Abstract

Introduction: Ovarian Apoplexy is the sudden rupture of the ovarium that commonly occurs at the site of the corpus luteum cyst, followed by hemoperitoneum. Extremely uncommon but potentially fatal presentation, hemoperitoneum owing to cyst rupture has just a few cases documented in the literature. We report a woman 6 weeks delaying menstrual period, the pregnancy test was positive (second pregnancy). In this article, we aimed to report a rare case of ectopic pregnancy and ovarian apoplexy. Case Presentation: A 26 Years old woman with a history of abortion in her first pregnancy came to the emergency room with 3 days history of vaginal spotting and mild (2/10) continous right lower abdominal pain. She was suspected of incomplete abortion from primary health care. She was in 6 weeks delaying her menstrual period which is an uncommon case for incomplete abortion. The pregnancy test was positive (second pregnancy). Surgery was performed after completing the consent. Upon exploration of the abdomen, approximately 500 MLS of intraabdominal blood was identified. The right ovary's ruptured corpus luteum cyst was discovered to be the cause of the intra-abdominal bleeding. The hemorrhage was simply stopped by electrocauterization. Conclusion: Ovarian Apoplexy concomitantly occurs with an ectopic pregnancy and has not been reported before. Clinical signs and symptoms, especially pain perception of acute abdomen in a person with high pain tolerance, may bias the diagnosis.

Abstract

Physiological change makes pregnant women more vulnerable to COVID-19 infection. It raises morbidity and mortality rates in pregnant women in comparison to the general population. Several morbidities have been reported such as earlier miscarriage, vaginal bleeding during pregnancy, edema in all extremities and also the face, hypertension during the gestational phase, foul-smelling vaginal discharge, and dysuria due to urinary tract infection, etc, with the effectiveness, of reducing morbidity and mortality, vaccines play a key role, especially for this group. Abstract

Introduction: Adenomyosis became challenging in the gynecological field and healthcare economic aspect. The prevalence of adenomyosis is widely ranged in different countries. Infertility is one of the frightening complications. Thus a radical adenomyosis tissue removal method using the triple-flap (the Osada technique) could be done. Using this technique the adenomyosis tissue can be removed as much as possible while maintaining normal uterine function. Thus, in this case, report, we performed a woman diagnosed with adenomyosis underwent the surgery with Osada technique. Case description: A 30-year-old woman went to the medical care of Kasih Medika. She has been married for 4,5 years and did not have any children. She had a history of abdominal pain every menstruation, and it has persisted since 2 months ago even though not on the menstrual schedule. Transvaginal sonographic examination revealed a diffusely enlarged uterus and thickening of the uterine wall. A laparotomy is performed using the Osada technique (triple flap. After that, around 1 month after surgery, the patient will undergo an ultrasound examination and the patient will undergo the GnRH agonist therapy (Tapros) 3 times. Conclusion: Adenomyosis is one of the challenging diseases. Adequate treatment must be done to minimalize the infertile probability Laparotomy with the Osada technique could be done. Infertility examination in both couples also must be done. Undergoing the GnRH agonist therapy also can give a better result for the patient.

Abstract

Introduction: Chorioangioma is the most common non-trophoblastic vascular tumor of the placenta. It was estimated that the incidence of chorioangioma was 1%. Although the incidence was not widely large, it has high mortality and morbidity. Commonly, chorioangiomas is a small asymptomatic lesion found only after birth after careful excision of the placenta. In making it easier to visualize the food vessels entering the placental mass and peritumoral diffuse vasculature we could use the color doppler. The wide range of outcomes and limited studies related to this case makes it quite difficult to handle. Thus, this study aimed to provide information on chorioangioma from diagnosis to the management. Case description: Mother A is our referral patient who was recommended to do an ultrasound, with a diagnosis of 20 weeks gestation with suspected IUFD and a cyst in the placenta measuring 6.5cm x 4.1cm x 5.4 cm, the mother has not felt fetal movement. Ultrasound examination revealed a hematoma at the time of insertion of the umbilical cord in the placenta. The location of the placenta in the corpus anterior grade 1. No heartbeat was found in the baby, and the baby's weight was 529 grams. Conclusion: The adverse outcome is known to be associated with chorioangioma. It depends on the mass size, and the existence of fetal hydrops. The worst prognosis that we found in this case was no heartbeat when the baby was born.

Abstract

Numerous studies have demonstrated how routine episiotomies can cause additional problems like poor postpartum sexual function, ongoing perineal pain, and increased anal sphincter damage resulting in flatus or fecal incontinence. To combat the high frequency of maternal labor morbidity, antibiotic prophylaxis is recognized as a method of intervention that can be applied. . On the other hand, several studies seem to support the use of preventive antibiotics in situations of severe perineal rupture. It is not advised to use antibiotics excessively due to the rising prevalence of their use, which increases the danger of antibiotic resistance. As a result, following a serious perineal tear, the use of prophylactic antibiotics is crucial to preventing infection and determining whether a side effect is present in the mother or infant. Therefore it is necessary to assess the effectiveness of antibiotic prophylaxis to reduce maternal morbidity and side effects of third and fourth-degree perineal rupture during vaginal delivery.

Abstract

Background: An aberrant accessory route is a hallmark of the rare heart rhythm condition called Wolff-Parkinson-White (WPW) syndrome. The onset of tachyarrhythmias, which can result in hemodynamic instability and even maternal mortality, is considered to be triggered by a variety of physiological changes that occur during pregnancy. Therefore, it is anticipated that effective management options for pregnant women who have WPW syndrome will be able to regulate tachyarrhythmias and give healthy pregnancies for both the mother and the fetus. Case Report: A 32-year-old pregnant woman with 34 weeks of gestational age arrived complaining of palpitations and breathing difficulties. The results of the ECG test revealed left branch bundle block (LBBB), a Wolff-Parkinson-White (WPW) type B pattern, and sinus tachycardia. Both an NST assessment and an echocardiographic evaluation revealed no fetal distress or structural abnormalities of the heart. The patient was treated with 300 mg of Propafenone, which was administered on a regular basis whenever the patient experienced palpitations. Due to a history of prior caesarean deliveries, the pregnancy had progressed to 40 weeks of gestation and caesarean birth under spinal anaesthesia was performed in this point. The infant was born healthy, full-term, and free from birth problems. Conclusion: One of the causes of tachyarrhythmias in WPW syndrome is pregnancy. Arrhythmias that could develop in women with WPW syndrome can be controlled with good pregnancy planning and management.

Abstract

Introduction: One of the risk factors seen most frequently in obstetric practice is maternal obesity. Women who were overweight or obese made up more than half of all female fatalities due to direct or indirect causes. Obesity increases the chance that the mother will experience obstetric problems during pregnancy, labour, and delivery, making fetal assessment more technically difficult. Methods: This study uses quantitative descriptive univariate analysis. The type of data is secondary data taken from the birth register. This study examines women who gave birth in 2022 and were identified as belonging to the obesity category by Prof. Dr. I. G. N. G. Ngoerah Denpasar General Hospital. Results: At Prof.dr.I.G.N.G Ngoerah Denpasar General Hospital, 21 obese pregnant women will give birth throughout 2022. Of the 21 people, 81% give birth by emergency C-section, 4,8% by elective C-section, and 14,2% give birth vaginally. Obese pregnant women have a higher risk of needing a cesarean section. Patients with obese class III had a significantly higher risk of infant morbidity than those with a normal BMI, according to their findings. Conclusions: Obese pregnant women are at increased risk of having a cesarean section. Therefore, health workers must provide knowledge to pre-pregnant or pre-marital women to maintain their BMI so they do not become obese during pregnancy. Thus, morbidity and mortality rates can also be reduced.

Abstract

Introduction: Successful breastfeeding depends on an infant's ability to latch onto the mother's breast correctly. A poor latch will cause insufficient milk transfer which will cause poor growth. Each type of mammal produces different components of milk which make it species-specific and adjusts to their needs, growth rate, and breastfeeding habits. Human breast milk has many benefits for human babies because it contains various substances both nutrition and protection. This study aims to present two infants who suffered from Ankyloglossia, which hindered the baby's ability to breastfeed and eat solid food Case Presentation: The study presented two infants who suffered from Ankyloglossia, which hindered the baby's ability to breastfeed and eat solid food. These babies were diagnosed with marasmus and kwashiorkor. After revision on both the lip- and tongue ties, supplementation with pasteurized human milk donor, and good complementary feeding, the babies finally reach optimal growth and development. Conclusion: The sucking ability of the baby during early breastfeeding was proven to be important and should be examined, to achieve further successful breastfeeding.

Abstract

Treponema pallidum, which causes the infectious disease syphilis, has a high rate of vertical transfer from mother to kid. Untreated maternal infection increases the risk of perinatal morbidity and mortality and unfavorable pregnancy outcomes, notably congenital syphilis. It is necessary to check for gestational syphilis during the prenatal period, and its diagnosis is straightforward. Nevertheless, this illness still affects two million pregnant women globally, indicating its significant prevalence. Congenital syphilis clinical symptoms are regulated by gestational age, maternal syphilis stage, maternal therapy, and fetus immune response. Early and late congenital syphilis have historically been used to describe it. The direct identification of treponemes, serological tests, and clinical signs all contribute to the diagnosis of maternal infection. Penicillin is typically used in the treatment, which also includes sexual partners. Fetal infection can be treated and prevented from spreading from the mother to the fetus with proper maternal infection management. To lower the prevalence of congenital syphilis, screening, early detection, and appropriate care are crucial during pregnancy and the preconception period.

Abstract

Today, providing complementary care in health services for the community is increasingly mushrooming and is in great demand by all groups. The same is true for midwives. The midwife's duty as an extension of the government has resulted in the midwife having an obligation to take care of the condition of pregnant women until she gives birth, especially those related to maternal psychology. The gentle birth delivery method is one of the methods of delivery that supports government programs related to complementary and integrated health services. Through a gentle birth, the mother will feel valued, safe and comfortable, and the pain will be reduced. Evidence-based midwifery shows that if mothers are cared for and given support during labor, they will feel safe and comfortable so that the results will be better. Maternal love care is like allowing the mother to determine the desired position and movement during labor and birth. Midwives play a very important role. As advocates for mothers, midwives must ensure the needs of mothers, use adaptation skills, be flexible, and implement innovative practices to increase awareness of the services available to mothers.
