Abstract

Introduction: The activities of garment workers who work with repetitive movements over a long time can influence the emergence of musculoskeletal complaints, one of which is low back pain, which is non-specific. Low back pain non-specific complaints impact functional activities and affect workers' work. Purpose: This study aims to determine the effect of the stretching exercise and McKenzie exercise in reducing pain and increasing functional activity in garment workers with non-specific low back pain. Methods: This type of research is experimental with a pre-test and post-test control group design. Treatment is given in 12 sessions over 4 weeks and carried out from April to May 2024. The research subjects were garment workers who met the inclusion and exclusion criteria with 22 people, divided into group I (n = 11 people) given the stretching exercise and group II (n = 11 people) given the McKenzie exercise. Results: The results of the difference test between groups I and II used the Mann-Whitney U test on the Visual Analog Scale (p < 0.05) and Oswestry Disability Index values (p < 0.05). Methods: It was concluded that self-stretching exercise was more effective than McKenzie exercise in reducing pain and increasing functional activity in garment workers with non-specific lower back pain

Abstract

Introduction: Green nail syndrome is a common bacterial infection of the nails and is characterized by green discolouration of the nail plate caused by Pseudomonas aeruginosa. It frequently occurs in people who are exposed to constant water and due to mechanical trauma, especially in elderly patients. Case Illustration: We reported 2 cases treated with a combination therapy of oral ciprofloxacin, 1% acetic acid dressing, gentamicin ointment and showed a significant clinical improvement. There are two cases: a 67-year-old man and a 64-year-old woman complained of black-greenish discolouration on his right big toenail, which has been accompanied by changes in the shape of the nail for 3 months. Both patients have a history of frequent and prolonged contact with water—dermoscopy results in greenish-black discolouration in the nail plate.  The culture results of both patients, Pseudomonas aeruginosa was found. Combination treatment showed significant clinical improvement in both patients. Discussion: Dressing of 1% acetic acid was useful as an adjuvant to improve the ointment penetration, inhibiting the formation of Pseudomonas aeruginosa biofilms. Gentamicin and ciprofloxacin are effective against aerobic gram-negative bacterial infections, such as Pseudomonas aeruginosa. Conclusion: The use of ciprofloxacin is recommended, especially in elderly patients. Combination therapy with oral ciprofloxacin, 1% acetic acid dressing, and gentamicin ointment is an effective option for patients with green nail syndrome

Abstract

Introduction: Soil Transmitted Helminths (STH) infection is a worm infection through the soil. Consumption of raw food such as fresh vegetables is a route of STH transmission to humans because soil is a growth medium for vegetables and an ideal place for worm development into an infective form so that there is a risk of contamination when the soil is contaminated with human feces containing worm eggs. Transmission through the use of vegetable fertilizers contaminated with human feces, vegetables that are not washed properly and poor food management. Research on STH eggs in fresh vegetables in Grogol Petamburan food stalls has never been conducted. Objective: The study aims to determine the presence or absence of STH, the prevalence of contamination, stages and types of STH eggs in fresh vegetables in Grogol Petamburan food stalls to provide the public with information to pay attention to the cleanliness of fresh vegetables. Method: The study was descriptive with microscopic examination using the sedimentation method. The sampling technique was a total sampling of 84 fresh vegetables samples taken from 38 Grogol Petamburan food stalls. Fresh vegetables samples, namely cabbage, lettuce and basil, were examined in the parasitology laboratory. Data on hygiene and sanitation practices were taken from interviews. Results: STH identification results in 9 samples (10.7%) were positive for STH contamination. Ascaris lumbricoides eggs were found in the contaminated samples, while Trichuris trichiura and Hookworm were not found. Conclusion: Based on the results of the study, the samples were positive for STH contamination in the Ascaris lumbricoides egg stage. The results indicate that the raw vegetables are not suitable for consumption.

Abstract

Introduction: Suicide rates continue to rise globally, with self-poisoning being a common method in some developing countries. This report reported a case of hydrogen peroxide ingestion leading to aspiration pneumonitis, a less-discussed complication compared to gastrointestinal toxicity. Case Illustration: A 41-year-old woman presented to the emergency unit 8 hours post-ingestion of a hydrogen peroxide-based toilet cleaner. Symptoms included vomiting, hoarseness, faintness, and frothing at the mouth. The physical examination revealed dyspnea, irritability, a cleaning solution odor in the oral cavity, rhonchi in the middle and lower lung fields, epigastric tenderness, and increased bowel sounds. Gastric lavage with 200 mL of saline was performed, and activated charcoal and sucralfate syrup were administered via a nasogastric tube. The patient had previously received ranitidine at another facility and was also treated with omeprazole and ondansetron. Laboratory results showed leukocytosis, and a chest x-ray indicated bilateral lung infiltration and increased broncho-vascular pattern. Discussion: Early and proper intervention by trained healthcare professionals, combined with preventive measures such as positioning the patient in a semi-upright position and avoiding emetogenic liquids, is essential. Conclusion: This case showed the importance of prompt management involving gastric lavage, mucosal protection, acid suppression, and absorption inhibition. Nonetheless, respiratory complications, such as aspiration pneumonitis, remain challenging to prevent.

Abstract

Introduction: Colorectal cancer is a global health problem with an increasing incidence. Monitoring cancer stage is crucial as it is closely related to therapeutic decisions, prognosis, and patient survival. Simple hematological parameters, such as hemoglobin, platelet count, and neutrophil-to-lymphocyte ratio, may aid in clinical evaluation. Objective: This study aimed to describe the profiles of hemoglobin, platelet count, and neutrophil-to-lymphocyte ratio in patients with colorectal cancer. Methods: The study employed a descriptive observational method with a cross-sectional approach. Secondary data were obtained from the medical records of 141 patients using the total sampling method. Data analysis was performed using descriptive statistics.  Results: Anemia was found in 83% of patients, with a higher incidence at higher stages. Normal platelet counts were observed in 80% of patients, with decreasing proportions in higher stages. Elevated NLR was found in 78% of patients and was more frequent at higher stages of disease.  Conclusion: These findings indicate the presence of anemia and a systemic inflammatory response, but not thrombocytosis. This research highlights the potential of these simple biomarkers in monitoring cancer stages and treatment strategies.

Abstract

Introduction: As individuals age, the incidence of osteoarthritis increases. Globally, it is reported that approximately 300 million individuals, particularly the elderly, are affected by osteoarthritis. Metabolic disorders related to lipid pathways have been associated with the development of osteoarthritis in the elderly. Purpose: This study aims to provide an overview of the relationship between haematological indicators, lipid profiles, and body fat proportion with osteoarthritis in the elderly population in Pusaka Kebon Jeruk, West Jakarta. Methods: This study used a cross-sectional design involving an elderly population, with 100 respondents aged ≥60 years who were willing to participate. Data were collected through interviews using an osteoarthritis questionnaire, blood draws, and measurements using BIA. The chi-square test (p<0.05) and multiple logistic regression (p<0.25) were used in this study. Results: Of the 100 respondents, it was found that 28% of the elderly respondents experienced osteoarthritis. Bivariate analysis showed a significant relationship between body fat proportion and osteoarthritis (p: 0.006; OR: 3.519; 95% CI: 1.394–8.878). Multivariate analysis indicated that body fat proportion was the dominant factor associated with osteoarthritis (p: 0.006; OR: 3.814; 95% CI: 1.475–9.865). Conclusions: We concluded that body fat proportion has a significant relationship with the incidence of osteoarthritis. However, this study did not find any significant associations between hemoglobin, white blood cells, RDW, LDL, HDL, triglycerides, and cholesterol.

Abstract

Introduction: Congenital Heart Disease (CHD) is one of the most common congenital disabilities, with an incidence of 8-10 per 1000 live births in full-term infants and ten times higher in preterm infants. In Indonesia, the incidence of CHD is estimated at around 8 per 1000 live births.  Understanding the anatomy and pathophysiology of the defect that occurs contributes to the accuracy of management planning.  Purpose: This study aims to identify the characteristics of pediatric patients with CHD featuring left-to-right shunt accompanied by left ventricular enlargement at RSPAL dr. Ramelan Surabaya from 2020 to 2023. Methods:  The descriptive study uses secondary data (medical records) of pediatric CHD patients with left-to-right shunt and left ventricular enlargement, specifically focusing on ventricular septal defect (VSD) and patent ductus arteriosus (PDA) at the pediatric clinic of RSPAL dr. Ramelan.  Results: Based on medical records from 45 samples, the majority of CHD patients were aged 1-12 months (23 children). CHD with VSD was more common in males (18 children), while PDA was more common in females (11 children). Most CHD patients showed no initial complaints (39 children) and did not have other types of congenital heart diseases, with 23 children having VSD and 10 having PDA. Additionally, most patients did not have comorbidities (39 children). Echocardiographic results indicated that most patients had an ejection fraction (EF) greater than 75% (27 children) and a fractional shortening (FS) greater than 45% (23 children). The main management involved echocardiographic evaluation and medication.  Conclusion: The predominant age group for CHD cases at the pediatric clinic of RSPAL dr. Ramelan was 1-12 months, with echocardiographic results showing normal left ventricular function, with EF >75% and FS >45%.

Abstract

Introduction: Vascular dementia is the second most common type of dementia in the world, following Alzheimer's dementia. One of the primary risk factors for vascular dementia is hypertension. Prolonged hypertension can impair kidney function, as indicated by the glomerular filtration rate (GFR). A decreased GFR can lead to vascular complications, such as endothelial dysfunction, which may result in brain tissue infarction and contribute to the development of vascular dementia in the future. Purpose: The purpose of this research is to examine the relationship between kidney function, measured through GFR values, and the incidence of vascular dementia at Gatot Soebroto Army Hospital during the 2023-2024 period. Methods :  This study employed a cross-sectional design and conducted a bivariate test using the Chi-square method with a sample of 47 individuals. The research utilized a total sampling technique. Results: Among the 47 patients, 35 (74.5%) were diagnosed with vascular dementia, while 12 (25.5%) had different types of dementia. The majority of patients, 31 (66%), exhibited a mild decrease in GFR values. The analysis revealed a relationship between kidney function and the incidence of vascular dementia, with a p-value of 0.596. Conclusion:  There was no significant relationship found between kidney function, as indicated by GFR values, and the incidence of vascular dementia. Future researchers can study larger populations.
