Abstract

Diabetes mellitus (DM) is a metabolic disorder that is accompanied  by an increased glucose in the blood or called hyperglycemia. Diabetes can be caused by problems with the pancreas not being able to produce insulin, insufficient insulin secretion, imperfect insulin action, or both. When blood glucose is left elevated for a long time, it can cause complications such as gangrene. Diabetic gangrene is one of the complications caused by tissue damage. Gangrene occurs due to neuropathy and vascular disorders, usually in the peripheral areas of the body. Bacteria found in diabetic wounds are bacteria that produce biofilms such as Staphylococcus aureus. Staphylococcus aureus can release exotoxins such as leukocidin. Leucosidine lyses PMNs and releases ROS which will then induce a cytokine response such as interleukin-6 which will increase CRP levels. hs-CRP is a high precision CRP measurement even at low concentrations. This study aims to determine the correlation between hs-CRP levels and the presence of Staphylococcus aureus in type II diabetes mellitus gangrene wounds. The research design used is observational analytical using a cross sectional approach and accidental sampling. There were 16 research respondents with increased hs-CRP levels in 15 respondents and 9 respondents with Staphylococcus aureus bacteria. Correlation test with a value of p = <0.05 (p=0.032) means that there is a correlation between hs-CRP and the presence of Staphylococcus aureus in type II diabetes mellitus gangrene wounds.

Abstract

The objective of this study is to determine the primary factors that contribute to the mortality rate of COVID-19 in Indonesia, with a specific focus on age, comorbidities, and vaccination status. A retrospective analysis was performed utilizing patient data obtained from the West Lombok Health Office in the year 2020. The findings indicated that individuals aged 59 years or older had a considerably higher risk of mortality, with an odds ratio of 0.065 (95% confidence interval: 0.014 - 0.305). Comorbidities, specifically hypertension and diabetes mellitus, were found to be substantial risk factors. The odds ratio for hypertension was 63.64 (95% CI: 1.232 - 30.608), while the odds ratio for diabetes mellitus was 2.00 (95% CI: 1.593 - 2.512). The study revealed that being vaccinated considerably decreased the likelihood of death, with an odds ratio of 0.071 (95% CI: 0.015 - 0.334). Nevertheless, engaging in self-isolation was found to greatly elevate the likelihood of mortality, as indicated by an odds ratio of 195.429 (95% confidence interval: 22.894 - 1668.196). The Chi-Square test revealed a statistically significant association between vaccination status and COVID-19 mortality, with a p-value of 0.000 < 0.05. The logistic regression analysis revealed that the type of isolation had a very significant and large effect on COVID-19 mortality. The odds ratio (Exp(B)) was 96.604, showing a strong association between the type of isolation and the mortality rate. The p-value was 0.000, further confirming the statistical significance of this relationship. Therefore, the type of isolation plays a critical role in determining the death rate of COVID-19 patients. These findings emphasize the significance of immunization programs and efficient isolation measures in decreasing COVID-19 mortality. In summary, this study offers significant information that can be used to develop more focused health policies in Indonesia. It emphasizes the crucial importance of vaccination and isolation measures in effectively managing the COVID-19 pandemic. Gaining insight into these factors can assist in customizing health treatments to mitigate risks and enhance clinical outcomes for COVID-19 patients in Indonesia

Abstract

Lempuk fish is a native Indonesian fish that is endemic in the Ranu Grati tectonic lake, Pasuruan Regency, East Java. This research aims to provide information in efforts to domesticate lempuk fish, especially on reproductive aspects related to the microscopic characteristics of gonads and reproductive type of Lempuk fish (Gobiopterus sp). This research uses the primary sexual characteristics method with gonadal histology. The variables observed included the male to female sex ratio, the histological structure of male and female gonads, and the reproductive type of lempuk fish. All data was analyzed descriptively, qualitatively and quantitatively by comparing with related literature. The samples taken in this research were 39 Lempuk Fish (Gobiopterus sp). The samples were then preserved in 10% formalin. The number of samples for histological analysis was 20 individuals, while for morphometric and meristic analysis there were 8 individuals. The results of the research show that morphologically the lempuk fish has a transparent body shape and there is a central line on the body with black or brown pigment and has a total length of around 2-3 cm. Lempuk fish have a type of reproduction that develops asynchronously (asynchronous) because the development of oocytes in the ovaries of female fish shows a variety of developmental stages, namely the initial growth stage (nuclear and perinuclear chromatin), cortical alveolar, vitellogenesis, and the mature stage. The sex ratio is predominantly female with a ratio of 0.64: 0.36. The histological structure of male and female gonads has histological differences that reflect different reproductive functions, where the ovaries play a role in egg cell production, while the testes are responsible for sperm formation. An imbalance in the sex ratio indicates the importance of carrying out domestication steps in order to prevent extinction or continuous population decline.

Abstract

Increasing cultivation activities and intensifying ponds has the potential to give rise to problems, namely increasing cases of disease in grouper fish, namely Zeylanicobdella, causing mass deaths of up to 100%. This study aims to analyze the histopathological picture of grouper skin infested with Zeylanicobdella arugamensis after administering papaya leaf juice with different soaking times and concentrations of papaya leaf juice. The method used was experimental Completely Randomized Design (CRD) with a factorial pattern consisting of two factors, namely factor A, different concentrations of papaya leaf juice consisting of 0% (A1), 0.1% (A2), 0.3% (A3) , and 0.5% (A4), as well as factor B for grouper rearing time consisting of the 0th minute (B1), 60th minute (B2), and 120th minute (B3) with five repetitions. The results of the study showed that the soaking time of papaya leaf juice in grouper fish had an effect on reducing the degree of Zeylanicobdella infestation, there were changes in the histopathological picture and water quality values before and after soaking. Treatments A2, A3, and A4 experienced a decrease in the degree of ectoparasite infestation at 60 and 120 minutes of immersion. The histopathological scoring values for treatments A2, A3, and A4 decreased at 60 and 120 minutes of maintenance. The best treatment combination was 0, 3% and soaking time at 120 minutes. In conclusion, giving papaya leaf juice with a soaking time of 120 minutes with a concentration of 0.3% can reduce the degree of infestation and the histopathological scoring value of Zeylanicobdella in grouper fish.
