Abstract

Introduction: Low back pain (LBP) is a common health issueexperienced by nearly 80% of people throughout their lives. Thiscondition often recurs and can become chronic low back pain (CLBP) ifit lasts for more than 3 months, with a prevalence of approximately 23%and causing disability in about 11-12% of the population. CLBP is oftenassociated with depression, which can worsen pain intensity, function, and patient recovery. Purpose: This study aims to analyze therelationship between depression and CLBP, as well as the factors thatinfluence it. Methods: The study uses a cross-sectional analyticalobservation method with consecutive sampling of 170 respondents. Datawere collected through interviews and questionnaires, including painintensity measurement using the Numeric Rating Scale, depression levelsusing the Beck Depression Inventory-II, and disability levels using theOswestry Disability Index. Data collection was conducted at SumberWaras Hospital and dr. Mintohardjo Naval Hospital. Results: The studyresults show the highest prevalence of LBP in individuals over 65 yearsold (47.65%), and the majority of respondents were female (70.59%).There is a significant relationship between pain intensity and depression(p < 0.001) and between disability and depression (p < 0.001). Painduration was not significantly associated with depression (p = 0.211).Conclusion: In conclusion, pain intensity and disability level aresignificantly associated with the incidence of depression in CLBPpatients.

Abstract

Introduction: Subcutaneous endometriosis is a rare condition with endometrial tissue localized outside the pelvis and often mimics general endometriosis. The novelty of this study is an in-depth understanding of the specific risk factors associated with subcutaneous endometriosis after cesarean section and how optimal therapeutic approaches can provide better outcomes in patients with a history of endometriosis. This study aims to explore the clinical presentation, outcomes, and managerial approaches to this condition, as well as provide evidencebased recommendations for its management. Case Illustration: This study involved seven patients at Cipto Mangunkusumo Hospital (January 2022–December 2024). Data collected included demographics, age, BMI, initial symptoms, time of symptom onset, history of cesarean section, history of endometriosis, complications, nodule size, therapeutic approaches, and clinical outcomes. Discussion: Of the seven patients, three underwent one cesarean section, two underwent two operations, and two underwent more than two cesarean sections. A history of diffuse adenomyosis was found in three patients. Subcutaneous nodules located mainly in the cesarean scar, had highly variable sizes and caused significant pain (VAS 5–6). Dienogest therapy was given to three patients and significantly reduced pain. Conclusion:Subcutaneous endometriosis is often associated with a history of abdominal surgery, especially cesarean section. Extrapelvic location can be mistaken for keloid or fibroma, which hinders timely diagnosis. The novelty of this study lies in emphasizing the importance of multidisciplinary collaboration in the management of subcutaneous endometriosis, especially involving plastic surgeons to improve aesthetic results and prevent recurrence. Treatment involving hormonal therapy or surgery, based on the size and location of the nodule, is essential for optimal results.

Abstract

Introduction: Ramsay-Hunt syndrome is a complication of the varicellazoster virus that causes inflammation of the geniculate ganglion cranialnerve VII. This disease is a rare case in children. This case report aims tohighlight the importance of diagnosis, early examination, andappropriate therapy to prevent complications and poor prognosis. CaseIllustration: A 9 year-old girl presented with clear fluid-filled blisters inher left ear for four days before going to the hospital. Two days after thelesions came out, she complained of her face feeling tilted to the right, difficulty close her left eye, and the corner of her mouth being moredrawn towards the normal side. The left auricular helix fossa regionshowed a rash of multiple vesicles in groups containing clear fluid withan erythematous skin base, distributed unilaterally according to thedermatome. The patient was diagnosed with Ramsay-Hunt syndrome. She was treated with a combination of oral acyclovir andmethylprednisolone injections. Discussion: Antiviral therapy within 24–72 hours after the onset of the rash is very effective for healing of the rashand reducing the duration of pain. Corticosteroid as an antiinflammatory can reduce peripheral nerve damage and edema.Combination of corticosteroid with acyclovir for 7 days in herpes zosteraccompanied by facial paralysis is very effective in significantly reducingacute pain and reducing the risk of complications. Conclusion:The exactdiagnosis and combination of antiviral with corticosteroid therapy inchildren can be beneficial in reducing the duration of lesions and pain aswell as the risk of complications.

Abstract

Introduction: An unhealthy lifestyle among medical students maycontribute to inflammation, reduced vascular elasticity, andincreased blood pressure. However, cardiovascular risk in theseyoung adults remains less explored compared to hypertensivepatients. Purpose: This study compares inflammatory markers andvascular elasticity in prehypertensive and normotensive medicalstudents. Methods: A case-control study utilizing simple randomsampling was conducted in the medical student population, included54 prehypertensive or normotensive subjects with no history ofhematological disorders, autoimmune diseases, cancer, diabetesmellitus, or cardiovascular medication use. Vascular elasticity, blood pressure, and inflammatory markers were assessed using anaccelerated photoplethysmograph, a digital sphygmomanometer, and a hematology analyzer. Results: A total of 25 subjects (46.3 %)had suboptimal vascular elasticity, 30 (55.6%) had a highneutrophil-to-lymphocyte ratio (NLR), and 40 (74.1%) exhibited alow-risk platelet-to-lymphocyte ratio (PLR). The Chi-square testrevealed a significant difference in NLR between prehypertensiveand normotensive students (p = 0.000; OR = 7.0; CI = 2.3–20.7),while no significant difference was observed in PLR (p = 0.525).Vascular elasticity also differed significantly between groups (p =0.000; OR = 5.2; CI = 2.0-13.2). Conclusion: Prehypertensivesubjects had higher NLR, and those with suboptimal vascularelasticity had higher prehypertension risk. These findings highlightthe need for cardiovascular risk assessment in young adults.

Abstract

Introduction: Mild Cognitive Impairment (MCI) is a condition characterized by cognitive decline that does not interfere with daily activities but increases the risk of progressing to Alzheimer's Dementia (AD). Early detection of MCI progression to AD is crucial for early intervention. Purpose: The purpose of this meta-analysis to evaluate the performance of Convolutional Neural Networks (CNNs), an artificial intelligence used to analyze complex data such as images, in predicting the conversion of MCI to AD using MRI data. Methods: A meta-analysis was conducted following PRISMA guidelines, utilizing articles from PubMed and Wiley Online Library. Inclusion criteria focused on studies that used CNN in conjunction with MRI to diagnose MCI. A total of 39 articles with 40 comparative studies were analyzed. Results: The CNN showed an average accuracy of 0.7910 (p < 0.0001), sensitivity of 0.7698, specificity of 0.8467, and an Area Under the Curve (AUC) of 0.8063. High heterogeneity (I² = 90.90%) indicated significant variation across studies. Sub meta-analysis revealed consistent performance despite the heterogeneity. Conclusion: CNN is potentially useful for predicting the progression of MCI to AD. Further research is needed to address heterogeneity, improve algorithms, expand datasets, and include more diverse populations.

Abstract

Introduction: Intussusception is a rare condition in adults. It can cause ischemic necrosis, perforation, and peritonitis. Objective: This case report highlights the clinical presentation, diagnosis, and management of bowel obstruction due to intussusception in an adult. Case Illustration: A 43-year-old female patient presented with watery stools mixed with blackish blood, abdominal pain, vomiting, and decreased appetite. Abdominal examination revealed mild distension, reduced bowel sounds, generalized tenderness, and a firm, mobile mass (10 × 10 × 5 cm) in the right lower quadrant. Digital rectal examination showed a dilated rectal ampulla, extra luminal protrusion, and blackish blood-stained feces. Imaging studies confirmed a high-level obstruction and ileocecal intussusception with bowel wall edema. The patient was diagnosed with partial bowel obstruction. Discussion: This unusual case involves a localized tumor in the ascending colon acting as a lead point, leading to ischemia and mucosal necrosis. Accurate diagnosis in adults relies on timely imaging examinations. While plain radiography (BNO) may show signs of intestinal obstruction, its sensitivity is limited. Initial management focuses on stabilizing the patient’s condition by using a gastric tube for decompression, providing intravenous fluids for rehydration, administering analgesics and antibiotics, and, if appropriate, performing a barium or air enema to help reposition the affected segment. Prompt management is essential to prevent serious complications. Conclusion: Adult intussusception is rare and requires diagnosis through history, physical examination, and radiologic studies. Management involves initial medical stabilization and surgical intervention.

Abstract

Background: As the elderly population increases, the incidence of osteoarthritis (OA) also rises. In Indonesia, the prevalence of OA is 15.5% in men and 12.7% in women. Research on OA in Indonesia remains limited, particularly regarding population-based studies. Purpose: This study aimed to provide an overview of knee OA and its risk factors among older adults living in Pusaka Kebon Jeruk, West Jakarta. Methods: This cross-sectional study involved 100 respondents. Data were collected through interviews using questionnaires. Bivariate analysis was conducted using chi-square tests, and multivariate analysis was performed using multiple logistic regression (p<0.05). Results: The study found that 28% of the elderly had knee OA, with the majority aged 60-74 years (75%), female (71%), unemployed (82%), educated for 12 years or more (90%), and married (55%). Bivariate analysis identified waist circumference (p<0.001) and depression (p=0.032) as variables significantly associated with knee OA. Multivariate analysis indicated that waist circumference was the most significant factor affecting knee OA, with an odds ratio (OR) of 24.239. The prevalence of knee OA requires serious attention from the government and healthcare providers. Conclusion: Knee OA is significantly related to waist circumference and depression, with waist circumference being the most critical factor. Central obesity is associated with a higher risk of knee OA. Therefore, it is essential to prevent central obesity through a balanced diet and regular exercise.

Abstract

Introduction: Sleep quality, organizational activities, and commuting distance may influence students' academic performance. Poor sleep reduces concentration, organizational involvement consumes study time, and long commuting distances increase fatigue, all of which can ultimately affect students' academic achievement.  Purpose: This study aims to analyze the relationship between sleep quality, organizational activities, commuting distance, and the Grade Point Average (GPA) of the 2021 cohort of medical students at UPN Veteran Jakarta.  Methods: A cross-sectional study was conducted with 118 respondents selected through simple random sampling. Data were collected using the Pittsburgh Sleep Quality Index (PSQI) questionnaire, along with information on organizational participation, commuting distance, and GPA. Data was analyzed using the Spearman test. Results: Most respondents had good sleep quality (82.2%), participated in 1–2 organizations (71.2%), lived within 1 km commuting distance (55.1%), and achieved a GPA categorized as very satisfactory (65.3%). There is no significant relationship between sleep quality and GPA (p=0.727) or between organizational activities and GPA (p=0.316). However, a significant relationship was found between commuting distance and GPA (p = 0.039; r = -0.191), with students living farther away tending to have lower GPAs. Conclusion: The results emphasize the importance of effective time management and residential location in enhancing students' academic success

Abstract

ABSTRACT In the high-stakes world of Intensive Care Units (ICUs), the judicious application of empirical antibiotic therapy is crucial for managing sepsis in critically ill patients, as its misuse is often linked with unfavorable patient outcomes. Thus, timely and precise treatment strategies are indispensable. This study was designed to evaluate the rationality of such treatments among sepsis patients at RSUD Dr. Mohamad Soewandhie between January 2021 and September 2024. Using an observational and descriptive approach, researchers scrutinized secondary data from 120 patient medical records, with expert reviewers employing the Gyssens method to assess treatment rationality. Findings revealed that 60% of patients received appropriate empirical antibiotic therapy, most commonly intravenous Ceftriaxone. Alarmingly, culture and antibiotic sensitivity tests were conducted on only 18 patients, yielding 11 positive results, and among these were 19 gram-negative and 2 gram-positive bacterial isolates, with a troubling 67.9% being multidrug-resistant organisms (MDRO). Despite the empirical therapy's rationality, the study noted a concerning mortality rate of 81.67%, possibly owing to delays in obtaining culture results and the use of outdated local guidelines for antibiotic therapy in sepsis management, highlighting gaps in existing approaches. Abstract

Introduction: Acute myocardial infarction (AMI) is a clinical manifestation of coronary atherosclerosis involving chronic inflammation. Neutrophilia, lymphopenia, and thrombocytosis due to systemic inflammation can be measured using hematological markers neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR). Previous studies have reported using NLR and PLR to determine the prognosis of cardiovascular disease. Purpose: This study aims to investigate whether NLR and PLR differ in the incidence of AMI, both NSTEMI (non-ST elevation myocardial infarction) and STEMI (ST elevation myocardial infarction), and to evaluate their predictive value. Methods: A cross-sectional study was conducted using complete blood count data from 141 AMI patients treated at Tzu Chi  Hospital from 2018 to 2024 upon their arrival at the emergency room. The data were analyzed using SPSS, utilizing the Mann-Whitney test to identify significant differences, and Receiver Operating Characteristic analysis to assess the predictive value and cut-off points for NLR and PLR. Results: There was a statistically significant difference in NLR in the incidence of AMI (p = 0.013) but no statistically significant difference in PLR in the incidence of AMI (p = 0.549). NLR with a cut-off of 4.34 (sensitivity 60.6%, specificity 60%) showed a low predictive value (AUC = 0.622), and PLR with a cut-off of 158.90 (sensitivity 53%, specificity 57.3%) had no predictive value (AUC = 0.529) to distinguish between NSTEMI and STEMI events. Conclusion: The difference in NLR was more significant and had better predictive value than PLR in AMI
