Abstract

Introduction: Contact dermatitis is one of the occupational skin diseases caused by exposure to irritants or allergens. This condition can arise during work activities or may be influenced by other factors within the work environment. Purpose: The purpose of this study was to determine the frequency of contact dermatitis symptoms on the hands and to identify factors associated with these conditions among workers engaged in wet work (such as cleaning, laundry, and kitchen) at Ukrida Hospital and Campus in West Jakarta.  Methods: This study utilized an analytic cross-sectional design conducted from August to September 2024. Individual characteristics and job information were collected through a questionnaire, while symptoms of contact dermatitis were assessed using the Nordic Occupational Skin Questionnaire 2002 (NOSQ-2002). Data were analyzed using the chi-squared test, the Mann-Whitney U test, and logistic regression. Results:  A total of 44 workers (45.8%) experienced symptoms of contact dermatitis on their hands, with the most common symptoms including dry, rough, scaly, itchy, and chapped skin. The study found that wet work and a history of allergies are associated with these symptoms of contact dermatitis.  Conclusion: Workers in wet environments and those with a history of allergies are at a higher risk of experiencing symptoms of contact dermatitis on their hands.  Implementing exposure control measures in the workplace is essentialn to managing symptoms of contact dermatitis.

Abstract

Introduction: While oral corticosteroids are not included in the psoriasis systemic therapy algorithm, they are still utilized in certain cases because of their anti-inflammatory and immunosuppressive properties. This case report describes a case of psoriasis vulgaris treated with oral corticosteroids as replacement therapy for first-line drugs. Case Illustration: A 53-year-old male presented to Ciawi Regional Hospital with a complaint of a red, scaly rash that had been itchy on his back for a year, which was spreading to his entire body and worsening over the last 3 months. The physical examination revealed erythematous papules and plaques with thick, silver scales throughout his body. He was diagnosed with psoriasis vulgaris, with a Psoriasis Area Severity Index (PASI) score of 32.6. The patient utilized National Health Insurance, which did not cover the first-line drugs; therefore, he was prescribed oral methylprednisolone instead. After two weeks of medication, his clinical manifestations improved, resulting in a PASI score of 5.5. Discussion: The oral corticosteroid provided a significant improvement in skin lesions for this patient. This improvement may be due to its anti-inflammatory and immunosuppressive effects. Close monitoring after treatment is crucial, as flare-ups and side effects may occur after treatment with oral corticosteroids. Conclusion: The use of oral corticosteroids in the treatment of psoriasis vulgaris is an option when first-line drugs are not available, followed by post-treatment observation

Abstract

Introduction: Knee osteoarthritis (OA) is a degenerative joint disease causing disability, especially in older adults. Flatfoot, marked by reduced medial arch height, alters lower limb biomechanics and may influence knee OA severity. Objective: To evaluate the association between foot arch, measured by Clarke’s angle, and knee OA severity based on the Kellgren-Lawrence (K-L) grading to provide clinical insight for guiding management decisions. Methods: A cross-sectional study of 57 knee OA patients at Siloam Karawaci Hospital was conducted. Clarke’s angle was measured using ink footprints, and K-L grading assessed radiological severity. Statistical tests included Kolmogorov-Smirnov, one-way ANOVA, and Kruskal-Wallis, with p < 0.05 considered significant. Results: Most participants were female (77.2%) with a mean age of 65.85 ± 9.69 years. OA severity was mostly mild to moderate (K-L 2 and 3). Flatfoot prevalence was 47.4%, and lower Clarke’s angles were significantly associated with higher K-L grades (p = 0.029). No significant differences in age or BMI were found across severity groups, though BMI trended higher with severity. Conclusion: Lower Clarke’s angle correlates with increased knee OA severity. Flatfoot may contribute to or result from OA progression. Further research is needed to clarify causation and inform management strategies.

Abstract

Introduction: Knee osteoarthritis is the most common articular disease in developed countries that caused chronic disability and reduction of the quality of life in the elderly. Intra-articular (IA) injections are selected if other conservative therapies are ineffective. Purpose: This study aimed to measure the effect of IA injection of triamcinolone acetonide on the quality of life in knee osteoarthritis’s patients. Methods: This observational research was conducted in three steps: filling out questionnaires by patients, interviewing patients, and retrieving patients' data from medical records. The patients' quality of life scores measure before and 4 weeks after therapy, using the AIMS2-SF score, VAS EQ-5D-5L, and Indonesian EQ-5D-5L. The result will be declared statistically significant if p<0.05. Results: No statistically significant difference found in AIMS2-SF scores (p> 0.05) for Symptoms, Social, Occupational domains, and in VAS EQ-5D-5L (p> 0.05). A significant difference was found in AIMS2-SF scores for Physical, Emotional domains and in the EQ-5D-5l Score. Multivariate analysis showed that patients' characteristics and knee osteoarthritis disease did not affect the AIMS2-SF, EQ-5D-5L, and VAS score. Conclusion: IA injection of triamcinolone acetonide has a potential effect to increase the number of domains in the patient's quality of life after 4 weeks of therapy.

Abstract

Introduction: Retinal detachment is the visual nerve layer in the eyeball from the lower layer or retinal layer of pigment epithelium with accumulation in subretinal fluid. The clinical manifestation of retinal detachment is that the patient's vision can decrease from mild to severe. The symptom that suf erers often complain about is floaters, which are visible objects floating around. Purpose: The research is to determine the knowledge of retinal detachment among medical students at the Faculty of Medicine and Health Sciences, Krida Wacana Christian University. Methods: The design of this research is cross-sectional. Participants completed the questionnaire via Google form from 7 December 2022 to 15 December 2022. This research used random sampling; sample selection was carried out randomly. Participants numbered 108 students. Results: Students who had a high level of knowledge 69 respondents (63.9%), followed by a low level of knowledge, namely 22 respondents (20.4%), and then for a medium level of knowledge there were 17 respondents (15.7%). Conclusion: Most students at the Faculty of Medicine and Health Sciences, Krida Wacana Christian University have a high level of knowledge. This study had variations in respondents' knowledge levels because there were pre-clinical and clinical clerkship students

Abstract

Introduction: Intussusception in adults, especially when caused by Meckel's Diverticulum, is a rare occurrence, often presenting with nonspecific symptoms such as abdominal pain and nausea, making it challenging to diagnose. When concurrent with conditions like diabetes mellitus, the situation need particular care. Case Illustration: A 27-year-old male with a medical history of diabetes presented with severe right abdominal pain, nausea, and a reduced appetite. Initial symptomatic treatment did not alleviate the condition. Subsequent CT scans revealed an ileo-ileal intussusception triggered by Meckel's Diverticulum and partial high obstruction ileus, leading to surgical intervention. Discussion: Ileo-ileal resection (end to end anastomosis) and adhesiolysis surgery were successfully performed, followed by vigilant postoperative monitoring for complications, effective pain management, maintenance of drain patency, and careful glucose level monitoring. The patient's significant postoperative improvement underscores the importance of early diagnosis and timely surgical intervention in managing Meckel's Diverticulum-induced intussusception in adults. Conclusion: The clinical course also highlighted the importance of tailoring postoperative care considering the patient's comorbidities, in this case, diabetes mellitus. This case served as a reminder for clinicians to consider Meckel's Diverticulum as a potential cause of intussusception in adults presenting with abdominal pain.

Abstract

Introduction: Cervical cancer is caused by persistent infection with HPV transmitted through sexual intercourse. Chemoradiation is often used in the treatment of cervical cancer because it can reduce recurrence and further cell growth and can increase survival. Increased Platelet Lymphocyte Ratio (RTL) can be used to track the progression of cervical cancer and the degree of differentiation of cancer cells. High RTL indicates a worse degree of cell differentiation and vice versa. Purpose: The aim of this study was to determine the difference in RTL in cervical cancer patients pre and post chemoradiation therapy. Methods: The research design is cross-sectional with total sampling. Data was taken from medical records of cervical cancer patients at the Department of Obstetrics and Gynaecology, RSPAL dr. Ramelan Surabaya in year 2000 - 2023. Results: The average RTL for cervical cancer patients before chemoradiation was 193.1293; and after chemoradiation 169.5459. The results of the Wilcoxon signed rank test showed a significant decrease (p < 0.001). This research proves that there is a significant difference in the Platelet Lymphocyte Ratio (RTL) of stage ≥ IIB cervical cancer patients pre- and post-chemoradiation therapy. Statistical tests show significant results. Conclusion: Chemoradiation has been proven to significantly reduce RTL, and it can be concluded that giving chemoradiation to cervical cancer stage IIIB and above provides good results.

Abstract

Introduction: Pangasius fish is known to contain antioxidants. Catalase is an enzymatic antioxidant whose role is to reduce oxidative stress. Oxidative stress results in fat oxidation which can be checked by Malondialdehyde (MDA) levels. Alloxan induction will cause oxidative stress. Purpose: This study aims to decide the role of Pangasius extract on catalase and MDA activity in alloxan-induced experimental animals. 24 hundred Rattus norvegicus were isolated into K0 the group without treatment, K1 was the group induced by an intraperitoneal (i.p) alloxan dose of 150 mg/kgBB on the 7th day, K2 was the group that was induced by an alloxan dose of 150 mg/kgBB i.p on the 7th day and given Pangasius fish oil extricate dosage of 73 mg/kgBB on the 10th day intragastric every day to 14 days. Results: The data investigation showed that there were significant differences in pancreatic catalase activity between groups K0 and K1 (p=0.001) and between groups K1 and K2 (p=0.001). These conditions indicate that Pangasius extract can scavenge free radicals so that there is no decrease in catalase activity in the pancreas. Pancreatic MDA levels showed significant results between groups K0 and K1 (p=0.010) and between groups K1 and K2 (p=0.010). This is due to the ability of the antioxidants contained in Pangasius fish extract to reduce free radicals thereby reducing oxidative stress resulting in decreased lipid peroxidation. Conclusion: the administration of Pangasius extract 73 mg/kgBW can decrease free radicals formed due to alloxan induction so that it can increase catalase activity and reduce pancreatic MDA

Abstract

Introduction: Uric acid (C5H4N4O3) is a product of purine degradation and can act as a pro-oxidant substance because of the formation of ROS through the enzymatic activity of xanthine oxidase. Previous research has shown that uric acid can affect lipid metabolism in the body. Purpose: This study aims to explore the relationship between lipid profiles in MCU patients and uric acid levels. This study employs a cross-sectional design, and we will analyze the data using linear regression. Methods: This study is an observational analytic study without intervention, using a cross-sectional study design. The research data came from secondary data, especially the medical records of MCU patients. Researchers selected the study sample using a random sampling technique. The study uses 86 secondary data samples. Results: Linear regression analysis reveals a significant positive correlation between uric acid and triglycerides (p-value < 0.001 and r = 0.464), total cholesterol (p-value = 0.018 and r = 0.255), and LDL (p-value = 0.011 and r = 0.273). A significant negative correlation exists between uric acid levels and HDL (p-value < 0.001 and r = 0.477). Increased uric acid levels are associated with higher levels of TG, TC, and LDL, as well as a decrease in HDL levels. The average levels of LDL and total cholesterol are 140.27 mg/dl and 206.66 mg/dl, respectively, which are at the high threshold, while all other variables are within normal limits. Abstract

Introduction: Mental health is a crucial concern globally and in Indonesia, especially among medical students who face higher rates of depression than the general population due to academic pressure, high expectations, and social stress. Depression can negatively affect academic performance and future careers. Studies show varying results regarding depression levels between early and end preclinical students. Purpose: This study aims to compare the level of depression in early to end preclinical students at Pelita Harapan University and to see whether there is a relationship between gender and social support and the level of depression. Methods: This cross-sectional study was conducted among 333 preclinical students from the Faculty of Medicine, Pelita Harapan University, in January-March 2024. The data was collected using the Beck Depression Inventory-II and the Multidimensional Scale of Perceived Social Support Questionnaire. Data analysis was performed using SPSS version 26 (chi-square statistical test). Results: The study found no statistically significant difference in depression levels between early and end preclinical students (p-value 0.348). There is a relationship between social support and the level of depression in preclinical students of the UPH Medical Faculty, with a (p-value of 0.001). However, no significant relationship was found between gender and the level of depression in preclinical students, with a p-value of 0.093. Conclusion: Conducted outside major metropolitan areas like Jakarta, the study offers insights into how depression manifests in different geographic and socio-economic contexts. The findings can be used as a consideration for making a faculty or university policy to improve mental health management for medical students, ensuring an effective learning process.
