Abstract

Introduction: Medical tourism refers to any person who travels cross-border to a foreign country to receive medical services. The Thai government has set a strategy to make Thailand a medical hub to drive economic growth in the future. The purpose of this research was to conduct an analysis of health system management for Cambodia, Laos, Myanmar, and Vietnam (CLMV) medical tourists in regional provinces of Thailand. Methods: This research was qualitative research. The in-depth interview was conducted with 25 cases, and the focus group was conducted with 30 representatives of government agencies and private entrepreneurs involved in policies, together with document analysis. The content analysis was applied for data analysis. Results: It was found that the guidelines for health system management for CLMV medical tourists in regional provinces, namely adjusting the medical payment system, tightening screening of treatment eligibility, cooperating internationally in the management of the International Health Insurance Fund, establishing a medical fund for those who do not have Thai citizenship; establish foreign patients department in government hospitals; and control appropriate and fair service rates. Conclusion: Practical short-term factors include promoting targeted niche markets, establishing marketing partnerships with other businesses, supporting medical tourism, making a database of expertise of hospitals in Thailand and training doctors with specialized expertise, especially international programs, promoting research, development and production of medical equipment and medical supplies, and encourage private hospitals in Thailand to be certified important in increasing Cambodia, Laos, Myanmar and Vietnam (CLMV) Medical Tourists in the Regional Provinces of Thailand.

Abstract

Introduction: Reproductive concerns by People Living with HIV (PLHIV) are matters that are important for both individual and general public health. We described fertility desire among HIV-positive women attending Care and Treatment Clinics (CTC) and determined factors for having such a desire to get children in urban areas of Dar es Salaam and Dodoma in Tanzania. Material and Methods: Cross-sectional study for which primary data were collected in a random sample of women of reproductive age (15-49 years) after accessing HIV care and treatment services. A survey was conducted among HIV-positive women receiving anti-retroviral treatment to document service needs, service quality, and fertility preferences. A Univariate model was used to examine the relationship between the dependent and the independent variables, and all associations were taken into account in the final model. Results: Among 691 participants, 282 (40.8%) women living with HIV have fertility desire. Women aged above 25 experienced a reduction in the odds of fertility desire by 56% (P value =0.003) compared to the reference group of women aged 25 years or less. Fertility desire was predicted to have two times as much growth with 1 to < 3 years (p- value=0.02) and 5 or 5+ years (P value =0.01) time lapse since the debut of ART, compared to the group of women who were on ART for 3 years but less than five years. Conclusion: Our data has not only reconfirmed the importance of ART and PMTCT services to PLHIV but also contributed to evidence on their value addition in terms of the creation of resilience to normal fertility family life after debut HIV diagnosis.

Abstract

Introduction: Endometriosis is a benign gynecological disorder characterized by ectopic growth of endometrial tissue. This condition may deteriorate patient’s quality of life due to pain as its main manifestation and its complications, including impaired fertility and risk of ovarian cancer. Current standard endometriosis treatment still yields numerous side effects and controversy. Thus, herbal medicine is hoped to be the safer alternative treatment with excellent efficacy for endometriosis. Methods: The search of literature published in English or Bahasa Indonesia was performed by incorporating the search queries “endometriosis”, “herbal therapy”, “silymarin”, “curcumin”, “Viburnum opulus”, “Pueraria”, “Allium sativum”, “black garlic”, “naringenin” on databases including PubMed, Cochrane, Springer, Science Direct, Nature, and Google Scholar. The studies included were in vitro studies, in vivo studies, and clinical trials. Results: Various herbal treatments have been widely studied regarding their anti-inflammatory, antioxidant, neuroprotective, hepatoprotective, cardioprotective, antiallergy, and immunomodulatory capacity. A lot of herbal compounds have the potential to suppress and manage endometriosis through various mechanisms, such as inhibiting the migration and adhesion of endometriosis to other tissues, suppressing inflammatory response in endometriosis pathogenesis, generating antioxidant, inhibiting estrogen activity, and inducing apoptosis of endometriosis cell through a number of signaling pathways. Conclusion: Herbal medicine has the potential to manage endometriosis through various mechanisms. Further research is needed regarding the efficacy of herbal therapy in managing endometriosis.

Abstract

Introduction: A high number of reports of preterm incidents in Probolinggo, which was the highest in East Java in 2018, has caused problems in the financing process claimed by the Indonesian National Social Health Insurance (Badan Penyelenggara Jaminan Sosial (BPJS)). Insufficient financing for maternity mothers, as the highest proportion of national delivery methods, can certainly be a direct or indirect obstacle to the maternal mortality rate, which is still very high in Indonesia. This research aims to analyze the influence of hospitalization period, gestational age, and delivery method in conservative preterm therapy on BPJS claim achievement at Waluyo Jati Hospital, Probolinggo Regency. Material and Methods: This study is a quantitative research with a cross-sectional approach. Sampling took place from May to September 2023 at Waluyo Jati Regional Hospital, Probolinggo Regency. Sampling used an accidental sampling technique. 33 respondents participated in this research. Results: The results of this study showed that the largest number of respondents with the criteria for a length of stay of 4-7 days was 23 respondents (69.6%), gestation age 31-33 weeks was 14 respondents (42.4%), and 23 respondents underwent cesarean section delivery (69.7%). The highest BPJS claim achievement value was in the range of 30-50%, with 18 respondents (54.5%). The results of the Multiple Linear Regression showed a p-value of 0.000. Conclusion: The conclusion from this research is that there is an influence of hospitalization period, gestational age, and delivery method simultaneously in conservative preterm therapy on the achievement of BPJS claims at Waluyo Jati Hospital, Probolinggo Regency.

Abstract

Introduction: Uterine fibroids are non-malignant neoplasms originating from the smooth muscle cells of the uterus, and they represent the most prevalent tumor seen within the female reproductive system. The primary established approach to manage this condition is surgical interventions, specifically myomectomy, hysterectomy, or hysteroscopic removal of small subserous nodules. Occasionally, submucous fibroids may protrude through the cervical os, thereby enabling the feasibility of performing a vaginal hysterectomy. Case Presentation: The clinical presentation of a 44-year-old multiparous woman who arrived with symptoms of vaginal bleeding and a sizable mass protruding through the vaginal opening. The patient underwent an emergency vaginal extraction to remove the mass measuring 24 cm in the emergency room and a scheduled for elective surgery. During the procedure, uterine inversion was found with a muscle bundle around the corpus uteri and a fourth-grade perineal tear. This patient was also planned for a total abdominal hysterectomy and bilateral-salphyngo-oophorectomy procedure, along with Haultain's procedure and perineal repair. The patient exhibited favorable surgical outcomes. Conclusion: Hysterectomy is the preferred therapy for prolapsed pedunculated submucous myoma. It has been seen that even exceptionally big nascent myomas can be effectively removed using vaginal means.

Abstract

Introduction: Pregnant women who experience miscarriage may experience health complications that lead to maternal morbidity or death. The incidence of miscarriage rose by 26% in women over 40 years old and by 12% in women under the age of 20. Due to their immature uteri, pregnant women under the age of twenty are particularly vulnerable to complications during pregnancy and childbirth, including miscarriage. In addition, women over 35 may be more susceptible to issues with their aging uterine organs, which raises the possibility of chromosomal abnormalities and pregnancy. Material and Methods: This type of research is an analytic survey with a cross-sectional approach. This research was conducted in the Sabang City Regional General Hospital in May 2022. The population in this study was 320 people, while the sampling technique was total sampling. Results: Test analysis of research results using the chi-square test. Based on the results of the study show a relationship between age and the incidence of miscarriage with a value of p = 0.000 < 0.05, and there is a relationship between pregnancy distance and the incidence of miscarriage with a value of p = 0.000 < 0.05. Conclusion: There is a relationship between age and pregnancy distance to the incidence of miscarriage.

Abstract

Endometriosis is a chronic inflammation with a specific characteristic being the appearance of endometrial-like tissue outside the uterus. Symptoms of endometriosis are usually non-specific and can be found to be similar to symptoms of other gynecological and even gastrointestinal disorders. The nature of the disease, enigmatic and needs more cost at every step, stimulate the economic burden in managing endometriosis. The important variable that has a significant role in the economic burden of endometriosis is the recurring nature of this condition, even up to 40-45%. I selected online articles using PRISMA and obtained 122 articles, 4 articles being deleted because of no full text, 8 articles being deleted because of duplication, 17 articles being deleted because of the age of articles more than 10 years, 80 articles being deleted because of not fully relevant with main topics, 4 articles being deleted because of the burden not specific in the economic aspect, and 9 articles are the most perfect choice related with the main topics. In conclusion, endometriosis is a chronic disease experienced by women mostly of productive age, which affects the quality of life, fertility, and even direct economic burdens in the form of recurring and long-term health expenses as well as indirect economic burdens in the form of productivity, many absences from work, and excessive leave. Often due to the treatment process. Based on several journals with similar discussions show significant results in the influence of endometriosis on economic burden.   .

Abstract

Introduction: Polycystic Ovary Syndrome (PCOS) is a common hormonal disorder in reproductive-aged women, characterized by hormonal imbalances such as Luteinizing Hormone (LH) and Follicle Stimulating Hormone (FSH). This study investigates the impact of Vitamin D3 supplementation on LH and FSH levels in female rats with a PCOS model. Material and Methods: In this laboratory experiment, female rats were induced with testosterone propionate for 28 days. They were divided into three groups: negative control, positive control (testosterone only), and treatment (testosterone + Vitamin D3 supplementation). LH and FSH levels were measured using the enzyme-linked immunosorbent assay method. Results: The results revealed that Vitamin D supplementation significantly reduced FSH levels in the treatment group compared to the positive control, while it increased FSH levels. Regression analysis demonstrated a negative correlation between Vitamin D supplementation and LH levels, as well as a positive correlation with FSH levels. Conclusion: Vitamin D3 supplementation appears to have a regulatory effect on LH and FSH levels in female rats with PCOS. Future research could explore the impact of higher Vitamin D3 doses and conduct further investigations at advanced stages of PCOS. These findings contribute to our understanding of PCOS and potential interventions involving Vitamin D3.

Abstract

Introduction: Osteoporosis is a medical condition characterized by increased bone turnover and decreased bone mass, which leads to bone fractures. Antiresorptive therapy, hormone replacement therapy (HRT), and bisphosphonates are used as first-line therapy related to numerous side effects. The osteoprotective properties of phytoestrogens are well known. This systematic review aims to explore the potential of phytoestrogen in the management of osteoporosis patients based on serum bone biomarker analysis. Methods: The literature search was conducted in six databases. The outcome of interest measures the mean changes in bone mineral density (BMD) and other serum bone biomarkers. Various forms of phytoestrogen intervention were used, including isoflavone extracts with an administered dose (tablets, capsules), genistein extract (tablets), resveratrol, and isolated soy protein (IBS) in powder form, beverages, food and snacks, and soy products. Quality appraisal was done using the Cochrane Risk of Bias Tool 2. Ten articles were included in the systematic review. Results: Seven studies found the mean changes in BMD values were significantly higher than the control group's after phytoestrogen intervention. Phytoestrogens dramatically boost numerous bone formation markers, including calcium, phosphorus, Ca/P ratio, and vitamin D, followed by a drop in BAP and osteocalcin levels. Furthermore, intervention may reduce bone resorption indicators such as CTX, RANKL, AKP, OPG, DPD, and PTH. Conclusion: Phytoestrogen intervention has demonstrated effectiveness in increasing bone mineral density and serum bone biomarkers.

Abstract

Introduction: Menstruation is periodic and cyclic bleeding from the uterus, accompanied by endometrial desquamation. In specific individuals, menstruation can cause pain called dysmenorrhea, which is characterized by mild, moderate, and severe menstrual pain. Primary dysmenorrhea is not a life-threatening disorder, but it can hurt women's quality of life. Prevalence of menopause in Indonesia An estimated 55% of women of reproductive age experience pain during menstruation. Material and Methods: We searched articles published from 2014-2020 in  PubMed, Proquest, and Google Scholar with the keywords "primary dysmenorrhea” AND “student lifestyle”. All data were gathered and analyzed. Results: The results showed that 8 articles showed the relationship between lifestyle and the occurrence of primary menstrual pain among female students. In comparison, the other two articles show that there is no relationship between lifestyle and primary dysmenorrhea in female students. Conclusion: This study concludes that an unhealthy lifestyle, such as high stress levels, infrequent exercise, and frequent consumption of fast food, is effective for primary dysmenorrhea during menstruation.
