Abstract

Hospital "X" as a Regional Public Service Agency (BLUD) has flexibility in financial management including the expenditure budget. In the budget implementation process over the past five years (2019-2023), there have often been changes in its implementation with an average adjustment of IDR 273,182,832,504 or 39.60% of the total allocated budget each year. Given the situation, this research aims to analyze the influence of knowledge and perceptions about budget management and leadership style on changes in budget implementation. The data was collected through questionnaires distributed to 42 heads of work units and secondary data taken from the 2019-2023 budget utilization documents. Data analysis was conducted using multiple linear regression tests to determine the influence of knowledge and perceptions of budget management, as well as leadership style on changes in budget implementation. The results of the research show that the higher the knowledge and the more accurate the perception of budget management among the heads of the work unit, the lower the changes in the implementation of the hospital budget. However, there is no influence of leadership style on changes in hospital budget implementation. In addition, knowledge and perceptions about budget management and leadership style simultaneously influence changes in the hospital budget. Knowledge, perception, and leadership style can influence changes in budget implementation with a contribution of 35.30%, while the remaining contribution of 64.70% is influenced by other factors not examined in this research. In conclusion, the factors that influence changes in the implementation of hospital budgets are knowledge and perceptions about budget management. Leadership style, on the other hand, is not a determining factor, since changes in hospital budget implementation remain high despite most heads of work units adopting a democratic leadership style.

Abstract

High workloads, the complexity of nursing tasks, declining care quality, increased risk of medical errors, and reduced patient satisfaction are persistent challenges in hospitals that require strategic solutions.  Work values may serve as motivational factors that encourage nurses to perform optimally, particularly under stressful conditions. This study aimed to analyze the effect of workload on nurse performance in delivering nursing care in low-care inpatient units, by considering work value as a potential moderating variable. This observational analytical study used a cross-sectional design. Data were collected from 195 nurses using proportional cluster random sampling based on unit assignment. Instruments included a demographic questionnaire, NASA-TLX to assess workload, an adapted career value scale to evaluate work values, and a nurse performance questionnaire developed from nursing practice standards. Data collection was conducted via Google Forms, and Partial Least Squares (PLS) moderation analysis was used to examine the relationships between variables. The results showed that most nurses experienced a moderate workload (49.7%) and demonstrated sufficient performance (78.5%). The highest contributing work value dimensions were intrinsic value and the competence sub-dimension. Workload and work value had a significant direct effect on nurse performance, but work value did not moderate the relationship between workload and performance. In conclusion, increased workload and stronger work values are associated with improved nurse performance.

Abstract

The nursing profession plays a crucial role in healthcare, requiring both physical and psychological preparedness. Registered nurses (RNs) specializing in infectious disease care are particularly vulnerable to heightened anxiety due to the physical demands of their work, uncertainty in patient care, and the risk of infection. Prolonged exposure to these stressors can lead to burnout, ultimately compromising the quality of patient care. This study examines the effect of anxiety levels on burnout, with work motivation as a moderating variable, among nurses in the Integrated Infection Service Installation at Saiful Anwar Hospital. This study employs an observational analytic approach, utilizing a cross-sectional design. The data were collected from 72 nurses through a questionnaire and analyzed using the Moderated Regression Analysis (MRA) method. The results indicate a significant positive association between anxiety levels and burnout (path coefficient = 0.803, t-statistic = 4.965, p = 0.000), while work motivation exhibited a significant negative effect on burnout (path coefficient = -0.405, t-statistic = 3.160, p = 0.002). Additionally, work motivation moderated the relationship between anxiety and burnout, mitigating its impact (path coefficient = 0.179, t-statistic = 2.134, p = 0.033). These findings underscore the importance of fostering a work environment that enhances motivation through performance-based rewards, professional training, and collaborative relationships. Furthermore, integrating anxiety management programs, such as counseling and mindfulness training, is essential for reducing anxiety and preventing burnout among nurses working in infectious disease care settings.

Abstract

Ventilator-associated pneumonia (VAP) is a major nosocomial infection in intensive care units (ICUs). Rotational therapy has been effective in reducing VAP incidence. The high incidence of VAP and the cost of ventilator maintenance indicate the need for a new management strategy. This study aimed to evaluate whether a rotational ventilator system based on lean management principles could reduce VAP incidence. The 5S method (Sort, Set in Order, Shine, Standardize, Sustain) was implemented to facilitate rapid change, while the Plan-Do-Study-Act (PDSA) cycle was used to control process variation and assess effectiveness over a 30-day period. The study involved 259 patients selected through total sampling. Data were analyzed using an independent t-test. Implementation of the revised management flow led to decreased waiting times, reduced non-clinical time wastage, improved patient and family satisfaction, minimized human error, and a 20% reduction in VAP rates within five days. The process was continuously evaluated, refined as needed, and formalized into a standard operating procedure (SOP) and clinical pathway for ICU ventilator management. The rotational ventilator system based on lean management, implemented in the ICU of Dr. Soebandi Hospital Jember, effectively reduced VAP incidence and management costs through the integration of 5S and PDSA methods.

Abstract

Solid medical waste management remains a significant challenge in Indonesia. According to data from the 2022 Indonesian Health Profile , only 38.9% of hospitals and health centers in Indonesia adhere to established standards for medical waste management. The impact of not managing medical waste according to standards is the increased risk of health and environmental problems. The present research design is of a qualitative nature, employing a case study approach in a government hospital. The data collection methods included in-depth interviews, document analysis, and field observations. The purposive sampling technique was employed, focusing on selecting participants who deeply understand the subject matter under investigation. The sample comprised seven individuals/informants from the Government Hospital, the Health Office, and the recipient of waste treatment services. The data derived from these interviews, document analysis, and field observations were systematically analyzed until the point of saturation was reached. The analysis yielded eight themes: The management delayed enforcing green hospital policies; The management neglected organizational governance of solid medical waste management without any policy; Technical governance of solid medical waste management was perplexing: policy or opportunity?; Inadequate communication and coordination between management and units; Hospital visitors and third parties slowed down the solid medical waste management process; The impact of management delays: solid medical waste accumulated; Inefficiency due to missing all recycling opportunities for solid medical waste; and The strategies for optimizing solid medical waste management governance. It can be concluded that the management of solid medical waste in government-owned hospitals is not yet at the most effective state, and there are still wastes and constraints in its governance. To address these issues, a number of strategies have been implemented, including adjustments to medical waste management and educational initiatives targeting the hospital community.

Abstract

Waiting time is one of the minimum standards in hospital pharmaceutical services. The waiting time for prescription services is measured as the interval between the submission of a prescription and the receipt of medication, which should be ≤30 minutes for non-compounded drugs and ≤60 minutes for compounded drugs. Timely service is closely related to customer satisfaction. However, prolonged waiting time for drug services still occurs at Dr. Saiful Anwar Hospital. Based on the 2023 community satisfaction index data, the score for the pharmaceutical installation was 83.88, and the lowest element was punctuality. Service quality is closely related to customer satisfaction, which is influenced by the clarity of information and services according to patients’ needs and expectations. The purpose of this study was to analyze the effect of information clarity on the length of waiting time for drug services and service quality on patient satisfaction in the JKN Outpatient Pharmacy Service Unit of Dr. Saiful Anwar Hospital. This study used a quantitative method with a cross-sectional approach. Samples were collected using consecutive sampling techniques with a total of 113 patients who accessed drug services at the JKN Outpatient pharmacy unit. The results showed that waiting time, tangibles, assurance, and empathy had a positive and significant effect on patient satisfaction. Reliability and responsiveness had a positive but not statistically significant on patient satisfaction. Among the variables, empathy had the strongest influence on satisfaction. Due to the dominant factor of empathy, it is recommended that Dr. Saiful Anwar Hospital improve its pharmaceutical services by providing training for pharmacists to deliver better and more empathetic service

Abstract

The timeliness of patient rounds by Attending Physicians (DPJPs) in hospitals significantly impacts various aspects of healthcare delivery, such as patient waiting times, treatment efficiency, and discharge processes. At Dr. Saiful Anwar Hospital, the level of compliance with punctual patient round standards remains below the national benchmark, which is recorded at 68.83%. This study aimed to analyze the influence of individual, environment, and motivation factors on the timeliness of rounds conducted by DPJPs at Dr. Saiful Anwar Regional Hospital, East Java Province. A quantitative, cross-sectional research design was used by involving 240 respondents selected through consecutive sampling. Data were collected using a questionnaire and analyzed using multiple linear regression. The results indicated that individual factors (gender, age, length of employment, and employment status), environment factors (access routes and medical facilities), and motivational factors (income, hospital policies, time pressure, and co-worker support) had no significant effect on patient round timeliness. This study found that timeliness was more influenced by adherence to professional ethics and a commitment to patient care, indicating that DPJPs continue to prioritize service quality despite operational challenges. In conclusion, external conditions and personal characteristics do not appear to be the primary determinants of timely rounds. To enhance service efficiency, hospitals are encouraged to optimize internal systems, such as scheduling, interprofessional coordination, and operational policy evaluation, to better support the timely execution of DPJP responsibilities.

Abstract

This study examines the impact of variations in clinical pathway (CP) implementation on potential financial losses in Sectio Caesarea (SC) services at Dr. Saiful Anwar Regional Hospital, East Java, Indonesia. Applying a quantitative approach with a cross-sectional study design, secondary data in the form of medical records and patient treatment costs were analyzed using the categorical comparative method from February to May 2024. The results showed that the majority of patients (78.4%) had good compliance with CP, while 21.6% did not comply. The group with appropriate CP implementation showed a lower average hospital cost (IDR 24,435,564.29) compared to the non-compliant group (IDR 30,996,638.89). CP implementation variance significantly affected cost containment, where the compliant group had a smaller average deficit (-IDR 15,741,421.43) compared to the non-compliant group (-IDR 20,243,246.30). Logistic regression tests showed that the more appropriate the CP, the greater the cost efficiency achieved. However, demographic factors and patient clinical conditions did not significantly affect the implementation of CP. This study concludes that proper CP implementation enhances cost efficiency and reduces potential hospital losses in SC actions. Finally, evaluation and development of evidence-based CP and periodic monitoring of its implementation are recommended to improve the efficiency of hospital services.

Abstract

Informed consent refers to a patient’s or their family’s approval of a medical procedure following a thorough explanation. It serves as a key indicator in hospital Minimum Service Standards, which aim for 100% completeness. In 2023, informed consent completeness at Hospital X reached 92.7%, failing to meet this target. One factor affecting completeness is the knowledge, attitude, and practice of Physicians in Charge of Services (DPJP).  This quantitative study adopts a cross-sectional design with observational data collection. The study used saturated sampling, targeting Medical Staff Group (KSM) physicians with low informed consent completeness. Conducted from April to August 2024, it involved 67 physicians. Data were analyzed using logistic regression, with informed consent completeness as the dependent variable and knowledge, attitude, and practice as independent variables.  Results show that knowledge, attitude, and practice influence informed consent completeness, both partially and simultaneously, but not significantly. Medical Records Unit data from 2023 indicate that the most frequent issue was missing physician signatures (78.8%). Questionnaire responses on physicians' practice indicate that 44.8% of respondents frequently delegated the task of completing informed consent forms to others, while 7.5% always did so.

Abstract

This study aimed to analyze the effect of service quality on inpatient satisfaction at Dr. Saiful Anwar Hospital. This study was conducted using a quantitative method, and data were collected through questionnaires from 204 inpatients selected using probability sampling techniques. The results showed that the tangibles, assurance, and empathy dimensions had a positive and significant effect on patient satisfaction, which indicates that improvements in these areas lead to higher satisfaction levels. In contrast, reliability and responsiveness had a positive but not statistically significant effect on patient satisfaction, thus improvements in these areas may lead to increased satisfaction, although the impact is not statistically significant. Among the five dimensions, assurance emerged as the most dominant factor influencing inpatient satisfaction. As the study identified assurance as the dominant factor influencing patient satisfaction, Dr. Saiful Anwar Hospital is expected to improve patient safety assurance. One potential initiative is the implementation of a QR-based medical record system to streamline access to patient information and minimize the risk of human error.
