Abstract

Tuberculosis remains a major public health problem in Indonesia, and medication nonadherence continues to reduce treatment success despite the availability of effective therapy. The Health Belief Model (HBM) provides a framework for understanding treatment adherence based on patients' health beliefs. This study aimed to analyze determinants of treatment adherence among pulmonary tuberculosis patients using the Health Belief Model approach in Pariaman City, Indonesia. A mixed-methods study with an explanatory sequential design was conducted from August 2023 to June 2024. The quantitative phase employed a cross-sectional design involving 107 pulmonary tuberculosis patients selected through proportional random sampling. The sample size was determined using the Lemeshow formula, yielding a minimum of 97 participants, with an additional 10% included to account for potential dropouts, resulting in a final sample of 107 patients. Independent variables included perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and self-efficacy. The qualitative phase involved in-depth interviews with 12 purposively selected informants to explore program implementation related to treatment adherence. Poor perceptions were reported for susceptibility (43.9%), severity (41.1%), benefits (46.7%), barriers (44.9%), and self-efficacy (52.3%). Bivariate analysis showed that perceived severity (p=0.007) and perceived benefits (p<0.001) were significantly associated with treatment adherence. Multivariate analysis identified perceived benefits as the strongest determinant of treatment adherence (OR=5.103; 95% CI: 2.165-12.027; p<0.001). Qualitative findings revealed that limited human resources, funding constraints, and inconsistent patient education contributed to poor perceived benefits, supporting the quantitative finding that perceived benefits was the strongest determinant of adherence. Health authorities should strengthen patient education, ensure adequate human and financial resources, and implement standardized monitoring and counseling programs to improve treatment adherence and tuberculosis control outcomes.

Abstract

Gastrointestinal disorders, particularly peptic ulcer and gastritis associated with Helicobacter pylori infection, remain a pressing global health burden that is compounded by rising antimicrobial resistance to standard eradication therapies. The genus Sterculia (Malvaceae) has long been used in traditional medicine to treat these conditions, yet no systematic synthesis of its efficacy and underlying phytochemical mechanisms has previously been available. This systematic review followed the PRISMA 2020 guidelines and a PICO framework, searching PubMed, ScienceDirect, Scopus, and Google Scholar, with study quality appraised using the Young and Solomon instrument. Ten eligible articles were narratively synthesized. Findings showed that a methanolic seed extract of Sterculia nobilis inhibited reserpine-induced gastric ulceration by up to 65.1% at 400 mg/kg, approaching the efficacy of ranitidine, through reduced gastric acid secretion, modulation of serum neuropeptides, and enhanced superoxide dismutase activity. Sterculia urens gum functioned dually as a gastroretentive drug-delivery matrix and an intrinsic antioxidant agent. Antimicrobial activity was consistently observed across species: fraxetin from Sterculia quadrifida significantly inhibited gastrointestinal bacterial pathogens, while Sterculia foetida exhibited notable antifungal potential against Candida spp. Three convergent mechanisms free-radical scavenging, neuropeptide modulation, and polysaccharide-mediated mucosal protection appear to underlie the gastroprotective effects of this genus. However, the absence of clinical trials and direct evaluation against Helicobacter pylori represent major evidence gaps. These findings provide a scientific basis for developing Sterculia-based gastrointestinal therapeutic agents, although further preclinical and clinical research is warranted to confirm their efficacy and safety.

Abstract

Urtica dioica L. (wild stinging nettle) is a plant traditionally used as a food source and medicinal plant and is known to contain various bioactive compounds. This study aimed to characterize a 70% ethanolic extract of U. dioica based on quality parameters, total flavonoid content, and antioxidant activity. The roots, stems, and leaves of U. dioica were combined and extracted by maceration for 3 × 24 h. Extract characterization included the determination of moisture and ash contents using gravimetric methods, total flavonoid content using an aluminum chloride colorimetric method with quercetin as the reference standard, and antioxidant activity using the DPPH radical-scavenging assay. The extraction yielded 9.95% based on the dry weight of the simplicia. The extract showed a moisture content of 19.71 ± 0.29%, ash content of 32.13 ± 0.96%, and total flavonoid content of 1.8765 ± 0.0079 g quercetin equivalents (QE)/100 g sample, corresponding to 18.77 mg QE/g extract. The antioxidant assay showed 66.54 ± 0.11% inhibition of DPPH radicals under the experimental conditions used. These findings provide baseline information on the physicochemical and phytochemical characteristics of the combined root, stem, and leaf extract of U. dioica, supporting further research on extract standardization and identification of its bioactive constituents.

Abstract

Acne vulgaris is a common skin disorder with a high prevalence among adolescents and young adults. Long-term use of topical antibiotics may contribute to antimicrobial resistance, highlighting the need for alternative topical agents such as propolis. This study aimed to formulate and evaluate the physical characteristics of a hydrogel acne patch containing nanopropolis (Trigona sp.) from Balikpapan. Propolis nanoemulsion was prepared using a self-nanoemulsifying approach and incorporated into four formulations: F0 (base control), F1 (nanoemulsion control), F2 (2.5% nanopropolis), and F3 (5% nanopropolis), using an HPMC-based solvent-casting method. Evaluations included organoleptic properties, folding endurance, thickness, weight uniformity, pH, and moisture absorption. The nanoemulsion showed a mean transmittance of 96.97 ± 0.34%, while all patch formulations had folding endurance >1000 folds, thickness of 0.405–0.494 mm, weight coefficient of variation <5%, pH of 6.26–6.32, and moisture absorption of 6.82–9.56%. All formulations met the predefined physical quality criteria. Formulations containing nanopropolis showed an increase in thickness, mean weight, and moisture absorption, while pH remained within the physiological range of skin. These findings indicate that Trigona sp. nanopropolis can be incorporated into an HPMC-based hydrogel acne patch with acceptable physical characteristics for further topical development.

Abstract

Background: Small frequent feedings (SFF) is a dietary approach intended to improve food tolerance in pregnant women experiencing nausea and vomiting. Objective: To assess changes in symptom severity following SFF at Vina Primary Care Clinic, Medan. Methods: A single-group pretest–posttest study included 36 pregnant women with a clinical diagnosis of hyperemesis gravidarum. The protocol prescribed six to eight small meals daily, supported by food diaries and WhatsApp monitoring. Symptoms were assessed on days 1 and 3 using the 24-hour Pregnancy-Unique Quantification of Emesis and Nausea (PUQE-24) scale. Paired scores were compared using the Wilcoxon signed-rank test. Results: Mean PUQE-24 scores decreased from 9.28 ± 3.248 to 6.83 ± 3.140, with a mean paired reduction of 2.44 points (Z = −5.103; p < 0.001; r = 0.85). Median scores decreased from 9.0 to 6.0. The proportion with mild symptoms increased from 25.00% to 61.11%, while the proportion with severe symptoms decreased from 22.22% to 8.33%. Scores decreased in 34 participants, remained unchanged in one, and increased by one point in one. Twenty participants (55.56%) met the reported adherence threshold. Conclusion: SFF accompanied by education and monitoring was associated with lower PUQE-24 scores during short-term observation. The uncontrolled design precludes causal attribution; controlled studies are needed before its specific effectiveness can be established.

Abstract

Fixed orthodontic appliances facilitate plaque retention and may disturb the oral microbial ecosystem, thereby increasing susceptibility to periodontal inflammation. Porphyromonas gingivalis is a keystone periodontal pathogen, whereas Lactobacillus reuteri has been proposed as an adjunctive probiotic strategy. This randomized controlled trial evaluated the effect of L. reuteri probiotic lozenges on salivary P. gingivalis counts in fixed orthodontic appliance users. Thirty-eight dental students aged 18–25 years were analyzed (19 per group). The probiotic group consumed one L. reuteri lozenge nightly for 14 days, while the control group received standardized oral-hygiene education. Unstimulated saliva was collected at baseline and after the intervention and quantified by quantitative polymerase chain reaction (qPCR). Median (IQR) P. gingivalis counts decreased from 10,503,530 (34,139,942) to 7,855,643 (15,907,250) copies/mL in the control group and from 10,329,601 (20,433,321) to 8,144,486 (11,820,528) copies/mL in the probiotic group. The between-group difference in change was not significant (Mann–Whitney U, Z = −0.131; p = 0.895; r = 0.02), although significant within-group reductions occurred in both the probiotic (p = 0.027) and control (p = 0.033) groups. A 14-day course of L. reuteri lozenges did not provide an additional reduction in salivary P. gingivalis beyond that observed with oral-hygiene education alone.

Abstract

Stunting is a condition of growth failure in children due to chronic malnutrition, characterized by a height-for-age z-score (HAZ) of < –2 SD. This study aims to analyze the determinants of stunting in children aged 24–59 months at the UPTD Puskesmas Uluidanotae, South Nias Regency, which is the area with the highest number of stunting cases in the regency. The research method is quantitative with an analytical observational design using a cross-sectional approach. The study population consisted of 162 mothers with children aged 24–59 months, with a sample size of 115 respondents selected using a purposive sampling technique. Primary data were collected through questionnaires and observations, then analyzed using univariate, bivariate (Chi-Square), and multivariate (multiple logistic regression) analyses. The results showed that the prevalence of stunting in the study location reached 24.3%. Based on bivariate analysis, a statistically significant relationship was found between maternal education level (p=0.029), breastfeeding practices (p=0.000), and immunization status (p=0.025) with the occurrence of stunting. However, family income did not show a significant relationship (p=0.624). The multiple logistic regression test concluded that breastfeeding is the most dominant factor and serves as a protective factor against stunting (p=0.000; Exp(B)=0.142), followed by immunization status (p=0.027; Exp(B)=0.325). The maternal education variable became insignificant in the multivariate model after controlling for other health behavior variables. The conclusion of this study is that exclusive breastfeeding practices and complete basic immunization are the primary determinants of a child's linear growth status in the UPTD Puskesmas Uluidanotae working area. It is recommended that health workers strengthen lactation counseling starting from pregnancy and increase the coverage of complete basic immunization services as priority strategies to reduce stunting rates.

Abstract

Patient-reported service evaluations provide a basis for improving inpatient care. This analytical cross-sectional study examined associations between five SERVQUAL dimensions and patient satisfaction at Stella Maris General Hospital, South Nias Regency, Indonesia. A purposive sample of 115 respondents completed a structured questionnaire. Associations were assessed using Pearson’s chi-square tests and exploratory Firth penalized logistic regression. Overall, 59 respondents (51.3%) were satisfied. All five dimensions were associated with satisfaction in unadjusted analyses (all p < 0.001). Satisfaction ranged from 90.2% to 100.0% among respondents with high service-quality ratings, compared with 1.8% to 7.4% among those with low ratings. The dimensions were strongly correlated (r = 0.808–0.966). Adjusted odds ratios ranged from 1.53 to 11.11, and all 95% confidence intervals included 1. The adjusted odds ratios were 6.55 (95% CI 0.040–184.08) for responsiveness and 1.53 (95% CI 0.008–46.66) for tangibles. A sensitivity analysis restricted to 77 respondents with a recorded stay of at least two days yielded the same pattern. Favorable ratings across all five dimensions were associated with inpatient satisfaction, whereas the adjusted analysis did not identify an independently dominant dimension. The findings support an integrated approach to service improvement encompassing timely assistance, consistent care, effective communication, and the ward environment

Abstract

Background: Continuous ambulatory peritoneal dialysis (CAPD) is a home-based option for kidney replacement therapy, but interest in this modality may depend on patients’ understanding, perceived risks, confidence, family resources, and the education they receive. Objective: To examine the associations of knowledge, perceived infection risk, family support, self-efficacy, and healthcare-provider education with interest in CAPD among patients receiving maintenance hemodialysis. Methods: This quantitative, cross-sectional correlational study was conducted at Murni Teguh Hospital, Medan, from November 2025 to February 2026. A purposive sample of 149 adults was selected from a reference population of 254 hemodialysis patients. Structured questionnaires assessed the five explanatory domains and interest in CAPD. Analyses included descriptive statistics, Pearson chi-square tests, crude prevalence ratios (PRs) with 95% confidence intervals (CIs), and exploratory simultaneous-entry binary logistic regression. Results: High interest in CAPD was reported by 59 participants (39.6%). The proportions with high interest were 90.0%, 43.8%, and 7.3% in the good-, fair-, and poor-knowledge groups, respectively. All five domains were associated with interest in unadjusted analyses (all p < 0.001). Good versus poor knowledge yielded a crude PR of 12.38 (95% CI, 4.78–32.04). No participant in the low-perception category reported high interest. The exploratory multivariable model did not converge, limiting interpretation of adjusted associations. Conclusion: Knowledge, perceived infection risk, family support, self-efficacy, and healthcare-provider education were associated with CAPD interest. These findings support individualized, family-inclusive counseling and prospective evaluation of balanced education and shared decision-making.

Abstract

Patient-reported service quality can identify priorities for improving inpatient care, but its association with satisfaction must be distinguished from a causal effect. This cross-sectional study examined the five SERVQUAL dimensions and inpatient satisfaction at Doloksanggul Regional General Hospital, North Sumatra, Indonesia. A purposive sample of 245 respondents completed a structured questionnaire. Descriptive statistics, Pearson’s chi-square tests, and Cramér’s V were used, and multiple logistic regression was attempted with all five dimensions entered simultaneously. Men accounted for 50.6% of respondents; intermediate education was the largest educational category (45.3%), and 46.1% had been hospitalized for 2–5 days. Overall, 126 respondents (51.4%) were satisfied. High service-quality ratings ranged from 50.6% for assurance to 53.1% for tangibles. All five dimensions were associated with satisfaction (all p < 0.001), with Cramér’s V ranging from 0.837 for tangibles to 0.984 for assurance. Satisfaction was reported by 124/124 respondents with high assurance and 2/121 with low assurance. However, the logistic model did not converge after 20 iterations, and collinearity diagnostics indicated substantial overlap among predictors. Consequently, interpretable adjusted odds ratios and a defensible ranking of independent determinants could not be established. The findings describe strong unadjusted associations between perceived service quality and satisfaction. Assurance, responsiveness, and the substantial proportion of dissatisfied respondents warrant attention in service evaluation, but intervention priorities should be supported by verified measurement, operational indicators, and prospective assessment.
