Abstract

This study investigates the legal consequences of medical negligence in Indonesia, focusing on Supreme Court Decision No. 590 K/Pid/2012 involving Dr. Wida Parama Astiti. The case centers on the death of a pediatric patient following the unsupervised administration of potassium chloride (KCl), a high-alert medication, in violation of national safety protocols. The research adopts a normative juridical approach, examining relevant statutory frameworks including Article 51 of Law No. 29 of 2004 concerning Medical Practice, Minister of Health Regulation No. 11 of 2017 on Patient Safety, and Articles 359 and 361 of the Indonesian Criminal Code. The analysis explores the application of the legal concept of culpa (negligence), particularly culpa lata (gross negligence), to determine the criminal liability of the defendant. Findings show that the failure to follow mandated medical protocols and provide direct supervision constituted a breach of professional duty, justifying the court’s custodial sentence. The study also highlights the role of systemic shortcomings in patient safety enforcement within hospital settings. This case serves as a critical reference for understanding how Indonesian law addresses gross medical negligence, setting a precedent for future litigation involving healthcare professionals. The research contributes to legal scholarship by clarifying the boundaries of professional accountability and emphasizing the need for institutional safeguards. It also provides practical recommendations for enhancing regulatory compliance and protecting patient rights through stricter enforcement of medical standards.

Abstract

Employee performance is a key indicator of organizational success, reflecting the ability of individuals or groups to meet established standards, responsibilities, legal norms, and ethical principles. At Citra Medika Clinic, a noticeable decline in employee performance has been observed, raising concerns regarding internal factors affecting productivity. Preliminary assessments suggest that low motivation, poor work discipline, and an unconducive work environment are major contributors to this decline. This study aims to empirically assess the influence of motivation, work discipline, and the work environment on employee performance at Citra Medika Clinic. A quantitative research method with a cross-sectional design was employed. The sample comprised 30 respondents selected from the clinic’s employees. Data were analyzed using univariate, bivariate (chi-square test), and multivariate (logistic regression) statistical techniques at a 95% confidence level (α = 0.05). The findings demonstrate that work motivation (p = 0.007; Exp(β) = 0.567), work environment (p = 0.003; Exp(β) = 0.706), and work discipline (p = 0.001; Exp(β) = 0.325) significantly influence employee performance. Collectively, these factors show a positive and simultaneous effect, with the work environment emerging as the most dominant influence. Based on these findings, it is recommended that management enhance employee motivation and discipline by implementing reward systems and recognizing achievements. Additionally, providing training and professional development aligned with job responsibilities can significantly improve employee competence and performance. These strategies are expected to foster a more productive and sustainable work environment at Citra Medika Clinic.

Abstract

Open Globe Injury (OGI) is common and can happen to anyone. Delayed treatment in children can lead to permanent blindness and reduce the child's quality of life. In this case, we inform about what was found and how it was treated. Case report: A 10-year-old girl complained of blurred vision, pain, and discharge from the right eye after being stabbed by a knife. The incident occurred 1 hour before the child was brought to the emergency room. Visual acuity in the right and left eyes was 1/~ with good light perception and 6/6, respectively. Soft palpation of the right eye during intraocular pressure (IOP) examination indicated decreased IOP. The results of the anterior segment examination of the right eye showed hyperemic conjunctiva, superior scleral laceration near the limbus, corneal laceration, shallow anterior chamber, brown iris with crypts, mid-dilated pupil, difficult to evaluate light reflex, and absent lens. The posterior segment of the right eye was difficult to evaluate. The Pediatric Penetrating Ocular Trauma Score (POTS) was 55. Initial management was performed in the ER and immediate cornea-sclera suturing surgery was performed, as well as iris claw lens placement was planned. The prognosis for the right eye was questionable. The outcome of OGI treatment varies from patient to patient. Appropriate and prompt treatment can prevent complications and improve the patient's visual outcome.

Abstract

Hypertension and hypercholesterolemia are two common comorbidities in the elderly and contribute greatly to the increased risk of cardiovascular disease. The two are often found together, but local data on the direct relationship between the two in the elderly population is limited. The purpose of this study is to find out whether there is a relationship between total cholesterol levels and blood pressure in the elderly. This study uses an observational analytical design with a cross-sectional approach. The subjects of the study were 16 elderly members of the posyandu in Balecatur, Sleman, who met the inclusion criteria. Blood pressure was measured using a sphygmomanometer, and total cholesterol levels were measured using a point-of-care device. Data were analyzed using the chi-square test to determine the relationship between cholesterol levels and hypertension. Of the total 16 respondents, as many as 13 people (81.25%) had blood pressure in the hypertension category, and 13 people (81.25%) had total cholesterol levels ≥200 mg/dL. The results of the statistical test showed that there was a significant relationship between cholesterol levels and blood pressure, with a value of p = 0.018 (p < 0.05). From this study, it was found that there was a significant relationship between cholesterol levels and blood pressure in the elderly. These findings confirm the importance of integrated screening for dyslipidemia and hypertension in promotive and preventive programs in the elderly population.

Abstract

Poor metabolic effects on blood pressure, cholesterol, triglycerides, and insulin resistance are caused by being overweight or obese. The study's objective was to evaluate how rice bran oil affected the central obese patient's changes in triglycerides and cholesterol. The study was a quasi-experiment with a control group and a non-randomized pre- and post-test.  Purposive sampling was used to choose the sample, which included 46 individuals split into two groups.  For 15 days, the treatment group received 25 milliliters of rice bran oil each day, while the control group did not receive any.  Triglyceride and cholesterol levels were assessed both before and after the intervention.  Using the SPSS software, the data were examined by comparing the lipid profile results before and after a particular intervention using the Wilcoxon paired t test and comparing the treatment between groups using the Man Whithney independent t test. The study's findings show that the intervention group's cholesterol levels significantly changed, with an average drop of 13.39 mg/dl and an average increase of 8.39 mg/dl in the control group. The difference between the two groups is not statistically significant (p=0.144).  This indicates that the intervention group and the control group are identical. There is an insignificant difference between the intervention group and control group (p=0.001), with the intervention group experiencing a considerable drop in triglyceride levels (mean decrease of 70.95 mg/dl) and the control group experiencing an increase of 10.73 mg/dl.  This indicates that the intervention group and the control group are different.
