Abstract

Background and Objectives: Elderly patients with hip fractures often experience severe pain and anxiety, which can impede recovery. Non-pharmacological interventions such as Benson's relaxation and lavender aromatherapy have shown potential in managing these symptoms, but evidence of their effectiveness in geriatric orthopedic settings remains limited. This case study aimed to evaluate the effects of a combination of both therapies on pain, anxiety, mobility, and vital signs in a geriatric patient following hip fracture surgery. Methods: This study used a single-subject case study design. The sample was a 72-year-old female diagnosed with a closed fracture of the right femoral neck post-hip replacement surgery. The combined intervention of Benson's relaxation (twice daily, 15 minutes) and lavender aromatherapy inhalation (twice daily, 30 minutes) was administered for four days alongside standard care. Measured variables included pain intensity (numerical rating scale 0-10), anxiety level (Hamilton Anxiety Rating Scale / HARS), lower extremity joint range of motion (ROM scale 0-5), and vital signs (blood pressure, pulse, respiration). Data collection instruments comprised observation sheets, the HARS questionnaire, and a numerical pain scale. Main Results: After four days of intervention, significant reductions were observed in pain intensity (from 5/10 to 2/10) and HARS anxiety scores (from 30 [severe] to 9 [mild]). Patient mobility improved, as indicated by an increase in lower extremity ROM from 3/5 to 4/5. Vital signs (blood pressure, pulse, respiration) showed a stabilizing trend within the normal range. No adverse effects or undesirable events were reported during the intervention period. Conclusion: Based on the findings, the combination of Benson's relaxation and lavender aromatherapy was effective in reducing pain and anxiety, improving mobility, and stabilizing vital signs in a geriatric patient post-hip fracture surgery. This combined therapy is safe and has the potential to be integrated as a complementary, evidence-based perioperative management strategy for the geriatric population in orthopedics. Further studies with more robust designs and larger samples are needed to generalize these findings.

Abstract

Indonesia is currently entering a phase of population aging, with an increasing proportion of elderly people. Elderly individuals are more susceptible to psychosocial issues such as stress, loneliness, low self-esteem, and anxiety. This condition is exacerbated by a lack of physical activity, making them susceptible to health problems such as joint pain, degenerative diseases, and cognitive decline. These various issues increase the risk of depression in the elderly. Gardening occupational therapy is a non-pharmacological therapy that can be applied to elderly people with depression. The purpose of this study was to determine the effect of gardening occupational therapy on reducing depression in the elderly. The study design was a quasi-experimental study with a pre-posttest without a control group. A sample of 20 elderly individuals was selected using a purposive sampling technique. The inclusion criteria were: no cognitive impairment, able to understand simple instructions, depression, no serious illness, and no severe physical mobility impairment. The study was conducted over a one-month period in Singonegaran Village, Kediri. The research instrument used was the Geriatric Depression Scale-15 (GDS-15). The results showed that gardening occupational therapy significantly reduced depression in the elderly, as indicated by a change in scores from an average of 9 to 4.3, with a p-value of 0.003. Gardening therapy, as an occupational intervention, provides simultaneous physical, social, and cognitive stimulation that plays a role in reducing depressive symptoms by increasing daily activity, social interaction, and a sense of personal accomplishment as participants engage in caring for and witnessing plant growth.

Abstract

Clubfoot, or congenital talipes equinovarus (CTEV), strikes about 1–2 in 1,000 newborns, featuring forefoot adduction, hindfoot varus, equinus, and cavus deformities that, if untreated, cause lasting disability. While the Ponseti method remains the gold standard, tough resistant cases call for surgery like posteromedial release (PMR) or complete subtalar release (CSTR) yet no clear winner emerges for long-term results or recurrence rates. This study compares their outcomes using recent evidence. This PRISMA-guided systematic review compared PMR and CSTR for CTEV treatment, analyzing peer-reviewed clinical studies from 2005–2025 on recurrence, complications, functional outcomes, and operative parameters; four studies met inclusion criteria. The search across PubMed, Google Scholar, and ScienceDirect used Boolean operators—(“CTEV” OR “Congenital talipes equinus varus”) AND (“Posteromedial release” OR “modified posteromedial release”) AND (“Complete subtalar release” OR “modified complete subtalar release”) including full-text English clinical reports/series on resistant clubfoot while excluding reviews, abstracts, non-English/out-of-timeframe works, duplicates, animal studies, and similar materials. PROSPERO (Registration ID: CRD420251159365). The four studies (short- and long-term follow-up) showed both PMR and CSTR effectively correct CTEV, with no significant differences in clinical outcomes, recurrence rates, radiological improvements, or functional results. Patient age, postoperative care, and rehabilitation influenced long-term outcomes. Complications included CSTR overcorrection and PMR scar issues, emphasizing careful surgical planning. This review suggests that both PMR and CSTR are effective treatments for resistant clubfoot, with no clear advantage of one over the other. Future studies should focus on larger sample sizes, standardized outcome measures, and longer follow-up to determine the most effective surgical approach for CTEV.

Abstract

Bone is a dynamic tissue that undergoes constant remodeling to maintain structural strength, mineral homeostasis, and hematopoietic function. Disorders like complex fractures, osteoporosis, and bone defects resulting from trauma or surgery pose significant clinical challenges. Traditional treatments such as autografts, allografts, and synthetic biomaterials have limitations in terms of availability, efficacy, and potential complications. This article aims to provide a comprehensive overview of the intricate relationship between bone microscopic anatomy, stem cell biology, and biomedical engineering technologies, while also outlining future opportunities and challenges in developing more effective, safe, and sustainable bone regenerative therapies. This article was prepared through an integrative literature review of scientific publications from the 2020–2025 period to analyze the interactions between the bone microenvironment, stem cell differentiation mechanisms, and the development of biomaterial and bioprinting technologies in the context of bone regeneration.  Recent advancements in cell biology and our understanding of microscopic anatomy have paved the way for stem cell-based regenerative therapies. Mesenchymal stem cells (MSCs) exhibit robust osteogenic capabilities through molecular regulation involving factors like Runx2, Osterix, BMP, and Wnt/β-catenin. Induced pluripotent stem cells (iPSCs) hold promise for personalized therapy, although safety concerns remain. The success of bone regeneration is heavily influenced by the bone microenvironment, including the vascular niche, extracellular matrix, and growth factors such as BMP-2, VEGF, PDGF, and TGF-β. Supporting technologies like biomaterial scaffolds, growth factor delivery systems, exosomes, and 3D bioprinting further enhance the potential for translating these therapies into clinical applications.

Abstract

Soft tissue sarcoma is a rare malignant tumor that can arise in various parts of the body, including the back. One of its subtypes, myxofibrosarcoma, is known for its relatively slow growth but high risk of local recurrence, making early diagnosis and appropriate management crucial. This case report aims to describe the diagnostic process, surgical management, and functional as well as aesthetic outcomes in a patient with back myxofibrosarcoma. This study used a descriptive case report design involving a 63-year-old male patient who presented with a back mass that had been developing for approximately 20 years. The observed variables included the clinical characteristics of the mass, anatomical pathology findings, type of surgical intervention, and postoperative condition. The assessment tools consisted of clinical physical examination, histopathological evaluation of tumor tissue, and observation of functional outcomes and the patient’s ability to perform daily activities after surgery. The examination results revealed a large soft tissue mass with dilated blood vessels on the back. Anatomical pathology confirmed a malignant mesenchymal tumor suggestive of myxofibrosarcoma. The patient underwent wide excision followed by flap reconstruction. Postoperatively, the patient’s general condition was stable without significant complications, the surgical wound improved, and the patient was able to return to normal activities as a construction worker after routine follow-up and care. This case demonstrates that soft tissue sarcoma of the back, although progressive in appearance and often concerning, can yield good clinical outcomes when managed with adequate surgical intervention according to indications. Patient education regarding the potential success of therapy plays an important role in improving treatment adherence, trust in medical procedures, and optimization of postoperative functional and aesthetic results.

Abstract

Acute pain is a common issue in post-appendectomy patients, potentially delaying recovery, increasing anxiety, and hindering early mobilization. Benson relaxation technique, as a non-pharmacological intervention, is believed to reduce pain, but its effectiveness in appendectomy patients requires further empirical evidence. This study aims to analyze the effectiveness of the Benson relaxation technique in reducing acute pain intensity in post-appendectomy patients. The research design used is a case study with a complete nursing care approach (assessment, diagnosis, intervention, implementation, evaluation). One post-appendectomy patient in Ward 10 of Mandaya Royal Puri Hospital received the Benson relaxation technique intervention for three consecutive days, in addition to standard pharmacological therapy. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Results showed a reduction in pain scale from 7 (severe) on the first day to 5 (moderate) on the second day, and finally to 0 (no pain) on the third day. The Benson relaxation technique proved effective as a supportive intervention in reducing acute pain, improving comfort, and reducing anxiety and mobility limitations in post-appendectomy patients. This intervention is recommended for routine implementation in post-abdominal surgery nursing care.

Abstract

Background Over time, one thing that is inevitable is the rapid advancement of science and technology, making it accessible to people of all ages around the world. Besides the impact of technological advancements on its users, the use of technology has provided significant benefits to various age groups, especially adults, and even children. This technology includes computers, mobile phones, laptops, and many other types of devices. Gadgets are the most well-known technology among the general public; a gadget is a set of electronic devices used for communication, documentation, social networking, and entertainment. Based on the issues above, the question posed in this study is, "Is there a relationship between gadget use in children and the social-emotional development of early childhood (ages 3-5)?" This study uses a quantitative research method with a Probability Sampling technique. The population in this study consists of children aged 4-5 years at TK Dharma Wanita 1, TK Dharma Wanita 2, and TK Dharma Wanita 3, totaling 127 children. The research results obtained a significance value of p = 0.000 < 0.05. Therefore, this indicates that there is an effect of the duration of gadget use on the social-emotional development of early childhood children. The r value = -0.663 indicates that the two variables have a strong negative relationship between the duration of gadget use and the social-emotional development of early childhood children, where long gadget use leads to low social-emotional development in young children. Based on the research results, the relationship between the duration of gadget use and the social-emotional development of early childhood children can be concluded as follows: The research results show that most respondents use gadgets for more than 1 hour a day. The research results also show that most respondents have low social-emotional development.

Abstract

Good vision is essential for students to be able to perform academically and achieve their educational goals. This paper is focused on how visual clarity affects learning improvement among ninth-grade students (14-16 years old) at an Islamic boarding school. The research focused on a quasi-experimental methodology involving a single group with a pre-test and post-test paradigm. Measuring learning achievement involves assessing standard academic test score differences pre-and post-intervention. These differences were then categorized into distinct ordinal classifications. Participants’ vision acuity was assessed using a Snellen chart and categorized into three different levels: good, moderate, and poor. Descriptive statistics were employed to analyze the data. From the study, it was concluded that visual acuity and learning achievement were unrelated (χ² = 6.34; df = 4; p = 0.1751). Descriptively, students with good visual acuity actually showed higher learning improvement compared to students with poor vision. Although the result is not statistically significant, the result shows that good vision should be in students for them to perform academically. For students to perform academically, it means supporting students with available resources. Affordable resources include routine vision test evaluation and the provision of glasses if needed.

Abstract

The increasing prevalence of Type 2 Diabetes Mellitus (T2DM) among young adults is closely associated with lifestyle changes and sleep disturbances. Poor sleep patterns contribute to impaired glucose regulation and an elevated risk of diabetes. This study aimed to examine the effectiveness of sleep hygiene management interventions in reducing the risk of T2DM among young adults. A quasi-experimental study employing a nonequivalent control group design was conducted involving 60 participants, divided into an experimental group (n = 30) and a control group (n = 30). The experimental group received an eight-week sleep hygiene intervention, while the control group maintained their usual routines. The variables measured included fasting blood glucose levels and sleep quality assessed using the Pittsburgh Sleep Quality Index (PSQI). Data were analyzed using the independent samples t-test. The results demonstrated that the experimental group experienced a significant reduction in fasting blood glucose levels by 15.42 mg/dL and an improvement in sleep quality by 6.28 points, with statistical significance at p < 0.001. In contrast, the control group showed no significant changes.  In conclusion, sleep hygiene management interventions are effective non-pharmacological strategies for reducing the risk of Type 2 Diabetes Mellitus among young adults.
