Abstract

Type 1 diabetes mellitus (DM T1) is a chronic autoimmune disease that is common in children and adolescents, characterized by damage to cells β pancreas that leads to absolute insulin deficiency. This condition contributes to metabolic disorders, immune dysfunction, and an imbalance of the body's microbiota, including the oral microbiota. The oral cavity as one of the important microbiota ecosystems, plays a central role in maintaining local and systemic health. In children with T1 DM, oral dysbiosis is often found and is associated with an increased risk of periodontal disease and caries. This literature review aims to understand the interaction between oral microbiota and T1 DM in children, by highlighting changes in microbiota composition, their impact on oral and systemic health, and the potential for microbiota-based therapeutic interventions. Discussion. Commensal microbes in the oral cavity play a role in maintaining the homeostasis of the oral environment. However, in the DM T1 condition, there is an increase in pathogenic bacteria such as Porphyromonas gingivalis, Prevotella intermedia (P. Intermedia) and Fusobacterium nucleatum that trigger periodontal inflammation and the release of pro-inflammatory cytokines (IL-1β, TNF-α, IL-6). This can worsen the regulation of glucose metabolism and increase susceptibility to infection. An overview of clinical manifestations due to dysbiosis that occur in the form of gingivitis, periodontitis, xerostomia, oral candidiasis, and delayed wound healing. Conclusions, The reciprocal interactions between the oral microbiota and T1 DM reflect the complex relationship between dysbiosis, inflammation, and immune dysregulation.

Abstract

The capability assessment of Indonesia’s Government Internal Supervisory Apparatus (APIP) has primarily relied on the Internal Audit Capability Model (IACM) as a formal framework. However, major corruption cases persisting in institutions with high IACM levels indicate a gap between formal capability scores and the actual effectiveness of APIP in preventing corruption. This study employs a qualitative case study approach and applies the McKinsey 7S Framework to analyze organizational factors that may not be optimally captured in the current IACM assessment. The study was conducted at three government levels (central, provincial, and local) through in-depth interviews and supporting document analysis. Findings reveal that elements such as leadership style, shared values, structure, and human resource quality significantly affect APIP effectiveness, yet are underrepresented in the IACM model. This research recommends integrating the McKinsey 7S Framework into the IACM assessment process to provide a more holistic and contextually accurate representation of APIP capabilities.

Abstract

Patient satisfaction has an impact on patient loyalty, especially on the intention to revisit and use the services offered by a hospital. The primary objective of this study is to analyze the impact of waiting period, price affordability, and service quality on patient satisfaction and revisit intention at Jala Ammari Navy Hospital in Makassar City. This study used a nonprobability sampling method with a consecutive sampling approach to select respondents, as it allowed researchers to collect data through questionnaires distributed to patients who visited the hospital on 1st - 30th April 2025. The data obtained were analyzed using Partial Least Square-Structural Equation Modeling (PLS-SEM) with SmartPLS software. The analysis revealed a clear hierarchy of importance among the examined factors, with service quality emerging as the most influential determinant (=0.423, p0.001). Regarding operational factors, waiting period showed a meaningful though relatively modest impact on satisfaction. The study's comprehensive measurement approach provided robust validation of the hypothesized relationships (Hair et al., 2022). The findings offer valuable evidence-based insights for hospital administrators seeking to improve patient care and foster long-term relationships with their patients within the context of a private hospital setting.

Abstract

Golden Visa is an investment-based immigration policy scheme that grants long-term residence permits for foreign nationals. Since 2023, Indonesia has officially adopted this model through regulations aimed at attracting foreign investors, global talents, world leaders, and the diaspora. This article aims to evaluate the initial implementation of the Indonesian Golden Visa, compare it with similar practices in other countries such as Portugal, the United Arab Emirates, Singapore, and Malaysia, and formulate policy strengthening recommendations. With a qualitative approach based on literature studies and regulatory analysis, this paper finds that the success of Golden Visa is highly dependent on the selectivity of applicants, the quality of investment contributions, and cross-sectoral governance capacity. In Indonesia, although until April 2025 the investment realization value from Golden Visa has reached Rp35.4 trillion, challenges such as transparency, policy integration, and supervision of investment realization are still the main obstacles. Lessons from other countries show the importance of policy designs that are not only administratively appealing but also impact long-term economic transformation. Therefore, Indonesia needs to formulate a Golden Visa model that is not only oriented towards the amount of investment but also accompanied by an impact-based selection mechanism, periodic evaluation of contributions, and integration of cross-sectoral policies such as immigration, taxation, and investment. This scheme also needs to be directed at strategic sectors such as technology, renewable energy, and education, in order to create long-term added value and support the national development agenda in an inclusive and sustainable manner.

Abstract

The reliability of IT systems is crucial for technology-driven businesses, as service disruptions can lead to financial losses, operational inefficiencies, and customer dissatisfaction. Despite having an incident management framework, organizations still experience recurring IT incidents, indicating systemic weaknesses in incident prevention, detection, and response. To identify the systemic root causes of major IT incidents and assess challenges in incident detection and resolution. By identifying recurring failure patterns, the research seeks to provide insights into improving IT incident management processes. The study uses a qualitative approach, utilizing thematic analysis on post-mortem reports of 26 major IT incidents that occurred at PT INUSA, a fintech company in Indonesia, between August 2023 and August 2024. Tags were assigned to categorize systemic failure points, and patterns were extracted to highlight deficiencies in software operations and incident management processes. Findings show that 80% of incidents were triggered by internal changes, with recurring issues such as insufficient testing, ineffective deployment and change control processes, and missing or misconfigured production settings. Additionally, 69% of incidents lacked proactive alerts, particularly on transaction success rates, CPU utilization, and system health metrics, leading to delayed detection. Incident response inefficiencies, including delayed incident reporting and slow debugging processes, further prolonged recovery times. The study highlights critical weaknesses in IT incident management and recommends improvements such as enhanced automated testing, stricter deployment validation, and standardized monitoring mechanisms. These insights provide guidance for fintech and technology companies to reduce incident frequency, improve detection capabilities, and optimize response efficiency.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder associated with an elevated risk of vascular complications due to enhanced platelet activation and hypercoagulability. Immature Platelet Fraction (IPF), a parameter measurable via automated hematology analyzers, has been proposed as a potential biomarker for thrombotic risk in DM patients. However, the utility of IPF in this context remains controversial due to inconsistent findings. This study aims to evaluate the clinical relevance of IPF in predicting vascular disorders in DM patients through a systematic review of existing literature, focusing on identifying its diagnostic accuracy and proposing directions for further research. Using a literature review approach, the study analyzed secondary data from peer-reviewed journals and clinical studies published between 2012 and 2025. A total of 14 key references were selected based on their relevance and methodological rigor. Data were thematically analyzed and interpreted using citation management tools. The results indicate that IPF levels tend to be elevated in DM patients with vascular complications, reflecting increased platelet turnover and activity. However, inconsistencies remain regarding its predictive validity, as some studies report significant correlations while others do not. Reference values for IPF also vary by population and instrument. This study suggests that although IPF holds promise as a thrombotic risk marker in DM, further empirical research is essential to establish standardized reference values and evaluate its integration with other biomarkers in clinical practice.

Abstract

This study analyzes the impact of cyber attacks on the National Data Center (PDN) on national resilience, with a case study of the Immigration Checkpoint at Soekarno Hatta Airport. Cyber attacks on the PDN, which paralyzed the immigration system, exposed the vulnerabilities of digital infrastructure that is crucial to the continuity of public services. This study examines how disruptions to immigration services impact mobility, the economy, and the country's image. The economic impact is analyzed through potential losses due to flight delays, decreased tourism, and disruption to the supply chain. The social aspect is examined through the psychological impact on society due to uncertainty and discomfort. The weaknesses of the National Data Center (PDN) are not only understood as technical problems that disrupt administrative services, but also as issues that have a direct impact on the vital function of the Immigration Checkpoint (TPI). TPI has a strategic role in managing the flow of population migration, both Indonesian Citizens (WNI) and Foreign Citizens (WNA), who leave and enter through the country's border areas. Furthermore, this study explores how the attack affects public trust in the government's ability to protect vital data and infrastructure. The analysis of national resilience focuses on how cyber attacks can weaken the stability of the state and damage the government's ability to carry out its functions. This study emphasizes the need for investment in strengthening cybersecurity systems and increasing public awareness to minimize similar impacts in the future.

Abstract

Malnutrition among children under five remains a significant public health issue in Pekalongan City, affecting their growth, development, and long-term quality of life. In response, the local government has implemented a Supplementary Feeding Program (PMT) using locally sourced, high-nutrient food ingredients that are affordable and accessible. This study aims to analyze the implementation of local food-based PMT and its impact on improving the nutritional status of malnourished children under five. Employing a mixed methods approach, the study combined quantitative research using a case control design with qualitative data from in-depth interviews. A total of 280 malnourished children were selected through purposive sampling. Key variables measured included nutritional status (based on weight-for-age) and the type and frequency of PMT provided. Data were collected via anthropometric assessments and direct observation, and analyzed using the Wilcoxon test. The findings revealed a statistically significant improvement in the nutritional status of the children following a 30-day local food-based PMT intervention (p < 0.05). The PMT menus—featuring ingredients such as tempeh, eggs, sweet potatoes, and local vegetables—were shown to meet daily energy and macronutrient requirements. The study concludes that local food-based PMT is an effective, sustainable strategy to address childhood malnutrition and can serve as a model for community-based nutrition interventions.
