Abstract

Background: Antimicrobial resistance (AMR) is a critical global health crisis exacerbated by persistent misuse and non-adherence to antibiotic therapies. Because comprehensive evidence detailing the specific factors driving these behaviors remains scarce, this systematic review aimed to identify and synthesize the multifactorial drivers influencing patient adherence and misuse across diverse populations. Method: Following the PRISMA guidelines, a systematic literature search was conducted to screen cross-sectional studies assessing antibiotic adherence, self-medication, and misuse. Eighteen studies met the inclusion criteria for final analysis. The methodological quality of the included studies was independently evaluated by two reviewers utilizing the Joanna Briggs Institute (JBI) Critical Appraisal Checklist for Analytical Cross-Sectional Studies. Result: The review identified complex factors across five domains: sociodemographic, knowledge and beliefs, medication-related, behavioral, and the health system. Lower education, younger age, and financial constraints consistently predicted non-adherence. Knowledge gaps were prevalent, particularly the misconception that antibiotics are effective for viral infections. Therapy-related factors, such as frequent dosing (e.g., three times daily) and long treatment durations, significantly reduced adherence. Behaviorally, premature discontinuation of therapy upon symptom relief was the most common form of non-adherence, leading to the retention of leftover antibiotics. Furthermore, health system factors, including easy access to non-prescription antibiotics and the lack of a regular doctor-patient relationship, facilitated inappropriate use. Conclusion: Patient adherence to antibiotics is influenced by a complex interplay of socioeconomic, cognitive, and structural factors. Effective interventions must be multifaceted, combining public education to correct misconceptions, prescriber-led simplification of treatment regimens, and stricter enforcement of dispensing regulations.

Abstract

Background: Antibiotic resistance is a major global health crisis exacerbated by the misuse of and poor adherence to treatments. Evidence on the behavioral factors driving this misuse remains scarce, particularly in rapidly growing urban areas of developing nations. Therefore, this study evaluates the community’s knowledge, attitude, and practices (KAP) regarding antibiotic use in South Tangerang City, Indonesia, aiming to understand the interconnections between these factors and sociodemographic characteristics. Materials and Methods: A cross-sectional survey was conducted using convenience sampling to recruit participants from the Pondok Jagung and Serpong 2 Health Centers. Data were collected via a validated questionnaire. Statistical analyses included descriptive statistics, chi-square tests, multivariate logistic regression, and Spearman’s rho correlation to assess relationships between variables. Results: Among 110 respondents, education level was significantly associated with knowledge regarding antibiotics (p = 0.044). While initial tests indicated gender and employment affected attitudes, multivariate logistic regression confirmed that only gender remained a strong independent predictor (p = 0.019), with men exhibiting significantly lower odds of holding positive attitudes (OR = 0.237). Notably, good knowledge did not automatically translate to better attitudes or practices. Instead, a positive attitude showed a statistically significant but weak correlation with appropriate antibiotic use (r = 0.193, p = 0.043).  Conclusion: A statistically significant but weak positive correlation was found between attitudes and practices (r = 0.193, p = 0.043), suggesting that while attitudes may influence behavior, other unmeasured factors likely play substantial roles. These findings provide essential baseline data for local health centers to design targeted interventions.

Abstract

Introduction: Vitamin C (ascorbic acid) plays an important role in supporting the immune system; however, it has low stability and is prone to degradation during distribution and storage, which may affect product quality. Objective: This study aimed to determine the content and content uniformity of vitamin C in four commercial generic tablet brands in Indonesia as a quality control effort to ensure compliance with established standards. Methods: Vitamin C content determination was performed using the 2,6-Dichlorophenol Indophenol (DCPIP) titration method, which was verified through precision and accuracy in four tablet brands, followed by a content consistency among tablets. Result: The DCPIP titration method showed good precision, with an %RSD value of 0.81% (criterion <2%), and good accuracy with %recovery ranging from 98.17% to 99.14% (criterion 98.0% to 102.0%). The vitamin C content in the four brands range from 93.63% to 95.12% of the labeled claim, meeting the required range of 90.0% to 110.0%. The Acceptance Value (AV) obtained from the content uniformity test ranged from 5.49 to 6.11, below the acceptance limit (L1<15.0), indicating that all four brands met the quality requirements. Conclusion: The DCPIP titration method demonstrated food precision and accuracy, making it reliable for vitamin C content determination. All four commercial generic tablet brands tested met both content and content uniformity requirements, indicating good quality and compliance with the claims started on label.

Abstract

Cyproheptadine hydrochloride is an antihistamine drug widely available in tablet dosage forms; therefore, a validated analytical method is required to ensure product quality. This study aimed to develop and validate a Reversed-Phase High Performance Liquid Chromatography (RP-HPLC) method for the determination of cyproheptadine hydrochloride 4 mg in tablets according to ICH Q2(R2) and USP 2024 guidelines. The analysis was performed using an XBridge C18 column (2.1 mm × 50 mm; 3.5 µm) with an isocratic mobile phase of acetonitrile:0.1% acetic acid (60:40), a flow rate of 1.0 mL/min, an injection volume of 10 µL, and PDA detection at 285 nm. Validation parameters included specificity, linearity, accuracy, precision, robustness, LOD and LOQ. The method showed excellent linearity with R² of 0.997, recovery of 100.26%, precision with % RSD of 1.42%, LOD 0.0022% and LOQ of 0.0066%. Specificity testing showed no interfering peaks, while robustness was confirmed for a ±5% change in mobile phase composition (%RSD 1.26% for retention time and 1.97% for peak area) but not for a ±5% change in flow rate, where retention time gave %RSD of 2.14%. The procedure is fit for routine quality control assay of cyproheptadine hydrochloride tablets, with the stated limitation on flow-rate robustness.

Abstract

Inflammation is a central component of numerous pathological conditions, and the limitations of currently available anti-inflammatory therapies continue to motivate the search for structurally diverse therapeutic candidates. Mitragyna speciosa Korth. contains bioactive indole alkaloids, particularly mitragynine and 7-hydroxymitragynine, that have been associated with anti-inflammatory effects. This study evaluated the predicted binding affinity and ligand-protein interaction profiles of mitragynine and 7-hydroxymitragynine against four inflammation-related targets—cyclooxygenase-1 (COX-1), cyclooxygenase-2 (COX-2), interleukin-1 beta (IL-1β), and tumor necrosis factor-alpha (TNF-α)—using molecular docking. Protein structures were obtained from the Protein Data Bank (PDB IDs 1EQG, 3LN1, 5R87, and 3EWJ, respectively). Protein and ligand preparation and interaction visualization were performed using Discovery Studio Visualizer, docking was conducted using PLANTS, and protocol validation was assessed by redocking and root mean square deviation (RMSD) analysis in Molegro Molecular Viewer. All protocols met the prespecified RMSD acceptance criterion of ≤2.0 Å, with values of 1.57 Å for COX-1, 0.77 Å for COX-2, 1.36 Å for IL-1β, and 0.55 Å for TNF-α. Within each protein target, 7-hydroxymitragynine showed the most favorable predicted score among the tested alkaloids for COX-1 and COX-2 (−17.39 and −19.56 kcal/mol, respectively), whereas mitragynine showed the more favorable predicted score for IL-1β and TNF-α (−16.72 and −20.42 kcal/mol, respectively). These results indicate target-dependent binding behavior and provide a computational basis for further structural, pharmacokinetic, and experimental evaluation of both alkaloids as potential anti-inflammatory candidates.

Abstract

Organic acid profiling provides a chemical basis for characterizing fruit fermentation. This study evaluated lactic and acetic acid profiles in Classic Enzyme on fermentation days 7 and 14. A descriptive laboratory study used one batch containing 300 g of honey, 900 g of 14 fruit types, and 3,000 mL of boiled drinking water. Qualitative screening, sensory assessment by five panelists, and high-performance liquid chromatography with a C18 column and UV detection at 210 nm were performed. Six analytical determinations were available per sampling day. Mean lactic acid concentrations increased from 0.2216 ± 0.0191% on day 7 to 0.3178 ± 0.0561% on day 14, whereas acetic acid concentrations decreased from 2.9293 ± 0.0966% to 0.6376 ± 0.0145% (mean ± sample standard deviation). Mean taste scores on the study-specific eight-point scale were 5.6 and 6.4, respectively. The opposing acid profiles describe this fermentation batch but do not establish a causal effect of fermentation time. Results remain exploratory because the concentration basis, chromatographic specificity, and calibration coverage require verification. These findings provide preliminary information for further analytical and fermentation-process standardization, rather than evidence of product safety or health benefits.

Abstract

Amusium pleuronectes shell waste was investigated as a source of chitosan for a topical hydrogel with activity against Malassezia furfur. Chitosan was isolated by deproteinization, demineralization, depigmentation, and alkaline deacetylation, characterized by Fourier-transform infrared spectroscopy, and incorporated into hydrogels at nominal concentrations of 2%, 4%, and 6%, with a chitosan-free base as control. Physical properties were assessed over 21 days, and antifungal activity was evaluated by agar-well diffusion. The recovered material represented 24.30% of the initial shell mass, and its estimated degree of deacetylation was approximately 77%. All formulations retained their appearance without visible phase separation. Across formulations and observation times, pH ranged from 4.55 to 6.09, spread diameter from 5.1 to 5.8 cm, adhesion from 1.11 to 1.61 s, and apparent viscosity from 31,310 to 38,180 cP; pH, adhesion, and viscosity declined during storage. Mean inhibition zones were 11.57 ± 0.21, 13.33 ± 0.15, and 15.00 ± 0.10 mm for the 2%, 4%, and 6% formulations, respectively (three readings each). The base produced no inhibition halo, whereas the reported ketoconazole comparator value was 21.3 mm. The 6% formulation showed the greatest in vitro inhibition among the chitosan hydrogels, while the changes in physical properties indicate a need for further formulation optimization.

Abstract

Postharvest deterioration of red bell pepper is closely associated with water loss, respiration, and microbial activity. Chitosan is a biodegradable, biocompatible polysaccharide with film-forming and antimicrobial properties that can be developed as an edible coating. This study aimed to isolate chitosan from roasted pulut cockle shells (Atrina pectinata) and evaluate its application as an edible coating for red bell pepper (Capsicum annuum L.). Chitosan was obtained through sequential deproteinization, demineralization, depigmentation, and deacetylation. The isolate was characterized by organoleptic properties, yield, moisture and ash contents, solubility, degree of deacetylation, and FTIR spectroscopy. Antibacterial activity against Staphylococcus aureus was evaluated by disk diffusion at chitosan concentrations of 3%, 6%, and 9%. Coating solutions containing 0%, 3%, 6%, and 9% chitosan were applied by dipping, and red bell peppers were evaluated after 8 days of storage. Triplicate data available in the thesis were reanalyzed descriptively and, where appropriate, by one-way ANOVA. The isolated chitosan was a white, odorless fine powder, soluble in 2% acetic acid, with a yield of 63.46%, moisture content of 4.26%, ash content of 3.84%, and a degree of deacetylation of 76.68%. Mean inhibition zones were 20.73 ± 1.45, 22.73 ± 2.75, and 23.83 ± 1.85 mm for 3%, 6%, and 9% chitosan, respectively; the difference among chitosan concentrations was not statistically significant (one-way ANOVA, p=0.260). After 8 days, weight loss was 19.20 ± 4.98% in the uncoated control and 5.59 ± 2.33% in the 9% chitosan group (overall ANOVA, p=0.008). The 9% coating group had a pH of 6.28 ± 0.15 and vitamin C content of 463.47 ± 56.58 mg/100 g. These findings support the potential of shell-derived chitosan as a postharvest coating, while several source-data units require verification before final publication.

Abstract

Background: Red cabbage sauerkraut (Brassica oleracea L.) is a fermented vegetable matrix that supports the growth of lactic acid bacteria (LAB), some of which produce bacteriocins with potential as natural food biopreservatives. Objective: This study aimed to isolate and phenotypically characterize LAB from red cabbage sauerkraut and evaluate the antibacterial activity and physicochemical stability of the partially purified bacteriocin against Staphylococcus aureus and Escherichia coli. Methods: LAB were isolated on MRS agar, characterized by colony and cell morphology, Gram staining, catalase activity, and motility, and screened for antibacterial activity. Bacteriocin from the selected isolate was partially purified by ammonium sulfate precipitation and dialysis. Stability was evaluated across pH 2–11, temperatures of 30–121 °C, pepsin exposure, Tween 20 and Tween 80, and UV-C irradiation. Antibacterial activity was assessed by agar diffusion. Results: Three presumptive LAB isolates (BAL1, BAL2, and BAL3) were obtained, with BAL1 showing the greatest antibacterial activity. Untreated bacteriocin produced inhibition zones of 11.2 mm against S. aureus and 12.8 mm against E. coli. Activity was retained most strongly at pH 3–5 and 30–40 °C, decreased after pepsin and UV-C exposure, and increased in the presence of Tween 20 or Tween 80. Conclusion: BAL1 from red cabbage sauerkraut produces a proteinaceous antibacterial substance with stability under acidic and moderate-temperature conditions, supporting its further characterization as a candidate food biopreservative.
