Abstract

The Emergency Severity Index (ESI) is a triage method that determines the escalation of treatment for patients based on the severity of their emergency condition. In order to improve the level of satisfaction of individuals receiving health services, it is necessary to provide quality services that are responsive to the expectations and needs of patients. This study aims to explore the relationship between the ESI (Emergency Severity Index) level and patient satisfaction in the Emergency Room of Pertamedika Ummi Rosnati Hospital. The research design applied is analytical with a cross-sectional method. From a total population of 500 patients in December, 51 respondents were selected using purposive sampling. The instruments used included the ESI (Emergency Severity Index) Questionnaire and CSQ-8 (Client Satisfaction Questionnaire), with univariate and bivariate data analysis using the Chi-Square test. The results showed that in the high ESI index category, most participants (37 people or 92.5%) expressed satisfaction with the services received, while 3 people (7.5%) were dissatisfied. In the moderate ESI group, 9 respondents (90.0%) reported satisfaction, while only 1 respondent (10.0%) reported dissatisfaction. Conversely, in the mild ESI level, there were no patients who were satisfied (0%), and one patient (20.0%) was dissatisfied. Through analysis using the Chi-Square statistical test, a ρ value of 0.009 (≤ 0.05) was obtained, indicating a significant relationship between ESI levels and patient satisfaction. Based on these findings, it can be concluded that there is a correlation between ESI classification and the level of satisfaction of service users in the Emergency Department. The researchers recommend that patients gain a better understanding of the ESI stages, and the Emergency Department is expected to provide education, such as putting up informative banners related to ESI (Emergency Severity Index) to increase patient understanding of the triage process in the Emergency Department.

Abstract

The incidence of violent behavior among mental health patients globally reaches 9%. One of the independent interventions carried out by nurses is the application of implementation strategies for patients with violent behavior tendencies, whose risk level is greater than that of other patients. This study aims to analyze the effect of implementation strategies on the risk of violent behavior in patients with mental disorders at the Aceh Mental Hospital. The implementation strategy consists of four main techniques: building trust, training in controlling violent behavior through medication, training in social/verbal risk control, and training in spiritual control. The study used a quasi-experimental design with a one-group pretest-posttest approach. A total of 2,166 inpatients at risk of violent behavior were identified over a one-year period, and 10 respondents were selected as samples using purposive sampling based on inclusion criteria. The research instrument was the BPAQ questionnaire, with aggression assessment ranging from low to very high. The results showed that the pretest average for signs and symptoms was 38.40, while the posttest score decreased to 11.60 after the implementation strategy intervention. Data analysis was performed using univariate and bivariate methods, with a paired t-test yielding a p-value of 0.000 (less than 0.05). This study concluded that the implementation strategy had a significant impact in reducing the risk of violent behavior in patients. It is recommended that patients' understanding of the implementation strategy therapy be continuously improved as an effort to control violent behavior.

Abstract

Introduction: Back pain during pregnancy is a problem that is often experienced by pregnant women and is complex because it involves not only physical changes, but also psychological aspects. Changes in body biomechanics, weight gain, and shifting of the center of gravity due to uterine enlargement are the main factors that trigger pain complaints, especially in the lower back area. This condition can have a negative impact on the quality of life of pregnant women, interfere with daily activities, and even worsen emotional states due to the emergence of anxiety, fear, and stress if not managed properly. Treatment of back pain in pregnant women can be done through safe non-pharmacological methods, one of which is prenatal yoga. Yoga practice for pregnant women has been proven to provide benefits in the form of relaxation, posture improvement, and increased muscle elasticity. One of the effective movements is the Angry Cat Pose because it is able to stretch the back muscles, improve blood circulation, and reduce pressure on the lower spine. Objective: This study aims to analyze the effect  of Angry Cat Pose  exercise on the intensity of back pain in pregnant women in the third trimester. Methods: This study used a quasi-experimental design with a one group pretest-posttest design. The study sample consisted of 31 pregnant women in the third trimester with complaints of back pain who visited the Gunungpati Health Center. The research instrument was in the form of a questionnaire, while the data analysis was carried out using the Wilcoxon test with a significance level of <0.05. Results: The results showed a significant effect of the Angry Cat Pose  exercise on the reduction of back pain in pregnant women in the third trimester with p=0.000. After the intervention, the majority of respondents experienced a decrease in pain to the mild category on a scale of 1–3. Conclusion: The Angry Cat Pose  exercise has been shown to be effective in reducing back pain in third trimester pregnant women, so it can be recommended as a safe, simple, and routine nonpharmacological intervention under the supervision of health workers.

Abstract

The long-term care required by schizophrenia patients places a heavy burden on caregivers who are responsible for meeting the patients' needs. Performing these caregiving tasks often causes physical and emotional stress on caregivers, especially due to the long-term nature of the care. In situations like this, coping strategies play a crucial role in supporting the well-being of caregivers so that they are able to face the challenges that arise during the process of caring for schizophrenic patients. This study focuses on examining the correlation between coping strategies and the quality of life of caregivers who provide care for schizophrenic patients at the Aceh Mental Hospital Polyclinic. The research design applied was analytical with a cross-sectional approach. A total of 115 caregivers were sampled through accidental sampling from a total population of 4,035 families. Research data were collected using two measuring instruments, namely the Brief COPE questionnaire and The Schizophrenia Caregiver Quality of Life Questionnaire (SCGQoL). The research process took place at the Aceh Mental Hospital Polyclinic from March 6 to 24, 2025. Data analysis used univariate and bivariate methods. Based on the research results, it was found that 108 caregivers (93.9%) were in the low coping strategy category, while 80 caregivers (69.6%) had a moderate quality of life. Statistical testing with Chi-Square produced a p-value of 0.001, which indicates a significant relationship between coping strategies and the quality of life of caregivers. Therefore, it is hoped that health care institutions can improve mental health nursing interventions for patients' families, particularly through educational programs on managing schizophrenia patients at home and assistance in identifying adaptive coping resources for patients' families.

Abstract

Background: Colostrum is the first natural food for newborns, containing essential nutrients and antibodies that protect against infection. Health education for pregnant women in their third trimester is crucial to enhance their knowledge and practices regarding colostrum provision. Despite the global decline in neonatal mortality, the rate remains high, partly due to limited access and low quality of neonatal care. In Indonesia, particularly in Semarang, although exclusive breastfeeding coverage has improved, many mothers still lack understanding of colostrum’s importance and remain influenced by misconceptions. Preliminary studies revealed insufficient knowledge among pregnant women about colostrum. Method: This study applied a quasi-experimental method with a pretest-posttest design and a control group. The intervention group received health education, while the control group did not. Results: Before intervention, most respondents in both groups had poor knowledge. After the intervention, knowledge in the intervention group increased significantly, with 88.8% reaching a good category, while the control group showed no significant change. Furthermore, 94.4% of mothers in the intervention group gave colostrum, compared with only 33.3% in the control group. Conclusion: Health education is an effective strategy to increase maternal knowledge and encourage colostrum practice, dispelling myths and promoting better neonatal outcomes.

Abstract

Low Back Pain (LBP) is often caused by spondylolisthesis, which causes pain, limited mobility, and disability. Conservative physiotherapy is the primary option to reduce symptoms and improve function. This study aims to evaluate the effectiveness of multimodal physiotherapy interventions in patients with LBP and spondylolisthesis. A case report study was conducted on a 59-year-old female patient diagnosed with LBP and spondylolisthesis. The patient underwent three physiotherapy intervention sessions: Infrared (IR), Short-Wave Diathermy (SWD), Transcutaneous Electrical Nerve Stimulation (TENS), William Flexion Exercise, Core Stability, and Strengthening. Evaluation was performed using the Numeric Rating Scale (NRS) for pain, a goniometer for joint range of motion (LGS), Manual Muscle Testing (MMT) for muscle strength, and the Oswestry Disability Index (ODI) for function. The results of the therapy showed a significant reduction in pain, namely silent pain from a score of 5 to 1 and pain on movement from a score of 6 to 3. Lumbar ROM increased with flexion from 70 degrees to 85 degrees. Lumbar muscle strength also improved, flexors from a score of 4 to 5 and extensors from a score of 3 to 5. ODI decreased from 55.5% (severe disability category) to 12% (minimal disability category). Multimodal physiotherapy interventions have been proven effective in reducing pain, improving LGS, muscle strength, and function in patients with LBP et causa spondylolisthesis.

Abstract

Background: Knee osteoarthritis is one of the most common degenerative conditions in older adults, often causing pain and limited range of motion (ROM), which negatively affects quality of life. Physiotherapy management plays an important role in reducing pain, improving flexibility, and maintaining joint function. Objective: This study aims to evaluate the effectiveness of physiotherapy interventions in reducing pain and improving ROM in elderly patients with knee osteoarthritis. Method: A case study was conducted on a 63-year-old patient at Dr. Soedjono Level II Hospital, Magelang, who underwent three therapy sessions consisting of Infrared (IR), Transcutaneous Electrical Nerve Stimulation (TENS), Proprioceptive Neuromuscular Facilitation (PNF) with the hold-relax technique, and static cycling. Assessments were carried out using the Numeric Rating Scale (NRS), Likert Gait Scale (LGS), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Results: The findings showed a significant reduction in pain on palpation (from 4/10 to 1/10), improvement in knee flexion ROM (from 110° to 135°), and functional enhancement indicated by a decrease in WOMAC scores from 39.58% to 27.08%. Conclusion: These results suggest that structured physiotherapy interventions provide therapeutic benefits in reducing pain, improving joint flexibility, and enhancing functional ability in elderly patients with knee osteoarthritis.

Abstract

Cardiac arrest is a medical emergency that results in the sudden cessation of heart function, leading to more than 7 million deaths worldwide annually. In Indonesia, its prevalence is not well-documented. Ischemic heart disease is the primary cause, with other risk factors like hypertension, diabetes, obesity, and smoking also contributing. Clinical signs include loss of responsiveness, impaired breathing, and the absence of a pulse. Diagnosis is based on history, physical examination, electrocardiography, laboratory tests, and imaging to determine the underlying cause. Treatment follows the 2020 American Heart Association (AHA) guidelines, starting with basic life support (BLS), followed by Advanced Cardiac Life Support (ACLS). ACLS includes high-quality cardiopulmonary resuscitation (CPR), early defibrillation for shockable rhythms, epinephrine administration, and airway management through intubation. Once Return of Spontaneous Circulation (ROSC) is achieved, post-cardiac arrest care is crucial, including hemodynamic stabilization, Targeted Temperature Management (TTM), and critical care to prevent recurrence and neurological damage. A clear understanding of cardiac arrest's diagnosis, risk factors, and management is vital for reducing mortality. Adherence to AHA guidelines, especially rapid resuscitation, improves survival rates, making continuous CPR education essential for healthcare providers and the public.

Abstract

Hypertension is a condition where blood pressure is persistently above the normal limit. This condition is a risk factor that can cause damage to vital organs, including the heart, brain, retina, kidneys, aorta, and peripheral blood vessels. This case study aims to describe the implementation of these management efforts in a 45-year-old female patient with hypertension in Blang Dalam Geunteng Village, Nisam District, North Aceh. Mrs. H, a 45-year-old woman, came to the General Clinic of Nisam Community Health Center with a primary complaint of headache since one week before the visit. The headache felt like a throbbing pain throughout the head and was intermittent. This complaint was quite disruptive to the patient's daily activities, especially when doing light household chores. In addition, the patient complained of a throbbing sensation in both eyes that began to be felt simultaneously with the onset of the headache. This complaint appeared mainly when waking up in the morning. The patient has a history of hypertension that has been diagnosed since 2024. Family history shows that the patient's biological mother also suffered from hypertension and one of the patient's siblings was also diagnosed with high blood pressure but had not started treatment. Physical examination showed a blood pressure of 150/90 mmHg. Data were collected through anamnesis, physical examination, supporting procedures, home visits, and family documentation. The holistic assessment included quantitative and qualitative aspects of the initial visit, the process, and the outcome of the visit. Interventions included hypertension education, medication adherence, a healthy lifestyle, and family support for blood pressure management and lifestyle. This case study demonstrates the importance of a holistic and continuous family medicine approach in the management of patients with grade II hypertension, particularly those with familial risk factors.

Abstract

(MMR) refers to the number of deaths of women that occur during the pregnancy period or within 42 days after delivery. According to WHO in (2022) MMR is an indicator that states the number of maternal deaths for 303,000 per 100.000 live births, According to the Brebes health office (2022) MMR 177.94/ 100.000 live birth, IMR 197 cases. According to data obtained from the Sirampog Health Center daring 2022, the maternal mortality rate was recorded at one case, and eleven infant dealths. The study aims to provide comprehensive midwifery care to patients with a history of cesarean section, using a qualitative descriptive case study approach. Based on integrate midwifery care carried out on patients who had a history of cesarean section, the results were obtained, namely Mrs. Saya’s pregnancy with an indication of CPD as well as high blood pressure and ureine protein approaching positive one. Delivery was carried out by secarean section as an effort to prevent complications in the mother anda baby. During the neonatal care period up to 28 days of age, no abdormalities or problems were found. During the postpartum period up to 42 days, on problem or complications were found. Mrs. I  chose to use an IUD as a contraceptive.
