Abstract

Background: Antenatal Care (ANC) services are a vital maternal health intervention for the early detection of pregnancy risk factors and the prevention of complications contributing to high maternal mortality rates. Although the national target for both the first visit (K1) and complete six visits (K6) is 100%, coverage remains suboptimal. Low knowledge regarding the benefits of prenatal check-ups often hinders adherence. Community Health Center X continues to face low complete K6 coverage at 60.4%. This study aims to analyze the relationship between pregnant women's knowledge level and distance to health facilities with the completeness of K6 ANC visits. Methods: An analytical study with a cross-sectional approach was conducted. The study sample comprised 47 postpartum women selected using total sampling. Data were analyzed using univariate and bivariate statistical analysis. Results: The findings revealed that 55.3% of respondents had not completed the K6 ANC visits, and 48.9% possessed a low level of knowledge regarding prenatal care. Although the majority of respondents (63.8%) had good geographic access living within a reasonable distance, this ease of access did not fully translate into service utilization due to continued reliance on traditional practices. Bivariate analysis demonstrated a significant association between knowledge level and K6 ANC completeness ($p = 0.003$), as well as between distance to health facilities and K6 ANC completeness ($p = 0.002$). Conclusion: Knowledge level, reproductive experience, and geographic accessibility are critical determinants influencing maternal adherence to standard complete antenatal care (K6) visits.

Abstract

Background: Hypercholesterolemia is one of the major risk factors for cardiovascular disease, with an increasing prevalence in Indonesia. The use of natural products as alternative therapies represents a promising approach. Lempipi leaves (Gymnema inodorum (Lour.) Decne) contain bioactive compounds, including flavonoids, saponins, tannins, and alkaloids, which may exhibit anticholesterol activity. Objective: This study aimed to formulate effervescent granules containing ethanolic extract of lempipi leaves and evaluate their physical quality and in vitro anticholesterol activity. Methods: Lempipi leaves were collected, processed into dried simplicia, and extracted by maceration using 96% ethanol. The resulting extract was formulated into effervescent granules. Physical quality evaluations included organoleptic properties, flow time, angle of repose, particle size distribution, degree of fineness, foam height, pH, and solubility. In vitro anticholesterol activity was assessed using a UV–Visible spectrophotometer at a wavelength of 627 nm. The activity was determined based on the percentage of inhibition and the half-maximal effective concentration (EC₅₀). Results: All effervescent granule formulations met the required physical quality standards. Formula 1 showed an EC₅₀ value of 125.587 ppm, indicating weak anticholesterol activity, whereas Formula 2 showed an EC₅₀ value of 76.99 ppm, indicating moderate anticholesterol activity. Formula 2 demonstrated stronger anticholesterol activity than Formula 1. Conclusion: Effervescent granules containing ethanolic extract of lempipi leaves exhibited in vitro anticholesterol activity and met the physical quality requirements. Formula 2 showed the highest activity and has the potential to be further developed as a herbal preparation to help reduce cholesterol levels. Suggestion: Further studies are recommended to optimize the formulation, determine the active compounds responsible for the anticholesterol effect, evaluate product stability, and conduct in vivo and clinical studies to confirm its efficacy and safety.

Abstract

Background: The postnatal period is a critical window for newborn survival, during which umbilical cord care directly influences separation timing and infection risk. Postpartum mothers serve as primary caregivers, yet their understanding of appropriate cord care varies considerably based on education, cultural tradition, and access to health information. Objective: This study examined the association between postpartum maternal knowledge of umbilical cord care and cord separation duration among newborns in Raya Kahean District, Simalungun Regency, alongside selected sociodemographic factors. Methods: An observational analytic cross-sectional study was conducted using total sampling of eligible postpartum mothers in the study area. Knowledge was assessed via a structured, validated questionnaire; cord separation duration was determined through direct observation and caregiver report; and associations between categorical variables were tested using the Chi-Square test. Results: Most mothers were of prime reproductive age with junior secondary education, predominantly identifying as homemakers. Cord care knowledge clustered in the moderate category, while most newborns achieved separation within the physiologically expected timeframe. A markedly strong association was found between maternal knowledge and cord separation duration, with better-informed mothers considerably more likely to experience prompt separation. Educational attainment also showed a significant association with separation outcomes, whereas maternal age and occupational status did not. Conclusion: Postpartum maternal knowledge emerged as the dominant determinant of cord separation timing, substantially outweighing age and occupation, while educational background served as a reinforcing structural enabler. These findings highlight the strategic importance of structured, culturally responsive health education within primary care services in this district.

Abstract

Introduction: Primary dysmenorrhea is menstrual pain without reproductive organ abnormalities experienced by most adolescent girls, with a prevalence reaching 45–95% globally and 60–70% in Indonesia. Untreated pain can limit physical activity and decrease the quality of life. This study aims to analyze the effect of cinnamon aromatherapy (Cinnamon Essential Oil) on reducing primary dysmenorrhea in adolescents. Methods: This study used a pre-experimental one-group pretest-posttest design. The population consisted of 56 third-year female students at the Health Polytechnic of the Ministry of Health Malang, Campus IV Kediri, with a sample of 36 respondents selected through a simple random sampling technique. Pain intensity was measured using the Numeric Rating Scale (NRS) observation sheet before and after the inhalation of cinnamon aromatherapy for 15–20 minutes. Data were analyzed using the Wilcoxon Signed-Rank test. Results: Before the intervention, nearly half of the respondents (47.2%) experienced moderate dysmenorrhea, 36.1% mild, and 16.7% severe, with no respondents being pain-free. After the administration of cinnamon aromatherapy, 30.6% of respondents no longer felt pain, 33.3% experienced mild and moderate pain respectively, while severe pain decreased to only 2.8%. The Wilcoxon test showed a p-value = 0.00$ with Z = -4.848, indicating a significant effect. This effect is attributed to the cinnamaldehyde and eugenol content, which possess antispasmodic and anti-inflammatory properties, thereby inhibiting prostaglandin synthesis and relaxing uterine muscles. Additionally, the aroma of cinnamon stimulates the release of serotonin and endorphins through the limbic olfactory pathway, inducing a relaxing effect and reducing pain perception. Conclusion: Cinnamon aromatherapy is proven effective in reducing the intensity of primary dysmenorrhea in adolescents. Suggestions: This intervention can be recommended as a safe, inexpensive, and easy-to-apply non-pharmacological therapy. Health education institutions are expected to utilize this intervention as a complementary alternative to reduce dependence on analgesic drugs in dysmenorrhea management.

Abstract

Background: Breast milk production among mothers following cesarean section is often suboptimal due to postoperative pain, limited mobility, anxiety, and fatigue, which can inhibit the lactation process. Non-pharmacological interventions, such as lactation massage using jasmine oil, may help stimulate breast milk production by promoting relaxation and hormonal responses. Objective: To determine the difference in breast milk production before and after lactation massage using jasmine oil among post-cesarean section mothers in the working area of Sukorame Community Health Center, Kediri City. Methods: This study employed a pre-experimental design using a one-group pretest-posttest approach without a control group. A total of 36 post-cesarean section mothers were selected through simple random sampling. The intervention consisted of lactation massage with jasmine oil for 15–20 minutes, administered once daily for three consecutive days. Breast milk production was assessed using an observation checklist before and after the intervention. Data were analyzed using the Wilcoxon Signed Rank Test. Results: The analysis showed a statistically significant difference in breast milk production before and after lactation massage with jasmine oil (p-value = 0.000; p < 0.05). The intervention was associated with improved breast milk production among post-cesarean section mothers. Conclusion: Lactation massage using jasmine oil is effective in improving breast milk production among post-cesarean section mothers. The intervention enhances blood circulation, stimulates prolactin and oxytocin secretion, and provides a relaxing effect. Additionally, the aroma of jasmine oil helps reduce maternal anxiety and increase comfort, thereby optimizing the milk ejection reflex. Suggestion: Healthcare providers, particularly midwives and nurses, are encouraged to incorporate lactation massage with jasmine oil as a complementary non-pharmacological intervention to support breastfeeding among post-cesarean section mothers. Future studies should employ randomized controlled trial designs with larger sample sizes to further confirm its effectiveness.
