Abstract

Background: Prescribing audits provide a basis for evaluating the appropriateness of antihyperlipidemic treatment in clinical practice. Objective: To describe prescribing patterns and assess documentation-based appropriateness at RSU Imelda Pekerja Indonesia, Medan. Methods: This descriptive study retrospectively reviewed medical records from March 2025 to February 2026. Data were collected in June 2026, and 51 patients were selected by purposive sampling. Patient, indication, drug, dose, and route appropriateness were assessed with reference to the 2021 Indonesian Society of Endocrinology (PERKENI) guidance. Data were summarized using frequencies, percentages, and exact 95% confidence intervals (CIs). Results: Women accounted for 32 patients (62.75%); the largest age group was 56–65 years (16; 31.37%). Simvastatin 20 mg, atorvastatin 20 mg, and fenofibrate 300 mg monotherapy were recorded in 26 (50.98%), 19 (37.25%), and four (7.84%) patients, respectively. Two patients (3.92%) received simvastatin 20 mg plus fenofibrate 100 mg. Statin monotherapy accounted for 88.24% of patients (95% CI, 76.13–95.56%). All 51 records were classified as appropriate for each criterion (100%; 95% CI, 93.02–100.00%). Conclusion: Statin monotherapy predominated, and no documented inappropriateness was identified within the five assessed criteria. These findings describe prescribing classifications, not comprehensive treatment safety or lipid-target attainment. Further evaluation should link prescribing decisions to individual cardiovascular risk and measured lipid outcomes.

Abstract

Introduction: Durian peel is a source of phytochemicals with antibacterial potential, and particle-size reduction may modify dispersion and measured in vitro activity. This study evaluated the operational minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of ethanol extract and a size-reduced ("nano extract") preparation of durian peel against Staphylococcus epidermidis. Methods: The ethanol extract was prepared by maceration with 96% ethanol; 30 g of the same extract was homogenized at 1,700 rpm for 1 h and ultrasonicated for 1 h. Particle size, phytochemical screening, broth absorbance at 600 nm, qualitative subculture, and disk diffusion were evaluated in triplicate (n=3). Inhibition-zone data were analyzed using one-way ANOVA. Results: The mean particle size was 645.72 nm. Operational MIC values were 6.25% for the ethanol extract and 0.625% for the nano extract, while no growth after subculture was observed at 50% and 5%, respectively. Mean inhibition zones ranged from 6.6-14.5 mm and 8.3-18.3 mm, with significant concentration-related differences within each series (p<0.001). Discussion and conclusion: The size-reduced preparation reached inhibitory endpoints at lower nominal concentrations within its test series. However, final concentrations were not analytically verified, and optical interference and qualitative bactericidal endpoints limit direct inference of intrinsic potency. These findings support in vitro antibacterial activity but do not establish a quantified increase in potency or a reduction in therapeutic dose

Abstract

Background: Hemodialysis adherence requires sustained engagement with a demanding treatment regimen. Objective: To examine factors associated with adherence at Murni Teguh Memorial Hospital, Medan. Methods: This analytical cross-sectional study included 172 purposively selected patients. Adherence was assessed using a modified six-item End-Stage Renal Disease Adherence Questionnaire. Associations were examined using chi-square tests and multivariable logistic regression. Results: Overall, 115 patients (66.9%) were adherent. Education, knowledge, and family support were associated with adherence (all p < 0.001), whereas age, sex, and dialysis duration were not (p = 0.866, 0.357, and 0.212, respectively). Adherence was 92.6% with good family support and 5.9% with poor support. Family support was the only significant adjusted term (Exp[B] = 416.36; 95% confidence interval [CI], 43.08–4024.31); sparse discordant observations and the wide interval limited precision. A post hoc unadjusted Firth analysis yielded an odds ratio of 164.10 (95% CI, 51.86–694.10). Conclusion: Family support was strongly associated with self-reported adherence. Family-centered counseling merits evaluation, but the cross-sectional design and imprecise estimates preclude causal interpretation.

Abstract

participation, yet routine monitoring remains inconsistent in many primary-care settings. KPSP Pro is a digital educational medium that presents age-appropriate developmental screening guidance derived from the Indonesian Kuesioner Pra Skrining Perkembangan (KPSP) framework. This quasi-experimental pretest-posttest study with a control group evaluated the effectiveness of KPSP Pro in strengthening mothers' roles in early detection and monitoring of growth and development among infants aged 0-24 months. Sixty mothers in the Pustu Pematang Lalang working area, Percut Sei Tuan District, Deli Serdang Regency, were analyzed, with 30 participants in the intervention group receiving KPSP Pro-based education and 30 in the control group receiving conventional education. Maternal-role scores increased from 49.27 +/- 6.68 to 82.53 +/- 13.51 in the intervention group and from 48.17 +/- 6.23 to 59.17 +/- 7.03 in the control group. Both within-group changes and the between-group difference in score change were statistically significant (p<0.001). These findings indicate that KPSP Pro-based digital education was associated with a markedly greater improvement in maternal participation in early growth and developmental monitoring than conventional education.

Abstract

Basil leaf extract is a promising botanical ingredient for antibacterial mouthwash development. This study formulated a mouthwash containing size-processed basil (Ocimum basilicum L.) leaf extract and evaluated its physicochemical characteristics and in vitro antibacterial activity against Streptococcus mutans and Escherichia coli. Dried leaf powder was macerated with 96% ethanol; the concentrated extract was subsequently homogenized and ultrasonicated, yielding a mean particle size of 424.9 nm before formulation. Mouthwashes containing 5%, 10%, and 15% w/v extract were assessed for organoleptic characteristics, pH, four-week visual stability, and antibacterial activity by disc diffusion (n = 3 per group). Mean inhibition-zone diameters for the 5%, 10%, and 15% formulations were 9.27, 11.63, and 16.33 mm against S. mutans and 9.77, 12.07, and 17.03 mm against E. coli, respectively. The extract-free vehicle produced no inhibition zone, whereas the commercial mouthwash comparator produced larger zones than the test formulations. Omnibus analyses identified differences among groups, and post-hoc analyses were used to characterize pairwise patterns. The 15% formulation produced the largest mean inhibition zones among the extract-containing mouthwashes and maintained its organoleptic characteristics during four weeks of observation. These findings support further formulation development, while longer-term stability, antibiofilm activity, and oral-mucosal safety require additional investigation.
