Abstract

We reported three cases of young adults with typical and atypical neurological manifestations, supported by positive brain and spinal imaging findings, along with cerebrospinal fluid (CSF) analysis consistent with McDonald's criteria 2017 for multiple sclerosis (MS). Clinical manifestations varied with optic neuritis, myelopathy, vertigo, ataxia, and urinating problems in relapsing-remitting patterns in two cases and progressive in one other. Brain MRI showed T2-hyperintense lesion in cortical/juxtacortical, periventricular, and infratentorial areas, coupled with a spinal cord lesion shown in spinal MRI. The CSF analysis for oligoclonal band type II was positive in all cases. All three patients received relapse therapy with post-therapy clinical improvement, followed by long-term therapy with DMD (disease-modifying drugs) fingolimod, interferon β-1a, and one patient was treated with mycophenolate mofetil. Careful clinical observation was maintained to evaluate different relapse rates.

Abstract

Introduction: Stroke is the biggest cause of death and disability in the world. Ischemic stroke is the most common type of stroke caused by the formation of atherosclerosis in the brain. Abnormal levels of lipids in the body, especially low density lipoprotein (LDL), cause the formation of atherosclerotic plaques that also affect the functional outcome of ischemic stroke. Aim: The purpose of this study was to determine the relationship between LDL levels and the functional outcome of ischemic stroke at RSUDZA Banda Aceh. Methods: This research is a cross sectional study using purposive sampling which was analyzed by Fisher's test. Samples were 40 ischemic stroke patients in the RSUDZA neurosurgery ward who met the inclusion and exclusion criteria. Data were obtained from medical records to see LDL levels and interviews to assess the patient's functional outcome using the modified Rankin Scale score. Results: There were 32 patients with abnormal LDL levels and all of them had poor functional outcome (100%) with a p value of 0.000 (p<0.05). Discussion: The results of this study indicate that there is a significant relationship between LDL levels and the functional outcome of ischemic stroke in RSUDZA. Abstract

PARADOXICAL REACTION IN A DEFINITIVE CASE OF TUBERCULOUS MENINGOENCEPHALITIS ABSTRACT Tuberculous Meningoencephalitis is an infection of the central nervous system (CNS) that is characterized by fever, headaches, nuchal rigidity, focal neurological deficit and altered consciousness. This is a case report illustrating the clinical progress of a 22-year old woman with HIV-negative status diagnosed with definitive tuberculous meningoencephalitis with a paradoxical reaction, which is a clinical syndrome where the patient experiences clinical deterioration after a phase of clinical improvement; syndrome of inappropriate antidiuretic hormone secretion (SIADH) and hydrocephalus, which are two relatively common complications in cases of tuberculous meningitis, which pose as challenges in clinical management of such cases. Ventriculoperitoneal (VP) shunt is an invasive treatment option for the management of hydrocephalus is cases of central nervous system infections that do not respond adequately to pharmacologic therapy. Abstract

ABSTRACT Stroke is leading cause of disability in Indonesia. Approximately 80% of stroke patients experience disabilities in their upper extremities. In recent years, immersive Virtual Reality (VR) technology has been used as a promising modality for neurorestoration. VR-based neurorestoration can induce neuroplasticity, Mirror-Neuron System (MNS) and Brain Reward System, leading to contrasted improvements in the upper extremity after stroke. This review prompts systematic understanding of the mechanisms of VR-based stroke neurorestoration and summarizes the emerging evidence for ongoing innovation of VR systems and application in stroke neurorestoration. Abstract

Introduction: Vertebrae are the most common sites for metastases, and spinal cord compression can be an early manifestation of malignancy. The incidence of spinal cord compression tends to increase as the survival rate of cancer patients increases. We report a case of spastic paraparesis in suspected metastatic spinal cord compression. Case presentation: A 62-year-old male presented to the emergency room complaining of weakness in lower limbs and lower back pain that began one month prior to hospital admission and progressively worsened. The patient appeared moderately ill and completely conscious, with a body temperature of 36.4°C, a pulse rate of 89 beats per minute, a respiratory rate of 18 breaths per minute, oxygen saturation of 99%, a blood pressure of 144/81 mmHg, and a pain scale of 7. Neurological examination demonstrated spastic paraparesis with loss of posture and vibration sensation in both lower extremities. There were masses in the bodies of the fourth to tenth thoracic vertebrae, along with paravertebral masses and destruction of the surrounding bones, as well as intracranial and extracranial medullary invasions with lytic lesions in the second lumbar vertebra. Consolidation was also detected on radiological testing in the left lung's apicoposterior region, leading to the appearance of a pulmonary mass on computed tomography (CT) thoracolumbar myelography. Ketorolac, methylprednisolone, and citicoline were administered as therapy. Within a few days, the patient demonstrated significant relief in pain. Conclusion: Spinal cord compression due to metastases is an oncological emergency where spastic paraparesis and proprioception deficits are included as the neurologic consequences.

Abstract

Introduction: Coronavirus Disease 2019 (COVID-19) is a global disease with a high mortality rate. In Indonesia, the mortality rate has significantly increased, accompanied by various manifestations, including neurological symptoms. Aim: We aim to assess the clinical characteristics, including common symptoms and neurological manifestations, of COVID-19 patients at the COVID-19 National Emergency Hospital, Wisma Atlet Kemayoran. Methods: This study used a descriptive observational method with a cross-sectional design at the COVID-19 National Emergency Hospital, Wisma Atlet Kemayoran, Indonesia. Data were collected from June 2021 to February 2022, focusing on patients’ age, sex, comorbidities, severity degree, common symptoms, and neurological symptoms. Results: Among 599 patients, 63.8% were aged below 50, 53.1% were female, and 34.8% had hypertension. Mild symptoms were noted in 71.1% of cases, primarily cough (38.7%), while anosmia prevailed as the most common neurological symptom (33.2%). Chi-Square analysis highlighted significant associations between COVID-19 severity and variables like age, sex, pregnancy, comorbidities, and symptoms. Cough, runny nose, shortness of breath, fever, and appetite loss were linked to severity. Neurological symptoms, including anosmia, headache, and loss of consciousness, correlated with COVID-19 severity. Severe cases exhibited a heightened likelihood of central nervous system symptoms (p < 0.001) but a reduced chance of peripheral nervous system symptoms (p < 0.001). Discussions: In summary, age, sex, comorbidities, and various symptoms demonstrated significant relationships with COVID-19 severity, impacting both nervous system categories. The findings indicate that COVID-19 leads to diverse clinical presentations, ranging from mild to moderate, and contributes to numerous deaths. Abstract

Introduction: Early detection and prompt treatment in Parkinson’s Disease (PD) can improve patients’ quality of life. The complexity of PD requires multidisciplinary team management. Nurses play roles in explaining nursing diagnosis and interventions such as motor function and fall risk, dysautonomia, disorder of communication and sleep, adherence to treatment, and advance stage PD treatment. Aim: To find out the description of the knowledge of outpatient and emergency department nurses regarding Parkinson's Disease at Ciawi Regional General Hospital. Methods: Observational descriptive study with a cross-sectional design using a questionnaire that has been tested for validity and reliability. Results: The majority of respondents are aged 30-40 years (60.4%), with educational level Diploma-3 (78.2%). About half of them have interacted with patients with PD (56.4%), but most of them have never received education about PD (62.4%). Majority level of knowledge about PD is sufficient (40.6%). The level of sufficient knowledge was obtained from respondents with educational level Diploma-3, aged 30-40 years, work experience > 10 years, and have interaction with patients with PD. Discussion: The level of knowledge of nurses at Ciawi Regional Hospital was mostly at 41 (40.6%), followed by a good level of knowledge at 32 (31.7%), and less at 28 (27.7%). This study is the first in Indonesia, and shows the need to increase the knowledge of PD among nurses as part of a multidisciplinary team. Education programs may have roles in increasing the level of knowledge regarding PD. Keywords: Knowledge, Multidisciplinary, Nurse, Parkinson’s Disease.

Abstract

Dalam rangka World Stroke Day yang diperingati setiap tanggal 29 Oktober, editorial kali ini akan membahas terkait stroke. Diperingatinya hari stroke sedunia ini dimaksudkan untuk meningkatkan kewaspadaan masyarakat, baik medis maupun non-medis, terhadap stroke dan bahaya yang mengancamnya. Sudah kita ketahui bahwa stroke merupakan penyakit tidak menular yang angka kejadiannya terus bertambah serta menjadi penyebab ketiga kecacatan dan penyebab kedua kematian secara global menurut data satu tahun terakhir. Oleh karena itu, kewaspadaan terus ditingkatkan dalam upaya pencegahan stroke di tingkat komunitas dengan kampanye hidup sehat dengan Gerakan Masyarakat Sehat (Germas), upaya pengenalan dini gejala stroke dengan kampanye jargon FAST (Face drooping, Arm weakness, Speech disturbance, Time to call help) dan SeGeRa Ke RS (Senyum tidak simetris, Gerak separuh anggota tubuh melemah, bicaRa pelo/sulit, Kebas sesisi tubuh, Rabun pandangan, dan Sakit kepala hebat), upaya penanganan stroke secara tepat dan cepat di tingkat Rumah Sakit dengan penggalakan code stroke, upaya monitoring stroke secara intensif selama perawatan akut dengan adanya tim perawatan stroke di unit stroke, upaya rehabilitasi pasien pasca-stroke dengan menggalakkan program neurorestorasi pasca-stroke, dan upaya prevensi sekunder pasien pasca-stroke jangka panjang untuk mencegah stroke ulang dan komplikasi.

Abstract

Penetrating brain injury differs markedly from blunt injury in dural tears and the transmission of kinetic energy to the surrounding structures along the penetrating tract. Unique clinical characteristics of penetrating brain injury (PBI) certainly require a case-by-case approach, however there are some principles that can be applied in almost every case and need to be considered to improve the outcomes. We present three cases of PBI with different mechanisms and outcomes. Two of the cases presented with non-missile PBI, while the third case presented with missile PBI. We also present a review of current literature highlighting various pathomechanisms, clinical implications and key approaches in managing PBI, particularly in resource-limited settings.

Abstract

Epilepsy is a chronic disorder of the brain characterized by recurrent seizures. In Indonesia, epilepsy is still often seen as a curse or possessed by an evil spirit, which in turn creates a negative stigma against epilepsy. This study aims to look at the stigma of epilepsy from the public and people with epilepsy (PWE). Using a descriptive method through questionnaires distributed to the Tarumanagara University area and to the PWE community for the period December 2022 - February 2023. With PWE criteria, students and employees at Tarumanagara University who willing to fill out the questionnaire. A total of 108 respondents from ordinary people and 57 PWE respondents participated in filling out the questionnaire. In terms of friendship: the general public wants to be friends with people with epilepsy (80.4%), while PWE are afraid of being shunned by friends and the surrounding environment (41.4%). In terms of education: the general public considers epilepsy can affect a sufferer's education (24.3%), while PWE find it difficult to reach their goals where epilepsy can interfere with a person's cognitive impairment (27.0%). In the field of work: ordinary people are willing to employ a person with epilepsy (56.1%), while PWE think that it is difficult for people with epilepsy to find work (45.0%). Based on this description, the stigma of epilepsy in society does not hinder PWE from socializing and working, but the stigma within PWE hinders PWE themselves from socializing and working. But with regard to education, there are not too many differences regarding education in PWE and how PWE achieves their goals. This is what seems to encourage many people with epilepsy who prefer to work as entrepreneurs rather than work as employees because they find it difficult to find work.
