Abstract

Background Coronary heart disease is one of the causes of morbidity and mortality worldwide, especially in Indonesia. Coronary heart disease is closely associated with dyslipidemia, particularly increased low-density lipoprotein (LDL), which plays an important role in the formation of atherosclerosis. High-intensity statin therapy is recommended for patients with coronary heart disease after percutaneous coronary intervention (PCI) to optimally reduce LDL and reduce the incidence of major adverse cardiovascular events (MACE) due to its pleiotropic effects. Objective To review the scientific evidence regarding the effectiveness, safety, and clinical outcomes of the use of high-intensity statins in patients with coronary heart disease after PCI based on the current literature. Methods The literature was obtained from the PubMed, Scopus, and Google Scholar databases published within the last ten years. The included studies consisted of randomized clinical trials, retrospective and prospective cohort studies, observational studies, systematic reviews and meta-analyses, and narrative reviews. The evaluated outcomes included LDL target achievement, major adverse cardiovascular events (MACE), and safety profiles including myopathy, hepatotoxicity, diabetes mellitus, and other adverse effects. Results Most studies showed that high-dose statins effectively reduced LDL levels and provided clinical benefits in patients with cardiovascular disease. A comparison of atorvastatin 40 mg and 80 mg showed similar effectiveness in reducing LDL, with a low incidence of adverse effects and still tolerable. In patients after PCI, atorvastatin 80 mg and rosuvastatin 40 mg had comparable effectiveness, although atorvastatin showed a better safety profile and cost efficiency. Several studies reported that the combination of moderate-dose statins with ezetimibe resulted in higher achievement of LDL targets compared with high-dose statin monotherapy. In addition, the combination was associated with lower risks of muscle-related adverse effects, increased liver enzymes, and diabetes mellitus. Nevertheless, most high-risk patients still do not achieve the recommended LDL target, so treatment intensification strategies are needed, including the addition of ezetimibe or PCSK9 inhibitors.

Abstract

Whitening hand and body lotions are widely used cosmetic products, but unregulated products may contain prohibited depigmenting agents such as mercury and hydroquinone. This laboratory study screened eight lotion products marketed in Banda Aceh, comprising three products purchased through online channels and five products obtained from offline outlets. Mercury and hydroquinone were screened qualitatively using 0.5 N potassium iodide and 1% ferric chloride, respectively. Hydroquinone was assessed by UV-visible spectrophotometry, whereas mercury was measured by atomic absorption spectrophotometry. Hydroquinone was not detected in any sample. Samples 4, 5, 7, and 8 produced positive qualitative reactions for mercury. The corresponding calculated readings were 0.0085, 0.0084, 0.0030, and 0.0032 mg/L, respectively; however, all values were below the reported method detection limit (0.058 mg/L) and quantification limit (0.175 mg/L). Accordingly, these values should be interpreted as estimated signals rather than validated quantitative confirmation. The findings indicate a presumptive mercury-related safety concern in four products and support confirmatory testing with a more sensitive, fully validated method. Negative findings for the two analytes do not establish the overall safety of the products.

Abstract

Potential drug–drug interactions (pDDIs) are a major cause of medication-related problems in critically ill patients; however, evidence from intensive care units (ICUs) in Indonesia remains limited. This prospective cross-sectional study aimed to evaluate the prevalence, severity, and predictors of pDDIs among 100 patients admitted to the ICU of Kendal Islamic Hospital, Central Java, Indonesia, from June to July 2025. Medication data were extracted from electronic medical records and systematically screened using the DrugBank® database. Descriptive analysis, chi-square test, and binary logistic regression were performed. All patients experienced at least one pDDI, with a total of 4,938 pDDIs involving 50 distinct drug pairs. Moderate-severity interactions predominated (66.5%), followed by minor (23.4%) and major (10.1%) interactions, with the severity distribution differing significantly (χ² = 2,577.38; p < 0.001). Fentanyl–ranitidine was the most frequently identified drug pair. Binary logistic regression identified age category as the only independent predictor of major pDDIs (OR = 3.531; 95% CI 1.564–7.975; p = 0.003), whereas sex and ICU length of stay were not significantly associated. These findings demonstrate the high burden of pDDIs among ICU patients and highlight the need for systematic pDDI screening and greater involvement of clinical pharmacists to improve medication safety.

Abstract

The skin is highly susceptible to free radical damage, including exposure to ultraviolet rays, which can trigger premature aging. Beets (Beta vulgaris L.) are known to contain secondary metabolites such as flavonoids, tannins, saponins, and alkaloids that exhibit antioxidant activity. This study aims to develop a physically stable cream formulation from beetroot ethanol extract that possesses antioxidant activity. The extraction method used in this study was maceration. The resulting concentrated extract was then formulated into an oil-in-water (O/W) cream base with varying extract concentrations (5%, 10%, and 15%). This study aimed to evaluate the physical stability of the creams, including organoleptic properties, pH, homogeneity, viscosity, spreadability, tackiness, and antioxidant activity using the DPPH (2,2-diphenyl-2-picrylhydrazyl) method. All formulations were homogeneous, with a pH of 5–6, viscosity of 3,537–3,544 cps, spreadability in the range of 5–7 cm, and adhesion time of 2.12–2.41 seconds. The IC50 value for pure beetroot extract was 190.50 ppm ((IC50 151-200 ppm weak), F1 cream (5%) at 732.85 ppm, F2 (10%) at 686.97 ppm, and F3 (15%) at 632.38 ppm (IC50 >200 ppm very weak). It can be concluded that beet fruit extract can be formulated into a physically stable cream preparation, even though its antioxidant activity is classified as very weak

Abstract

Background: Three-dimensional (3D) culture models have emerged as a promising preclinical platform for breast cancer research because they better mimic the tumor microenvironment (TME) of conventional two-dimensional cultures. With advances in tissue engineering, research related to 3D models has grown rapidly, necessitating a comprehensive evaluation of global research trends. Methods: A bibliometric analysis was performed using the Scopus database covering publications from 2007 to 2025, and only original research articles were included. Bibliometric indicators, including annual scientific output, leading countries, institutions, authors, journals, highly cited publications and collaboration patterns, were analyzed using Biblioshiny (R package), while keyword co-occurrence and temporal evolution were visualized using VOSviewer. Results: A total of 207 research articles published by 1,245 authors were analyzed. Scientific output showed a consistent annual growth rate of 13.32%, reflecting the increasing global interest in 3D breast cancer modeling. The United States was the leading contributor in publication productivity and scientific impact, while Biomaterials was the most productive journal. Highly cited publications predominantly focused on advanced tissue engineering technologies, including bioprinting, hydrogels, extracellular matrix engineering and microfluidics. Keyword co-occurrence analysis identified seven thematic clusters, while overlay visualization revealed a shift in research from conventional 3D cell culture toward more advanced tissue engineering approaches, particularly in biomaterials, tumor microenvironment reconstruction, drug screening and precision medicine. Conclusion: Research on 3D breast cancer models has evolved into a multidisciplinary field driven by tissue engineering innovations. This study provides a comprehensive knowledge map of the field, highlights emerging research directions and offers valuable guidance for future interdisciplinary collaborations and the development of physiologically relevant in vitro breast cancer models.

Abstract

Polycystic ovary syndrome (PCOS) requires individualized pharmacotherapy because reproductive and metabolic manifestations vary among patients. This study evaluated the associations of age, documented menstrual complaints, and pregnancy-related presentations with medication-count category among patients with PCOS at Kencana Clinic, Sorong City. A retrospective cross-sectional analysis was conducted on 118 eligible records selected by computer-generated simple random sampling from 412 records dated January 2022-December 2023. Associations were analyzed using the chi-square test and expressed as odds ratios (ORs) with 95% confidence intervals (CIs). Patients aged >25 years were more likely to receive >2 medications than those aged ≤25 years (40/73 vs 16/45; OR = 2.197; 95% CI = 1.022-4.721; p = 0.042). Menstrual complaints and pregnancy-related presentations were not significantly associated with medication-count category (p = 0.297 for each comparison). Older age was associated with greater medication-count complexity, but did not establish causality. Treatment planning should integrate age with metabolic status, comorbidities, and reproductive goals.

Abstract

Introduction: Peripheral neuropathy contributes to pain, functional impairment, and reduced quality of life in patients with diabetes mellitus and hypertension. This study evaluated changes in neuropathic screening status and quality of life during the use of complementary therapies alongside standard pharmacotherapy. Methods: This uncontrolled pre-post observational study included 147 patients at RSUD Nurdin Hamzah: 65 with hypertension and 82 with diabetes mellitus. Neuropathic characteristics and quality of life were assessed using the Douleur Neuropathique 4 (DN4) and WHOQOL-BREF, respectively. Paired DN4 observations were analyzed using the Wilcoxon signed-rank test, while associations with WHOQOL-BREF domains were analyzed using chi-square or likelihood-ratio tests. Results: After one month, DN4-positive screening decreased from 100% to 96.9% in the hypertension group and from 100% to 63.4% in the diabetes group. Paired DN4 observations differed significantly in both groups (p<0.001). No significant associations were found between complementary-therapy modalities and the physical, psychological, social, or environmental WHOQOL-BREF domains (all p>0.05). Conclusion: DN4 screening status improved during combined standard pharmacotherapy and self-selected complementary-therapy use, particularly among patients with diabetes, without a corresponding association with quality-of-life domains.

Abstract

Introduction: Oral candidiasis is a frequent opportunistic infection in people living with HIV and is closely associated with cellular immunosuppression, particularly a reduced CD4 count. Although antiretroviral therapy (ART) suppresses HIV replication and supports immune restoration, oral candidiasis may persist when immune recovery is incomplete or when other clinical factors are present. This study aimed to describe oral candidiasis according to CD4 count and ART duration among patients with HIV/AIDS at Dr. M. Djamil General Hospital, Padang. Methods: This retrospective descriptive study reviewed medical records from 2022 to 2025. Records were screened consecutively according to patient registration order. A total of 502 records with complete information on clinically diagnosed oral candidiasis, CD4 count, and cumulative ART duration met the eligibility criteria. The data were analyzed univariately and presented as frequencies and within-category percentages. Results: Oral candidiasis was documented in 35 of 502 patients (7.0%). Within the CD4 categories, the proportions were 13.8% (23/167), 4.9% (8/163), and 2.3% (4/172) among patients with CD4 counts <200, 200-400, and >400 cells/mm³, respectively. According to cumulative ART duration, the proportions were 7.7% (1/13) for <6 months, 10.3% (3/29) for 6-12 months, and 6.7% (31/460) for >12 months. Although the >12-month group contained the largest absolute number of cases, its within-category proportion was not the highest. Discussion: The decreasing proportion across increasing CD4 categories is consistent with the clinical relationship between oral candidiasis and impaired cellular immunity. Conversely, the distribution according to ART duration did not show a monotonic pattern. Because adherence, treatment interruption, ART regimen, viral load, resistance, nutritional status, and mycological confirmation were unavailable, the findings should not be interpreted as evidence of an effect of ART duration. Conclusion: Oral candidiasis was documented in 7.0% of the study population and showed the highest descriptive proportion among patients with CD4 counts <200 cells/mm³. ART duration alone cannot be used as an indicator of immune recovery or oral candidiasis risk.

Abstract

Resistensi antimikroba menjadi tantangan dalam pengembangan terapi antibakteri sehingga diperlukan eksplorasi sumber mikroorganisme baru penghasil senyawa antibakteri. Penelitian ini bertujuan untuk mengeksplorasi, mengisolasi, mengkarakterisasi, dan mengevaluasi potensi antibakteri bakteri indigenous dari sedimen Sungai Batanghari, Danau Sipin, sedimen mangrove, dan limbah cair kelapa sawit (palm oil mill effluent/POME) terhadap bakteri patogen. Isolat bakteri diperoleh melalui metode isolasi dan pemurnian, kemudian dilakukan karakterisasi morfologi, pewarnaan Gram, uji biokimia, serta pengujian aktivitas antibakteri menggunakan metode difusi cakram terhadap Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa, dan Salmonella Typhi. Hasil penelitian menunjukkan adanya variasi karakter morfologi dan fisiologi pada isolat dari berbagai sumber lingkungan. Beberapa isolat memiliki aktivitas antibakteri dengan tingkat penghambatan yang berbeda. Isolat SBHD 44 dan SBHD 67 menunjukkan aktivitas kuat terhadap S. aureus, sedangkan isolat LCM 7 dari POME menunjukkan aktivitas tertinggi terhadap E. coli dengan zona hambat 21,07 mm. Isolat dari Danau Sipin dan sedimen mangrove juga menunjukkan aktivitas antibakteri terhadap bakteri uji tertentu. Hasil penelitian menunjukkan bahwa sedimen akuatik dan POME berpotensi sebagai sumber bakteri indigenous penghasil senyawa antibakteri yang dapat dikembangkan lebih lanjut.

Abstract

Background: Coronary heart disease (CHD) remains a major cause of morbidity and mortality, and sustained lifestyle modification is essential for secondary prevention. The Indonesian Ministry of Health promotes CERDIK, a six-component healthy-lifestyle strategy comprising regular health checks, avoidance of cigarette smoke, regular physical activity, a healthy balanced diet, adequate rest, and stress management. Evidence regarding structured CERDIK education among patients with established CHD in hospital outpatient settings remains limited. Objective: This study evaluated the effect of CERDIK-based health promotion on knowledge, attitudes, and healthy lifestyle practices among patients with CHD. Methods: A quasi-experimental two-group pretest-posttest study was conducted in the cardiac outpatient clinic of Murni Teguh Hospital, Medan. Forty patients were recruited by purposive sampling and allocated to an intervention group (n=20) and a control group (n=20). The intervention group received structured CERDIK-based health education for four weeks, whereas the control group received routine education. Outcomes were measured before and after the intervention using questionnaires assessing knowledge, attitudes, and practices. Change scores were compared between groups using the Mann-Whitney U test. Results: After the intervention, 85% of participants in the intervention group had good knowledge, 85% had positive attitudes, and 45% had good practices; the corresponding proportions in the control group were 0%, 0%, and 10%, respectively. Mean change scores were substantially greater in the intervention group than in the control group for knowledge (25.75 vs 2.95), attitudes (27.30 vs 1.85), and practices (23.35 vs 1.85). Between-group differences were statistically significant for all outcomes (all p<0.001). Conclusion: Structured CERDIK-based health promotion significantly improved short-term knowledge, attitudes, and healthy lifestyle practices among patients with CHD. The intervention may be incorporated into outpatient secondary-prevention education, although larger randomized studies with longer follow-up are needed to assess sustained behavioral effects.
