Abstract

Bureaucracy is a government instrument to realize efficient, effective, fair, transparent and accountable public services. This means that in order to be able to carry out government functions properly, bureaucratic organizations must be professional, responsive, and aspirational to the various demands of the community being served. The author's aim in this research is to find out how much influence the work culture and job competence have on improving medical personnel performance both partially and simultaneously at Doctor Suyoto Hospital Emergency Departement. The method used in this research is a quantitative research method with descriptive and verification methods. The sampling technique used in this research was the Slovin formula, and the data analysis technique used was the linier regression analysis technique using the SPSS version 27.0 application. Based on the analysis carried out, it can be concluded that the work culture and job competencet can significantly, either individually or collectively, have an influence on performance at Doctor Suyoto Hospital Emergency Departement. Based on respondents' perceptions, the work cultuten and job competence are in the good category. Likewise, the performance variable is included in the good category. Partially there is a strong and significant influence of the wor culture in improving performance, likewise job culture has a large and significant influence on performance. Taken together there is a large and significant influence of the work culture and job competence on medical personnel performance at Doctor Suyoto Hospital Emergency Departement.

Abstract

Modern leaders are ideally expected to possess firm, intelligent, caring, and integrity-driven characters, as well as the ability to adapt to change and leverage technology to promote collaboration and innovation within organizations. This study aims to examine the influence of job pressure and demands on leadership character. The research employs a quantitative explanatory approach with a cross-sectional design to explain the impact of job pressure and demands on leadership character. A purposive sample of 185 respondents was selected from leaders or prospective leaders who have served for at least one year in various organizations. Data were collected using a closed-ended questionnaire with a 5-point Likert scale that had been tested for validity and reliability, and analyzed descriptively and through binary logistic regression. The analysis results present regression coefficients, odds ratios, and significance values to test the relationships between variables at a 0.05 significance level. The findings indicate that job pressure and demands have a significant negative relationship with leadership character (p<0.05), meaning that higher pressure and demands reduce the likelihood of leaders having strong character. Specifically, individuals experiencing high job pressure or demands have an 82% to 82.1% lower chance of exhibiting strong character compared to those with lower pressure or demands. The constant B value shows that at low levels of pressure and demands, the likelihood of having strong leadership character is 5.395 times greater. Since job pressure and demands significantly negatively affect leadership character, organizational management needs to carefully manage workload and job expectations to avoid damaging leadership integrity.

Abstract

Background: Nursing services are an integral part of the primary health care system provided by community health centers (Puskesmas). The success of these services is not only measured by the number of patient visits but also by the quality of nurse-patient interactions, adherence to standard operating procedures, and clinical outcomes achieved. The Ber-Beru Puskesmas in Kalukku Subdistrict, Mamuju Regency, is one of the Puskesmas serving rural communities with geographical challenges and limited resources. Objective: This study aims to evaluate the effectiveness of nursing services at the Ber-Beru Community Health Center through a multi-indicator approach, namely patient satisfaction, compliance with Standard Operating Procedures (SOPs), and clinical outcomes. Method: This study used a quantitative descriptive design with a cross-sectional approach. The sample consisted of 100 patients selected through purposive sampling. Data were collected through patient satisfaction questionnaires, observations of nurses' compliance with SOPs, and reviews of clinical outcomes from medical records. Data analysis used descriptive statistics and Spearman's correlation test. Results: Patient satisfaction with nursing care was high (average score of 4.2 on a 5-point scale). Nurse compliance with SOPs reached 88%, indicating good implementation of procedures. Clinical results showed that 75% of patients experienced improvement in their condition. Correlation tests showed a positive and significant relationship between SOP compliance and patient satisfaction (ρ=0.65; p<0.01), as well as between patient satisfaction and clinical outcomes (ρ=0.52; p<0.01). Discussion: These findings indicate that nursing services at the Ber-Beru Community Health Center are quite effective, particularly in terms of nurse-patient interaction and SOP implementation. However, there are still challenges in achieving optimal clinical outcomes, particularly due to limitations in resources and supporting facilities. This means that although nurses have worked according to procedures and patients are satisfied, the effectiveness of the service has not been fully reflected in patient recovery outcomes. This highlights the importance of synergy between healthcare staff competence, availability of facilities and infrastructure, and quality management systems for service delivery. Conclusion: Nursing services at the Ber-Beru Community Health Center have demonstrated sufficient effectiveness, but still require improvement, particularly in terms of clinical outcomes and the provision of supporting facilities. Regular evaluations and a systemic approach are essential to maintain the quality of nursing services at rural community health centers.

Abstract

Malaria is usually spread by the bite of a female Anopheles mosquito that has the disease., which feeds on blood to support egg development. Anopheles mosquitoes are typically active at night. During the blood-feeding process, the mosquito’s saliva containing Plasmodium parasites in the gametocyte stage enters the human body. Within the body, the male and female sexual forms (gametes) of the parasite unite to form a cyst-like sporozoite. These sporozoites then travel to the liver, where they multiply and develop into exoerythrocytic schizonts. Malaria can have negative impacts on pregnant women and their unborn babies. These effects may include maternal and infant death or a range of complications affecting the mother, fetus, and newborn. This research seeks to explore how malaria during pregnancy affects the weight of the placenta and the weight of newborns. A quantitative method with a descriptive approach was used. The study subjects were pregnant women infected with malaria who gave birth at the Sentani Health Center. Among the 12 respondents infected with malaria, all had placental weights of less than 500 grams. Meanwhile, the 20 respondents who were not infected with malaria all had placental weights of 500 grams or more. The Chi-square test for placental weight yielded a p-value of 0.000, indicating a significant effect of malaria infection in pregnant women on placental weight at Sentani Health Center, Jayapura Regency, in 2024.

Abstract

The large number of activities carried out and the responsibility for the work carried out, means that administrative staff experience a high workload so that it must be a concern for the Mamajang Makassar Community Health Center to take important steps, namely by adding administrative staff, or by adding facilities and infrastructure such as: computers, printers and Wifi as well as transportation accommodation so that work can be facilitated at the Mamajang Community Health Center. Research Objective To find out in depth the workload of BPJS Health claims administration officers at the Mamajang Community Health Center. This type of research is qualitative descriptive research. The data collection methods used were observation, interviews, and document review. And the data analysis used in this research uses a qualitative descriptive data analysis method, with steps for data reduction, data presentation, and drawing conclusions. Workload analysis shows a mismatch between time pressure and workload, caused by factors such as working hours, limited facilities, additional tasks, and problems managing patient information. Analysis of the workload of BPJS Health claims administration officers at the Mamajang Community Health Center highlights information overload, including limited facilities, work outside working hours, lack of data analysis, and stress from interactions with patients. Analysis based on responsibilities shows the complexity of the responsibilities of BPJS Health Mamajang administrative officers, from claims to interactions with patients. Environmental constraints, limited facilities, and difficulties in communicating with patients are the main obstacles.

Abstract

Aging is a natural part of human life. Aging is one of the factors that cause hypertension, as with increasing age, bodily functions decline, marked by a decrease in the elasticity of arterial blood vessels and increased stiffness of blood vessels, making them more susceptible to elevated blood pressure in the elderly. Blood pressure is one of the vital signs, along with respiratory rate, heart rate, oxygen saturation, and body temperature. One of the common issues related to blood pressure is hypertension, which contributes to premature mortality worldwide. A person is considered to have hypertension if their systolic blood pressure is > 140 mmHg and diastolic blood pressure is > 90 mmHg. From a health perspective, as we age, we become more vulnerable to various physical, mental, social, and economic issues. High blood pressure not only affects your quality of life but can also be life-threatening. Lack of attention to lifestyle factors, such as poor dietary habits or insufficient physical activity, can contribute to the development of high blood pressure in the elderly. To prevent the risk of more severe complications, blood pressure can be controlled by changing lifestyle habits and engaging in regular physical activity, one of which is ergonomic exercise, which is very practical, efficient, and easy to perform. This literature review aims to analyze the application of ergonomic exercise in lowering blood pressure in hypertensive patients. Based on several reviews of various articles, it is evident that the application of ergonomic exercise has proven effective in lowering blood pressure in hypertensive patients.

Abstract

Background: Geriatric patients are a vulnerable group to drug side effects due to decreased physiological function and the presence of multiple chronic diseases that require polypharmacy therapy. The combination of various drugs in one prescription increases the possibility of potential drug-drug interactions (pDDIs) which can have a negative impact on patient safety. Objective: This study aims to analyze the frequency, type, and severity of potential drug interactions found in geriatric patient prescriptions at the pharmacy installation of the Regional General Hospital (RSUD), and to identify the drugs that most often cause interactions. Methods: This study used a retrospective descriptive design with a quantitative approach. Data were collected from 250 copies of prescriptions for patients aged ≥60 years who received outpatient and inpatient care during January–March 2025. Interaction analysis was performed using the Medscape Drug Interaction Checker, and the results were classified based on severity (minor, moderate, major). Results: From a total of 250 prescriptions analyzed, 418 potential drug interactions were found, consisting of 38.5% major interactions, 50.2% moderate interactions, and 11.3% minor interactions. The drugs most frequently involved in interactions included amlodipine, simvastatin, furosemide, aspirin, and metformin. Conclusion: The results of the study indicate that the incidence of potential drug interactions in geriatric patients is relatively high. This indicates the need for a more careful prescription monitoring system, active involvement of clinical pharmacists, and the application of system-based technology to detect and manage drug interactions effectively. This step is important to improve patient safety and the effectiveness of therapy among the elderly.

Abstract

Background: Paracetamol is a widely used antipyretic and analgesic drug, particularly in syrup form for children. The stability of syrup preparations is significantly influenced by storage conditions, particularly temperature. Objective: This study aimed to analyze the physical and chemical stability of paracetamol syrup preparations stored at different temperatures. Methods: An experimental laboratory study was conducted on three storage groups: room temperature (25±2°C), cold temperature (4±2°C), and high temperature (40±2°C) for 30 days. Physical stability evaluation included color, clarity, pH, and viscosity, while chemical analysis was performed using UV-Vis spectrophotometry to measure paracetamol levels. Results: The results showed that syrup stored at high temperatures experienced a significant decrease in active ingredient levels (up to 20%) and changes in pH. Preparations stored at cold and room temperature remained physically and chemically stable throughout the test period. Conclusion: High storage temperatures accelerate the degradation of paracetamol in syrup preparations, so it is recommended to store at room temperature or refrigerated to maintain stability.

Abstract

Background: Long queues and waiting times exceeding the standard in the Emergency Department (ER) are indicators of low hospital operational efficiency. This problem not only impacts patient satisfaction but also the overall safety and quality of healthcare services. The role of operational management is crucial in addressing this issue through resource management, workflow, and the integrated use of information technology. Objective: This study aims to analyze the role of operational management in managing queues in the ER and identify factors influencing the effectiveness of this queuing system. Methods: The study used a quantitative-descriptive approach using observation, semi-structured interviews, and document review in the ER of RSUD X. The sample consisted of 30 patients and 10 medical personnel selected purposively. Data were analyzed descriptively and linked to operational management theory. Results: It was found that operational management contributes significantly to queue management, through the implementation of a triage system, scheduling of healthcare personnel, and the utilization of hospital information systems. However, challenges such as limited bed capacity, shortage of medical personnel, and lack of information system integration remain major obstacles. Conclusion: Optimizing operational management in the emergency department (ED) has been shown to improve service flow and reduce waiting times. Continuous evaluation of operational systems and strengthening of human resources and information technology capacity are recommended to improve the quality of emergency services.
