Abstract

Purpose: Obesity is a growing public health problem in Indonesia, with prevalence influenced by several factors such as unhealthy diet, physical inactivity, and sociodemographic factors such as urbanization and socioeconomic status. It is associated with an increased risk of non-communicable diseases, including diabetes and asthma, and is more common in urban and affluent areas compared to rural areas. Although traditional markets may increase access to healthier food options, their direct impact on reducing obesity remains unclear. This study aims to conduct a multifactorial and bivariate analysis of obesity risk factors by involving analysis of the influence of gender, age, and traditional markets. Methodology: This cross-sectional study analyzed data on Gatak Sub-district Health Center visits (May-July 2024) and the presence of traditional markets from Statistics Indonesia reports, using Pearson Chi-Square and logistic regression to test the impact of age, gender, and market availability on BMI. Results: This study analyzed 9,639 patients, with fewer than half under 50 years old, two-thirds female, and nearly 80% living in areas without traditional markets. Multivariate and bivariate analyses revealed a higher proportion of obesity among patients aged 50 and older and females (p = 0.000). However, the presence of traditional markets showed no significant impact on obesity rates. The presence of traditional markets does not have a significant influence in causing obesity. Age 50 years and above and female gender have a higher risk of obesity. Applications/Originality/Value: This research contributes to understanding the factors that influence BMI in rural areas in Indonesia.

Abstract

Abstract Introduction: Polio can lead to Post-Poliomyelitis Syndrome (PPS) due to the long-term effects of poliovirus on motor neurons. After poliomyelitis's initial recovery, patients may experience neuromuscular symptoms 15 years post-infection. Polio survivors often deal with residual muscle atrophy and joint problems, acute flaccid paralysis, postural deformities, and respiratory muscle weakness. There are various rehabilitative treatments to manage PPS, including occupational therapy, exercise, medications, and surgery that shown effectiveness in improving patients' mobility and QoL. Purpose: This study aims to identify and evaluate the effectiveness of various global treatment strategies for Post-Poliomyelitis Syndrome. Methods: This study adheres to Cochrane Collaboration's methodological guidelines for literature reviews. Five databases were utilized, guided by PICO (Population, Intervention, Control, Outcome) framework. The inclusion criteria specified original articles published in English between 2020-2024, with full-text availability. Data regarding treatment outcomes were synthesized, leading to the formulation of conclusions. Results and Discussion: The search obtained 2040 studies, and 1835 abstracts were reviewed. Following full-text assessment, 187 papers were examined, and 19 studies were included. Most of the studies had an RCT design in outpatient setting. Interventions with self-management programs, Hinge-Knee Replacement (HKR), interactive videogame exercises, Radio Electric Asymmetric Conveyer (REAC), Whole-Body Vibration (WBV), Total-Knee Arthroplasty (TKA), and Knee-Ankle-Foot Orthoses (KAFO) showed significant results (p<0.005). While L-citrulline supplementation and PVP-lavage intervention showed no significant effect with post-polio syndrome. Conclusion: This research highlights the significance of tailored healthcare management of PPS according to the patient's severity, offering a variety of approaches that can improve the patient's QoL and independence.

Abstract

Purpose: This study purpose is to discuss cigarette smoke exposure as a risk factor for tuberculosis in children. Methodology: This study is a literature review, conducted through an extensive search of international literature using the BMC, Nature, PubMed, Scopus, and JEPH databases. The inclusion criteria: 1) Published in the last 5 years; 2) Sought for retrieval; 3) Abstract recording to the theme; 4) Free full text; 5) Content of the journal match the theme. Results: The filtering processes produced 15 articles that were relevant to this study. 13 studies stated that there was a remarkable relationship between smoking and TB in children. Cigarette smoke diminishes the effector cytokine response and hampers the ability of infected human macrophages to contain mycobacteria, increasing susceptibility to TB. Applications/Originality/Value: This study can be use for a reference regarding the influence of cigarette smoke exposure to tuberculosis in children.

Abstract

Purpose: This study purpose is to determine the correlation between the number of absolute eosinophils, absolute basophils, and eosinophil-basophil ratio (EBR) with the length of stay of pediatric patients with dengue virus infection. Methodology: The study design used cross-sectional. The samples used in this study were taken from routine hematologic laboratory medical record data for the period January-October 2024, using consecutive non-probability sampling technique. Results: 49 samples were obtained and met the restriction criteria as subjects of this study. The results of the Spearman correlation bivariate test showed that the number of absolute eosinophil count, absolute basophil count, and EBR with length of stay had statistical data results respectively p = 0.139, p = 0.993, and p = 0.536. Applications/Originality/Value: This study provides a reference in laboratory results to determine the length of stay (LOS) in children with dengue virus infection.

Abstract

Purpose: To conduct an analysis of the relationship between academic stress and awareness in the pre-help-seeking behavior stage with the level of depression in students. Methodology: This study utilized an analytical observational study design with a cross-sectional approach. The technique used was purposive sampling and 50 respondents were obtained, and three types of questionnaires were used to measure academic stress, help-seeking behavior and depression. The data analysis methods used in the research are univariate, bivariate (Chi-Square test), and multivariate (logistic regression test). Results: According to the findings of the univariate analysis, the majority respondents were female (74.0%), moderate academic stress (52.0%), moderate help-seeking behavior (54.0%), and moderate depression (46.0%). Meanwhile, bivariate analysis found that academic stress had a significant relationship with depression levels (P=<0.001), while the awareness in the pre-help-seeking behavior stage was not related to depression levels (P=0.726). Applications/Originality/Value: To provide a new perspective on the relationship relationship between academic stress and awareness in the pre-help-seeking behavior stage with the level of depression in students.

Abstract

Purpose: Mitral stenosis (MS) is a common valvular heart disease that often causes heart failure (HF). The right ventricle (RV) is frequently involved in MS, which contributes to clinical outcomes in patients. NT-pro BNP (N-terminal pro-B-type natriuretic peptide) is a widely used biomarker in the diagnosis and prognosis of heart failure. However, its relationship with RV parameters in patients with HF due to MS remains insufficiently explored. This study aimed to explore the correlation between NT-pro BNP levels and right ventricular parameters in patients with mitral stenosis. Methodology: A total of 40 outpatient subjects with severe MS were included in this study. NT-pro BNP examination was performed using the ELISA method. RV parameter assessment was performed using echocardiography by assessing TAPSE (function) and RV basal diameter (dimension). Results: RV function parameters (TAPSE) and NT-pro BNP showed a significant weak correlation (r=0.324; p=0.041). RV dimension parameters and NT-pro BNP did not show a significant relationship. Patients with RV dysfunction had greater increases in NT-pro BNP levels compared to patients with normal RV function (6441.25±773.18 vs 7671.87±788.72 pg/mL; p=0.172). Applications/Originality/Value: Provides valuable insights into the pathophysiology of right heart involvement in MS and provides clinicians with potential biomarkers for early identification of RV dysfunction in MS patient population.

Abstract

Purpose: To determine the medical and management steps for building a code stroke at Rizki Amalia Medika Hospital. Methodology: his research is a descriptive qualitative study using a case report approach using observation and interview methods. Observations were carried out at in January-June 2024. The sample was obtained by purposive sampling method. Results: Landscape analysis is carried out on internal and external factors. The creation of a multidisciplinary team involves medical and nonmedical service units. The stroke protocol is carried out starting from pre-hospital, emergency unit, stroke code activation and administration of alteplase therapy for ischemic stroke. Training and education are carried out internally and externally to the hospital. Community socialization and education involve public campaigns and collaboration between elements. Improving the quality and analysis of data focuses on collecting medical data, medical audits, and feedback loops. In January to June 2024, there were 7 ischemic stroke visits with 57% receiving alteplase therapy. The success of alteplase therapy in ischemic stroke patients at Rizki Amalia Medika Hospital was 100%. Conclusion: Building a code stroke requires a comprehensive approach. By prioritizing rapid stroke identification and treatment, hospitals can significantly improve patient outcomes.

Abstract

Purpose: Toddler development is a condition of change in the form and function of organ and individual maturation. Under-five developmental disorders are still a case that requires special attention. The first 1000 days of a child's birth is the golden period for child development. Based on data Kemenkes in 2021 shows that there are 7,512.6 per 100,000 population (7.51%) of toddlers in Indonesia who experience developmental disorders. There are several factors that influence development, including nutritional status and family environmental sanitation. Nutritional status as measured by Body Mass Index (BMI) affects the development of toddlers. The relationship with nutritional status, if the nutritional status is poor then it becomes a factor that further disrupts the physical development of toddlers with optimal nutritional status. Methodology: This study used a analytic design, with a cross-sectional approach. Sampling using fixed disease sampling method from Development Pre-Screening Questionnaire and Clean and Healthy Living Behavior questionnaire data and medical records of toddlers at Gatak Health Center. Data analysis using the chi-square test with SPSS 25. The sample size used was 91 samples. Results: The results of chi-square nutritional status with the development of toddlers obtained OR = 6.618 (with p = 0.050). While the relationship between family environmental sanitation with the development of toddlers obtained OR 5.661 (with p=0.022). Conclusion: Nutritional status and environmental sanitation are significantly associated with the development of toddlers.

Abstract

Purpose: Hospital management is multidisciplinary which includes planning, organizing, coordinating, supervising, and controlling various resources to achieve hospital goals. Effective managerial characteristics can have a significant impact on the performance of health workers. This study will explore and analyze the current relevant literature on the managerial characteristics of hospitals and their relationship to the performance and satisfaction of health workers. Methodology: In this study, data was collected on Scopus through the PRISMA method. The results of this study obtained 117 appropriate and relevant documents. Results: Managerial characteristics such as managers in age, education, expertise, leadership, good communication and relationships with other workers affect the productivity and performance of health workers. Applications/Originality/Value: Good managerial characteristics, it can also create a good working environment and climate that improves the performance and satisfaction of health workers.

Abstract

Purpose: Analyzing the influence of agricultural environment on inflammation and breast cancer. Methodology: PECOS by mentioning terms in MeSH as search keywords. Inclusion criteria include: observational cohort, cross sectional or case control study design. Exclusion criteria included various forms of reviews, experimental design studies, RCTs and in animals, in-vitro studies and in-silico studies. Results: Google Scholar platform search obtained 112 articles and 177 articles from the Scopus database. After identification and screening using PRISMA-P, 6 articles were obtained. Benign and malignant breast lesions were significantly associated with exposure to environmental estrogens including polychlorinated biphenyls (PCBs) and organochlorines. There was an increase in inflammatory biomarkers, namely NF-κB, PPAR-γ, IL-12 dan TNF-α levels in breast lesions from areas with high pesticide spending. Applications: The agricultural environment is a risk factor for breast cancer mediated by inflammatory factors.
