Abstract

Penelitian ini bertujuan untuk menganalisis Kedudukan Hukum Fatwa DSN-MUI No:107/DSN-MUI/XI/2016 Tentang Pedoman Penyelenggaraan Rumah Sakit Berdasarkan Prinsip Syariah) dan Hukum Positif.. Metode penelitian yang digunakan dalam penelitian ini adalah penelitian deskriptif kualitatif dan penelitian normative atau studi kepustakaan didukung dengan penelitian empiris, yang dilakukan dengan proses wawancara mendalam (in-depth interview), diskusi dan studi literatur dalam rangka mendapatkan informasi dari sumber primer dan sekunder. Hasil penelitian menunjukkan bahwa Fatwa DSN-MUI Nomor 107 Tahun 2006 saat ini merupakan sebuah anjuran atau rekomendasi, saat ini belum merupakan peraturan perundang-undangan atau hukum positif atau hukum nasional yang berlaku di Negara Republik Indonesia, walaupun pada saat ini Penyelenggaraan Rumah Sakit Syariah di Indonesia mendasarkan pada Fatwa DSN-MUI Nomor 107 Tahun 2016 tersebut. Fatwa DSN-MUI Nomor 107 Tahun 2016 tersebut baru bisa dinyatakan sebagai peraturan peraturan perundang-undangan atau hukum positif atau hukum nasional atau hukum yang berlaku di Negara Republik Indonesia, yaitu apabila Fatwa Nomor 107 Tahun 2016 tersebut tersebut sudah dipositifasi dalam bentuk Peraturan Perundang-undangan sebagaimana dimaksud dalam Undang-Undang tentang Pembentukan Peraturan Perundang-Undangan sehingga perlu dibuat Peraturan dan Perlu Penguatan Kedudukan Fatwa DSN-MUI Nomor 107 Tahun 2016 dalam Pengaturan Hukum Positif

Abstract

ABSTRACT Belleza Kedaton Hospital Bandar Lampung faces challenges in ensuring the quality of health services in the midst of its transformation into a general hospital and the implementation of a partnership with BPJS Health since 2024. This study aims to examine the relationship between service quality dimensions (Tangibles, reliability, responsiveness, assurance, and empathy) with patient satisfaction, and its impact on hospital operational performance. This study uses quantitative with a Cross Sectional approach. The sample was 100 general patient respondents and 100 BPJS patients, with a Stratified Random Sampling technique. Data were collected through SERVQUAL (Service Quality) questionnaire and numeric scale for satisfaction and analyzed using Univariate. The results showed the gap score of attributes - attributes of statements based on the SERVQUAL dimension in patients before BPJS in 2023 the highest gap score was at attribute statement number 27 “The nurse informs and explains the actions to be taken” of 1.75. While the lowest gap score is on statement attribute number 41 “Ease and speed of the administrative process” of 0.06. In 2024, the highest gap score is on statement attribute number 31 “Patient administration procedures are easy and straightforward” by 2.17. While the lowest gap score is on statement attribute number 41 “The appearance of health workers and personnel (doctors, nurses) is clean and neat” by 0.17. Based on the results of the study, it can be seen that the analysis of customer perspectives based on the emphaty dimension decreased satisfaction in 2023 respondents who stated that they were satisfied by 71 people (71.0%) and decreased in 2024 to 47 people (47.0%), the Tangible dimension found a decrease in satisfaction in 2023 respondents who stated they were satisfied by 65.0% and decreased in 2024 to 49.0%, the reliability dimension of service performance satisfaction decreased satisfaction in 2023 respondents who stated they were satisfied by 77. 0% and decreased in 2024 to 46.0%, the assurance dimension of service performance satisfaction decreased satisfaction in 2023 respondents who expressed satisfaction of 74.0% and decreased in 2024 to 42.0%, the responsiveness dimension of service performance satisfaction decreased satisfaction in 2023 respondents who expressed satisfaction of 70.0% and decreased in 2024 to 40.0%. Based on the analysis of service performance satisfaction, it can be seen that there was a decrease in satisfaction in 2023 respondents who stated that they were very satisfied by 46.0% and decreased in 2024 to 14.0%.  Keywords: Customer Perspective, Satisfaction, Service PerformanceABSTRAK Rumah Sakit Belleza Kedaton Bandar Lampung menghadapi tantangan dalam memastikan mutu pelayanan kesehatan di tengah transformasi menjadi rumah sakit umum dan penerapan kemitraan dengan BPJS Kesehatan sejak 2024. Penelitian ini bertujuan untuk mengkaji hubungan antara dimensi mutu pelayanan (Tangibles, reliability, responsiveness, assurance, dan empathy) dengan kepuasan pasien, serta dampaknya terhadap kinerja operasional rumah sakit. Penelitian ini menggunakan kuantitatif dengan pendekatan Cross Sectional. Sampel yaitu sebanyak 100 responden pasien umum dan 100 orang pasien BPJS, dengan teknik sampel Stratified Random Sampling. Data dikumpulkan melalui kuesioner SERVQUAL (Service Quality) dan skala numeric scale untuk kepuasan dan dianalisis menggunakan Univariat. Hasil penelitian menunjukkan gap skor atribut – atribut pernyataan berdasarkan dimensi SERVQUAL pada pasien sebelum BPJS tahun 2023 gap skor yang paling tinggi ada pada atribut pernyataan nomor 27 “Perawat menginformasikan dan menjelaskan tindakan yang akan dilakukan” sebesar 1,75. Sedangkan gap skor yang paling rendah ada pada atribut pernyataan nomor 41 “Kemudahan dan kecepatan proses administrasi” sebesar 0,06. Tahun 2024 gap skor yang paling tinggi ada pada atribut pernyataan nomor 31 “Prosedur administrasi pasien mudah dan tidak berbelit-belit” sebesar 2,17. Sedangkan gap skor yang paling rendah ada pada atribut pernyataan nomor 41 “Penampilan petugas dan tenaga kesehatan (dokter, perawat) bersih dan rapi” sebesar 0,17. Berdasarkan hasil penelitian dapat diketahui analisis perspektif pelanggan berdasarkan dimensi emphaty terjadi penurunan kepuasantahun 2023 responden yang menyatakan puas sebesar 71 orang (71,0%) dan menurun di tahun 2024 menjadi sebesar 47 orang (47,0%), dimensi Tangible didapati terjadi penurunan kepuasan tahun 2023 responden yang menyatakan puas sebesar 65,0% dan menurun di tahun 2024 menjadi sebesar 49,0%, dimensi reliability terhadap kepuasan kinerja pelayanan terjadi penurunan kepuasan tahun 2023 responden yang menyatakan puas sebesar 77.0% dan menurun di tahun 2024 menjadi 46,0%, dimensi assurance terhadap kepuasan kinerja pelayanan terjadi penurunan kepuasan tahun 2023 responden yang menyatakan puas 74.0% dan menurun di tahun 2024 menjadi sebesar 42,0%, dimensi responsiveness terhadap kepuasan kinerja pelayanan terjadi penurunan kepuasan tahun 2023 responden yang menyatakan puas sebesar 70.0% dan menurun di tahun 2024 menjadi sebesar 40,0%. Berdasarkan analisis kepuasan kinerja pelayanan dapat diketahui bahwa terjadi penurunan kepuasan tahun 2023 responden yang menyatakan puas sekali sebesar 46,0% dan menurun di tahun 2024 menjadi sebesar 14,0%.  Kata Kunci: Perspektif pelanggan, kepuasan, kinerja pelayanan

Abstract

Highlights: 1. As there is a scarcity of publications on the use of metformin for COVID-19 in Indonesia, the findings of this present study may contribute more insight to the existing body of research and provide data specific to the Southeast Asian population. 2. This study revealed a decreased mortality rate associated with metformin use in diabetic patients with mild to moderate COVID-19 infection. 3. This study suggests that diabetic patients may continue metformin treatment during a COVID-19 infection as the medication has sustained therapeutic effects. Abstract COVID-19 patients with comorbidities, such as type 2 diabetes (T2DM), have a higher mortality rate compared to those without any comorbidities. T2DM patients usually receive metformin as their first-line treatment. However, the effectiveness of metformin in reducing mortality rates still requires further analysis. The objective of this study was to analyze the effectiveness of metformin in reducing mortality rates among COVID-19 patients with T2DM. An analytic observational design with a retrospective cohort approach was used in this study. Samples were acquired from hospitalized COVID-19 patients with T2DM medical records at Fatmawati Central General Hospital, Jakarta, Indonesia, throughout 2020–2021. The samples were collected using a purposive sampling technique and analyzed using Chi-square test (p<0.05; RR<1). This study comprised 137 samples, with 56 samples receiving metformin and 81 not receiving metformin. The mortality rate in the sample group that received metformin was lower (19.6%) compared to the group that was not given the medication (38.3%). The Chi-square test results indicated a statistically significant relationship between metformin treatment and a lower mortality rate among COVID-19-contracted individuals with T2DM (p=0.020; RR=0.513). Therefore, this study concludes that the administration of metformin treatment reduces mortality among COVID-19 patients with T2DM.

Abstract

Rumah Sakit Umum Daerah (RSUD) Koja sebagai salah satu rumah sakit rujukan di Jakarta Utara menghadapi tantangan dalam pengelolaan limbah cair non-medis yang berkelanjutan. Pengelolaan limbah cair non-medis seperti dari dapur, laundry, dan sanitasi belum sepenuhnya terintegrasi dengan prinsip Green Hospital, sehingga berpotensi mencemari lingkungan dan meningkatkan beban operasional. Penelitian ini bertujuan untuk merumuskan strategi implementasi konsep Green Hospital melalui pengelolaan limbah cair non-medis guna mendukung manajemen lingkungan dan pemenuhan standar akreditasi rumah sakit. Pendekatan yang digunakan adalah mixed method dengan kombinasi analisis kualitatif dan kuantitatif, serta analisis SWOT untuk memetakan kekuatan, kelemahan, peluang, dan ancaman. Hasil penelitian menunjukkan bahwa tantangan utama meliputi keterbatasan teknologi IPAL, rendahnya efisiensi penggunaan air, dan belum optimalnya pemanfaatan kembali air limbah. Strategi implementasi yang disarankan mencakup penguatan regulasi internal, penerapan teknologi pengolahan limbah berkelanjutan, pelibatan seluruh pemangku kepentingan, serta integrasi kebijakan lingkungan dengan sistem manajemen rumah sakit. Penelitian ini memberikan kontribusi terhadap pengembangan rumah sakit ramah lingkungan dan dapat dijadikan acuan dalam penerapan konsep Green Hospital di rumah sakit lain di Indonesia.

Abstract

The COVID-19 pandemic has revealed how poorly prepared the world is for a pandemic and how easily viruses spread in our interconnected world. The response to COVID-19 in Indonesia has been hampered by a surge in infection and re-infection cases, an ill prepared of healthcare system, low booster vaccination coverage, the emerging of new sub variant, inconsistent implementation of public health measures, and communication barriers. Nevertheless, as infectious disease outbreaks become more frequent, it is hoped that national health security resources will be identified and strengthened. This will enable the nation to keep pace with spreading diseases and become more resilient to future threats. COVID-19 exposes strengths and weaknesses while also creating threats and opportunities. An analysis of the COVID-19 pandemic strategy was conducted using SWOT (Strengths, Weaknesses, Opportunities, and Threats) analysis based on Indonesia's experience with the response and countermeasures to COVID-19. The objective of this study is to analyse all the factors involved in the COVID-19 pandemic response so that lessons can be learned towards an effective public health emergency response. The results highlighted strengths, identified weaknesses, and provided recommendations for the future. This would be useful in responding to current threats to global health security and preparing for future ones.

Abstract

Fermented rice water (FRW) has been traditionally used for skin care, but its biological mechanisms on fibroblasts, key cells in wound healing, have not been fully characterized. This systematic review aims to evaluate the efficacy of FRW on fibroblast proliferation and migration and to identify the molecular pathways involved. A literature search was conducted through PubMed, Scopus, and Google Scholar using the keywords "fermented rice water," "fibroblast," and "wound healing" in publications from 2015 to 2025. Studies were included if they evaluated the effects of FRW on fibroblasts in vitro or in vivo. Risk of bias was assessed using SYRCLE RoB (Risk Of Bias). Four studies met the inclusion criteria. FRW increased fibroblast viability by up to 116% and accelerated diabetic wound closure from 27% to 71% compared to controls. Molecular analysis showed upregulation of Collagen I mRNA by 17%, Collagen III by 47%, and Elastin by 159%. FRW activated the NRF2 pathway, which increased the expression of antioxidant enzymes (SOD, CAT, HO-1) and decreased inflammatory mediators including TNF-α, NF-κB, and MMP-1. Bioactive metabolites from Lactobacillus plantarum, Bacillus cereus, and Aspergillus oryzae played a role in these therapeutic effects. FRW shows potential as a fibroblast bio-stimulator through multi-pathway modulation involving extracellular matrix synthesis, antioxidant, and anti-inflammatory effects, with prospects for application in chronic wounds and anti-aging that require further clinical validation.

Abstract

Indonesia’s Free Nutritious Meal Program (Program Makan Bergizi Gratis, MBG) is rapidly expanding in scale, yet evidence on its readiness to serve older adults, persons with disabilities, and rural communities remains limited; this study provides a novel risk-based assessment of expansion readiness across nutritional, disability-specific, food-safety, and rural implementation domains. This study aimed to identify critical implementation gaps and propose a differentiated, phased framework for MBG expansion. A PRISMA-lite literature review was conducted using PubMed and Google Scholar and supplemented by national regulations, government guidelines, Badan Gizi Nasional technical documents, epidemiological surveillance data, and credible media monitoring. Thematic synthesis was undertaken using a risk-based policy framework. Since implementation began in January 2025, 16,109 MBG beneficiaries were reportedly affected by food-borne illness across 70 documented incidents by November 2025, indicating substantial food-safety vulnerabilities. Older adults comprise approximately 12% of Indonesia’s population, with frailty affecting 26.8% in community settings, while approximately 6 million persons with moderate-to-severe disabilities require heterogeneous feeding adaptations. Rural communities face additional barriers involving cold-chain limitations and shortages of certified food handlers. MBG expansion therefore represents a high-consequence policy undertaking requiring beneficiary stratification, condition-specific nutritional and texture standards, strengthened food-safety systems, and outcome-based monitoring. A “Quality First, Scale Later” phased approach is essential to minimize preventable harm.

Abstract

Sharia hospitals in Indonesia are rapidly evolving under Fatwa No. 107/DSN-MUI/X/2016 and MUKISI/SIRSMA standards; however, governance remains focused on contractual, financial, and halal compliance, leaving environmental health and patient safety underrepresented as measurable outcomes. This study synthesises normative and empirical evidence to assess the integration of these domains and proposes a maqasid al-shariah-based framework to render them operational. Utilizing a normative–empirical narrative synthesis, the research triangulated Indonesian regulatory and fatwa documents, certification standards, four domestic case studies, and comparative literature from Malaysia and Gulf countries. Results reveal that while environmental health and infection control are not entirely absent from current standards, their requirements remain fragmented across national accreditation and environmental regulations. Furthermore, case evidence highlights persistent gaps in halal supply chains, Sharia Supervisory Board engagement, and standardized safety indicators, whereas mature international models successfully embed these elements as core maqasid-linked components. Ultimately, Indonesian sharia healthcare governance possesses sufficient foundations to explicitly integrate environmental health and patient safety, where a unified maqasid-based indicator set would significantly enhance accountability, compliance, and overall safety performance.

Abstract

Patient loyalty is crucial for antenatal care (ANC) continuity, yet physician arrival punctuality remains a challenge. This study investigates the impact of physician punctuality on patient loyalty, with patient satisfaction as a mediating variable, at the Obstetrics and Gynecology Outpatient Clinic of RS Permata Bekasi. A sequential explanatory mixed-method design was employed. The quantitative phase involved a cross-sectional survey of 98 ANC patients using a Likert-scale questionnaire. Patient satisfaction was measured using non-tangible SERVQUAL dimensions. Data were analyzed using linear regression and mediation testing (Baron & Kenny, Sobel test). Qualitative interviews provided explanatory insights. Quantitative analysis demonstrated that physician arrival punctuality significantly influenced both patient loyalty (B=0.210; p=0.002) and patient satisfaction (B=1.798; p=0.000). Patient satisfaction significantly predicted loyalty. Furthermore, satisfaction fully mediated the relationship between punctuality and loyalty. The Sobel test produced a z-value of 4.72. Qualitative findings revealed that empathetic communication could mitigate negative perceptions of delays. Clear information delivery could also mitigate negative perceptions of delays. Professional conduct could further mitigate negative perceptions of delays. These findings reinforce satisfaction as the key mechanism. The results confirm that physician punctuality is important. However, its effect on loyalty is fully channeled through patient satisfaction. Qualitative data elucidate how positive interpersonal interactions can compensate for waiting times. Professional interactions can similarly compensate for waiting times. Therefore, fostering satisfaction through reliable service and empathetic communication is the primary pathway to building patient loyalty. This pathway remains effective even when punctuality challenges occur. Healthcare providers should prioritize enhancing patient satisfaction. This serves as a buffer against systemic punctuality issues.
