Jurnal Kedokteran Komunitas merupakan salah satu jenis jurnal akademik bidang kedokteran atau kesehatan di mana penulis mempublikasikan artikel ilmiah hasil penelitian maupun kajian. Artikel ilmiah ini juga mencangkup hasil-hasil penelitian terbaru dan tulisan ilmiah lainnya di bidang biomedis, kedokteran klinis, kedokteran komunitas, dan kesehatan masyarakat. Abstract

Background: One of the academic achievement of Medicine Faculty students of UNISMA is determined by curriculum block score. Students with curriculum block score under 68 have to take remediation. Students who take remediation indicate that they aren’t ready to attend lectures. Academic readiness was shaped by several indicators, including achievement motivation orientation, m-score and non-academic activities of students is a factor that influenced academic achievement; that is curriculum block score. This research aims to prove whether academic readiness related to achievement motivation orientation, m-score and non-academic activities affect academic readinesss.Method: This descriptive analytic research with cross-sectional approach applied sampling in students at the year of 2017, 2018 and 2019 of Medicine Faculty of UNISMA as many as 253 respondents. The data of achievement motivation orientation of the students used questionnaires of Academic Readiness Questionnaire (ARQ). The data of M-Score was the result of students’s final national examination when they were in senior high school. The data of non-academic activities of the students was related to the questionnaires made by the researcher based on the condition in the Medicine Faculty of UNISMA. To prove the relevance of academic readiness dimension and the impact on academic achievement, researcher used path analysis with SEM-PLS method.Result: Academic readiness was formed by Achievement Motivation Orientation (AMO), M-Score and Non-Academic Activities (NAA) dimension with p = 0.00 (p <0.05) and R2 AMO=0,671; R2 NAA=0,659. Academic readiness could be positively influenced by the academic achievement by p = 0.00 (p<0,05)  and explained by 26.9% with R2  = 0.269.Conclusion: The academic readiness model related to achievement motivation orientation, m-score and non-academic activities affect to academic achievement is a fit or apropriate.Keywords: Academic Readiness, Achievement Motivation Orientation, M-Score, Non-Academic Activities, Academic Achievement.

Abstract

Introduction : Diabetes mellitus (DM) is a glucose metabolism disorder that is able to cause organ damage if the patient has inadequate glycemic control. Organ damage in patient with DM leads to micronutrient metabolism disorder includes calcium. Decreased serum calcium level in DM patient could reduce the insulin sensitivity. The effect of glycemic control in type 2 DM on calcium serum level in Indonesia has never been studied. Therefore, further research is needed, especially in Malang City.Method : : A cross sectional study with control group post-test-only design was conducted using purposive non-random sampling. Total subject was 40 patients consist of 15 patients with good glycemic control and 25 with poor glycemic control. Each group examined for serum calcium level using Atomic Absorption Spectrophotometry (AAS). Data were analyzed by using Independent T-Test followed by Pearson correlation test with a significance level of p <0.05Results : Calcium level in type 2 DM patients with good glycemic control (4,25±1,54 mg/dL) were higher than poor glycemic control (3,32±1,43 mg/dL) with signifikasi p=0,404. Glycemic control is moderately correlated on serum calcium with correlation level r=-0,358 and signification level p=0,03.Conclusion : Glycemic control in type 2 diabetes mellitus moderate correlated with serum calcium level.Keywords: Glycemic control in type 2 DM is moderately correlated with serum calcium level.

Abstract

Background: Diabetes Mellitus (DM) is a degenerative disease caused by impaired insulin function, marked by hyperglycemia. Chronic hyperglycemia may decrease the nutritional status of DM patients by measuring Body Mass ndex (BMI) and Upper Arm Circumference (UAC). This study aimed to look at the role of glucose control in BMI and UAC in Type 2 DM patients.Methods: This reserach was cross sectional studies. The respondents are male and female of diabetic patients without complications over the age of 40 years. The subjects were divided into the controlled group and uncontrolled group. Each group performed weight measurements with weight scale, body height with microtoise and UAC with a measuring tape. Independent T-Test was used to analyze the data , followed by a Pearson correlation test with a significance level of p<0.05.Results: There was no significant difference between glucose control, BMI (p=0.921), right UAC (p=0.611) and left UAC (p=0.406) in controlled group and uncontrolled group. Glucose control had no correlation to BMI (r=-0.061). There was also no correlation between glucose control, right UAC (r=0.161) and left UAC (r=0.197).Conclusion: Glucose control had no role in body mass index and upper arm circumference towards patients with diabetes mellitus type 2 in Malang.Keywords: Glucose control, body mass index, upper arm circumference, type 2 diabetes mellitus

Abstract

Introduction: The tuber of garlic contains of various compounds that has anthelmintic effect of unknown mechanism.  Therefore, this research aimed to determine the active compounds and find their anthelminthic mechanism of action. Method: The potency of garlic active compounds to inhibit acetylcholinesterase, beta tubulin and to activate voltage dependent L type calcium channel were evaluated by molecular docking, compared with pirantel pamoat, mebendazole and praziquantel.Result:  Garlic’s active compound, S-allyl-cysteine sulfoxide (alliin) has the best affinity to target protein acetylcholinesterase, beta tubulin and calcium channel with free binding energy of -4.84 kcal / mol, -5.89 kcal / mol and 6.50 kcal / mol respectively under control values. Active compounds of bawang putih tuber have lower affinity values compared to controls of pyrantel pamoate, mebendazole and praziquantel.Conclusion: Active compounds of bawang putih (Alliium sativum L) have low antihelmintic potential through target protein acetylcholinesterase, beta tubulin and voltage dependent L type calcium channel. Abstract

Background: Helminthiasis are a pandemic and has a high prevalence in Indonesia. Turmeric rhizome contains curcuminoids and essential oils as its active compounds. However, their mechanism as an anthelmintic is unknown. Therefore, further research is needed to find out the exact mechanism of turmeric rhizome as an anthelmintic.Method: The effectivity of turmeric rhizome’s active compounds against Acetylcholinesterase, Microtubulin (Beta Tubulin), and Calcium Channel was evaluated by Docking Server with Pirantel Pamoat, Mebendazole, and Praziquantel as sequential control.Results: Curcumin Sulfate was identified as having a low affinity for Acetylcholinesterase with -5,62 kcal/mol free bond energy. Cyclocurcmin has a high affinity for Beta Tubulin and Calcium Channel with -7,39 and -8,36 kcal/mol free bond energy sequentially. Meanwhile Dihydrocurcumin and Curcumin Sulfate have a high affinity for Calcium Channel with -7,19 and -7,41 kcal/mol free bond energy sequentially.Conclusion: The active compound of turmeric rhizome has a low affinity for the inhibition of Acetylcholinesterase in Ascaris lumbricoides. Cyclocurcumin has a high affinity for the inhibition of Microtubulin (Beta Tubulin) and Calcium Channel in Ascaris lumbricoides. Meanwhile Dihydrocurcumin and Curcumin Sulfate have a high affinity for the activation of Calcium Channel in Schistosoma sp.Keywords: Anthelmintic, Turmeric Rhizome, In silico

Abstract

Introduction : Chronic hyperglycemia increase ROS production and risk of diabetic macroangopathy complications. Abelmoschus manihot (L.) Medik has potential as an antidiabetic and anti-inflammatory, but research on Abelmoschus manihot (L.) Medik inhibiting elevated levels of TNF-α tissue and aortic lumen diameter that contribute to complications has not been widely reported. This research aimed to determine the effect of ethanol extract Abelmoschus manihot (L.) Medik by observing the level of TNF-α tissue and aortic lumen diameter in diabetic rats.Methods : This study use 4-6-weeks-old Sprague dawley male rats which divided into normal group, diabetes mellitus group, and ethanol extracts of gedi merah leaves doses I (200 mg/kgBW), II (400 mg/ kgBW), and III (800 mg/kg BW) (n = 5 rats). The rats were induced HFFD and 25 mg/kgBW of STZ injection i.p with multiple dose. The rats were administrated orally EEDGM for 4 weeks. The level of TNF-Alfa aortic tissue was measured using microplate reader λ = 450 nm, while the aortic lumen diameter was examined using a trinocular microscope at 40x magnification. Statistical analysis using One Way ANOVA and LSD test (p <0.05).Results : EEDGM dose II and III inhibit the increase levels of TNF-α rat aortic tissue. EEDGM doses I, II, and III were not significant inhibit the decrease of aortic lumen diameter compared to KDM (p> 0.05). DTLF and STZ induction increases tissue TNF-α levels and decreases the aortic lumen diameter.Conclusion : Ethanolic extract of Abelmoschus manihot (L.) Medik could inhibit the increase in TNF-α levels of aortic tissue and did not significant inhibit the decrease of aortic lumen diameter.Keywords : High fat diet, high fructose diet, streptozotocin, diabetes, inflammation

Abstract

Introduction: SRL (Self-Regulated Learning) is one of some factors influencing the college student’s academic achievement. By applying SRL well, academic performance will be improved, but, there are some difficulties to apply it such as setting the strategy of motivation. Some strategy of motivation formed by performance goals which includes good grade, employment prospect, and successful future. Hence, this study aims to observing the effect of performance goals forming the SRL and the  effect of SRL formed by performance goals to academic achievement.Methods: This research used analytical descriptive observational design with explanatory squencial mixed  method by collecting and analyzing data quantitatively with quitionairre and then qualitatively with focusec group disscusion. The sample is 175 college student of General Medicine in Malang Islamic University from the purpossive sampling counted by the amount of minimal criteria (Morgan and Kretjie Table), then analyzed the quality and relationship among the constructs by using PLS. After that, qualititative data was collected by the focused group disccusion with the 18 sample choosen by maximum variation (6 girls and 6 boys) and analyzed by content analysis.Result: In forming the SRL, performance goals show 0,985 for R square and >1,97 (P value 0)for T statistic so  it prove the significant effect. While in affecting academic achievement, the SRL formed by performance goals shows non-significant effect because the the R square is 0,006 dan the T statistic is <1,97 (P value 0,255).Conclusion: The motivation of achieving good grades, employment prospect, and successful future (performance goals) are factors that form SRL, while the SRL formed by those performance goals does not affect the academic achievement. Keywords: Self-Regulated Learning, performance goal, academic achievement, student of general medicine

Abstract

Introduction: One simple and rapid DNA isolation method is by heat treatment. However, it unknown whether the result of this method has the same quality compared to the more commonly used filter based kit. This research aims compare heat treatment and filter based kit DNA isolation methods based on its limit of detection (LOD) and limit of quantification (LOQ) from DNA isolation yield from E. coli.Method: This research is an in vitro laboratorium experimental research method. The E. coli concentration in this study is obtained through purposive samplingwith a concentration of 101 – 104 CFU/ml. Heat treatment and filter based kit method are DNA isolation methods used in this study. The data analysis uses ANOVA and Least Significant Diference (LSD) test.Result: Optimal yield of heat treatment method was found at a bacterial concentration of 10.000 CFU/ml (p=0.000) and 1000 CFU/ml (p=0.002). While the optimal yield of filter based kit was found at 10.000 CFU/ml (p=0.003), 1000 CFU/ml (p=0.009) and 100 CFU/ml (p=0.033). The purity value of both heat treatment and filter based kit method did not reach 1,7-2,0. The heat treatment's LOD was 2,39 ng/µl, while the filter based kit's LOD was 4,06 ng/µl. The heat treatment's LOQ was 6, 43 ng/µl, while the filter based kit's LOQ was 8, 10 ng/µl.Conclusion: According to limit of detection, the heat treatment method has the minimum bacterial concentration of 1000 CFU/ml. Whereas, the filter based kit method has the minimum bacterial concentration of 10 CFU/ml. Based on the method comparison, filter based kit method has higher yield than heat treatment method. Keyword: DNA isolation, heat treatment, filter based kit, Escherichia coli

Abstract

Introduction: Human Immunodeficiency Virus (HIV) is a retrovirus group in the family Retroviridae, a genus of lentiviruses that attack white blood cells that cause a decrease in immunity in humans.The immunity of HIV patients can be assessed based on CD4 levels and clinical symptoms which are divided into four clinical stages of HIV. Each level of HIV stage can affect the use patterns of antiretrovirals and antibiotics used by HIV patients during treatment. This study aims to determine the relationship between the effect of HIV staging on antiretroviral profiles, types of opportinistic infections that accompany, antibiotics used for opportunistic infections and the duration of antibiotic administration used by HIV patients in one hospital in Malang.Method: Non-experimental research was carried out in a Analytic Observational Cross Sectional Retrospective study using a Descriptive Analytic design, using a total sampling method by taking secondary data or medical records on HIV patients at Hospital X Malang in 2015-2018.Results: The total population are 112 medical records and data that inclusion criteria are 73 medical records obtained by the HIV stadium did not affect the use of ARVs  HIV stage did not affect the use of antiretroviral drugs (p = 0.473) (p> 0.05), opportunistic infections (p = 0.135) (p> 0.05), antibiotic use (p = 0.175) (p> 0.05) and duration of antibiotic administration (p = 0.864) (p> 0.05).Conclusion: HIV stage does not affect the pattern of ARV use, opportunistic infections that accompany patients, patterns of antibiotic use and duration of antibiotic administration of HIV patients at VCT Hospital X Malang in 2015-2018.Keywords: HIV stage, ARVs, opportunistic infections, antibiotics

Abstract

Introduction: Single Garlic (Allium satuvum L.) can be an alternative cure for the Ascariasis disease because it has compound anti-helmintik like saponin, allicin, and flavonoid that can kill a worm contained in the human body.Method: This research uses the in vitro experimental laboratory. In the sample collection Ascaris suum , goeze used method of random purposive sampling. Ascaris suum , goeze roundworm used in this study amounted to 28 tail included in 7 groups tube which each canister contains 4 worms. 7 groups tube consist of the negative control group (NaCl 0.9 %), 5 group consist single garlic ethanol extract (0.5 % , 1 % , 2 % , 4 % , 8 % ) , and positive control group (pirantel pamoat 5 mg / ml).Result: The results obtained in the linear regression show significant impact (P>0,05) with the required PC50 0,81% to make the paralysis 50% of the total number of worms and it takes concentration Single Garlic extract LC50 2,09% to make the death of 50% of the total number of worms.Conclusion: Extract ethanol of Single Garlic is having an effect on anti-helmintik Ascaris suum, Goeze roundworms.Key Word: Single Garlic Extract, PC50 Paralysis, LC50 Death, Ascaris suum Goeze Adult Worm
