Abstract

Labor pain is an unavoidable part of childbirth, often causing significant distress and anxiety in parturient women. While pharmacological pain management options exist, there is growing interest in non-pharmacological alternatives, such as aromatherapy, due to their potential to provide relief with minimal side effects. Lavender essential oil, in particular, has been recognized for its calming and analgesic properties. This study aimed to investigate the effectiveness of lavender essential oil aromatherapy in reducing pain intensity during the first stage of labor. A randomized controlled trial was conducted, involving 100 participant women experiencing their first stage of labor. Participants were randomly assigned to either the intervention group (n=50), receiving lavender essential oil aromatherapy, or the control group (n=50), receiving a placebo aromatherapy. Pain intensity was assessed using the visual analog scale (VAS) before and after the intervention. The results demonstrated a statistically significant reduction in pain intensity in the intervention group compared to the control group (p < 0.05). The mean VAS score decreased from 7.2 ± 1.5 to 4.3 ± 1.2 in the intervention group, while the control group showed a decrease from 7.0 ± 1.6 to 6.2 ± 1.4. In conclusion, lavender essential oil aromatherapy appears to be an effective non-pharmacological method for reducing pain intensity during the first stage of labor. The findings suggest that it can be a valuable addition to the pain management strategies offered to women during childbirth.

Abstract

Infant massage has been recognized for its potential benefits in promoting infant health and development, as well as enhancing maternal-infant bonding. However, the adoption of infant massage practices can be influenced by various factors, including maternal attitudes and knowledge. Health education interventions have been proposed as a means to improve maternal attitudes and practices related to infant massage. This study aimed to investigate the impact of a structured health education program on maternal attitudes and practices towards infant massage. A randomized controlled trial was conducted involving 120 mothers of infants aged 3-6 months. Participants were randomly assigned to either an intervention group (receiving a structured health education program on infant massage) or a control group (receiving routine care). Data on maternal attitudes and practices towards infant massage were collected at baseline and three months post-intervention using validated questionnaires. The intervention group demonstrated a significant improvement in maternal attitudes towards infant massage compared to the control group (p < 0.001). Additionally, the intervention group reported a higher frequency of infant massage practices and increased confidence in performing infant massage techniques. In conclusion, the findings suggest that a structured health education program can effectively enhance maternal attitudes and practices toward infant massage. The implementation of such programs in healthcare settings may contribute to increased adoption of infant massage, potentially leading to improved infant health and well-being.

Abstract

Tuberculosis (TB) remains a global health challenge, with significant implications for patients' quality of life (QoL). While clinical outcomes are crucial, understanding the broader impact of TB on patients' psychosocial and physical well-being is essential for comprehensive care. This study aimed to explore the multifaceted nature of QoL in TB patients receiving outpatient treatment, delving into both psychosocial and physical dimensions. A cross-sectional study was conducted among TB patients attending outpatient clinics at a tertiary care hospital. The Short Form-36 (SF-36) questionnaire was used to assess QoL across eight domains: physical functioning, role limitations due to physical health, bodily pain, general health perceptions, vitality, social functioning, role limitations due to emotional problems, and mental health. Sociodemographic and clinical data were also collected. Descriptive statistics, correlation analysis, and regression models were employed to analyze the data. The study included 150 TB patients (mean age: 42.3 years, 58% male). The overall QoL was significantly impaired, with the mental health domain showing the most pronounced impact. Factors associated with poorer QoL included female gender, lower socioeconomic status, presence of comorbidities, and longer duration of TB treatment. Notably, social support emerged as a significant predictor of better QoL across multiple domains. TB significantly affects patients' QoL, extending beyond physical symptoms to encompass psychosocial well-being. Addressing mental health concerns, providing social support, and considering socioeconomic factors are crucial components of holistic TB care. The findings underscore the need for interventions that go beyond clinical management to enhance the overall QoL of TB patients.

Abstract

Uterine involution, the process of the uterus returning to its pre-pregnancy state, is a critical physiological event in the postpartum period. Suboptimal involution can lead to complications such as postpartum hemorrhage and prolonged lochia. Postpartum exercise has been suggested to promote uterine involution, but its efficacy in primary care settings remains under-explored. This study aimed to investigate the impact of a structured postpartum exercise program on uterine involution in women receiving care in primary care clinics. A randomized controlled trial was conducted involving postpartum women attending primary care clinics. Participants were randomly assigned to either an intervention group, receiving a structured postpartum exercise program, or a control group, receiving standard postpartum care. The primary outcome was the rate of uterine involution, assessed through serial measurements of fundal height. Secondary outcomes included duration of lochia, postpartum pain, and maternal quality of life. The study enrolled 120 women (60 per group). The intervention group demonstrated a significantly faster rate of uterine involution compared to the control group (p < 0.05). Additionally, the intervention group reported a shorter duration of lochia and improved quality of life scores. No significant differences were observed in postpartum pain between the groups. A structured postpartum exercise program implemented in primary care settings can effectively accelerate uterine involution and improve maternal well-being. These findings underscore the importance of integrating postpartum exercise into routine primary care for postpartum women.

Abstract

Prostate cancer is a leading cause of cancer-related death in men globally. Early detection is critical for improving patient outcomes. Prostate-specific antigen (PSA) testing is a widely used screening tool, but its accuracy in predicting prostate cancer on transrectal biopsy remains a topic of debate. This meta-analysis aims to evaluate the diagnostic accuracy of PSA in predicting prostate cancer on transrectal biopsy. A systematic search of electronic databases (PubMed, Scopus, Web of Science) was conducted to identify relevant studies published between 2013 and 2024. Studies reporting the sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) of PSA for predicting prostate cancer on transrectal biopsy were included. Data were extracted, and the pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR) were calculated using a random-effects model. Six studies met the inclusion criteria, comprising a total of 1,245 patients. The pooled sensitivity and specificity of PSA for predicting prostate cancer on transrectal biopsy were 0.77 (95% CI, 0.75 - 0.82) and 0.68 (95% CI, 0.62-0.73), respectively. The pooled PLR, NLR, and DOR were 3.27 (95% CI, 2.45-4.37), 0.20 (95% CI, 0.14-0.28), and 16.39 (95% CI, 10.27-26.21), respectively. The pooled AUC was 0.87 (95% CI, 0.84-0.90). In conclusion, PSA demonstrates good diagnostic accuracy in predicting prostate cancer on transrectal biopsy. However, it is essential to consider its limitations, including false positives and negatives. Further research is needed to identify strategies to improve the accuracy of PSA testing and reduce unnecessary biopsies.

Abstract

Upper tract urothelial carcinoma (UTUC) and bladder is a rare malignancy with varied histopathological features that influence treatment strategies and patient outcomes. This systematic review comprehensively analyzes the histopathological heterogeneity of UTUC and bladder, focusing on its impact on diagnosis, prognosis, and therapeutic implications. A systematic search of PubMed, Google Scholar, NCBI, and ScienceDirect was conducted to identify relevant studies published in the last 20 years. Articles were included if they focused on the histopathological characterization of UTUC and bladder. A total of 10 studies were included in the review. Histopathological analysis revealed diverse subtypes, including papillary, micropapillary, nested, plasmacytoid, and sarcomatoid variants, each with distinct features and prognostic implications. In conclusion, UTUC and bladder exhibits significant histopathological heterogeneity, which influences tumor behavior and patient outcomes. Integrating histopathological evaluation with molecular profiling can refine risk stratification and guide personalized treatment decisions. Further research is needed to elucidate the complex interplay of these factors and develop targeted therapies for improved management of UTUC and bladder.

Abstract

Continuity of midwifery care (CoMC) is a model of care in which a woman is supported by the same midwife, or a small team of midwives, throughout her pregnancy, childbirth, and the postpartum period. This model has been shown to have several benefits for women and their babies, including reduced rates of intervention, increased satisfaction with care, and improved maternal and neonatal outcomes. This case report describes the experiences of a 21-year-old primiparous woman who received CoMC from a midwife in a community setting. The woman was highly satisfied with the care she received, reporting that she felt empowered and in control of her pregnancy and childbirth experience. She also felt that the midwife was a trusted source of support throughout her journey. In conclusion, this case report provides further evidence of the benefits of CoMC for women and their babies, particularly for young mothers who may require additional support and guidance. It highlights the importance of providing woman-centered care that is responsive to the individual needs of each woman.

Abstract

Neonatal sepsis is a major cause of illness and death in newborns worldwide. Early diagnosis is critical for quick treatment and better outcomes. However, traditional markers like white blood cell count and immature-to-total neutrophil ratio are often not sensitive or specific enough. This meta-analysis reviewed PubMed, Embase, and the Cochrane Library from January 2013 to December 2023. It included studies that assessed the accuracy of procalcitonin (PCT), interleukin-6 (IL-6), and C-reactive protein (CRP) in diagnosing neonatal sepsis. The QUADAS-2 tool was used to check the quality of the studies. Six studies, including 1245 newborns, were analyzed. PCT had the highest pooled sensitivity (0.85, 95% CI 0.80-0.90) and specificity (0.82, 95% CI 0.75-0.89), followed by IL-6 (sensitivity: 0.78, 95% CI 0.70-0.85; specificity: 0.75, 95% CI 0.65-0.83). CRP was less sensitive (0.70, 95% CI 0.60-0.79) and specific (0.68, 95% CI 0.58-0.77). In conclusion, new biomarkers, especially PCT, are promising for early diagnosis of neonatal sepsis. However, more large-scale studies are needed to confirm these findings.

Abstract

The consumption of chocolate, especially varieties high in sugar, can disrupt the delicate pH balance in the oral cavity, increasing the risk of dental caries. Pineapple, with its rich enzymatic and organic acid profile, presents a potential natural solution to counteract this acidic challenge. This study investigates the efficacy of pineapple consumption in mitigating the pH-lowering effects of chocolate and its impact on saliva volume, a crucial factor in maintaining oral health. A quasi-experimental design with a pre-test and post-test control group approach was employed. Participants included children aged 6-9 years (n=40), divided into two groups: a control group (chocolate consumption only) and an intervention group (chocolate consumption followed by pineapple consumption). Saliva volume and pH were measured before and after treatment, and the data were analyzed using paired t-tests. Pineapple consumption significantly increased saliva volume in the intervention group (from 2.01±1.29 mL to 5.3±0.88 mL, p<0.05), exceeding the volume observed in the control group. Furthermore, the decrease in saliva pH in the intervention group (from 7.45±0.13 to 6.25±0.37) was more controlled compared to the control group (from 7.30±0.09 to 5.28±0.25). In conclusion, the consumption of pineapple after chocolate effectively stimulates saliva production and helps maintain a balanced oral pH, mitigating the acidic effects of chocolate and potentially reducing the risk of dental caries. These findings suggest the potential of pineapple as a natural, dietary approach to support oral health.

Abstract

Phosphorus is a vital mineral required for the proper mineralization of bones and teeth, particularly during childhood development. Deficiency in phosphorus intake can lead to delayed dental development, including the eruption of permanent teeth, which may result in oral health complications such as malocclusion and increased risk of dental caries. A cross-sectional study was conducted among 50 children from SD N 15 Koto Lalang, Padang City. Participants were categorized into two groups based on the eruption status of their permanent canines: normal eruption and delayed eruption. Phosphorus intake was measured through dietary surveys, and statistical analysis, including the Mann-Whitney test, was applied to assess the relationship between phosphorus intake and tooth eruption timing. The median phosphorus intake in the normal eruption group was 597.30 mg, significantly higher than the 440.00 mg observed in the delayed eruption group (p = 0.000). The interquartile range (IQR) for the delayed group was narrower, suggesting a more uniform phosphorus deficiency. In conclusion, phosphorus deficiency is a significant factor contributing to delayed eruption of permanent teeth in children. Ensuring adequate phosphorus intake through proper nutrition is crucial for timely dental development and the prevention of long-term oral health issues. Further studies should explore the interaction of phosphorus with other micronutrients to provide a comprehensive understanding of its role in dental health.
