Abstract

The coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). Due to its heterogeneous disease severity and rapid transmission, the use of antiviral and immunomodulator drugs is crucial. Favipiravir, an antivirus, can treat SARS-CoV-2 due to its action on RNA Genome ACE2 receptor, therefore halting the viral replication. However, Adverse Drug Reactions (ADRs) monitoring of Favipiravir is recommended. The study design is a descriptive cross-sectional approach with prospective sampling. The study was held in May-July 2021 and aimed to determine the adverse effects of Favipiravir in confirmed COVID-19 patients in Dr. Mohammad Hoesin General Hospital Palembang. Adverse events such as gastrointestinal symptoms (nausea, vomiting, and decreased appetite) occurred in 2 of 170 patients (1.18%). Several side effects, such as an elevated liver enzyme, leukopenia, neutropenia, hyperuricemia, and increased triglyceride levels, cannot be assessed due to a lack of laboratory results before and after Favipiravir administration. Other side effects such as skin rash, asthma, rhinitis, nasopharyngeal pain, and oropharyngeal pain, were not found in the study.

Abstract

Background: More than 60% of stroke patients suffered from long-term disability due to motor impairment despite underwent intensive rehabilitative therapies. Non-invasive brain stimulation such as repetitive Transcranial Magnetic Stimulation (rTMS) has been reported in improving motor function after stroke through the mechanism of neuroplasticity. Research related to the use of rTMS as adjunctive therapy in ischemic stroke cases has mostly been carried out in chronic stroke patients and until now only a few studies have assessed the effectivity of rTMS in patients with acute stroke. Therefore, this study aims to assess whether rTMS can improve motor function in patients with acute ischemic stroke. Methods: This research was a randomized controlled trial and carried out acute ischemic stroke patients in Dr. Mohammad Hoesin Hospital Palembang. A total of 22 patients were randomly divided into a control group with standard therapy (11 subjects) and a treatment group with standard therapy and 5 sessions of rTMS (11 subjects). Improvement of motor function was assessed with motor strength using the Medical Research Council (MRC) scale performed before and after the intervention on the four limb segments. Bivariate analysis with Paired T-Test was used to compare the strength before and after the intervention in each group and the Mann Whitney Test used to compare the differences of motor strength between groups. Results: All subjects in both groups completed the study and no adverse effects were found in the rTMS group. The MRC scale after the intervention increased in all segments in both groups, but significant differences were only found in the treatment group (hand grip (p=0.011), shoulder abduction (p=0.001), hip flexion (p=0.001), and toe dorsiflexion (p =0.002)). Comparison of differences in motor strength between the two groups using the Mann Whitney Test revealed that the improvement was more significant in the rTMS group compared to the control group (p=0.024, p=0.031, p=0.016, and p=0.021). The motor strength in the rTMS group start to increase on the second and third day and particularly reached the peak on the fourth day after the procedure. Multivariate analysis showed no effect of confounding factors such as age, sex, BMI, comorbidities, onset, lesion location, NIHSS, and mRS inthe improvement of motor strength. Conclusion: rTMS therapy can improve motor function in acute ischemic stroke patients and can be considered as an adjuvant therapy in assisting the recovery of post-stroke motor impairment.

Abstract

Ovarian tumors account for 30% of all malignancies in the female genital organs and can be benign, borderline, and malignant tumors. According to World Health Organization (WHO) based on the origin of tumor cells are divided into three groups, namely epithelial tumors, cord-stromal sex, and germ cells where the most types are epithelial types. This study aims to determine the clinicopathological characteristics of ovarian tumors carried out in a descriptive observational manner. Samples using secondary data of ovarian tumor patients who came to the Anatomical Pathology Section of Dr. Mohammad Hoesin Hospital Palembang for the 2018-2020 period who met the inclusion criteria using the total sampling technique obtained 325 secondary data all included as research samples. The results of the study found proportional morbidity rate of ovarian tumors based on the type of histopathology most epithelial tumors obtained by 55%. The highest ovarian tumor patients in the age group of 20-34 years are 91 people, the most common type of tumor histopathology is epithelial tumors as many as 179 cases where mucinous tumor most occur at the age of 20-34 years as many as 23 people, the most growth patterns are cystic 140 cases, the most ovarian tumor behavior is malignant tumors as many as 143 cases. Malignant ovarian tumors are most common in the age group of 20-34 and 45-54 years, which is 37 people each. The cystic growth pattern is most prevalent in benign ovarian tumors, which are as many as 87 cases. It can be concluded that the most common ovarian tumors are epithelial tumors. Based on age and growth patterns, most ovarian tumors occur at the age of 20-34 years with cystic growth patterns. The behavior of most ovarian tumors is malignant tumors.

Abstract

Dr. Mohammad Hoesin Hospitals Cancer Incidence: Hospital-Based Cancer Registration Data. Cancer worldwide is summarized through a complex process called cancer registries. Based on data from the Global Burden of Cancer Study (GLOBOCAN), there will likely be approximately 10 million cancer-related deaths and 19.3 million new cases of cancer globally in 2020. However, Indonesia still does not have cancer incidence data based on population, as well as hospital-based cancer incidence data, especially the Dr. Mohammad Hoesin General Hospital Palembang, the main referral center for cancer in the South Sumatra region. This study aims to determine the distribution of cancer cases based on gender, age, organs, and cell origin in Dr. Mohammad Hoesin General Hospital Palembang for the period 2012-2019. This is an observational-descriptive study with 3413 samples taken from hospital cancer registry data, including topography, cell origin, age, and gender data from January 1, 2012, to December 31, 2019. The data is then processed and presented as distribution and frequency on tables and graphs using the SPSS application version 26. Cancer is prevalent in 63.5% of women and 36.5% of men. More than 53% of cases of cancer are diagnosed in the age range of 41-60 years. Acute lymphoblastic leukemia, infiltrating duct carcinoma, and adenocarcinoma are the most common cases of cancer in the age groups 0-20 years, 21-40 years, and >40 years, respectively. Based on location, breast, cervix-uteri, and colon occupy the 3rd highest position. By origin, 72% of cancer came from epithelial cells (carcinoma), 281 cases came from lymphoid cells (lymphoma), and 96 cases came from mesenchymal cells (sarcoma). Cancer generally occurs in females, patients aged 41-60 years, the most common location of the breast, and originates from epithelial cells (carcinoma).

Abstract

Patients with Multiple Drug-Resistant Pulmonary Tuberculosis at Dr. Mohammad Hoesin Palembang: Association of Body Mass Index on Drug Side Effects. Tuberculosis (TB) is an infectious disease transmitted by acid-resistant bacteria, namely, Mycobacterium tuberculosis (MTb). TB enters the respiratory tract and infects the lungs, and it can spread to other organs both hematogenously and lymphogenously. Pulmonary TB is the most common occurrence. Based on resistance, there are 2 types of pulmonary TB infection: drug-sensitive and drug-resistant. Multiple drug-resistant pulmonary TB (MDR-TB) is a type of pulmonary TB bacteria that is resistant or immune to rifampicin and/or isoniazid treatment. In the treatment of pulmonary TB, MDR uses a multi-drug combination therapy regimen in which each drug has both major and minor side effects. One of the risk factors for side effects is body mass index (BMI). In this study, conducted using a retrospective descriptive method during the 2019–2021 period at Dr. Mohammad Hoesin General Hospital Palembang, 52 subjects met the inclusion and exclusion criteria. 33 subjects (63.5%) were under weight, and 19 subjects (36.5%) were normoweight. Of the 52 subjects, 27 experienced minor ESO and 25 experienced major ESO. The most common major ESO is hearing loss, with 12 events, and in the underweight group, with 10 events. The most common minor ESO was nausea, which occurred 14 times in the overweight group and 9 times in the underweight group. The chi square test showed that there was a significant relationship between underweight BMI and the incidence of major ESO with a p value of 0.029 and an odds ratio (OR) of 3.67x. We concluded that a low body mass index had a 3.67x effect compared to a normal BMI on the increased incidence of major side effects of anti-tuberculosis drugs in MDR-TB patients at Dr. Mohammad Hoesin General Hospital Palembang.

Abstract

Comorbidities in Psoriasis: A Narrative Literature Review. Psoriasis is a chronic inflammatory disease with multisystemic morbidities. The most common comorbidities of psoriasis include psoriatic arthritis, cardiovascular disease, metabolic syndrome, overweight/obesity, inflammatory bowel disease, and mental disturbance in terms of depression. Evidence shows an association of psoriasis with comorbid diseases and have been proposed as related to the chronic inflammatory status of psoriasis. The presence of such comorbidities affects the therapeutic choices for clinicians in comprehensive way. The understanding of these pathogenesis relation and regular screening for early diagnosis of the comorbid diseases will certainly lead to better management of the disease. This is because the management paradigm of psoriasis does not only target repair of skin lesions but also works together with other scientific disciplines to prevent and treat comorbid diseases holistically. Patient often visit the clinician regularly and it is important to recognize and address early signs of psoriatic comorbidities to prevent further deterioration and improve quality of life. The purpose of this review is to summarized pathogenesis and screening recommendation of comorbidity associated with psoriasis, which may be essential for future clinician guideline in order to achieve holistic therapy of psoriasis and quality of life improvement.

Abstract

Phenomenological Study: Spiritual Needs of Cancer Patients at Dr. Mohammad Hoesin General Hospital, Palembang. Cancer is the main cause of death in the world. Cancer does not only cause problems physically, but also psychologically, socially and spiritually. In holistic care, not only focusing on physical care alone, psychological, social and spiritual care is also very important. Fulfillment of spiritual needs that are not optimal causes a decrease in the quality of care and quality of life for patients. This study aims to explore in depth the phenomenology of the spiritual needs of cancer patients at Dr Mohammad Hoesin General Hospital, Palembang. The research method uses a qualitative phenomenological approach. The research sample consisted of ten cancer patient respondents using purposive sampling. Data was collected using in-depth interviews from October to December 2022. All respondents were women, aged 25-51 years. There are four main themes, 1) acceptance responses to illness experienced and during illness, 2) changes in aspects of life experienced during illness, 3) obstacles to worship during illness and 4) hope for the future in living life. The spiritual aspect is an important aspect that needs attention, especially in the care of cancer patients in hospitals.

Abstract

Relationship between Patient Profile and Quality of Life for Palliative Cancer Patients at Dr. Mohammad Hoesin General Hospital, Palembang. Cancer treatment affects all aspects of the patient's life, both the physical and psychological condition of the patient. Measurement of quality of life is important to do to improve treatment or rehabilitation, medical decision making, and facilitate communication between health workers and palliative cancer patients. This research is an analytic descriptive study using a cross-sectional design with a quantitative approach. The purpose of this study was to determine the relationship between cancer palliative patient profile and quality of life. The instrument for assessing the quality of life is the WHOQOL-BREF instrument. The research sample was 64 adult patients with palliative cancer who were treated in inpatient ward. The majority of the sample is female (79,9%), aged over 46 years old (54,7%), married (85, 9%) and ongoing chemotherapy (78,1%). Based on the results of this study, the quality of life of 60 patients was in a good category (93.8%) and the remaining had a moderate quality of life (6.3%). The majority of poor quality of life occur in domain 2 (psychological) as much as 68.8% and domain 4 (environment) as much as 59.4%. Type of therapy has a significant relationship with quality of life with p value = 0.006. The patient profile (education and cancer stage) is significantly related to the psychological domain and the cancer stage profile is related to the environmental domain. The conclusion of this study is that the type of therapy is related to the patient's quality of life. The quality of life of cancer palliative patients needs to be considered by nurses, especially related to the psychological domain and the environmental domain.

Abstract

Wounds are conditions of damage and partial loss of body tissue caused by physical trauma, sharp objects, blunt objects, temperature fluctuations, chemical exposure. Wound healing is a complex interaction between cells, cytokines, mediators, and blood vessels as a natural physiological response to tissue damage. TNF-α has a crucial role in the wound healing process, facilitating body immunity, immune cell mobilization, fibroblast cell proliferation, keratinocytes, and growth factor expression. Although TNF-alpha has benefits in the early stages of wound healing, excessively high levels or a sustained inflammatory response can lead to problems, such as excessive scar tissue formation or blockages, which can impede the wound healing process. This study utilized the PRISMA-ScR (Scoping Review) protocol. A review was conducted of articles discussing the Role of TNF-Alpha in the Wound Healing Process: Molecular and Clinical Perspectives within the last 10 years. Literature search in this study using PubMed, ScienceDirect, Elsivier and Google Scolar databases Search using English, abstract and full text. After searching with the keywords: The Role of TNF-Alpha, 19,022 articles were found. The search continued using the keyword The Role of TNF-Alpha in the Wound Healing found 346 articles. The last search using the keyword The Role of TNF-Alpha in the Wound Healing: Molecular and Clinical found 26 articles. TNF-α plays an important role in the early stages of cutaneous wound healing, significantly delayed after day 3, but not by day 7. TNF-α has a crucial role in the early stages of skin wound healing, and its administration is recognized to have a positive impact on the healing response, which may pave the way for innovative strategies to address chronic skin wound healing problems.

Abstract

Cancer is currently the second leading cause of death in the world after heart disease. Global number of deaths from cancer in 2020 reached 10 million people. In Indonesia, the number of cancer sufferers has reached more than 369 thousand cases. Cancer treatment consists of various types such as intravenous chemotherapy. However, intravenous chemotherapy can cause complications at the injection spot known as extravasation. The incidence of extravasation at RSUP Dr Mohammad Hoesin Palembang is quite high. In 2019 there were 11 extravasation incidents, in 2020 there were 4 incidents, in 2021 there were 17 incidents and in the period January to October 2022 there were 32 extravasation incidents. This study aimed to determine what factors are associated with the incidence of extravasation in cancer patients with chemotherapy at RSUP Dr. Mohammad Hoesin Palembang. The research design used was a cross sectional purposive sampling technique with a total sample of 78 outpatient chemotherapy (One Day Care). Data analysis was carried out using the Chi Square test (X2). Statistical test results showed that gender, age and type of medicine was not related to the incidence of extravasation, while the vein's spot where the IV catheter inserted was significantly associated with the incidence of extravasation with a p value 0.043. It can be concluded that the incidence of extravasation is related to the vein spot where the intravenous catheter is inserted.
