Abstract

Human echinococcosis is a zoonotic disease caused by a species of medical importance, the Echinococcus granulosus , an etiological agent of cystic echinococcosis (CE). A radiological examination will be performed for these patients to diagnose Echinococcus granulosus parasite by find echinococcal cysts. All of these patients were involved in this study and during the period starting from October 2023 to the end of January 2024, from all ages of patient from both sex (Males and Females At Al-Sader Medical City, AL-Hakeem General Hospital, AL-Haidarya general Hospital and Al-Hayat Hospital. The mean level of TNF-α in patients is 163.27 pg/ml, which is significantly higher than the mean level of 38.58 pg/ml in controls (p-value < 0.001). The prevalence of hydatid disease in Al-Najaf Al-Ashraf in this study was 33%. Levels of the cytokines IL-6, TNF-α, IL-1ß, and IL-4 are more important in patients with Echinococcus granulosus under 40 years of age than in those over 40 years of age.

Abstract

A significant increase in visits, visitor complaints about employees, and turnover rates that exceed the established standards indicate problems related to workload and motivation that will impact employee performance. The purpose of this study was to determine the relationship model of workload and motivation to employee performance and job satisfaction. The method of conducting this research is using a questionnaire with a Google form. The data analysis technique uses Structural Equation Modeling with maximum likelihood estimation (MLE) so that the sample requirements used must meet a minimum of 100 data. Research results at the final stage, namely model modification, obtained the goodness of fit value and cut-off value at chi-square = 176.653, Significance Probability = 0.054, Cmin/df = 1.194, RMSEA = 0.044, GFI = 0.868, AGFI = 0.794, TLI = 0.975, and CFI = 0.982. From the model test results compared to the cut-off value, all indicators have achieved good results, except AGFI. Indicators reach good results (marginal) so that the next analysis test can be continued. Based on the results of hypothesis testing, the results show that workload has a significant effect on employee performance, work motivation has an insignificant effect on employee performance, workload has a significant effect on job satisfaction, work motivation has a significant effect on job satisfaction, and job satisfaction has a significant effect on employee performance.

Abstract

Clay-shoveler's fracture is a relatively rare stress-type avulsion fracture of the lower cervical or upper thoracic spinous processes, most commonly occurring at C7 and T1. The term of this injury comes from its historical association with manual laborers who lifted and threw big objects, as well as with scooping huge loads of clay in the early 20th centuryThe case report describes A 29 year old male experienced severe back pain after playing golf. at the time of the golf swing, the golf swing did not hit the ball but hit the ground. the patient experienced radiculopathy pain to both hands. A "pop" sound was audible in the spine at the moment of the hitting fault, and there was no paralysis in the lower limbs.The primary method for managing a clay-shoveler fracture is conservative. In most cases, traction of the rhomboid and trapezius muscles on the spinous process leads to nonunion. In rare cases if the pain persists, surgically removing the bone fragment may be a possibility.

Abstract

Anxiety problems experienced can be caused by situations or conditions that a person will face that are considered threatening and endangering themselves so that pharmacological treatment is needed, namely in the form of antianxiety and non-pharmacological. Nonpharmacological therapy in generalist nurses that can be given to anxiety patients includes slow deep breathing. The purpose of this study was to determine the effect of slow deep breathing relaxation therapy on anxiety scores in anxiety patients in the Limboto Health Center working area. The research method used pre-experimental with one group pre-post test design. The study population was patients with anxiety who were in the Limboto Puskesmas working area. The sample in this study was the total population of 15 people. Based on the results showed that the average score of symptoms before slow deep breathing relaxation was 25.73 with a standard deviation of 1.668 and the average score after slow deep breathing relaxation was 15.33 with a standard deviation of 3.222, and the p-value obtained was 0.001 (<α 0.05), meaning that there was an effect of slow deep breathing relaxation therapy on anxiety scores in anxiety patients in the Limboto Puskesmas Working Area.

Abstract

Mood changes are caused by a functioning thought process that experiences maladaptive behaviors that generate negative thoughts that cause the patient to descend behaviors such as withdrawal from the environment associated with depressive episodes, which results in maladaptive coping mechanisms that increase the risk of suicide, so that Mood management can be given to reduce the burden of the problem. The aim of the study was to analyze the effectiveness of the application of mood management to SAD PERSON screening with suicide risk. The research method used a pre-experimental research design with one group pre posttest design, the research subjects of patients at risk of suicide were 15 patients with a total sampling technique, the research instrument used a SAD PERSON observation sheet and the data was analyzed by the Wilcoxon non-parametric statistical test. The results showed that the average score of symptoms before the implementation of mood management was 6.73 with a standard deviation of 1.033 and the average score after the implementation of mood management was 4.80 with a standard deviation of 1.207, and the p-value was obtained with a value of 0.001 (<α 0.05). Conclusion of the effective application of mood management to SAD PERSON screening with suicide risk in the Limboto Health Center.

Abstract

An information system serves as a formal mechanism that supplies management with precise and timely data essential for decision-making, facilitating successful planning, oversight, and operational functions. This research employs a quantitative methodology with a cross-sectional design. The research population comprises employees utilizing computer systems in units linked to SIiMRS, with a sample size of 125 individuals. This research indicates that human resources significantly influence the implementation of SIMRS on service quality at RSU Bethesda Gunungsitoli, with a p-value of 0.000, which is less than 0.05. The hardware significantly influences the implementation of SIMRS on service quality at RSU Bethesda Gunungsitoli, evidenced by a p-value of 0.001, which is less than 0.05. The implementation of software in SIMRS affects the quality of service at RSU Bethesda Gunungsitoli, evidenced by a p-value of 0.014, which is less than 0.05. Data management significantly influences the implementation of SIMRS on service quality at RSU Bethesda Gunungsitoli, evidenced by a p-value of 0.035, which is less than 0.05. The LAN conditions significantly influence the implementation of SIMRS on the quality of service at RSU Bethesda Gunungsitoli, evidenced by a p-value of 0.023, which is less than 0.05. The findings of the multivariate analysis indicated that human resource factors exerted the most significant impact on service quality at RSU Bethesda Gunungsitoli. The continuity variable is indicated as 7.807, signifying that respondents with proficient human resources possess a 7.807 likelihood of enhancing the quality of service at RSU Bethesda Gunungsitoli.

Abstract

Stunting has a big influence on children's growth and development, both motoric and verbal development. Stunting can be prevented by fulfilling quality energy intake. Modification of additional food for toddlers by using local food through a touch of technology is the strategy offered in this research. This study aims to evaluate the effect of fortification of Moringa leaf flour in milkfish crackers on organoleptic characteristics, including color, aroma, texture and taste. The cracker samples were fortified with Moringa leaf flour at concentrations of 0%, 2.5%, 5% and 7.5%. The assessment is carried out by panelists who provide scores for each parameter. The results showed that fortification up to 2.5% did not affect color and aroma, and improved texture, but reduced taste acceptability. Fortification at levels of 5% and 7.5% reduced the acceptability of taste and color although the texture remained good.

Abstract

PT. Rajawali Sumber Rejeki is one of the manufacturing companies that produces cigarettes in Indonesia. The cigarette production process involving large machines has a high potential risk of accidents for employees. This study assesses and proposes improvements to the company's safety culture maturity as a material for evaluating the company's K3 policy so that the number of work accidents in the production process can be minimized. The safety culture maturity assessment involves safety culture variables, namely commitment, leadership, responsibility, competence, engagement & involvement, information & communication, risk, and organizational learning. Data collection was carried out by distributing questionnaires to 60 employees working in the company. Three expert judgments were also involved as determinants of the weight of the safety culture maturity variable using the AHP (Analytical Hierarchy Process) method. In addition, fuzzy theory is also used to reduce the subjectivity of the company's safety culture maturity assessment results. Then the improvement proposal is given with a safety model canvassing based on the results of the safety culture maturity assessment. The results of the study showed that the safety culture maturity of PT. Rajawali Sumber Rejeki is 3.5 which is included in the Complaint level. Based on the safety canvassing model that has been created, the priority for improving the safety culture maturity variables from the first to the last is organizational learning, competence, commitment, information & communication, risk, engagement & involvement, responsibility, and finally the leadership variable.

Abstract

Esophageal atresia is a congenital disorder characterized by a disruption in the continuity of the esophageal lumen. Esophageal atresia may be accompanied by tracheoesophageal fistula, which is a lumen connection between the proximal and or distal part of the esophagus and the airway (trachea). Type C atresia is the most prevalent, which is approximately 88.5% to 90% of cases, where there is proximal esophageal atresia with distal tracheoesophageal fistula. In this case, a 9-day-old infant was admitted to Arifin Ahmad Hospital for thoracostomy and esophagostomy surgery due to continuous mucus discharge. The patient underwent awake intubation and was positioned intraoperatively, with a tilt to the right when an orogastric tube was placed. During surgery, the patient maintained stable hemodynamic and was subsequently admitted to the NICU using a ventilator. Infants with esophageal atresia often show symptoms of hypersalivation and shortness of breath caused by aspiration pneumonia. When the nasogastric tube cannot pass through the esophagus, atresia can be suspected. Radiology studies play an important role in diagnosing esophageal atresia, confirming esophageal atresia with or without fistula and diagnosing other anomalies associated with VACTERL.

Abstract

A 39 year old female patient with a diagnosis of mandibular tumor underwent mandibulectomy and Free Fibular Flap reconstruction. Preoperative evaluation is carried out on patients, namely by carrying out anamnesis, physical examination and supporting examinations to determine the problem and conclude the patient's physical status. The patient's physical status is ASA III. The patient was intubated awake with a video laryngoscope. After that, sedation and muscle relaxants are given. The operation lasted 10 hours with stable hemodynamics. Post-operatively, patients are treated in the ICU, vital signs are observed and post-operative pain is managed. A closed study conducted by the Professional Committee of the American Society of Anesthesiologists (ASA) revealed that the risks and serious complications of anesthesia are most often closely related to airway management problems. Success in dealing with complications depends on early detection of symptoms and corrective action to prevent the situation from getting worse. In this patient, airway management was based on the Difficult Airway Algorithm.
