Abstract

Diabetic wounds are one of the complications of diabetes mellitus and are often accompanied by bacterial infection, which may delay the healing process. The use of natural ingredients as topical antibacterial agents is considered a potential alternative. Robusta coffee beans (Coffea canephora L.) are known to contain bioactive compounds with potential antibacterial activity. This study aimed to formulate Robusta coffee bean extract into an emulgel dosage form and evaluate its antibacterial activity against Staphylococcus epidermidis. This research was an experimental laboratory study. Extraction was carried out using the maceration method with 70% ethanol as the solvent. The emulgel was formulated in three extract concentrations, namely 20%, 30%, and 50%, and its antibacterial activity was evaluated using the well diffusion method, with clindamycin gel as the positive control and emulgel base as the negative control. The results showed that Robusta coffee bean extract contained flavonoids, alkaloids, and tannins. The antibacterial activity test of the extract showed inhibition zone diameters ranging from 15.09 to 18.21 mm, while the emulgel preparation produced mean inhibition zone diameters of 8.03 ± 0.73 mm at 20%, 10.08 ± 0.34 mm at 30%, and 13.08 ± 0.11 mm at 50%. The 50% emulgel formula showed the best antibacterial activity, although it was still lower than the positive control. Based on these findings, Robusta coffee bean extract has the potential to be developed as a natural active ingredient in topical antibacterial emulgel preparations.

Abstract

Free radicals play a crucial role in the development of premature aging and degenerative diseases, thereby necessitating the use of antioxidants to mitigate oxidative damage. Moringa oleifera Lam. leaves and Clitoria ternatea L. flowers are recognized as natural sources of antioxidants with strong activity and potential synergistic effects. This study aimed to formulate a stable facial serum containing a combination of Moringa oleifera and Clitoria ternatea extracts and to determine the formulation exhibiting optimal antioxidant activity. An experimental laboratory approach was employed by preparing serum formulations with varying extract ratios, followed by evaluations of physical stability and antioxidant activity using the DPPH method. The results demonstrated that all serum formulations remained homogeneous before and after the cycling test. Physical parameters, including pH, spreadability, and viscosity, exhibited changes during stability testing but remained within acceptable ranges for serum preparations. The IC₅₀ values indicated strong antioxidant activity for formulations F1, F2, and F3, weak activity for the negative control, and very strong activity for the positive control. Among the combination formulas, the serum containing a 1:1 extract ratio showed the highest antioxidant activity. In conclusion, the combined extract serum of Moringa oleifera and Clitoria ternatea demonstrated good physical and chemical stability, with optimal antioxidant potential observed in the 1:1 formulation.

Abstract

Hepatic cirrhosis is a chronic liver disease often accompanied by serious complications, one of which is spontaneous bacterial peritonitis (SBP), which plays a significant role in increasing morbidity and mortality. Third-generation cephalosporin antibiotics, such as cefotaxime and ceftriaxone, are widely used as first-line therapy in patients with hepatic cirrhosis at risk of SBP. However, the increasing incidence of bacterial resistance and the high cost of treatment require an evaluation of the clinical effectiveness and cost-efficiency of the therapy provided. This study aims to assess the effectiveness and cost of using cephalosporin antibiotics in patients with hepatic cirrhosis at risk of SBP at Royal Prima Hospital Medan during the period 2022–2025. This study uses a pharmacoeconomic approach in the form of Cost-Effectiveness Analysis (CEA) to assess the cost-effectiveness of cephalosporins based on the relationship between treatment costs and patient clinical outcomes. Cost-effectiveness is assessed through the calculation of the Cost-Effectiveness Ratio (CER). The results show that the characteristics of patients are predominantly male and in the adult to elderly age group. This finding is in line with the epidemiological characteristics of liver cirrhosis which is more common in men, especially due to long-term risk factors such as chronic liver disease and alcohol consumption. The study shows that the analysis of the Cost-Effectiveness Ratio (CER) and Cost-Effectiveness Analysis (CEA) values has a positive and significant effect on total medical costs, which indicates that increasing costs per unit of clinical outcomes will have a direct impact on increasing total treatment costs.

Abstract

Hypertension is one of the most common non-communicable disease (NCDs) worldwide. This condition has the potential to lead to serious complications such as stroke, heart disease, and kidney failure. Effective hypertension management involves regular use of antihypertensive medications. However, patient compliance with medication is still relatively low. This non-compliance can lead to suboptimal blood pressure control and increase the risk of complications. This systematic review aims to describe and analyze various factors influencing adherence to treatment in hypertensive patients. The results are expected to serve as evaluation material to help patients improve their medication adherence. The method used was aticle review. Selected article met the following criteria: published between 2020 and 2025, available in full-text form, and employed both qualitative and quantitative research designs. Following a selection process based on these criteria, relevant articles were further analyzed. Findings from 12 studies indicate that the factors most frequently influencing adherence are knowledge, education, motivation, and family support. Additionally, other factors such as occupation, distance from residence to healthcare facilities, the role of heathcare professionals, and perceptions of the disease also contribute to patient adherence. Therefore, patients are advised to be more proactive in increasing self-awareness and practicing self-care, thereby better managing their hypertension and preventing potential complications. The implication of this systematic review is that efforts to improve medication adherence among hypertensive patients must be multidisciplinary and comprehensive, involving patients, families, healthcare professionals, and healthcare services

Abstract

Diabetes mellitus is a chronic metabolic disease with an increasing global prevalence and remains a major cause of microvascular and macrovascular complications. Plant-derived antidiabetic agents have attracted considerable attention due to their therapeutic potential and relatively low adverse effects.Yakon (Smallanthus sonchifolius) leaves contain numerous bioactive compounds that may regulate glucose homeostasis through multiple molecular pathways. This study systematically reviewed the available scientific evidence regarding the antidiabetic activity of Smallanthus sonchifoliusleaves based on in vitro and in vivo studies. A Systematic Literature Review was conducted following PRISMA 2020 guidelines. Literature searches were performed in the Scopus database using the keywords "Smallanthus sonchifolius" OR "yacon leaves" AND antidiabetic OR diabetes mellitus for articles published between 2016 and 2025. Eligible studies included English-language original research articles available in full text investigating the antidiabetic effects of yacon leaves. Twenty-seven articles were identified, and fifteen studies met all inclusion criteria. The findings demonstrated that extracts, fractions, and infusions of yakon leaves consistently reduced blood glucose levels through antioxidant activity, enhanced insulin sensitivity, activation of the IRS-1/PI3K/Akt signaling pathway, inhibition of α-glucosidase and α-amylase enzymes, and modulation of DPP-IV activity. Major bioactive compounds responsible for these effects include polyphenols, flavonoids, chlorogenic acid, sesquiterpene lactones, and fructooligosaccharides. Although current evidence is predominantly preclinical, yakon leaves represent a promising phytotherapeutic candidate for diabetes management and warrant further clinical investigation.

Abstract

Type 2 diabetes mellitus is a global metabolic disorder that requires safer and more effective therapeutic alternatives. Inhibition of α-glucosidase activity is an effective method for treating type 2 diabetes mellitus because it can delay carbohydrate hydrolysis and inhibit postprandial hyperglycemia. Guava fruit (Psidium guajava L.) contains various bioactive compounds with great potential to be developed as an alternative therapy for diabetes mellitus. This study aims to analyze the potential of active compounds in guava fruit as α-glucosidase inhibitors through molecular docking and ADME evaluation. A total of 27 bioactive compounds reported in guava fruit were tested against human α-glucosidase obtained from the Protein Data Bank (PDB ID: 2QMJ). Toxicity analysis was performed using ProTox-II, ADME prediction using pkCSM, and molecular docking using AutoDock Tools. Toxicity analysis revealed that 85% of the tested compounds had an acceptable safety profile, with LD₅₀ values greater than 500 mg/kg. ADME evaluation showed that most tested compounds had good intestinal absorption above 80%. Method validation through acarbose redocking yielded an RMSD value of 1.506 Å. Ligand docking results indicated that citral has potential as an antidiabetic drug candidate, with Gibbs free energy of -4.59 kcal/mol and an inhibition constant (Ki) of 431.62 μM. The interaction of citral with key residues TRP A441, PHE A450, and ASP A542 indicates possible binding to the α-glucosidase active site. This study demonstrates the potential of guava active compounds as α-glucosidase inhibitor candidates for developing type 2 diabetes mellitus therapy and supports further preclinical studies for future clinical application.

Abstract

The prevalence of Diabetes Mellitus (DM) continues to increase globally, including in Indonesia, prompting research to explore natural compounds as alternative therapeutic agents. Oil palm (Elaeis guineensis Jacq.) has attracted attention because it contains antioxidant compounds. This study aimed to evaluate the effect of Elaeis guineensis fruit extract on renal histopathological changes in male Wistar rats (Rattus norvegicus) induced with streptozotocin as a diabetic model. This study used an in vivo experimental approach with a post-test only control design. The samples were divided into five groups: one positive control group, one negative control group, and three treatment groups receiving different doses of the extract. Observations were conducted at the post-test stage by comparing renal histopathological features among all groups. Data were analyzed using the Kruskal-Wallis test, followed by the Mann-Whitney test, with a significance level of p<0.05. Results showed significant differences between the positive control group and the negative control group, as well as with treatment group 2 (P2) and treatment group 3 (P3). The negative control group showed significant differences compared to treatment group 1 (P1) and treatment group 2 (P2). However, treatment group 3 (P3) did not show a significant difference compared to the negative control group. Significant differences were observed between P1 and P3 and between P2 and P3, while no significant difference was found between P1 and P2. In conclusion, Elaeis guineensis fruit extract demonstrated a positive effect in improving renal histopathological damage induced by streptozotocin, indicating its potential as supportive therapy in diabetes mellitus management.

Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia resulting from impaired insulin secretion, insulin resistance, or both. The increasing prevalence of diabetes and limitations of current therapies have encouraged the development of plant-based antidiabetic agents. One promising medicinal plant is Tinospora crispa, which contains various secondary metabolites with reported antihyperglycemic activity. This study aimed to investigate the antidiabetic potential of Tinospora crispa bioactive compounds through network pharmacology and molecular docking approaches. The study was conducted in silico using PubChem, SwissADME, pkCSM, SuperPred, GeneCards, and STRING databases, as well as Cytoscape, PyRx, AutoDockTools, and Discovery Studio Visualizer software. A total of 51 secondary metabolites were identified, of which 33 compounds had available SMILES data and 21 compounds met drug-likeness and favorable pharmacokinetic criteria. Target prediction identified 495 protein targets, while disease target screening yielded 615 diabetes mellitus-related genes. Intersection analysis revealed 16 overlapping genes potentially involved in the antidiabetic mechanism of Tinospora crispa. Protein–protein interaction network and topological analyses identified mTOR as the primary hub gene, showing the highest degree centrality, betweenness centrality, and closeness centrality values. Molecular docking demonstrated that β-stigmasterol showed the strongest binding affinity toward the mTOR receptor, with a binding energy of −8.29 kcal/mol. These findings suggest that Tinospora crispa has potential as an antidiabetic agent by modulating glucose metabolism, energy homeostasis, and insulin signaling pathways, providing a scientific basis for further molecular dynamics simulations and in vitro and in vivo evaluations.

Abstract

Diabetes mellitus is a metabolic disorder characterized by hyperglycemia resulting from impaired insulin secretion, insulin action, or a combination of both. The increasing prevalence of diabetes has encouraged the development of plant-based therapeutic agents capable of exerting multitarget effects. One promising medicinal plant is Nigella sativa, which contains various bioactive compounds with potential antidiabetic activity. This study aimed to analyze the antidiabetic mechanisms of secondary metabolites from Nigella sativa using a network pharmacology approach. The study was conducted in silico using several databases and bioinformatics tools, including KNApSAcK, PubChem, pkCSM, ADMETlab 3.0, SuperPred, GeneCards, Venny, STRING, and Cytoscape. The identification process revealed 47 secondary metabolites with available SMILES data. ADME and toxicity analyses showed that 34 compounds met the safety criteria, demonstrated good oral bioavailability, and were predicted to be non-mutagenic and non-hepatotoxic. Target prediction identified 178 protein targets and 598 glucose metabolism-related genes, with 25 overlapping genes identified through intersection analysis. Protein-protein interaction (PPI) network analysis indicated that PPARG was the primary hub gene, exhibiting the highest degree centrality, betweenness centrality, and closeness centrality values. Additionally, STAT3, FASN, HIF1A, SLC2A1, and GSK3B showed high connectivity within the network. The findings suggest that the bioactive compounds of Nigella sativa may exert multitarget antidiabetic effects through the regulation of insulin sensitivity, glucose metabolism, energy homeostasis, and lipid metabolism.

Abstract

Insulin resistance is a condition characterized by decreased cellular sensitivity to insulin, leading to impaired glucose metabolism and contributing to the development of type 2 diabetes mellitus. The development of plant-based therapies has become an increasingly attractive alternative, one of which is Moringa oleifera, known to contain various bioactive compounds with potential antidiabetic properties. This study aimed to analyze the potential of Moringa oleifera against insulin resistance using network pharmacology and molecular docking approaches. The study was conducted through active compound identification, protein target prediction, protein–protein interaction (PPI) analysis, hub gene identification, and molecular docking simulations using the mTOR receptor (PDB ID: 4DRI). The results revealed 11 overlapping genes between compound-related targets and insulin resistance-associated targets, with mTOR identified as the primary hub gene based on network topology analysis. Docking validation showed an RMSD value of 0.525 Å, indicating that the docking protocol used was valid. Molecular docking results demonstrated that lupeol exhibited the best binding affinity among the tested compounds with a binding energy value of −8.46 kcal/mol and showed interactions with amino acid residues within the receptor binding site. These findings suggest that the active compounds of Moringa oleifera have the potential to modulate protein targets involved in insulin resistance and may serve as promising candidates for insulin resistance therapy through an insilico approach.
