Abstract

No studies have analyzed the path of predicting the experience of cognitive dysfunction by considering various characteristics in elderly, especially focusing on sleep duration. Thus, this study aimed to predict the experience of cognitive dysfunction according to sleep duration in older individuals. This cross-sectional study used data from 3,361 older individuals from the 2021 Community Health Survey (CHS). Participants were included in two groups according to their experience of cognitive dysfunction (yes or no). Sleep duration was categorized into the following three groups: lack of sleep(<6h), normal sleep (6 to <10h), and oversleep (≥10h). Decision tree and logistic regression analyses were used to identify factors related to cognitive dysfunction in elderly. According to the decision model, those who slept for ≥10h had depression and experienced the highest rate (89.2%)of cognitive dysfunction. In contrast, people aged 65-74 years with a lack of sleep or average sleep duration and low stress levels were the least likely to experience cognitive dysfunction (63.0%). Older individuals who were asleep for ≥10h and had depression showed the highest rate of cognitive dysfunction. Community-based programs to improve cognition in the elderly or healthcare providers caring for the elderly need to continuously assess and consider their age, sleep time, and depression to prevent and manage cognition dysfunction in elderly.

Abstract

Cryptosporidium sp. is one of the waterborne protozoa involving water as the main medium of transmission causing cryptosporidiosis in animals and humans and its presence is influenced by various things including hygiene and sanitation of the community. This cross-sectional study aims to determine the association between the implementation of the community-led total sanitation and the presence of Cryptosporidium oocysts in clean water sources. The research was conducted from October 2020 to June 2021 in Sukokerto Village, Sukowono, Jember Regency. The data were obtained from interview with the respondents using structured questionnaires. The presence of oocysts was examined by microscopic observations with sedimentation methods and Modified Ziehl=Neelsen staining, and the data were analyzed using Spearman rho statistical test. The results showed that there were 12 (20.3%) household with good category at the implementation of community-led total sanitation, 28 (47.5%) household with moderate category, and 19 (32.2%) household with poor category. The presence of Cryptosporidium oocysts was found in 6 (10.2%) samples of clean water sources. The results of statistical tests with Spearman rho showed a p-value=0.098, so that there was no significant association between the implementation of community-led total sanitation and the presence of Cryptosporidium oocysts in clean water sources in study location.

Abstract

Early adolescents are experiencing various changes in themselves and must adapt to them. In this process, many problems can lead to deviant behavior, one of which is self-harm behavior. Self-harming behaviors can cause harm to oneself and can even lead to suicide. The purpose of this study was to determine the relationship between parenting styles and self-harm behavior in early adolescents in the agricultural area of Jember Regency. This study used a correlational design with a cross-sectional approach. The sampling technique used was proportionate stratified random sampling with 376 respondents. The instruments used were the Parenting Authority Questionnaire (PAQ) and the Deliberate Self-Harm Inventory (DSHI)—data analysis using the contingency coefficient correlation test. The results showed that the most common parenting style was authoritative parenting, with as many as 242 respondents (64.4%), and the most common self-harm behavior was low self-harm, with as many as 319 respondents (84.8%). The correlation test results show that the ρ value is 0.000, which means a significant relationship exists between parenting styles and self-harm behavior in the agricultural area of Jember Regency. Parenting provided by parents ideally or means not restraining too hard and facilitating adolescents to express their opinions can trigger early adolescents' coping mechanisms to be more adaptive by behaving, not deviating, or not committing self-harm behavior. This study can be used as a guideline for schools, mental health nurses, and communities to develop appropriate interventions to address the problem of self-harm and parenting in early adolescents.

Abstract

Sibling rivalry is a competitive feeling that occurs between siblings. Children will show their sibling rivalry by behaving temperamentally, such as crying loudly for no reason, extreme behavior to bind their parents' attention, or even behaving violently and even committing violence against their younger siblings. This jealousy does not always arise in the first child who will have a younger sibling, where some research results mention that second and third children can also feel jealous of their younger siblings. Factors associated with the incidence of sibling rivalry are the level of knowledge, parental attitudes, age gap and gender of the child. The purpose of this study was to determine the factors associated with the incidence of sibling rivalry in children aged 3-10 years in the agricultural area. This study used a descriptive analytic research design with a cross-sectional approach with a sample of 108 respondents. The results showed that the level of knowledge and attitudes of parents were related to the incidence of sibling rivalry with the results of the level of knowledge p-value 0.02 and parental attitudes with p-value 0,03 which means a significant relationship between the two variables. Advice for parents to increase cooperation in overcoming sibling rivalry and find ways so that it does not have a negative impact on child development.

Abstract

The most common medical issue that elderly people face in a long-term care facility is pain. Nurses play an important role in assisting residents with pain management. This study looked at nurses' pain management practices in long-term care facilities. This study took place at a long-term care facility in Ontario, Canada, a 160-bed nursing home for the elderly that offers a variety of nursing and medical care services. A one-hour semi-structured focus group interview was conducted. The population of this study included 45 nurses. Using a purposive sampling strategy, the researcher selected a sample of 25 nurses. To identify recurring issues, the data was reviewed using qualitative data analysis. This study revealed the importance of identifying and overcoming barriers to effective pain management and reinforcing good practices in long-term care homes; better pain management practises are required to manage pain in a long-term care home. This study demonstrated the significance of identifying and overcoming barriers to effective pain management and reinforcing best practices in long-term care homes. As a result, improved pain management practises are required to manage pain in a long-term care home effectively. The overall benefits of pain management practice in long-term care homes increase nurses' clinical knowledge in the care of nursing home residents.

Abstract

Ventilator is a medical used to help patients to breath in the Intensive Care Unit (ICU). Patients with ventilator are at risk of experiencing Ventilator Associated Pneumonia (VAP). VAP is one of the Health Care Associated Infections. VAP contributes to the morbidity and mortality of patients treated in the ICU. One of the efforts to prevent and control VAP is to implement a VAP bundle. This study aims to describe the implementation of the VAP bundle in the ICU in July-September 2022. This type of research is descriptive analytic which describes the application of the VAP bundle in ICU, used purposive sampling with inclusion criteria of patients who had used a ventilator for more than 48 hours and did not experience pneumonia at the first time they were put on ventilator. The research sample obtained was 30 respondents. The results of the study showed that 50% of respondent were in the age range of 50-59 years old, 60% were female and 66.7% were a post-surgical patient. The implementation of hand hygiene was 86.7%, the implementation of head up 30-45 was 96.7%, the implementation of giving peptic ulcer disease prophylaxis was 93.3% and the implementation of giving deep vein thrombosis prophylaxis was 26.7%. VAP in July-September still occurs. The average VAP incidence rate in July-September 2022 is 2.53‰. Conclusions and recommendations, the VAP bundle has been implemented in the ICU, progressing but not yet optimal, efforts to prevent VAP are needed.

Abstract

Open fracture of the tibia is a complex injury with multifactorial outcomes and variable prognosis. This final scientific work by nurses aims to determine nursing care for physical mobility disorders with range of motion free active exercise in post-ORIF surgery tibia fracture patients. This scientific work uses a descriptive method with a case study approach which was carried out on 30 October – 02 November 2023. The results of the study showed data that the patient said he had pain in his left leg, did not dare to move his left leg, the back of his left leg was swollen. The nursing diagnosis of impaired physical mobility is related to pain. Nursing interventions are ambulation support, mobilization support and pain management with the criteria of increasing physical mobility. Innovation intervention range of motion free active exercise with 2 times the frequency of 2x8 steps. The nursing evaluation showed that physical mobility problems had decreased. Conclusion: range of motion free active exercise can increase range of motion, increase muscle strength, reduce complaints of pain, reduce anxiety when moving, and reduce edema. Suggestions for health workers can apply the results of non-pharmacological therapy research, especially range of motion free active exercise, to treat post-operative patients with complaints of physical mobility problems.

Abstract

The Covid-19 pandemic has made hospital screening more stringent. All patients undergo an antigen swab examination, and this examination can cause anxiety for the patient and family. This study aims to identify family support for the anxiety of patients undergoing Rapid Tests while in hospital. The independent variable in this study is family support, while the dependent variable is the anxiety of patients undergoing rapid COVID-19 screening tests. This study had 154 respondents using an accidental sampling technique. This research design uses a correlation type with a cross-sectional approach. Bivariate statistical tests use Kendall's Tau. The results of this study were that respondents had good family support of 71.4%, the anxiety of patients undergoing the COVID-19 rapid test screening was mild anxiety of 50.6%, and there was a relationship between family support and the anxiety of patients undergoing the COVID-19 rapid test screening in Hospital with p-value = 0.000 (p<0.05). Family support has a positive impact in reducing patient anxiety when undergoing a rapid COVID-19 screening test. Therefore, interventions involving the family in the patient care process can be considered a strategy to improve the patient's psychological well-being. Further studies are recommended to explore specific mechanisms by which family support may influence patient anxiety in other medical contexts. Keywords: Covid-19, Family Support, Anxiety, Rapid Test

Abstract

Preeclampsia in pregnancy is a condition that can threaten the health of both mother and baby. Optimal midwifery service standards are necessary and very important to be implemented so that they have an impact on reducing maternal and infant mortality rates. The aim of the research is to analyze the influence self-efficacy and the assertiveness of midwives in early detection of preeclampsia in pregnant women. Type of observational research design cross sectional. The research subjects were 100 independent practicing midwives in the work area of ​​the Jombang Regency Health Center, taken using techniques Multistage random sampling. Exogenous variables self-efficacy and assertiveness and the endogenous variable is preeclampsia detection. The instrument uses a questionnaire. Data were analyzed quantitatively with correlation tests Chi-Square and logistic regression. The research results showed that midwives with self-efficacy good ones (72%) and midwives with high assertiveness (72%). The analysis results show that the T-statistic value self-efficacy of 4.196 and the T-statistic value of assertiveness is 4.099, which is the significance level self-efficacy of 0.032 and assertiveness of 0.028 (p<0.05). Thus, H0 is rejected, and Ha is accepted, this shows that self-efficacy and assertiveness influence early detection of preeclampsia in pregnant women. Self-efficacy and the positive assertiveness of midwives has a strong direct influence on midwives' performance in early detection of preeclampsia in pregnant women. Self-efficacy and assertiveness have equally important contributions in building individual characteristics so that they do not have significant differences.

Abstract

Biomedical waste (BMW) poses significant health risks due to its potential to spread infections and environmental harm. Effective management is crucial to mitigate these risks. This study aimed to assess the knowledge and practices of senior nurses at a tertiary hospital in Bangladesh regarding BMW management. A descriptive correlational study was conducted from July 2019 to July 2020. Data were collected using validated questionnaires on sociodemographic characteristics, knowledge (14 items), and practices (13 items) related to BMW management. Statistical analyses, including descriptive statistics and bivariate analyses, were performed using SPSS. The study included 112 senior nurses with an average age of 29.26 years. Majority were female (88.4%) with basic B.Sc. or post basic B.Sc. education (59.8%). The nurses demonstrated varied levels of knowledge (low: 36.6%, moderate: 39.3%, high: 24.1%) and practices (low: 41.1%, moderate: 25.9%, high: 33.0%) regarding BMW management. A significant correlation was found with higher education level in both knowledge (p=0.024) and practice (p=0.049) levels. However, no significant correlations were observed based on age, sex, years of experience, or specific workplaces. The study highlighted gap in knowledge and practices among senior nurses regarding BMW management at the tertiary hospital. Targeted training programs and policy improvements are recommended to enhance BMW management practices, thereby reducing health risks and environmental impact associated with BMW.
