Abstract

Emergency department overcrowding was a obstacle of providing timely and effective care in emergency department. This condition had negatively impact on many aspect  of care proccess. There were different finding related to impact of emergency department overcrowding in care process. The aim of the study was to analyze research articles that discusses emergency department overcrowding and its potential impact on care proccess. The study was literature review using database journal resources from PubMed, Proquest, and ScienceDirect. Emergency department (ED), Overcrowding, and impact we used as keyword. Inclusion criteria of the study were original article, Published at 10 years later (2012-2022), written in english language, focus on patient, staff, or health care system level. Whereas, the exclusion criteria were overcrowding because of other condition (endemic or infection disease), and not focus on patient, staff, or health care system. 9 selected articles that met inclusion criteria were analysed based on author, years, title, and main idea of the study. The finding of the study were emergency department overcrowding had negatively impact on care process including patients, staff, and healthcare system. Impact on patients including increasing mortality and admission rate, impact on staff including increasing workload and non-adherence for practicing best guidelines, meanwhile impact on healthcare system including innapropriate triage proccess and increasing Length of Stay (LOS). Need an effort to decrease impact of ED overcrowding especially on patient, staff, and healthcare system. The result of the study could be used as considering to formulate an effort to reduce impact of ED overcrowding by controlling patient, staff, and healthcare system factors.

Abstract

The elderly is a life cycle experienced by every human being and is generally susceptible to diseases, especially degenerative diseases that affect their “health status.” The results showed that the prevalence of chronic diseases in the elderly is increasing over time. In 2015, the elderly morbidity rate was recorded at 28.62% which had an impact on increasing disability and decreasing quality of life. Family support is the attitude, action, acceptance of the family towards its family members in the form of emotional, instrumental, informational, rewarding support. This study was conducted to explain family support for the “health status” of the elderly in the work area of the Pucang Sewu Health Center in Surabaya. Descriptive research method, data collection is carried out directly using questionnaires, descriptive data analysis using percentages. The results showed emotional support was mostly good (70%), instrumental support, informational support and award support were almost entirely good (84%, 86%, and ,90%). This research is useful so that families provide optimal support to the elderly so that the elderly can achieve optimal “health status.” It is recommended to increase family support and the quality of health services so that it is beneficial for the elderly in improving their “health status” and reducing their health complaints.

Abstract

The COVID-19 pandemic has revealed how poorly prepared the world is for a pandemic and how easily viruses spread in our interconnected world. The response to COVID-19 in Indonesia has been hampered by a surge in infection and re-infection cases, an ill prepared of healthcare system, low booster vaccination coverage, the emerging of new sub variant, inconsistent implementation of public health measures, and communication barriers. Nevertheless, as infectious disease outbreaks become more frequent, it is hoped that national health security resources will be identified and strengthened. This will enable the nation to keep pace with spreading diseases and become more resilient to future threats. COVID-19 exposes strengths and weaknesses while also creating threats and opportunities. An analysis of the COVID-19 pandemic strategy was conducted using SWOT (Strengths, Weaknesses, Opportunities, and Threats) analysis based on Indonesia's experience with the response and countermeasures to COVID-19. The objective of this study is to analyse all the factors involved in the COVID-19 pandemic response so that lessons can be learned towards an effective public health emergency response. The results highlighted strengths, identified weaknesses, and provided recommendations for the future. This would be useful in responding to current threats to global health security and preparing for future ones.

Abstract

The immortal phenotype of cancer cells is a significant trait, and in the vast majority of malignancies, the enzyme telomerase is essential for sustaining the cancer cells' limitless capacity for replication. The absence of telomerase expression and the lack of an immortal phenotype in normal adult tissues suggest that telomerase is a prospective therapeutic target for the treatment of a variety of tumor forms. At every cell division, telomeres will shrink if telomerase is not active. Apoptosis, cell senescence, and chromosomal instability are initiated when the telomeres become critically short. Telomeres are stabilized in the majority of rapidly expanding cancers by reactivating telomerase. In numerous tumor forms, it has been demonstrated that telomerase inhibition inhibits the development of cancer cells. Telomerase inhibitors have been discovered over the past 10 years as a result of substantial basic research into the mechanisms regulating telomeres, which may offer a potent, nearly universal cancer treatment approach. A short-chain oligonucleotide called imetelstat [GRN163L, Geron Corporation] has a high affinity and specificity for the RNA component of telomerase's template region [hTR or hTERC]. Researchers conduct an examination of articles that are in accordance with the issue to be studied. Articles used in literature review are obtained through the database of international journal providers through PubMed, we investigated clinical studies and discussed what happened in these clinical studies and the extent of the effectiveness of imetelstat in treatment of cancer. Articles proved that imetelstat could enhance cancer treatment. Articles proved that Imetelstat is promising therapeutic agents for cancer treatment. In this review, we suggest that formulating and following treatment, Further studies are needed to determine the related mechanisms to enhance Imetelstat efficacy.

Abstract

Cultural diversity in the Australian population is significant. It challenges health professionals in Australia's acute and critical care settings to provide culturally competent care for ethnic minority patients. Chinese Australians are the largest ethnic minority group in Australia. However, their experiences in acute hospital settings remain largely unknown. Studies on this patient group are much needed to address the research gap and add research evidence on transcultural nursing for the international community. The aim of this study was to explore Chinese patients’ and their family caregivers’ perceptions of care quality in Australian hospitals. A qualitative descriptive study design was used in this study. Adult Chinese patients born in mainland China and who had an acute and critical care admission were recruited from metropolitan hospitals in South Australia in 2019 using the snowball method. Semi-structured interviews were conducted in Mandarin. Data were transcribed verbatim in simplified Chinese for thematic analysis. Results were translated to English and checked by a bilingual team member. Ten participants participated in the study. Four main themes were identified from the data analysis: 1) the feeling of being disoriented in the ICU; 2) desire for comfort; 3) lack of engagement in treatment and care plans; and 4) lack of participation in quality improvement feedback to the care system. Chinese people have unique cultural beliefs, communication patterns, and health-seeking behaviors. To provide culturally competent care for acute and critically ill Chinese patients, health professionals are required to incorporate transcultural nursing theory, knowledge, skills, and attitudes into everyday practice.

Abstract

This review article aimed to assess the existing literature regarding the digital transformation of healthcare in the digital economy. The review employed narrative synthesis. Moreover, this systematic review made use of a number of databases, including EBSCO, Google Scholar, Scopus, Web of Science, and ScienceDirect. The documentary method and content analysis were used via systematic review to analyze the data. The review of literature suggested that digital technologies have the potential to improve healthcare delivery, enhance patient outcomes, and increase access to healthcare services. The studies reviewed in this article demonstrate that digital transformation in healthcare can lead to improved efficiency, reduced costs, increased patient satisfaction, and better patient outcomes. The use of digital technologies such as telemedicine, electronic health records, wearables, and mobile health applications has already started to revolutionize healthcare delivery. However, the adoption of these technologies is not without its challenges, including privacy and security concerns, regulatory barriers, and resistance from healthcare providers. It is therefore important for healthcare organizations and policymakers to address these challenges and implement strategies to ensure the effective integration of digital technologies into the healthcare system. Overall, the findings suggest that the digital transformation of healthcare is a critical area of research that has the potential to significantly transform the healthcare industry and improve health outcomes for individuals and communities.

Abstract

Anxiety is a judgement and emotional response to something dangerous. Anxiety is strongly instability, decreased income, sudden layoffs, difficulty finding employment, and loss of life in a matter of days after contracting Covid-19 which could cause anxiety. The purpose of this study was to determine the level of anxiety about Covid-19 in the community of RT I RW XII Manyar Sabrangan Village. The sampling technique in this study used an incidental sampling technique with a total of 110 people. The results of this study indicate that people with anxiety levels based on age mostly experience severe anxiety with the highest presentation at the age of 18-25 years (57.15%), while based on education most experience moderate anxiety with the highest presentation in junior high school education (33.3%) and anxiety levels based on work almost all experience moderate anxiety with the highest presentation as self-employed (88.88%). It is expected for every community to continue to discipline themselves in implementing health protocols such as wearing masks, washing hands, maintaining distance and carrying out Covid-19 vaccinations.

Abstract

Electrocardiography (EKG) is a method commonly used to measure the performance of the human heart through the heart's electrical activity. To obtain the ECG signal, a lead is carried out using electrodes attached to the skin surface which are recorded in the frequency range 0.05-150Hz. The characteristics of the ECG signal consist of the PR interval, QRS complex and QT interval, from these waveforms doctors can diagnose a disease. However, to get a quality ECG signal, there are often disturbances, such as interference with the 50Hz frequency from electrical grids, respiratory movements, or it could also be caused by the ECG processing algorithm, so it is feared that an error will occur in the diagnosis. From these problems, an appropriate ECG filter is needed to reduce the error rate in generating a wave and maintain signal quality so that it can be accepted clinically. This study aims to design an ECG using a 3rd order filter. The ECG input signal is amplified by an instrument amplifier with 100-fold gain, then a filter process is carried out using a 3rd order Low pass filter to reduce noise interference and then a notch filter is used to ward off the 50Hz network frequency. The results of making a 3rd order filter are able to reduce noise and be able to detect ECG signals properly. It is hoped that this research can be used as a reference for filter design for EKG production and can be utilized in clinical use.

Abstract

Osteoarthritis (OA) is a degenerative joint disease that affects millions of people worldwide. The disease frequently results in discomfort, edema, and stiffness, which can impair movement, make everyday tasks challenging, and even render a person disabled. Since osteoarthritis is presently incurable, researchers are working hard to find new treatments. Adult Mesenchymal stem cells (MSCs) are the most common stem cells used in the therapy of OA in clinical trials. They are simple to extract for researchers from bone marrow or adipose. Depending on the kind of tissue that contains them, MSCs can develop into cartilage, bone, muscle, tendon, ligaments, or fat. For this reason, scientists are researching MSCs in OA stem cell therapies. According to research, MSCs produce substances that promote healing and reduce pain. According to some research, they may aid in reducing pain, swelling, and loss of motion when injected into a joint. Researchers conduct an examination of articles that are in accordance with the issue to be studied. Articles used in literature review are obtained through the database of international journal providers through PubMed, we investigated clinical studies and discussed what happened in these clinical studies and the extent of the effectiveness of stem cells in treatment of osteoarthritis. Different patients have responded differently to this therapy. While some people have experienced pain others have not. Research is still required to determine whether stem cell therapies for OA are effective and why some people respond to the therapy more favorably than others.

Abstract

The growth of the fetus in the womb is one indicator that determines fetal well-being. This study aims to assess the influence of education on the care of pregnancy in pregnant women with chronic energy deficiency in Kendari City. This was a quasi-experimental design using the pre and posttests. Group 1 was given the education intervention using module. Group 2 was given the education intervention using MCH book. The research samples were as many as 60 people. Data collection instruments were in the forms of the questionnaire, centimeter tape. The data were analyzed by the Wilcoxon and Kruskal Wallis’ tests. The results showed that Education increases the knowledge of pregnant women about the care of pregnancy. Education increases the attitude of pregnant women about the care of pregnancy. Education improves the behavior of pregnant women about the care of pregnancy.
