Abstract

Chronic Pulmonary Aspergillosis (CPA) is a progressive respiratory syndrome of severe fungal infection typically found in immunocompetent or slightly immunocompromised patients with an underlying pulmonary disease. The underlying pulmonary disease typically includes active or post-tuberculosis, chronic obstructive pulmonary disease, or a history of surgery for lung cancer, for which tuberculosis remains the most common underlying condition. A 60 years old woman came with worsening dyspnea for the last 1 week, with hemoptysis and weight loss for the last 3 months, she has a history of pulmonary tuberculosis for 39 years, which was fully treated and she had completed the treatment regimen. A computed tomography (CT) scan was conducted, which showed solid fibroxanthoma with an air bronchogram in the upper left lung area. Transthoracic lung biopsy showed hyphae and fungal spores of Aspergillosis, which was supported by a positive Galactomannan test. Eventually, she was diagnosed with pulmonary aspergillosis and was advised to undergo surgical treatment with left pneumonectomy, but the patient refused surgical treatment. Chronic pulmonary aspergillosis is a progressive respiratory condition of severe fungal infection and is mostly found in post-tuberculosis patients. The symptoms may be similar to other chronic pulmonary diseases, making the diagnosis difficult. There are several reports that said pulmonary aspergillosis was misdiagnosed and only diagnosed only on autopsy. The mortality rate of this condition is quite high, and it typically worsens the quality of life.

Abstract

Tuberculosis is the second leading cause of death from infectious diseases in the world and Indonesia ranks third in the country with the highest burden of TB that can affect the success of treatment is the role of the Drug Swallowing Supervisor (PMO) in helping pulmonary TB patients to recover. Pulmonary TB treatment takes time which is very long, to ensure regularity of treatment required a PMO who will help sufferers during the treatment phase. World Health Recommendations Organization (WHO) in the Directly Observed Treatment Shortcourse (DOTS) strategy for TB control since 1995 is the involvement of PMO. PMO's Role It is very important for adherence and regularity to take medication to achieve healing, prevent transmission and avoid cases of drug resistance. The purpose of this research is to determine the relationship between the role of PMO on the success of treatment of pulmonary TB patients in Indonesia Kupang City. This research method is a research with observational method analytic and cross sectional approach. The population in the study includes all patients Pulmonary TB who has completed treatment in Kupang City. The sample is 79 sample, taken using probability sampling technique. The results of the Chi Square test results obtained p-value: 0.000 (p 0.05). The nutritiona status of children becomes one of the progress of a nation that is influenced and determined by the level of heath. Breast milk companion food is a food or drink containing nutrients, given to infants or children aged > 6-24 months to meet nutritiona needs other than breast milk. Meanwhile, this can be a hoard of triggers from heath problems for babies caused by the provision of complementary food breast milk too early. Babies become more easily sick, ranging from cough, runny nose, fever, constipation, or diarrhea. This study was conducted in Posyandu Anjelir Desa Sei Semayang in April 2022 and the sample taken was a mother who had a baby aged 0-6 months as many as 31 samples. The design of the research carried out is cross sectiona anaitic with the type of research is quantitative. Data capture using questionnaires, research, univariate and bivariate data anaysis. The results of research conducted with the Chi-Square statistica test found a significant relationship between the level of knowledge and influencing factors (education, employment, parity, and family support) in the provision of breast milk companion food too early with the value obtained p-value (< 0.05). Advice that can be shared for heath workers working in Posyandu is that it can be expected to continue to provide guidance and insight into Exclusive Breast Milk and MP-ASI in a timely manner. With this, the mother and family do not get the wrong information for the heath of the mother and the baby.

Abstract

Community empowerment is the process of giving individuals the capacity and autonomy to realize their inherent potential. The purpose of this research is to investigate community empowerment tactics based on community activities and to identify variables that limit community empowerment. This form of research is qualitatively descriptive and employs a case study design. Through in-depth interviews, observation, and recording, qualitative data was gathered and evaluated. The findings indicate that several strategies have been implemented in community empowerment, including increasing public awareness of the significance of groups and communities and addressing several obstacles that impede community empowerment, such as a lack of community participation, a lack of knowledge and capital, a lack of assistance and counseling, and the absence of the village government. This research suggests numerous alternate activities for enhancing the application of community empowerment and group members' knowledge and abilities. During the execution of this plan, numerous impediments to community empowerment were identified, notably a lack of community engagement, a lack of expertise and capital, a lack of support and counseling, and a lack of village government involvement. And highlighted that, in essence, village empowerment has a strategic component to explore the potential and capability of development and to offer the creation, execution, monitoring, and evaluation of the topic of local activities. This demonstrates the function and well-being of the village community itself.

Abstract

Poor oral hygiene might lead to Candida albicans infection. Since long term used of synthetics antifungal therapy could lead to fungal resistance, an alternative of antifungal therapy was needed. It has been reported that Gambier (Uncaria gambir ROXB) contains catechins and tannins which have antifungal effects invitro, but research using Gambier extract as a toothpaste for daily oral hygiene maintenance has not established. This study was done to determine the antifungal effect and the synergy of the combination of 10% gambier and fluorides in toothpaste preparation against Candida albicans. A true experimental laboratory study with a post-test-only design group was established. Three groups namely 10% gambier extract toothpaste, the combination of 10% gambier extract and 0.0048% fluorides toothpaste, lastly the 0.0048% fluoride toothpaste were tested. Gambier extract was obtained using the soxhletation method and the separation of the solvent from the extract was carried out by the distillation method. Agar Well method antifungal test was used to measure the inhibition zone value of tested groups. Data were analyzed using one-way Anova. Inhibition test of tested groups showed that only combination of gambier and fluorides toothpaste was effective in inhibiting Candida albicans with an average inhibition zone value of 8.35 mm. Toothpaste preparation of 10% Gambier extract and 0.0048% fluorides combination produce a synergistic therapeutic effect in antifungal therapy.
