Abstract

Introduction: Cardiovascular diseases are a major global health concern, with atherosclerosis being a significant contributor. Diabetes mellitus is a well-known risk factor for atherosclerosis, and SGLT2 inhibitors are more effective in reducing cardiovascular disease and mortality compared to other antihyperglycemic agents in patients with type 2 diabetes. Endothelial dysfunction, VSMC phenotypic alterations, inflammation, autophagy, oxidative stress, signaling hormones and dysbiosis are crucial factors and predictors of atherosclerosis and other cardiovascular events. SGLT2 inhibitors have been shown to alleviate these components in atherosclerotic models. Material and Methods: To proceed we analyzed and took into account all publications, including clinical trials, animal studies, in vitro observations, reviews, and meta-analyses from various scientific databases. Results: Evidently, SGLT2i attenuate the progression of atherosclerosis by repressing vascular inflammation, oxidative stress, endothelial dysfunction, mitigating vascular smooth muscle cells function, impairing  foam cell formation, preventing platelet activation and restoration of normal autophagic function. Conclusion: Based on these findings, it can be inferred that SGLT2 inhibitors exhibit anti-atherosclerotic characteristics and may have positive implications for the management of atherosclerosis, particularly in individuals with diabetic complications.

Abstract

Introduction: Ectopic pregnancy (EP) describes a pregnancy that develops outside the uterine cavity. The fallopian tube, specifically the ampulla is the most common site. Positive blood β-hCG levels can identify EP with no ultrasound confirmation of intrauterine or extrauterine pregnancy. Case Presentation: We report a 37-year-old woman was referred from primary health care with a diagnosis of ileus. The patient complained of abdominal pain, spotting, vomiting, and no defecating since the day before; gestational age was estimated at around seven weeks. Vital signs indicated shock; the physical examination revealed slight abdominal distention, decreased bowel sounds, tenderness throughout the abdominal area, slightly defans muscular, and hyper tympanic. Vaginal touch didn't reveal cervical dilation, absent portio swaying pain. The laboratory found anemia and leukocytosis with a predominance of neutrophils. The pregnancy test was positive in the emergency department. Previously, the primary health care facility did not undergo a pregnancy test. Abdominal x-ray showed low-lying obstructive ileus and ascites in the pelvic cavity. Transabdominal ultrasound revealed free fluid in the rectouterine cavity. The patient underwent an emergency laparotomy, and a rupture in the ampulla segment was found, then a salpingectomy was performed. Conclusion: Clinicians must be able to diagnose EP correctly and as quickly as possible. In women of childbearing age presenting with acute abdominal complaints and a known or unknown history of menstruation, we strongly recommend a qualitative β-hCG examination, which can be carried out in health facilities with limited resources and at affordable costs, will be very useful for diagnosing and screening ectopic pregnancy.

Abstract

Human immunodeficiency virus (HIV) infection and sexually transmitted infections (STIs) remain a public health problem and extend to social, economic, and cultural issues. HIV cases in the Southeast Asia region account for 10% of the total global HIV burden. In Indonesia, there are five provinces with the highest number of HIV cases as of December 2021, including DKI Jakarta (73,442), East Java (68,112), West Java (49,435), Central Java (42,012), and Papua (40,277). In Indonesia in 2021, the number of people living with HIV (ODHIV) reported was 36,902 and in 2022 up to the third quarter (July - September 2022) there were 34,213 people. New HIV infections in Indonesia continue to decline, in line with the global decline in new HIV infections. However, this decline has not been as large as expected.

Abstract

Introduction: Bullous Cellulitis is initially characterized by erythema and rapid development of bullae. This disease is most often caused by Beta-hemolytic streptococci and rarely by other bacteria such as Coagulase-Negative Staphylococcus (CoNS) type Staphylococcus haemolyticus. Immunocompromised state and history of malignancy are at high risk of infection with this bacterium. Local complications such as bullae can occur in obesity. Case Presentation: A 48-year-old woman with complaints of tensed and loose-walled blisters with a reddish base on the right thigh. The patient had a medical history of Thyroid Cancer, Acute Renal Failure, and Bulging Mass pelvic femur. On physical examination, the patient was unconscious with a ventilator. Dermatological examination of the dextra inferior extremity showed loose bullae with a positive Nikolsky sign and tensed bullae over the erythematous patches, ill-defined, irregular edges, and warm palpable. Laboratory results of leukocytosis, hypoalbumin, and CoNS bacteria from blood culture. An ALT-70 score obtained a total score of 5. Conclusion: Cases of bullous cellulitis are quite rare; cases like this usually occur in patients with an immunocompromised predisposition and obesity. CoNS is a commensal bacteria but, in some cases, could be a pathogen causing cellulitis. Poor prognosis that ends in death could occur in patients who have various complications.

Abstract

Introduction: Menopause, a significant event in a woman's reproductive lifespan, entails hormonal changes and a range of symptoms affecting various bodily systems. Conventional treatments like hormone replacement therapy (HRT) have limitations and associated risks, prompting interest in alternative therapies such as resveratrol. Materials/Methods: The literature search was conducted in six databases. The outcome of interest measures the mean changes in bone mineral density (BMD), Quality of Life (QoL), bone biomarkers, menopause symptoms, and cognitive and mood functions pre- and post-interventions. Resveratrol was administered in various forms and doses, including trans-resveratrol 75 mg twice or once daily, 250/500/1000 mg resveratrol tablets, and a combination of fermented soy containing 10 mg equol and 25 mg resveratrol. Quality appraisal was done using the Cochrane Risk of Bias Tool 2. Results: Resveratrol supplementation in post-menopausal women, involving 745 participants across various studies, has shown remarkable health benefits. These include improved cerebral vasoreactivity (CVR) to hypercapnia, a 10% decrease in overall pain, and enhanced quality of life. Additionally, resveratrol boosts flow-mediated dilation (FMD) by 23% and increases serum sex hormone-binding globulin (SHBG) concentrations by 10%, benefiting bone mineral density (BMD). Over the long term, it leads to a significant 35% increase in FMD and a substantial 33% enhancement in overall cognitive performance, highlighting its diverse benefits. Conclusion: For women in the post-menopausal stage, resveratrol supplementation shows potential for improving cardiovascular health, relieving pain, enhancing quality of life, supporting bone health, boosting cognitive function, and promoting overall mental well-being.

Abstract

Introduction: At the age of 3-6 months, the baby's total sleep time is around 13-15 hours/day, and babies aged 3-6 months usually tend to have irregular sleep duration. Good quality sleep in babies affects not only the baby's intelligence and brain development but also the baby's attitude when he wakes up. Al-Quran murrotal treatment is a useful tool for treating sleep issues in infants. Our auditory organs receive Murottal Qur'an therapy, which is subsequently sent to the limbic system, a region of the brain in the middle of the brain that controls hormones in developing children. Material and Methods: Pre-experimental methodology with a single group pretest-posttest design is used in this kind of study. Purposive sampling was used to choose 16 infants, ages 3-6 months, from the Federer formula. Results: Comprised of sixteen infants who had poor sleep quality. Following murottal qur'an therapy, 15 babies (93.8%) were able to sleep through the night, while one baby (6.2%) continued to have issues with the quality of their sleep. The Wilcoxon test research yielded a p-value of 0.000, indicating that there is a significant effect of delivering murottal Al-Qur'an therapy on the quality of a baby's sleep (p < 0.005). Conclusion: There is an effect of providing murottal Al-Qur'an therapy on the sleep duration of babies aged 3-6 months.

Abstract

Introduction: Menopause frequently coincides with age-related hormonal decline, elevated blood pressure, and unfavorable body composition changes, collectively heightening the risk of cardiovascular disease. Addressing the adverse effects of menopause is crucial through the integration of suitable lifestyle interventions, such as exercise. Nonetheless, there remains controversy within studies regarding the significance of exercise in improving Quality of Life (QoL), reducing Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), Total Cholesterol (TC), and Low-Density Lipoprotein (LDL), as well as maintaining Bone Mineral Density (BMD). Thus, this review aims to determine and clarify the influence of exercise on patient outcomes. Materials/Methods: This meta-analysis followed the PRISMA statement guidelines, utilizing databases such as PubMed, ScienceDirect, and Google Scholar over the past ten years. Inclusion criteria adhered to the PICOS framework, and the literature quality assessment was based on the Risk of Bias 2.0 Tool. Results: Out of 14 included studies, the majority indicated no significant improvements in menopause women undergoing exercise regarding QoL [MD: -6.62 (95% CI: -16.90 – -3.66, I2=91%)], SBP [MD: -4.74 (95% CI: -10.54 – 1.05, I2=100%)], DBP [MD: -4.52 (95% CI: -8.59 – -0.46, I2=99%)], TC [MD: -4.06 (95% CI: -8.75 –0.64, I2=57%)], LDL [MD: -2.26 (95% CI: -7.37 – 2.86, I2=65%)], and BMD [MD: 0.06 (95% CI: -0.01 – 0.13, I2=86%)]. Significant improvements were seen only in DBP outcomes Conclusion: Menopause exercise women may effectively raise QoL, reduce SBP, DBP, TC, and LDL, also maintain BMD.

Abstract

Introduction: A predisposing factor for fecal incontinence in women is vaginal birth, impacting between 24 and 44% of the female population. Fecal incontinence, characterized by the inadvertent expulsion of gas or excrement, constitutes a multifactorial condition. It causes discomfort and diminishes the quality of life for women. Owing to its occurrence in 0.6–9% of vaginal deliveries, obstetric anal sphincter injury (OASI) has been identified as the most prevalent cause of AI in young women, with a 15–61% incidence rate following primary repair. Case Presentation: The case presented in this report involved a 26-year-old woman who presented with symptoms of difficult-to-control flatulence and defecation following vaginal delivery. The patient has a history of giving birth to her first child spontaneously at a midwife in June 2023; she found a tear in the birth canal, which was then stitched up. Another history of past illness has been unremarkable. Upon examination, vital signs were within normal limits. The patient was obese I, with a BMI of 27.3 kg/m2. Local examination of external genitalia within normal limits. A digital rectal examination showed that the anal sphincter was not palpable, the rectal mucosa was smooth, and other examinations were within normal limits. Lab examination showed normal results. Conclusion: After an exhaustive examination, a diagnosis of fecal incontinence was made for the patient. The patient was treated with colporrhaphy and sphincterorrhaphy, both of which produced favourable outcomes.

Abstract

Introduction: The Covid-19 pandemic situations affect all groups of people in society. Covid-19 presented disruptions in work routines and unprecedented job loss for millions of people all over the country, including Persons with disabilities (PWDs). During the pandemic, PWDs harmed their physical, mental, and social lives. Material and Methods: This study used a cross-sectional design, and data were obtained from 96 PWDs who received training from the Vocational Training Institute of the Centre for the Rehabilitation of the Paralysed (CRP). The telephone survey was done with self-administered questionnaires. Descriptive statistics were used for the data analysis. Results: Among the total participants were men (76%) and women (24%). During the pandemic, 93% followed basic protection against infection, 32% received government emergency support and 97% of participants shared that their shops/factories and jobs were shut down for 1-2 months. On the other hand, 82% of participants reported having no monthly wage, and 91% of participants’ families had significantly negative impacts on their physical and mental health and family income and were unable to access all sectors. Conclusion: The Covid-19 pandemic followed by the national lockdown to mitigate the transmission of the virus, but the sudden disruption of social support and assistance to the groups of PWDs had a serious impact on their daily living activities, livelihood, and also on economic status that impacted the overall quality of life. So, the government or non-government organizations should take the lead to initiate a disability-inclusive approach along with improving access for PWDs while planning for pandemic responses.
