. After screening for conformity with the inclusion and exclusion criteria, only 5 research articles were found that explained the implementation of lean six sigma in the pharmaceutical inventory supervision area in hospitals. From the five research articles, there are several implementations of lean six sigma such as DMAIC, 5 R's, Pareto Chart, Turnover Ratio, and waste analysis. Conclusion: Several methods of implementing lean management can be applied in pharmaceutical logistics to improve inventory oversight in pharmaceutical logistics.

Abstract

The quality of health services plays a crucial role in achieving better health outcomes globally. Despite the availability of healthcare, poor service quality leads to millions of deaths annually. Entrepreneurial leadership, characterized by innovation, risk-taking, and strategic adaptability, is vital in improving service quality. This literature review examines the role of entrepreneurial leadership in enhancing healthcare service quality, particularly in hospitals. Secondary data was gathered from Google Scholar, PubMed, and ScienceDirect, focusing on studies published between 2020 and 2025. The review reveals that entrepreneurial leadership fosters innovation, increases work engagement, and improves hospital adaptability to evolving patient needs. The findings suggest that hospitals with entrepreneurial leaders can achieve continuous improvements in service quality by adapting to changes, enhancing efficiency, and fostering creativity within their teams. This research emphasizes the need for further studies to explore the direct impact of entrepreneurial leadership on healthcare service quality, as the current literature lacks in-depth analysis of this relationship.

Abstract

This study explores the perceptions of medical students regarding the role of podcasts and YouTube in enhancing their understanding of medical knowledge, while also examining the factors influencing their usage and reviewing these media from an Islamic perspective. Using a descriptive qualitative approach, data were collected through in-depth interviews with third-year medical students at YARSI University who actively utilize these platforms as self-directed learning tools. The findings reveal that students perceive podcasts and YouTube as effective, flexible, and engaging resources for visualizing complex medical concepts, revising course material, and supporting individual learning preferences. Internal factors such as motivation, digital literacy, and learning styles, as well as external factors like internet access, content quality, and institutional support, significantly affect usage. From an Islamic perspective, the majority of students believe that the use of digital media for learning aligns with Islamic values, provided it is used with sincere intentions and does not interfere with religious obligations. This study highlights the growing relevance of digital media in modern medical education and supports its integration into curriculum design with consideration of ethical and religious values.

Abstract

Sepsis is a life-threatening organ dysfunction due to dysregulation of the host response to infection which causes systemic changes, namely an increase in the serum lactate/albumin ratio which is associated with mortality. Method:This study is a clinical observational analytical study with a prospective cohort design at fittingAdult patients diagnosed with sepsis in the integrated inpatient ward and intensive care ward at Haji Adam Malik General Hospital, Medan. The subject sampling technique used was non-probability sampling with a consecutive sampling method where 50 subjects were required. Bivariate analysis used the Pearson correlation test and the linear relationship between the lactate-albumin ratio value and the SOFA score used linear regression. A 95% confidence interval with a p value <0.05 was considered significantly significant Results :The results of the research on the first day found that the average LAR H1 was 0.59 + 0.51 (0.12-2.26) and the average SOFA H1 score was 7.32 + 1.88 (7) where an r value of 0.726 was obtained. indicates a high and significant correlation between Lactate Albumin Ratio and SOFA Score as a predictor of sepsis patients (p value = 0.000, p<0.05). On the third day, the average LAR H3 score was 0.12 + 0.29 (0.21-0.47) and the average SOFA H3 score was 9.58 + 2.01 (10) where an r value of 0.33 was found which indicates Very low and insignificant correlation between Lactate Albumin Ratio and SOFA Score as a predictor of sepsis patients (p value = 0.818, p > 0.05). Conclusion:The lactate-albumin ratio was strongly correlated with the SOFA score on the first day of adult patients diagnosed with sepsis in the integrated inpatient ward and intensive care ward.

Abstract

This narrative review examines the factors influencing revisit intentions among outpatient care patients in private hospitals in Indonesia. The study highlights the significance of service quality, patient satisfaction, trust, and effective complaint handling in shaping patients' willingness to return. Key findings indicate that while technical service quality is vital, non-clinical aspects such as communication, social media engagement, and queue management also play critical roles. Understanding these determinants is essential for hospitals aiming to enhance patient loyalty and ensure competitive healthcare services.

Abstract

CAP continues to be a significant infectious disease among individuals living with HIV, even in the era of ART. HIV-positive individuals are at heightened risk for CAP due to altered immune function and exposure to a broader range of potential pathogens compared to the general population. This study aimed to examine the clinical characteristics, causative agents, and outcomes of CAP in HIV-infected patients to inform more accurate and evidence-based management practices.  A descriptive qualitative approach was employed, with data obtained from various documented sources relevant to the topic. The analysis process followed the three-step model by Miles and Huberman, which includes data reduction, data display, and conclusion drawing. The findings reveal that HIV-infected patients with pneumonia often present with typical acute symptoms and can be appropriately evaluated using the PSI. Diagnostic and therapeutic procedures were consistent with those used for non-HIV patients, involving chest X-rays, sputum or BAL sampling, and blood tests prior to the initiation of empirical antibiotic therapy.  Key strategies such as consistent use of ART, smoking cessation, and vaccination (particularly against pneumococcus and influenza) were found to be critical in both the treatment and prevention of CAP among HIV-positive individuals. Nevertheless, further research is necessary to refine prevention strategies and determine the most effective approaches for long-term disease management in this vulnerable population.

Abstract

Stroke is the second leading cause of death worldwide, with ischemic stroke comprising 87% of cases. Metabolic acidosis from hypoxia promotes anaerobic glycolysis, raising lactate dehydrogenase (LDH) levels, which reflects neuronal tissue injury and inflammation in acute ischemic stroke. The Alberta Stroke Program Early CT Score (ASPECTS) is used to assess ischemic brain injury on non-contrast CT, aiding early prognosis and treatment decisions. This cross-sectional study at Dr. Soetomo Hospital, Surabaya (February–May 2023), included 30 acute ischemic stroke patients (53.3% male, mean age 60.2 ± 7.1 years, onset 2–5 days). Exclusion criteria were prior thrombolysis, cancer, or organ failure. LDH levels were measured at admission using the Alinity C analyzer, and ASPECTS was calculated from initial CT scans. Spearman’s correlation was used for analysis. Results showed a significant inverse correlation between LDH and ASPECTS (r = -0.279, p = 0.003), indicating that higher LDH levels correspond with lower ASPECTS (larger infarcts). Mean LDH was elevated (258.75 ± 50.65 U/L, normal 120–190 U/L). Comorbidities included hypertension (90%), dyslipidemia (83.3%), and diabetes mellitus (56.7%). These findings suggest that serum LDH may be a valuable adjunct biomarker for early assessment of ischemic stroke severity when advanced imaging is unavailable, helping clinicians estimate infarct size rapidly. Further research involving larger populations is recommended to confirm LDH’s utility and to examine its combination with other biomarkers for acute ischemic stroke management.

Abstract

During the Covid-19 pandemic, people with heart disease are patients who are at high risk of severe severity if exposed (Indonesian Association of Cardiovascular Specialists, 2020). Chairman of the Central Board of the Indonesian Association of Cardiovascular Specialists (PERKI), Isman Firdaus, said that the Covid-19 pandemic which has been going on since the end of 2019 has become a concern for heart disease sufferers. Considering that exposure to any infection, including Covid-19 infection, can worsen cardiovascular diseases such as the recurrence of coronary heart disease or chronic heart failure (RI, 2019). One of the important efforts in preventing the occurrence of cardiovascular disease that can be done is to identify the risk factors for cardiovascular disease from each individual. There are several models to detect risk, one of which is by using the Jakarta cardiovascular score [2]. Health promotion is also one of the efforts in managing modifiable risk factors for heart disease (Handayani, 2010). Health promotion is expected to prevent risky behaviors that lead to an increased incidence of heart disease in the long term. This study aims to see the effectiveness of early detection and health promotion on changes in attitudes and lifestyles of residents at risk of heart disease after the covid-19 pandemic. The research method in this study uses a quasi-experimental design with a total of 30 respondents in a group of 1. The research was conducted in the working area of the Puskesmas of Pasar Minngu District, Jakarta. The results showed that after intervention by conducting early detection and providing health promotion, there was a change in attitude of 0.8 points, from 88.13 (before the intervention) to 88.95 (after the intervention). The results of the T test were obtained p value = 0.004, meaning that statistically there was a significant difference between before and after the intervention. The respondents' lifestyle also changed by 5.5 points, from 66.03 (before the intervention) to 71.62 (after the intervention) with p value = 0.004 which means that statistically there is a significant difference between before the intervention and after the intervention was carried out.

Abstract

Carpal Tunnel Syndrome (CTS) is a musculoskeletal disorder caused by the compression of the median nerve, with an incidence of 1 to 3 cases per 1000 people annually and a prevalence of 50 per 1000 in the United States. The two main surgical treatments for CTS are open carpal tunnel release (OCTR) and endoscopic carpal tunnel release (ECTR). This study aims to compare the outcomes of ECTR and OCTR based on randomized controlled trials (RCTs). A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, focusing on RCTs published between 2013 and 2023. The outcomes analyzed included the Boston Carpal Tunnel Questionnaire Symptom Severity Scale (BCTQ-S), Boston Carpal Tunnel Questionnaire Functional Status Scale (BCTQ-F), VAS score, and postoperative complications. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) guidelines were used to assess the level of evidence. Seven studies, including 271 hands treated with ECTR and 285 hands treated with OCTR, were reviewed. The comparison results showed minimal differences in BCTQ-S (MD=0.06), BCTQ-F (MD=0.02), VAS (MD=0.03), and complications (ECTR vs OCTR = 5 vs 9). Both ECTR and OCTR produced similar results, with only slight differences observed. In conclusion, ECTR and OCTR offer comparable outcomes based on BCTQ-S, BCTQ-F, VAS score, and postoperative complications. Despite some differences, these findings should be interpreted cautiously and not considered conclusive.
