Abstract

Indonesians traditionally consume Katuk (Sauropus androgynus (L) Merr) leaf to treat various diseases, improve weight loss, and as vegetable dishes. Sauropus androgynus, also known as a medicinal plant with high antioxidant potential, is also traditionally utilized to increase breastmilk production during lactation. Lactation is a process of milk synthesis and secretion, in which Prolactin responsible for the synthesis of breastmilk. Recent studies have proven that Katuk leaf extract can increase the breastfeeding quality and prolactin level in breastfeeding women. This article provides a brief summary of evidence supporting the use of Katuk leaf to improve breastmilk production in breastfeeding mothers.

Abstract

Introduction: Many mothers who give birth are less interested in giving birth normally because of the intense labor pain. Actually it can be overcome by doula assistance. The doula's presence provides emotional support during the preparation for childbirth, and the doula provides physical support by accompanying to overcome hunger, thirst, and pain. The aim of this study was to determine the effect of doulas assistance on the success of delivery. Methods: This study is a descriptive observational study with a cross-sectional design conducted at a private hospital in Bali in the period January 2020 to January 2021. The inclusion criteria in this study were all pregnant women with gestational age 34 to 40 weeks who were willing to undergo the study. While the exclusion criteria in this study were pregnant women who experienced complications during childbirth and pregnant women with incomplete clinical and demographic data. All of the data was analyzed descriptively by using SPSS 25 and was presented by frequency. Result: This study managed to collect as many as 100 pregnant women who will give birth with the help of a doula. It was found that as many as 60 people (60%) gave birth vaginally. Then followed by 31 people (31%) who underwent a Cesarean section with the help of a doula. There were also 5 people (5%) and 4 people (4%) who underwent epidural delivery and vacuum extraction with the help of a doula. Conclusion: Doula assistance during labor can increase the confidence of the mother who will give birth and support the success of normal delivery.

Abstract

Introduction: All human worldwide had a risk infected by a severe acute respiratory syndrome–coronavirus-2 (SARS-CoV-2), including pregnant woman. Nowadays, SARS-CoV-2 transmission is still not fully understood. Thus, this situation created anxiety particularly for mothers that planned to breast-feed their baby and did their routine hospital visit. Thus this research aimed to evaluate the impact of COVID-19 pandemic with the routine visits and breastfeeding plan. Methods: This study was carried out with cross sectional observational study at Kasih Medika Hospital. The routine visits were conducted on the 5th-10th of August 2020. This study used a questionnaire instrument with samples of pregnant and postpartum mothers. The inclusion criteria of the sample were all pregnant women and post-partum that registered in Kasih Medika Hospital, complete questionnaire data filled in. The exclusion criteria were the patient agreed to be the research sample. All of the data was analyzed descriptively by using SPSS 25 and was presented by frequency. Results: All of the patients felt anxious with different stages of anxiety. COVID-19 affects 18% of patients who reduce their routine visits to Kasih Medika, while 82% keep visiting as scheduled. The COVID-19 pandemic had an insignificant effect on breastfeeding plans. This can be seen from only 27% of all respondents who are afraid of their breastfeeding plans. However, in the primigravida population, 45.45% of respondents were worried about their breastfeeding plan. Although in multigravida and post-partum, it was found that 90.90% were not afraid of their breastfeeding plans during the COVID-19 period.  Conclusion: Our research established that COVID-19 reduces pregnant women's attendance to hospital, and patient primigravida was mainly afraid to do breastfeeding plans rather than multigravida patients. Abstract

Introduction: Low birth weight infants contribute to neonatal deaths. Kangaroo mother care (KMC) is a treatment with various benefits as an alternative to an incubator. In Depok General Hospital, its implementation was not yet optimal. This study aimed to assess kangaroo mother care knowledge, attitude, and practice among health providers. Methods: This descriptive study with a cross-sectional approach was conducted from November to December 2015 at Depok General Hospital. The population was all health providers in the study site. This study used total sampling (37 persons). The inclusion criteria were: health providers of Depok General Hospital who worked in the perinatology, delivery, rooming-in rooms, and obstetric polyclinic, had a minimum of 6 months working experience, agreed to become respondents. The exclusion criteria were: on work leave during the study period, on study assignment without working. The data were collected using a questionnaire. Univariate analysis was conducted using SPSS. Result: The mean age of respondents was 31.38 years, ranging from 23-41 years. The average working experience was 7.95 years. All respondents were female, 81.1% graduated from Diploma III, and 72.97 % had experienced or trained in KMC. About 75.7% of respondents had good knowledge of KMC, 51.4% had a positive attitude towards KMC, and 64.9% had a low level of KMC practice. Conclusion: The majority of the respondents had good knowledge of KMC, around half had a positive attitude towards KMC, and more than half had a low level of KMC practice. Training, facilities, written SOP, and management commitment for KMC are needed for successful KMC implementation.

Abstract

Introduction: The infant mortality rate is one indicator of the public health quality in a country. One of the main causes of infant mortality is low birth weight babies (LBWBs). Kangaroo Mother Care (KMC) is a treatment method for LBWBs by making direct contact between the baby’s skin and the mother’s skin. Budi Kemuliaan Mother and Child Hospital already practicing KMC since 2010 and it has succeeded. The purpose of this study was to determine the implementation of KMC for LBW mothers at Budi Kemuliaan Mother and Child Hospital. Methods: This study was qualitative research with Rapid Assessment Procedures (RAP) design. This research was conducted on mothers who gave birth with LBWB (<2500 grams) from 9 to 18 December 2011, and practice KMC at Budi Kemuliaan Mother and Child Hospital, Jakarta. Result: The implementation of KMC for LBW mothers at Budi Kemuliaan Mother and Child Hospital was affected by seven factors. Those factors were; mothers and their husbands have sufficient knowledge of KMC from health workers explanation, good acceptance attitude, given accountable information sources of KMC, there is the presence of KMC policy that supporting KMC program, health workers support, husband support, and sufficient health education and counseling of KMC for mother and their husband before discharge. Conclusion: There are seven factors that determine the successful implementation of KMC for LBW mothers at Budi Kemuliaan Mother and Child Hospital, those are; sufficient knowledge of KMC, acceptance attitude of KMC, information sources of KMC, the presence of KMC policy, health workers support, husband support, and sufficient health education of KMC for mother before discharge. This study suggested the formation of a support group to build social support between LBW families.

Abstract

Introduction: The entire or partial rotation of the adnexa around its vascular axis or pedicle is known as ovarian torsion. Predisposing factors include short length, unrestricted motion, and a long pedicle. It’s tough to pinpoint the exact cause. Dermoid and serous cyst adenomas are the most common. The complete torsion blocks the venous and lymphatic systems, resulting in stasis, venous congestion, bleeding, and necrosis. The cyst becomes irritable and may explode. A soft cystic mass separate from the uterus can be seen on examination of the affected person, who usually complains of abrupt severe discomfort in the stomach and pelvis. At some time during pregnancy, the risk of ovarian torsion increases by a factor of five. The incidence rate is five per 10,000 pregnancies. Torsion of ovarian tumors was more common among women in their reproductive years. The majority of the instances were seen in pregnant women (22.7%) rather than non-pregnant women (6.1%). Case Description: In this case, we report a 27-year-old primigravida with 36 weeks and 5 days of pregnancy. She presented to the hospital with acute pain abdomen in the lower right (lumbar). She has a history of pregnancy control at a private clinic and there are no abnormalities in her pregnancy. She was hospitalized at 35 weeks 3 days of pregnancy due to the same complaint and has collaborated with the surgeon. No abnormalities were found on the abdominal ultrasound and urology at that time. The baby was born at 36 weeks 6 days of gestation by cesarean section, a twisted ovarian cyst was found on the right ovary, a dextra salpingectomy was performed for oophorectomy and an appendicectomy was also performed at the same time in collaboration with the surgeon. A healthy baby with a weight of 2300 grams and full of breastfeeding. Conclusion: This example demonstrates the problem of manufacturing desirable excellent radiological ultrasound imaging of the pelvic organs in late pregnancy. ultrasound exam in early pregnancy has to also be geared toward the cervix and adnexa which ends up in early analysis and management of ovarian hundreds, thereby fending off future emergency conditions and possible preterm transport.

Abstract

Introduction: A type of pregnancy that occurs in monochorionic twin pregnancies which results in the coexistence of normal "pump" twins and cardiac twins is called Twin Reverse Arterial Perfusion Syndrome (TRAP). The low oxygen pressure going to the baby causes many unique changes in the twins' physiology. These changes can lead to high prenatal mortality. Case Presentation: We present the case of a 25-year-old woman, primigravida, whose obstetrician referred to Kasih Medika Bali with suspicion of a large placenta. However, upon arrival at 35 weeks and 2 days of gestation, an ultrasound showed a twin pregnancy with suspicion of a TRAP pregnancy. Then, a baby girl was born at Payangan Hospital, Gianyar, Bali by Sectio Caesarea with a birth weight of 3155 grams, an Apgar score of 6-8-9, and a placental weight of 815 grams. The results of the examination of the second fetus that did not develop, weighing 1,705 grams in the placenta did not have an attached umbilical cord. The doctor's suspicions about the TRAP pregnancy were indeed true according to the conditions at the time of the twins' birth. The doctor found 1 healthy baby and 1 other like mass with a size large enough on ultrasound at 35 weeks 2 days of gestation. Conclusion: The incidence of pregnancy like this is very rare and needs further study in order to detect early abnormalities that occur in pregnancy. Thus, we can be aware of the risks for both mother and fetus and even treat them early. Couples can be counseled optimistically that the recurrence rate of TRAP syndrome tends to be low so that they can better plan future pregnancies in all aspects.

Abstract

Introduction: Planned home births are often a safe option for low-risk pregnancies, according to prior studies. This study aims to ascertain the characteristics of planned home deliveries with skilled medical staff in low-risk pregnancies as well as the outcomes on mother and infant health in Denpasar, Bali. Methods:  The planned home births from 2010 to 2019 were examined in this study using a retrospective descriptive design. The study included all intended home deliveries, including those that required hospital transfers owing to difficulties. As many as 168 planned home births with private doctor assistance made up the study’s sample. Sociodemographic and clinical variables were both examined in this analysis. Results: The mean age of the sample in this study was 32 years.  The findings indicate that women planning a home delivery with a licensed doctor in low-risk pregnancies have a better likelihood of spontaneous vaginal birth and favorable outcomes for maternal health. Additionally, there is a reduced likelihood of hospital transfer (10%). Furthermore, evidence demonstrates that planned home births are typically linked to fetal wellbeing. Conclusion:  The majority of women gave birth between 38 and 40 weeks gestation. The majority of samples were deliveries were normal vaginal births. The main reasons for transfer were due to the arrested cervical dilation or the arrested progress of the fetal head.

Abstract

Introduction: A neonatal death clinical audit is needed to improve the quality of care and prevent similar preventable neonatal deaths. This study aims to develop a neonatal death clinical audit instrument. Method: We developed a neonatal clinical death audit instrument for all death cases admitted to the nursery. The audit was started from the place of delivery for the inborn and arrival at the emergency room for the outborn. The instrument contains all aspect all aspects of case management. The audit was carried out by comparing the patient's management practices with the standard operating procedures. The last part of the instrument was the death information, assessment of deviations, and determination of avoidability. The instrument was tested twice, and pediatricians and neonatologists conducted an audit at several types of hospitals in Indonesia. The instrument was tested to audit 20 cases at six hospitals in 4 Provinces, and the second test was 37 cases at six hospitals in East Nusa Tenggara. The auditors of the first test were neonatologists and general pediatricians, while for the second test were general pediatricians. The average time of audit was 30 minutes. Results: The instrument could detect the deviations, cause of death, and avoidably. Conclusions: The instrument could identify deviations and determine avoidably neonatal death. Recommendations based on the deviations may improve the quality of care and prevent similar neonatal death.

Abstract

Introduction: Holoprosencephaly (HPE) is a rare congenital malformation of the brain; the incidence rate was 0.49-1.2 cases per 10,000-20,000 term births. HPE occurs due to failure of the prosencephalon division at the stage of brain development during the 4-5 weeks of pregnancy. Alobar HPE is one of the most severe types compared to other types. Most of the fetuses affected by this anomaly will die, and those born alive generally cannot survive for more than a year. This study presented a rare case of a baby with alobar HPE. Case report: A 33-year-old woman referred from Karang asem hospital Bali, G3P0020, 23 weeks gestation, has a poor obstetric history. The ultrasound examination results show no falx cerebri, even cerebellum and hypoechoic picture of the cerebrum. Ultrasound of the face was found a flat nose and hypotelorism. Termination at 28 weeks gestation, a baby boy was born 1000 g with the Apgar score 1-1. Multiple congenital abnormalities were found: flat nose, labiopalatoschizis, polydactyly manus dextra and omphalocele. Conclusion: Alobar HPE is a very rare congenital anomaly. The cause of the disease has not been fully explained. Current therapy is just supportive and has not been able to resolve the source of the problem. Alobar HPE disease has a poor prognosis.
