Abstract

Pendahuluan: Keluarga berencana melalui penggunaan kontrasepsi modern tetap menjadi strategi kesehatan masyarakat yang penting untuk meningkatkan kesehatan ibu dan anak sekaligus mengendalikan pertumbuhan penduduk. Meskipun Indonesia telah mencapai cakupan kontrasepsi yang relatif tinggi secara nasional, kesenjangan antarwilayah masih terjadi; hal ini mencerminkan perbedaan dalam pelaksanaan program, aksesibilitas layanan, dan kapasitas sistem kesehatan daerah. Oleh karena itu, memahami tren regional sangat penting untuk mengevaluasi kinerja program dan mendukung perumusan kebijakan berbasis bukti. Studi ini bertujuan untuk menganalisis tren penggunaan kontrasepsi di kalangan wanita kawin usia subur (15–49 tahun) di Kabupaten Tulang Bawang, Tulang Bawang Barat, dan Mesuji, Provinsi Lampung, selama periode 2020–2024. Metode: Studi deskriptif kuantitatif ini menggunakan data sekunder dari publikasi *Lampung dalam Angka 2025* yang diterbitkan oleh Badan Pusat Statistik (BPS) Provinsi Lampung. Indikator utama yang dianalisis adalah persentase wanita berstatus kawin berusia 15–49 tahun yang menggunakan kontrasepsi. Analisis komparatif dilakukan untuk mengkaji perbedaan besaran dan pola perubahan di ketiga kabupaten tersebut. Hasil: Temuan menunjukkan adanya tren regional yang beragam. Tulang Bawang menunjukkan peningkatan paling konsisten, naik dari 63,36% pada tahun 2020 menjadi 68,71% pada tahun 2024, yang mencerminkan peningkatan kumulatif sebesar 5,35 poin persentase. Tulang Bawang Barat menunjukkan tren yang fluktuatif namun mencatat peningkatan keseluruhan sebesar 4,08 poin persentase. Mesuji mengalami penurunan tajam pada tahun 2021, diikuti oleh pemulihan bertahap hingga mencapai 69,41% pada tahun 2024, sedikit di atas angka dasar tahun 2020. Kesimpulan: Temuan ini menyoroti perlunya strategi yang berkelanjutan dan spesifik sesuai konteks wilayah untuk mendorong peningkatan penggunaan kontrasepsi secara merata di berbagai daerah.   Introduction: Family planning through modern contraceptive use remains an important public health strategy for improving maternal and child health outcomes while managing population growth. Although Indonesia has achieved relatively high contraceptive coverage nationally, disparities persist across regions, reflecting differences in program implementation, service accessibility, and local health system capacity. Therefore, understanding regional trends is essential for evaluating program performance and supporting evidence-based policymaking. This study aimed to analyze trends in contraceptive use among married women of reproductive age (15–49 years) in Tulang Bawang, Tulang Bawang Barat, and Mesuji, Lampung Province, from 2020 to 2024. Method: This quantitative descriptive study used secondary data from Lampung in Figures 2025, published by the Central Statistics Agency (BPS) of Lampung Province. The primary indicator analyzed was the percentage of currently married women aged 15–49 years who were using contraception. Comparative analysis was conducted to examine differences in the magnitude and patterns of change across the three regencies. Results: The findings indicated heterogeneous regional trends. Tulang Bawang showed the most consistent increase, rising from 63.36% in 2020 to 68.71% in 2024, representing a cumulative increase of 5.35 percentage points. Tulang Bawang Barat showed fluctuating trends but an overall increase of 4.08 percentage points. Mesuji experienced a sharp decline in 2021, followed by gradual recovery, reaching 69.41% in 2024, slightly above its 2020 baseline. Conclusion: These findings highlight the need for context-specific and sustainable strategies to promote equitable increases in contraceptive use across regions.

Abstract

Pendahuluan: Penggunaan pestisida organofosfat (OP) di sektor pertanian menimbulkan ancaman serius bagi kesehatan lingkungan dan manusia. Artikel tinjauan pustaka ini bertujuan untuk mengkaji efek toksikologis dari paparan OP, mengevaluasi efektivitas metode pengendalian yang telah diterapkan, mengidentifikasi kesenjangan penelitian, serta menilai relevansinya dengan kondisi pengelolaan pertanian di Provinsi Lampung. Metode: Tinjauan ini dilakukan melalui kajian pustaka terhadap 15 artikel ilmiah yang diterbitkan antara tahun 2021 dan 2026, yang diperoleh dari sumber-sumber ilmiah yang relevan. Hasil: Temuan menunjukkan bahwa paparan OP berkaitan dengan penurunan aktivitas enzim kolinesterase pada petani, gangguan mikrobioma usus, efek neurotoksik, serta kontaminasi tanah dan air tanah. Metode pengendalian, seperti penggunaan alat pelindung diri (APD) dan pencucian produk pangan, belum sepenuhnya efektif akibat rendahnya tingkat kepatuhan dan keberadaan produk transformasi OP di lingkungan yang tetap bersifat toksik. Kesenjangan penelitian meliputi terbatasnya data longitudinal mengenai paparan kronis dosis rendah dan kurangnya studi mengenai produk transformasi OP di tingkat lokal. Kesimpulan: Temuan ini relevan bagi Provinsi Lampung karena residu OP masih terdeteksi pada produk pertanian lokal, sehingga menyoroti perlunya peningkatan edukasi mengenai penanganan keracunan pestisida. Oleh karena itu, pemantauan rutin residu pestisida dan biomonitoring perlu diperkuat guna mendukung pengelolaan pestisida yang lebih aman dan berkelanjutan. Namun, interpretasi mengenai kondisi di Lampung masih terbatas karena sebagian besar literatur yang ditinjau berasal dari luar Lampung, sehingga menekankan perlunya studi multisitus dengan cakupan geografis yang lebih luas.   Introduction: The use of organophosphate (OP) pesticides in the agricultural sector poses serious threats to environmental and human health. This literature review article aims to examine the toxicological effects of OP exposure, evaluate the effectiveness of control methods that have been implemented, identify research gaps, and assess their relevance to agricultural management conditions in Lampung Province. Method: The review was conducted through a literature review of 15 scientific articles published between 2021 and 2026, obtained from relevant scientific sources. Results: The findings indicate that OP exposure is associated with decreased cholinesterase enzyme activity in farmers, gut microbiome disturbances, neurotoxic effects, and contamination of soil and groundwater. Control methods, such as the use of personal protective equipment (PPE) and washing of food products, have not been fully effective due to low compliance and the presence of OP transformation products in the environment that remain toxic. Research gaps include limited longitudinal data on chronic low-dose exposure and a lack of studies on OP transformation products at the local level. Conclusion: These findings are relevant to Lampung Province because OP residues have still been detected in local agricultural products, highlighting the need for improved education on pesticide poisoning management. Therefore, routine pesticide residue monitoring and biomonitoring should be strengthened to support safer and more sustainable pesticide management. However, the interpretation of conditions in Lampung remains limited because most of the reviewed literature originated from outside Lampung, emphasizing the need for multisite studies with broader geographical coverage.

Abstract

Pendahuluan: Hipertensi merupakan penyebab kematian nomor satu di dunia. Data dari Organisasi Kesehatan Dunia (WHO) tahun 2023 menunjukkan bahwa sekitar 1,28 miliar orang di seluruh dunia menderita hipertensi. Jumlah kasus hipertensi di Indonesia mencapai 63.309.620 jiwa. Sementara itu, di Riau, prevalensi hipertensi tercatat sebesar 33,1% dan di Kota Pekanbaru sebesar 21,1%. Gejala yang paling umum menyertai hipertensi adalah sakit kepala. Penerapan terapi pijat punggung slow stroke (SSBM) terbukti efektif untuk mengatasi sakit kepala pada pasien hipertensi. Penelitian ini bertujuan untuk mengetahui pengaruh terapi pijat punggung slow stroke terhadap penurunan intensitas sakit kepala pada pasien hipertensi. Metode: Penelitian ini menggunakan metode kuantitatif dengan desain penelitian kuasi-eksperimen dan rancangan one-group pre-test-post-test. Sampel penelitian berjumlah 29 orang yang diambil menggunakan teknik non-probability sampling. Instrumen yang digunakan adalah lembar observasi dengan skala NRS (Numeric Rating Scale). Hasil: Hasil uji Wilcoxon menunjukkan nilai p = 0,000, yang berarti terdapat pengaruh terapi pijat punggung slow stroke terhadap penurunan intensitas sakit kepala pada pasien hipertensi. Kesimpulan: Berdasarkan temuan penelitian, dapat disimpulkan bahwa sebagian besar responden menderita hipertensi stadium 1 (86,2%). Sebelum menerima terapi SSBM, sebagian besar responden mengalami sakit kepala tingkat sedang (75,9%), sedangkan setelah menerima terapi SSBM, sebagian besar mengalami sakit kepala tingkat ringan (69,0%). Hasil uji Wilcoxon menunjukkan nilai p sebesar 0,000 (p < 0,05), yang mengindikasikan bahwa terapi SSBM berpengaruh terhadap penurunan intensitas sakit kepala pada pasien hipertensi di Puskesmas Rejosari, Pekanbaru.   Introduction: Hypertension is the number one cause of death worldwide. Data from the World Health Organization (WHO) in 2023 showed that approximately 1.28 billion people worldwide suffer from hypertension. The number of cases of hypertension in Indonesia reached 63,309,620. Meanwhile, in Riau, the prevalence of hypertension was 33.1% and in Pekanbaru City, it was 21.1%. The most common symptom accompanying hypertension is headache. The application of slow stroke back massage therapy is effective for treating headaches in patients with hypertension. This study aims to determine the effect of slow stroke back massage therapy on reducing the intensity of headaches in patients with hypertension. Method: This study used a quantitative method using a quasi-experimental research design with a one-group pre-test-post-test research design. The sample obtained was 29 people who were taken using a nonprobability sampling technique. The instrument used was an observation sheet with an NRS (Numeric Rating Scale). Results: The results of the Wilcoxon test obtained a p-value = 0.000, which means there is an effect of slow stroke back massage therapy on reducing the intensity of headaches in patients with hypertension. Conclusion: Based on the research findings, it can be concluded that the majority of respondents suffered from stage 1 hypertension (86.2%). Prior to receiving SSBM therapy, the majority of respondents experienced moderate headaches (75.9%), whereas after receiving SSBM therapy, the majority experienced mild headaches (69.0%). The Wilcoxon test results showed a p-value of 0.000 (p < 0.05), indicating that SSBM therapy has an effect on reducing headache intensity in hypertensive patients at the Rejosari Community Health Center in Pekanbaru.
