Abstract

Regional Disaster Management Agency (RDMA) workers must be on standby and respond for 24 hours, especially if there are emergency. This causes demands on workers and workers become fatigue. The aim of this study is to analyze the factors that influence work fatigue in Tuban Regency’s RDMA workers. This research includes analytical and observational with a cross sectional design. The study was conducted on 56 workers with a simple random sampling system. The dependent variable studied was physical and mental fatigue, while the independent variables consisted of work factors (working hours and workload), emotional demands (responsibility), and organizational demands (work shifts). Data collection was carried out by direct measurement, questionnaires, interviews, documentation and observation then processed with Structural Equation Modeling-Partial Least Square (SEM-PLS). The factors that affect work fatigue are work factors (t statistics = 3.643 and p-values = 0.000) and organizational demands (t statistics = 3.086 and p-values = 0.002), while emotional demands have no effect (t statistics = 0.950 and p-values = 0.342). Loading factors of physical work fatigue (0.917) and mental work fatigue (0.916) are almost the same, so they have the same contribution as a measure of fatigue. Workers who experience work fatigue should conduct regular medical examinations, know the workload limit for one day, organize sufficient work time, regularly rotate work time, stretch in the middle of work activities, prepare work music and videos during breaks.

Abstract

Hyperthyroidism is an endocrine disorder characterized by excessive production of thyroid hormones, with Graves' disease being the most common cause. This condition can lead to serious complications, including thyroid crisis or storm, which is a medical emergency with a high mortality risk. This article aims to review the literature related to the definition, epidemiology, etiology, clinical manifestations, pathophysiology, diagnosis, management, complications, and prognosis of hyperthyroidism and thyroid crisis. The writing method used a narrative literature review based on national and international literature from 2014 to 2024 obtained through PubMed, ScienceDirect, Google Scholar, and Sinta-indexed national journals. The results of the review show that hyperthyroidism has a broad impact on organ systems, with cardiovascular and metabolic complications being the main burden. Thyroid crisis, although rare, remains a serious threat that requires prompt, aggressive, and multidisciplinary management. In conclusion, early detection, accurate diagnosis, and guideline-based therapy are key to improving patient prognosis and reducing mortality.

Abstract

Background: The government has implemented a program to increase the use of Long-Term Contraceptive Methods (LTCM) with a national target of 28% in 2024; however, the current achievement is only 23.5%. Batam City is one of the government’s priority areas to increase contraceptive use, but several factors, such as satisfaction levels and health service quality, are still considered low. Objective: This study aims to analyze the relationship between health services and types of contraceptive methods with the satisfaction level of family planning acceptors in Rempang Cate Sub-district, Batam City. Methods: This quantitative research used an analytical observational design with a cross-sectional approach. The population consisted of fertile couples (PUS) in Rempang Cate Village. A total of 34 respondents were selected using purposive sampling techniques. Data were collected through observation sheets and questionnaires and analyzed using the Chi-square test. Results: The findings show that among 17 acceptors with poor health services, only 1 (5.9%) was satisfied, while among those with good health services, 16 were satisfied, and 1 (5.9%) was dissatisfied. Statistical analysis using SPSS showed a p-value = 0.000 (p < 0.05), indicating a significant relationship between health services and family planning acceptor satisfaction. Conclusion: There is a significant relationship between health service quality and the satisfaction of family planning acceptors, but no significant relationship was found between the type of contraceptive method and satisfaction.

Abstract

Work fatigue is one of the factors that contribute to decreased productivity and increased risk of occupational accidents. This study aims to analyze various factors associated with work fatigue among production workers at PT. X in 2025. The research employed an analytical quantitative design with a cross-sectional approach. The sample consisted of 53 respondents selected using total sampling. Data were collected through questionnaires covering worker characteristics (age, nutritional status, medical history, smoking habits, years of service, and work stress) as well as levels of work fatigue. Data analysis was conducted using the chi-square test. The findings revealed that 56.6% of workers experienced high levels of fatigue. There were significant associations between age (p=0.023; PR=1.836), nutritional status (p=0.021; PR=1.786), medical history (p=0.025; PR=1.789), smoking habits (p=0.039; PR=1.706), years of service (p=0.015; PR=1.912), and work stress (p=0.032; PR=0.574) with work fatigue. The results indicate that workers who are at-risk age (≥35 years), have abnormal nutritional status, a history of illness, smoking habits, and longer years of service are more likely to experience work fatigue. However, high work stress showed a negative relationship, suggesting a protective effect against fatigue. In conclusion, there is a relationship between age, nutritional status, medical history, smoking habits, years of service, and work stress with work fatigue among production workers at PT. X.

Abstract

Hyperkeratotic hand dermatitis (HHD) is a chronic hand eczema subtype marked by thick hyperkeratotic plaques, painful fissures, and minimal erythema or vesiculation. It is diagnostically challenging due to overlap with palmoplantar psoriasis and keratoderma and is strongly linked to repeated irritant exposure in wet work. A 60-year-old male taro leaf farmer presented with itching and burning on the palms and backs of both hands, spreading to the forearms for 2–3 weeks. Examination revealed papules, hyperpigmented plaques, erythematous macules, irregular scaling, and about 1 cm palmar fissures on both hands. The working diagnosis was chronic hyperkeratotic hand dermatitis. Initial treatment included oral cetirizine, topical betamethasone valerate, and Vaseline gel, plus education on using long rubber gloves at work. HHD results from skin barrier dysfunction and keratinocyte hyperproliferation caused by repeated irritant exposure. In this case, exposure to taro leaves and prolonged rubber glove use likely maintained irritation and occlusion. Differential diagnoses include palmoplantar psoriasis, contact dermatitis, and tinea manuum. Management involves potent corticosteroids, antihistamines, occlusive emollients, and occupational modifications such as replacing gloves and limiting occlusion time. Identifying occupational factors in HHD is essential. Effective management requires combined topical therapy, symptom control, and strict workplace modifications, along with patient education and allergy screening if needed.

Abstract

Tele-Herbal Medicine is a technology-driven service designed to deliver herbal medicine consultations and therapies through wellness clinics, aiming to improve access to personalized and affordable care. This study systematically assesses its effectiveness, implementation, and challenges as part of integrated health services by reviewing relevant literature published in the past five years. Findings indicate that Tele-Herbal Medicine significantly enhances patient access, offering tailored and cost-efficient herbal treatments. Despite these promising outcomes, implementation faces key obstacles, including inadequate regulatory frameworks, the absence of standardized herbal product validation procedures, and concerns over patient data security. These limitations underscore the urgent need for comprehensive policies, quality assurance mechanisms, and strict data protection protocols, consistent with broader telemedicine guidelines. By addressing these issues, Tele-Herbal Medicine can evolve into a reliable and sustainable model, enabling wellness clinics to expand services without compromising safety or quality. Ultimately, its success depends on clear legal support, standardized validation systems, and robust monitoring tools to ensure effectiveness, safety, and full integration into the healthcare ecosystem, thus positioning it as an innovative solution that bridges traditional herbal practices with modern digital health services.

Abstract

Osteoarthritis is a degenerative joint disease whose exact cause is not yet fully understood. This condition is characterized by damage to the articular cartilage, resulting in pain, joint stiffness, decreased muscle strength, muscle spasms, and reduced functional activity of the patient. Several factors contribute to the development of osteoarthritis, including age, gender, race, genetic factors, smoking habits, history of trauma, type of occupation or physical activity, exercise habits, and obesity. The prevalence of osteoarthritis increases with age; among individuals aged 65–74 years, approximately 39% experience knee osteoarthritis and 23% experience hip osteoarthritis. In those over 70 years old, almost all individuals, both men and women, show symptoms of this condition. Osteoarthritis is a chronic progressive disease characterized by inflammation of the articular cartilage and synovial membrane, which causes persistent pain, joint stiffness, decreased muscle strength, and reduced mobility. Physiotherapy management for this condition aims to reduce pain, improve muscle strength, and enhance joint function. Common therapeutic modalities used include Shortwave Diathermy (SWD), Transcutaneous Electrical Nerve Stimulation (TENS), and the Hold Relax exercise technique, which helps relax muscles and increase joint range of motion

Abstract

Musculoskeletal disorders (MSDs) represent a prevalent health issue among clinical medical students, with global studies reporting incidence rates of 73% in India and 81.9% in Saudi Arabia. Pain is most frequently reported in the neck, lower back, and shoulder areas. However, similar data from Indonesia, particularly Papua, remains limited. This study aims to identify the prevalence and risk factors of MSDs among clinical medical students at the Faculty of Medicine, Cenderawasih University. The research employed a cross-sectional design with a sample of 78 clinical students selected through purposive sampling. Data collection utilized the Standardized Nordic Musculoskeletal Questionnaire (NMQ) to measure prevalence and the Nordic Body Map to localize pain. Data analysis used Chi-Square test to evaluate the relationship between risk factors (such as Body Mass Index/BMI and physical activity) and musculoskeletal complaints. Results showed the majority of respondents were female (74.4%) and aged ≥25 years (51.3%). Most respondents experienced pain at the "moderate pain" level (57.7%), with the most frequently reported pain areas being the back, waist, calves, knees, shoulders, and wrists. There was a statistically significant relationship between BMI and pain complaints (p=0.035), but no significant relationship was found between physical exercise and musculoskeletal complaints (p=0.129). This study concludes that BMI is a risk factor for MSDs in the studied population. Evidence-based recommendations are needed to reduce MSD risk and improve students' quality of life during clinical education.

Abstract

Patient satisfaction refers to the degree of emotion felt by an individual, arising from an assessment of the quality of medical care received, compared to initial expectations. Suspected dissatisfaction among visitors to the Huta Padang Community Health Center, Asahan Regency, appears to be due to the inadequacy of the primary care facilities provided at the institution. Information gathering was conducted from July to August 2023. This analytical study aimed to measure patient satisfaction with essential health care at the Regional Technical Implementation Unit (UPTD) of the Huta Padang Community Health Center, Asahan Regency. The target group of the study included all polyclinic patients visiting the Saribudolok Community Health Center. Respondents were selected using a purposive sampling technique, with a total of 64 participants. The study findings were summarized through a frequency distribution table, followed by statistical data processing via a Chi-square test at a 95% significance level (α = 0.05). The analysis revealed a relationship between primary care quality and patient satisfaction, indicated by a p-value of 0.018. Therefore, it is recommended for Community Health Center leaders to actively improve the standards of assistance provided, for example by providing encouragement or incentives for staff to carry out their duties optimally, so that they can realize patient anticipation.
