Abstract

Background: Methods used to diagnose and monitor chronic hepatitis B (CHB) by quantitation of hepatitis B virus-deoxyribonucleic acid (HBV-DNA) levels is expensive. Cheaper laboratory test as an additional markeris needed, thus we studied serum quantitative HBsAg to be used as surrogate marker in CHB patients. This study was aimed to investigate correlation between serum quantitative HBsAg and HBV-DNA in CHB patients.Method: In this cross-sectional study, we enrolled 62 CHB patients between January 2010 and December 2012 who had quantitative HBsAg and HBV-DNA assays in a private laboratory at Denpasar. HBV-DNA was measured by real-time polymerase chain reaction and quantitative serum HBsAg was measured by chemiluminescent microparticle immunoassay (CMIA). Stastistical analysis was performed by Mann-Whitney and Spearman’s correlation.Results: Of 62 patients, most subjects were males (82.26%). Mean HBsAg titer of CHB in HBeAg positive and negative patients were 281,000 and 4,900 IU/mL, respectively; while mean HBV-DNA in HBeAg positiveand negative patients were 59,000,000 and 7,530,000 IU/mL, respectively. We found that quantitative HBsAg and HBV-DNA in HBeAg positive and HBeAg negative patients were statistically signi cant (p = 0.0001, p = 0.0001, respectively). Signi cant correlation was found between serum quantitative HBsAg and HBV-DNA (r = 0.737; p= 0.000). Quantitative HBsAg was signi cantly correlated with HBV-DNA in HBeAg-positive subgroup (r = 0.717; p = 0.0001); and signi cant correlation was also found in HBeAg-negative subgroup (r = 0.443; p = 0.006) although the correlation was weak. Abstract

Lemmel’s syndrome, also known as duodenal diverticulum obstructive jaundice,  is a rare cause of benign obstructive jaundice that should be included in the differential diagnosis of biliary obstruction when PAD is present, in the absence of cholelithiasis or other detectable obstacle. Diagnosing Lemmel’s syndrome could be challenging, but being aware of this condition is important to avoid mismanagement and it begins with identification of PAD, while interpreting any bile duct imaging. It can be misinterpreted as periampullary tumors, biliary stones, or pancreatic pseudocyst. Symptomatic patients can be successfully managed endoscopically in many cases but surgical management would be necessary in selected cases.We present a patient with benign obstructive jaundice caused by Lemmel’s syndrome who was successfully treated with endoscopic sphicterectomy. A 67 years old female presented to the emergency department with chief complaint of jaundice. The patient was assesed to have obstructive jaundice cause by a duodenal mass, elevation of transaminase enzime supected caused by drug induced liver injury, hypertension (controlled), and anterior extensive coronary ischemia. Endoscopic retrograde cholangiopancreatografi (ERCP) showing mutiple giant diverticle in second part of duodenum, stenosis of the distal CBD with compression of diverticular extra luminal as a differential diagnosis. Endoscopic ultrasound (EUS) was performed to exlude a periampullary tumor, resulting distal CBD stenosis due to compression of multiple periampullary diverticula (PAD). We performed an endoscopic sphinterectomy (EST) and the stent was removed. A further evaluation of the tuberculous lymphadenitis was planned as outpatient setting. One month follow-up, no recurence of jaundice was observed.

Abstract

Background: Colitis is a gastrointestinal disease that poses a major problem in Indonesia. Zinc (Zn) is an important trace element which plays role as antimicrobial in intestinal mucosa, increases gastrointestinal barrier, and immune function. Colitis can cause histologic changes in gastrointestinal tract epithelial which will disrupt the absorption and excretion of Zn causing zinc deficiency. This study aims to evaluate the difference of Zn level in colitis and healthy subjects, as well as the fecal analysis profile of colitis patients in Cipto Mangunkusumo General Hospital. Method: The design of this study was cross-sectional and was performed in colitis and healthy subjects. Colitis patients were recruited consecutively in patients in Gastroenterology Clinic and Gastrointestinal Endoscopy Centre in Cipto Mangunkusumo General Hospital. Zinc examination from blood sample was performed using spectrophotometry. Fecal analysis was performed manually. The level of zinc from colitis subjects was compared to healthy subjects. Fecal analysis examination consists of macroscopic, microscopic, and chemical examinations of the feces.Results: There were 40 colitis subjects and 16 healthy subjects in August 2019 - May 2020. There were significant differences in Zn levels between colitis and healthy subjects, which were 10.9 ± 1.9 mol/L and 12.3 ± 1 mol/L, respectively (p = 0.011). There were 45% (18/45) colitis subjects who were also suffering from Zn deficiency. The stool analysis in colitis subjects showed soft stool consistency in 90% subjects, slimy stools in 17.5%, increased in erythrocyte count in 60%, increased in leukocyte count in 5%, positive stool starch in 20%, positive stool fat in 7.5%, acid pH in 97.5%, positive stool occult blood in 37.5%, and fungi in stool in 7.5%.

Abstract

Adenoid cystic carcinoma is an extremely rare case of the esophagus. We present a female patient, aged 76 years who present with dysphagia and weight loss for the last three month. On endoscopy there was a luminal narrowing in the middle third of the esophagus. Diagnosis was challenging due to the stenosis and the tumor size.   Histopathological   confirmation   was   obtained   by  subcarinal   fine-needle aspiration biopsy. This type of cancer is very aggressive with short survival time. Further studies are needed to define optimal treatment. Keywords: dysphagia, adenoid cystic carcinoma

Abstract

Pregnancy is an important risk factor for growth of choledochal stones. Since choledocholithiasis encountered during pregnancy, which is also a possible cause of pancreatitis and cholangitis, may be the reason for serious morbidity and mortality both for the mother and the fetus, it should be treated. In this article, the results and reliability of endoscopic retrograde cholangiopancreatography (ERCP) application on a pregnant woman accompanied with percutaneous biliary procedures are presented.            We report a case of 33-year-old woman at 19th week of gestation with cholestatic jaundice due to a common bile duct (CBD) stone managed by endoscopic retrograde cholangiopancreatography (ERCP). The patient had post ERCP pancreatitis which resolved with medical management. Percutaneous cholecystostomy was also performed to control source of infection in the gallbladder.            ERCP is the first procedure that will be preferred in the treatment of choledocholithiasis in pregnancy with the right indications provided that proper precautions have been taken. Possible harmful effects of ionized radiation on fetus during fluoroscopy should be minimalized by giving in short periods and low doses.Keywords:

Abstract

Background: Gastroesophageal reflux disease is a disease with heterogen symptoms, with no gold standard available to diagnose this condition. Questionnaire is a diagnostic tool that may enable an objective symptom assessment. GERD-Q questionnaire consist of 6 components of questions, formed from 4 various components of reflux symptoms (positive predictors of GERD) and 2 as negative predictors of GERD. This study aimed to observe the relationship between GERD-Q score and reflux oesophagitis in GERD suspected patients.Method: This study was an analytic cross-sectional study, conducted in March to July 2019. Study participants were patients visiting the Endoscopy Unit in Hasan Sadikin Hospital, Cibabat District General Hospital, and Al Islam General Hospital, who were then evaluated using GERD-Q and later underwent endoscopy examination. The severity of reflux oesophagitis observed through endoscopy was evaluated using the Los Angeles classification. Endoscopy readings was performed by one experienced doctor.Results: Thirty-six patients were examined; 17 (47.2%) patients suffered from reflux oesophagitis, including 13 cases with grade A (76.5%) and 4 cases with grade B (23.5%). There were no cases with grade C or grade D was found. Patients who suffered from reflux oesophagitis with GERD-Q score ≥ 8 were 15 patients (65.2%) and GERD-Q score 8 were 2 patients (15.4%). Chi Square test analysis with confidence interval of 95% revealed p = 0.004 (p 0.05). Biserial point correlation coefficient was 0.643 with p 0.001.Conclusion: There was a strong relationship between GERD-Q score and reflux oesophagitis. The higher the GERD-Q score, the higher the possibility to suffer from reflux oesophagitis.

Abstract

Based on its relation to the liver sinusoid, increased pressure of portal vein can occur at three levels: presinusoid, sinusoid ,and postsinusoid. Obstruction of the presinusoid veins can be caused by extra- hepatic condition such as venous thrombosis. We reported a case of portal hypertension with esophageal varices bleeding was a result of obstruction due to thrombosis of the splenic vein and portal vein under hypercoagulant conditions due to thrombocytosis. The management of esophageal varices was sclerotherapy while for overcome the thrombosis the patient was given hydroxy urea.    Keywords: esophageal varices bleeding, portal hypertension, vein obstruction

Abstract

During the last two decades, non-alcoholic fatty liver disease (NAFLD) has been a topic in many discussions. The major risk factors for NAFLD is metabolic syndrome, which include obesity, insulin resistance andhypertension. Beside insulin resistance, oxidative stress has been linked with the disease. There is accumulating evidence that intestinal bacterial overgrowth plays an important role in NAFLD pathogenesis. Intestinal bacteria influence the progression of NAFLD through endogenous ethanol production and cytokine that would eventually induce hepatic oxidative stress. Probiotic intervene pathogenic intestinal flora so it is a potential treatment for NAFLD. Many animal studies documented the beneficial effect of probiotic in NAFLD. Probiotic reduce hepatic inflammation, reduce hepatic steatosis and improve insulin resistance. There is still limited human studies upon this topic. However, preliminary result showed potential role of probiotic in NAFLD treatment. Probiotic is safe, cheap and widely available therefore it is a promising new approach for NAFLD therapy. Upcoming study would hopefully provide firm foundation regarding the use of probiotic for NAFLD on human.Keywords: NAFLD, probiotic, metabolic syndrome

Abstract

Cytochromes P450 (CYP450) is a super-family of multigenes bound to heme and also a catalysator enzyme. Cytochrome P450s (1, 2, and 3) are the most important enzymes for biotransformation of drugs administered through gastrointestinal tract. The gastrointestinal tract is the first part of immune system against all of oral xenobiotics. Drug interaction may be predicted but it is hardly prevented. Thus, it frequently becomes clinical problem. CYP450 polymorphism may influence effective drug metabolism, which consequently will affect drug response and good therapeutic effect. Poor metabolizers need only a small dose of drug to bring on drug response but extensive or ultra-rapid metabolizers will need a large dose of drug. The unexpressed CYP2E1, one of the CYP families, may influence cancer incidence. Abstract

Background: In colorectal carcinoma (CRC), myofibroblast are the main component cells in tumour stroma which have an important role in the metastases process. The precentage between carcinoma and desmoplastic stroma known as carcinoma percentage (CP), can be used as an independent predictor metastases. D2-40/Podoplanin (PDPN) known as a spesific marker for lymphatic endothelial cell (LEC), which used to assess lymphatic microvessel density (LMVD) and lymphatic vessel invasion (LVI). This study aims to determine correlation and association between CP, LMVD and LVI with the metastases process to lymph node (LN).Method: CP assessment conducted on 44 samples of adenocarcinoma not otherwise specified (NOS) colorectal were divided into 22 cases with CP-High and 22 cases with CP-Low and examination D2-40/Podoplanin to assess LMVD and LVI. The statistical test is performed to find the correlation between CP and LMVD, as well as the relationship between CP, LVI and metastasis KGB.Results: There were a strong correlation between CP and LMVD intratumoral and peritumoral area with the negative correlation. There were a significant association (p=0,00) between LMVD (intratumoral and peritumoral area) with the LVI. There was a significant association between LVI and lymph node metastases (p = 0,03). Intratumoral area showed significant association with lymph node metastases (p = 0,04), whereas peritumoral area showed no significant association (p = 0,17).Conclusion: CP examination in histopathology specimen can be used to predict high/low rate of tumour cells metastases to the lymph node, based on a strong correlation between CP and LMVD.
