Abstract

Introduction: Nephrotic syndrome (SN) is a glomerular disorder characterized by severe proteinuria, hypoalbuminemia, hyperlipidemia, and peripheral oedema. The annual incidence of nephrotic syndrome in adults is three per 100,000 people. Case: A 27-year-old man came to the Internal Medicine Polyclinic with complaints of swelling throughout the body. Swelling was felt since 2 weeks ago, initially swelling in the face and eyelid area then increased in weight to the whole body. The patient often takes stall medicine Paracetamol when headache is felt. The eyes were found to have palpebral swelling (+/+). Physical examination of the abdomen revealed distension, undulation test was positive and shifting dullness was positive indicating ascites. Examination of the inferior and superior extremities revealed pitting oedema on all four extremities (+/+/+/+). Examination of genitalia on inspection found the scrotum looks erythema, swollen, scrotum painful when moved, negative phren sign, negative cremaster reflex.

Abstract

Purpose: The purpose of this report is to present a case of a left anterior epistaxis in a 39-years-old female, analyze its etiology, risk factors, clinical symptoms, diagnostic examinations, and the treatment provided. This report also highlights the importance of prevention and proper management to improve patient prognosis. Methodology: The methodology used in this report is a clinical case study. Data were collected through anamnesis, physical examination, and supporting examinations such as CT scan and histopathology. The patient's management was analyzed based on existing medical theories and appropriate treatment was provided according to the patient's condition. Results: The patient was diagnosed with a left maxillary abscess, presenting with persistent pain and swelling in the left cheek for two months. CT scan findings suggested left-sided sinusitis with a possible abscess, while histopathology confirmed chronic sinusitis.The patient received antibiotic therapy (Ceftriaxone and Metronidazole), symptomatic treatment (Santagesic, Methylprednisolone, and Ranitidine), and education on oral hygiene and follow-up care.The patient had a history of left maxillary surgery and smoking habits, both of which were identified as significant risk factors for developing the condition. Applications/Originality/Value: This report provides insights into the importance of early diagnosis and proper management in maxillary abscess cases.The findings can be useful for healthcare professionals to understand clinical signs and effective treatment approaches for similar cases.The study emphasizes the need for preventive measures, such as maintaining good oral hygiene and avoiding risk factors like smoking.

Abstract

Purpose: The purpose of this study was to determine the correlation between Neutrophil to Lymphocyte Ratio (NLR) and Lymphocyte to Monocyte Ratio (LMR) with the severity of ischemic stroke. Methodology: According to the criteria, a cross-sectional sample of ischemic stroke patients at Yarsis Islamic Hospital in Surakarta (January 2023–November 2024) participated in this observational analytical study, which used the National Institutes of Health Stroke Scale (NIHSS) as a severity measure. Results: 44 subjects were obtained with an equal ratio of gender and hypertension was the most common comorbidity. A substantial relationship between the NLR with the NIHSS score (p value = 0.001) and between LMR with the NIHSS score (p value < 0.001) was discovered using bivariate analysis. NLR (OR 4.713; p = 0.044) and LMR (OR 10.780; p = 0.043) had a 42.2% (R square = 0.422) impact on the severity of ischemic stroke, according to the findings of the logistic regression test. Applications: Based on this study, there is a significant relationship between NLR and LMR with the severity of ischemic stroke. As a consequence, NLR and LMR test findings may serve as a substitute indicator of acute ischemic stroke patients' severity.

Abstract

Background: HIV/AIDS is a global health issue with widespread impact, affecting not only individuals infected but also society and healthcare systems. Efforts to combat HIV/AIDS require active involvement from the healthcare system in prevention, diagnosis, treatment, and psychosocial support. The main challenges in these efforts include social stigma, limited access to healthcare services, and the continuous increase in the number of cases. Method: This study uses a literature review approach, analyzing journals and articles published between 2019 and 2024, sourced from databases such as PubMed, SageJournals, and SpringerLink. The analysis focuses on the role of the healthcare system in addressing HIV/AIDS, the challenges faced, and effective strategies to overcome these issues. Results: The study found that the biggest challenges in HIV/AIDS management include a lack of public awareness, low early detection rates, and limited resources at healthcare facilities. Effective strategies include public education, improving access to healthcare services, and strengthening referral systems. Conclusion: The healthcare system plays a crucial role in the fight against HIV/AIDS. Despite the significant challenges, the implementation of appropriate strategies, such as education, better access to healthcare services, and combating stigma, can improve the effectiveness of HIV/AIDS control programs.

Abstract

Purpose: This literature review aims to compare accurate biopsy techniques for establishing a diagnosis of nasopharyngeal carcinoma. Methodology: The method used in this research is the systematic literature review method. Results: Several types of biopsy techniques that can be performed to establish a diagnosis of nasopharyngeal carcinoma include nasopharyngeal biopsy and neck biopsy. Nasopharyngeal biopsy is performed if the primary tumor has not spread to other areas. Neck biopsy is performed to determine whether the lump in the neck is a metastasis of nasopharyngeal carcinoma or cancer that originates from the lymph nodes. Applications/Originality/Value: Further research is needed to understand and develop better methods of early detection and diagnosis. With the right approach, the prognosis for patients with nasopharyngeal carcinoma can be significantly improved.

Abstract

Purpose: The purpose of this study is to evaluate the association between high blood osmolality and neurological status in acute ischemic stroke. Methodology: This is a cross-sectional study. Subjects of this study are acute ischemic stroke patients admitted at Dr. Sardjito Hospital. Subjects who met the inclusion and exclusion criteria were analysed mainly the blood osmolality and NIHSS score at the time of admission. Results: The study involved 58 participants, with 29 (50%) exhibiting blood osmolality exceeding 300 mOsm/kg. A bivariate analysis revealed several factors significantly linked to poor neurological condition upon admission. These included elevated blood osmolality (OR=5,296, p=0,014, 95% CI, 1,29-21,7), presence of infection (OR=11,67, p=0,007, 95% CI, 1,95-6,99), increased white blood cell count (OR=4,750, p=0,028, 95% CI, 1,21-18,5), and an ASPECT Score below 7 (OR=14, p<0,001, 95%CI, 3,34-59). Multivariate analysis revealed that high blood osmolality (p=0,031) significantly associated with poor neurological status at the time of hospital admission. Applications: This study highlighted the importance of evaluating blood osmolality in acute ischemic stroke patients as it is associated with neurological deficit.

Abstract

Purpose: To examine the relationship between socioeconomic status and nutritional condition with the prevalence of helminthiasis among students at SDN 02 Selokaton. Methodology: This research employs a quantitative observational analytic approach with a cross-sectional design. A total of 130 fifth- and sixth-grade students were selected using total sampling. The independent variables (socioeconomic status and nutritional status) were assessed through questionnaires and anthropometric measurements, respectively. The dependent variable, helminth infection incidence, was determined through microscopic stool analysis. Data were analyzed using Spearman's test. Results: Among the 65 students who met the inclusion criteria, 20% (13 students) tested positive for helminth infections. Poor nutritional status was found in 6.2% of students, while 43.1% had low nutritional status. Most students came from families with a middle socioeconomic status (78.5%). No significant correlation was found between socioeconomic status or nutritional status and the incidence of helminth infections (p > 0.05). Applications/Originality/Value: Despite the lack of a significant correlation, the high prevalence of helminth infections suggests that factors like environmental hygiene and hygienic behaviors may play a more influential role. Health education and improved sanitation are needed to reduce the prevalence of helminth infections in the area.

Abstract

Purpose: The objective of this study is to determine whether parental knowledge about worms and environmental sanitation are related to the incidence of worms (helminthiasis) among students in SDN 02 Selokaton. Methodology: This study uses an analytical observational study research method with a cross-sectional research design. Results: there is no significant relationship between parental knowledge about worms and the incidence of worm infections, there is no significant relationship between environmental sanitation and the incidence of worm infections. Applications/Originality/Value: The novelty in this study is that no data has been found on the prevalence of worms in elementary school students in Karanganyar, so this study can be an initial screening of the prevalence of worms in elementary school students in Karanganyar.

Abstract

Abstract Purpose: The purpose of the article is to evaluate and emphasize the role of primary health care (PHC) in managing hypertension. It systematically reviews the integration of various pharmacological, non-pharmacological, and technological strategies used in PHC to improve hypertension outcomes. By analyzing studies from different regions and methodologies, the article identifies effective interventions, challenges, and areas requiring improvement in hypertension management. Methodology: This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standard statement. Our search strategy used electronic databases, namely Pubmed, Scopus, and Google Schoolar. Journal searches were conducted in November 2024. Pubmed and Scopus searches used the search keywords "Primary Health Care" OR "Primary Care" AND "Hypertension" AND "Management". Results: Our search strategy identified a total of 2698 articles. After full-text reading, 19 relevant journals were found. Applications/Originality/Value: The article's originality lies in its comprehensive review of primary health care's (PHC) role in managing hypertension, integrating recent evidence (2019–2024) from diverse regions. It highlights the importance of combining medications, lifestyle changes, and technology (e.g., telemonitoring, ABPM) while addressing barriers like poor access, low adherence to guidelines, and socio-economic disparities. By focusing on practical solutions, such as better training, patient education, and programs like Indonesia's PROLANIS, the article offers valuable insights for improving hypertension management, particularly in low-resource settings, emphasizing a holistic, actionable, and globally relevant approach.

Abstract

Purpose: Identify the relation between FBG and triglyceride with GFR in T2DM patients. Methodology: This observational quantitative research used a cross-sectional design. Data were obtained from the medical records of 50 Prolanis T2DM patients in 2023 at the Puskesmas Karangnongko, Klaten Regency, using consecutive sampling technique. The bivariate analysis used was the Spearman test. Results: Spearman test results show that there is no significant relation between FBG with GFR in type 2 diabetes mellitus patients (p-value=0.695; r-value=0.057). Apart from that, there is also no relation between triglycerides with GFR in type 2 diabetes mellitus patients (p-value=0.654; r-value=-0.065). Applications/Originality/Value: This research provides information regarding considerations for FBG and triglycerides as important components for detecting a decrease in GFR which can be an indicator of diabetic nephropathy.
