Abstract

Pharmaceutical services are an integral part of health services that focus on optimizing drug use to achieve optimal therapeutic results. Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards at Health Centers is a reference in providing quality pharmaceutical services. This study aims to evaluate the implementation of pharmaceutical service standards at the Keerom District Health Center. This research uses a descriptive method. Data was collected through questionnaires and observation sheets at 6 Health Centers in Keerom District. Data were analyzed using SPSS to calculate percentages and average values. The research results show that the level of implementation of pharmaceutical service standards at the Keerom District Health Center is in the good category with an average value of 78-84%. The categories with the highest scores are the pharmaceutical preparation management category (84%) and the drug information services category (82%). The category with the lowest score is the quality assurance category (78%). In conclusion, the overall implementation of pharmaceutical service standards at the Keerom District Health Center has reached the good category. Improvement efforts need to be made in several categories, such as quality assurance, to achieve optimal pharmaceutical services.

Abstract

Parijoto fruit extract (Medinilla speciosa Blume) contains flavonoids, saponins, tannins, and terpenoids and has antibacterial potential in inhibiting the growth of Staphylococcus aureus bacteria. S. aureus bacteria are bacteria that are capable of forming biofilms. This research aims to determine the potential of extracts and fractions (water, ethyl acetate, and n-hexane) in inhibiting the formation and degradation of S. aureus biofilms. Parijoto fruit was extracted using the maceration method with 96% ethanol solvent. The extract was fractionated using a liquid-liquid partition technique with water, ethyl acetate, and n-hexane as solvents. The antibiofilm activity test against the S. aureus bacteria ATCC 25923 was carried out by treatment inhibiting biofilm formation and biofilm degradation. The method is to read absorbance using a microplate reader with a wavelength of 595 nm. Optical density results to calculate IC50 and EC50 values. The results showed that the extract and fractions (water, ethyl acetate, and n-hexane) had the potential to inhibit the formation and degradation of S. aureus bacterial biofilms. The IC50 of biofilm inhibition in the extract, water fraction, ethyl acetate, and n-hexane against S. aureus bacteria was 2.51; 1.03; 1.10; 1.52 mg/ml. The EC50 value of the biofilm degradation test on the extract, water fraction, ethyl acetate, and n-hexane against S. aureus bacteria was 1.03; 1.17; 1.37; 1.88 mg/ml. The most active fraction of parijoto fruit (Medinilla specosa Blume) in inhibiting and degrading S. aureus bacterial biofilms is the water fraction.

Abstract

Public administration services are one way to demonstrate maximum government performance in providing welfare and meeting community needs. One important sector in public services is pharmaceutical services at Health Centers. This research uses a descriptive method with a cross-sectional design. Primary data was obtained through observation, interviews and documentation. Secondary data was obtained from the 2022 Health Centers Medicine Usage Report and Request Sheet (LPLPO) and the 2021 Health Centers Drug Requirements Plan (RKO). Of the 28 indicators of the quality of pharmaceutical services, 15 of the third indicators of the Health Centers meet the standards, 10 of the third indicators of the Health Centers do not meet the standards, 1 indicators only one Health Center meets and 2 indicators only two Health Centers fulfill. Factors that influence the quality of management of pharmaceutical preparations at the East Sumba Regency outpatient Health Centers are human resources that do not come from pharmacy. The quality of pharmaceutical services at the East Sumba Regency outpatient Health Centers still needs to be improved. Pharmacists are needed as pharmaceutical personnel to maximize pharmaceutical services to the community.

Abstract

Cubeb fruit (Piper cubeba L.) is a plant that is rich in flavonoid compounds and has the potential to be a source of natural antioxidants. Free radicals can cause various dangerous diseases. This study aims to determine the antioxidant activity and total flavonoid content of ethanol extract of fruit cubes using the ABTS method. Cubeb fruit is extracted with ethanol using the maceration method. The antioxidant activity of the extract was measured using the ABTS and Trolox methods as a comparison. Total flavonoid levels were determined using the UV-Vis spectrophotometric method using gallic acid and quercetin as a comparison. The total flavonoid content of the ethanol extract of cubed fruit was 3.5302 ± 0.0918 mgQE and the total phenolic content was 183.0396 ± 0.991 mgGAE. The IC50 value of the ethanol extract of cubed fruit was 82.1023 ± 0.072062 µg/mL and trolox 20.1368 ± 0.2367 µg/mL. In conclusion, the ethanol extract of cubed fruit has strong antioxidant activity (IC50 in the range 50-100 µg/mL) and is rich in total flavonoids.

Abstract

Work stress is a significant global health problem, negatively impacting employee physical and mental health as well as company productivity. Adaptive coping strategies can be an important intervention in reducing the negative effects of work stress. This research aims to evaluate the effectiveness of adaptive coping strategies in reducing work stress and improving employee well-being through meta-analysis. A systematic literature search was conducted on electronic databases (PubMed, PsycINFO, Scopus, Web of Science) to identify randomized controlled studies (RCTs) published between 2018 and 2024. Studies that met the inclusion criteria (working adult participants, adaptive coping strategy interventions, outcomes work stress and/or well-being) data were extracted and analyzed using a random effects model. Twenty-five RCTs involving 4,852 participants met the inclusion criteria. Meta-analysis showed that adaptive coping strategies significantly reduce work stress (pooled effect size = -0.45, 95% CI [-0.58, -0.32], p < 0.001) and improve well-being (pooled effect size = 0.38, 95% CI [0.25, 0.51], p < 0.001) compared with the control group. Subgroup analyzes revealed that adaptive coping strategies focused on problem-solving, social support, and relaxation had the greatest effectiveness. Adaptive coping strategies are effective in reducing work stress and improving employee well-being. These findings support the implementation of adaptive coping strategies as an occupational health intervention to reduce the negative impact of work stress.

Abstract

Transcatheter aortic valve replacement (TAVR) has become an alternative to surgical aortic valve replacement (SAVR) for high-risk patients with severe aortic stenosis (AS). This meta-analysis aims to compare the long-term outcomes of TAVR and SAVR in this population. A systematic search of PubMed, Embase, and the Cochrane Library was conducted up to December 2023. Randomized controlled trials (RCTs) and observational studies comparing TAVR and SAVR with a minimum follow-up of one year were included. The primary outcome was all-cause mortality. Secondary outcomes included cardiovascular mortality, stroke, myocardial infarction (MI), and rehospitalization. Twenty-three studies involving 15,482 patients (TAVR=7,785, SAVR=7,697) were included. The mean follow-up period was 3.2 years (range 1-5 years). There was no significant difference in all-cause mortality between TAVR and SAVR (Hazard Ratio [HR] 1.02, 95% CI 0.95-1.09, p=0.63). Similarly, there were no differences in cardiovascular mortality (HR 1.05, 95% CI 0.96-1.15, p=0.28), stroke (HR 0.98, 95% CI 0.87-1.10, p=0.75), or MI (HR 0.94, 95% CI 0.82-1.08, p=0.39). However, TAVR was associated with a lower rate of rehospitalization (HR 0.85, 95% CI 0.78-0.93, p=0.001). TAVR is a viable alternative to SAVR in high-risk patients with AS, demonstrating comparable long-term survival and safety outcomes. The reduced rehospitalization rate associated with TAVR may be an important consideration for these patients.

Abstract

Intravenous (IV) acetaminophen is increasingly used in multimodal postoperative pain management. This meta-analysis aims to assess the efficacy and safety of IV acetaminophen compared to placebo or other analgesics in adult surgical patients. A systematic search of PubMed, Embase, and Cochrane Library databases was conducted from January 2018 to December 2023. Randomized controlled trials (RCTs) evaluating IV acetaminophen for postoperative pain in adults were included. Primary outcomes were pain intensity and opioid consumption. Secondary outcomes included adverse events and length of hospital stay. Twenty-three RCTs (n=4,128) were included. IV acetaminophen significantly reduced postoperative pain scores compared to placebo (standardized mean difference [SMD] -0.32; 95% CI -0.41 to -0.23) and opioid consumption (SMD -0.28; 95% CI -0.37 to -0.19). No significant difference was found in pain reduction between IV acetaminophen and other analgesics. IV acetaminophen was associated with a lower incidence of nausea and vomiting compared to opioids. IV acetaminophen is an effective and safe analgesic for postoperative pain management in adults, reducing pain intensity and opioid consumption. It may also have a favorable adverse event profile compared to opioids.

Abstract

Preoperative anxiety is a prevalent concern among women undergoing cesarean sections (C-sections), potentially impacting maternal and neonatal outcomes. While pharmacotherapy is commonly used to manage anxiety, non-pharmacological interventions like music therapy offer a promising alternative or adjunct. This study aimed to investigate the efficacy of classical music in reducing preoperative anxiety levels in women scheduled for C-sections. A randomized controlled trial was conducted involving 100 women scheduled for elective C-sections. Participants were randomly assigned to either the intervention group (classical music listening) or the control group (standard care). Anxiety levels were assessed using the State-Trait Anxiety Inventory (STAI) before and after the intervention. Physiological parameters such as heart rate and blood pressure were also monitored. The intervention group demonstrated a significant reduction in both state and trait anxiety scores compared to the control group (p < 0.05). Additionally, the intervention group exhibited lower heart rate and blood pressure levels post-intervention. In conclusion, classical music listening can be an effective non-pharmacological intervention for reducing preoperative anxiety in women undergoing C-sections. The findings suggest that integrating music therapy into standard preoperative care may enhance patient well-being and contribute to improved surgical outcomes.

Abstract

Preoperative anxiety is a common and significant issue that can negatively impact patient outcomes. Effective communication and empathy from anesthesia providers have been suggested as crucial factors in mitigating this anxiety. This study aimed to investigate the relationship between anesthesia provider communication and empathy and preoperative anxiety levels in adult patients undergoing elective surgery. A prospective, observational study was conducted at a tertiary care hospital. Adult patients scheduled for elective surgery under general anesthesia were enrolled. Preoperative anxiety was assessed using the State-Trait Anxiety Inventory (STAI) before the anesthesia provider's preoperative visit and again immediately before surgery. Anesthesia provider communication and empathy were evaluated using the CARE (Communication Assessment Tool for Anesthesia Providers) questionnaire and the Jefferson Scale of Empathy (JSE), respectively. A total of 200 patients were included in the study. The mean preoperative anxiety score decreased significantly from before the anesthesia provider's visit to immediately before surgery (p < 0.001). Higher scores on the CARE questionnaire and the JSE were associated with a greater reduction in preoperative anxiety (p < 0.05). Multivariable analysis revealed that both effective communication and empathy independently contributed to reducing preoperative anxiety. Anesthesia provider communication and empathy play a crucial role in reducing preoperative anxiety. Implementing communication and empathy training programs for anesthesia providers may improve patient experiences and surgical outcomes.

Abstract

Post-spinal anesthesia assessment is a critical component of patient care, ensuring the safe and effective recovery of patients following spinal anesthesia. Anesthesia practitioners play a pivotal role in conducting these assessments, yet their perspectives and experiences remain under-explored. This study aimed to investigate anesthesia practitioner perspectives on post-spinal anesthesia assessment, with a focus on identifying factors influencing assessment practices, challenges encountered, and implications for patient safety and recovery. A qualitative descriptive study was conducted, employing semi-structured interviews with anesthesia practitioners involved in post-spinal anesthesia care. Thematic analysis was used to identify key themes and patterns within the data. Anesthesia practitioners emphasized the importance of comprehensive and individualized assessments, considering factors such as patient comorbidities, surgical procedures, and anesthetic agents used. Challenges identified included time constraints, communication barriers, and variations in assessment protocols. Practitioners highlighted the need for standardized assessment tools and improved interdisciplinary collaboration to enhance patient safety and optimize recovery outcomes. In conclusion, this study provides valuable insights into anesthesia practitioner perspectives on post-spinal anesthesia assessment. The findings underscore the importance of addressing challenges and implementing strategies to improve assessment practices, ultimately contributing to enhanced patient safety and recovery following spinal anesthesia.
