Abstract

A 37-year-old woman, G3P2A0, at 16 weeks of gestation, came to the community health center for her first antenatal check-up. The patient had no characteristic clinical symptoms, but the triple elimination screening results showed reactivity for HBsAg, with negative HIV and syphilis results. An ultrasound examination revealed a viable intrauterine fetus with normal findings. The patient had not received Hepatitis B immunization or vitamin supplements previously, and the patient's partner had not undergone Hepatitis B screening despite having a work-related risk factor. The patient was given iron and vitamin supplements and referred to the hospital for further evaluation. Comprehensive education was provided regarding modes of transmission, risks of maternal and fetal infection, prevention of vertical transmission, as well as the importance of Hepatitis B immunization and the administration of HBIG to the newborn within 12 hours after birth. This case emphasizes the importance of early antenatal screening, comprehensive education, psychosocial support, and collaboration between the patient, family, and healthcare providers to prevent vertical transmission of Hepatitis B. The education provided included essential preventive measures and infection control steps to protect both the mother and the baby. Proper management and follow-up monitoring are necessary to ensure the health of the mother and baby and prevent further complications.

Abstract

The prevalence of social media addiction is continuously increasing worldwide, including in Indonesia. Instagram has become the most widely used platform among students aged 18–25 years. Although Instagram provides many benefits, spending too much time on the application can cause learning concentration problems, procrastination, and a decrease in Grade Point Average (GPA). The purpose of this study was to determine whether there is a correlation between Instagram social media addiction and GPA among students of the Diploma Three Nursing Program at Universitas Abulyatama. This study applied a cross-sectional quantitative correlational design. The sampling technique used proportional random sampling, involving 78 students from a total population of 362 students. The research instruments consisted of cumulative GPA data and The Instagram Addiction Scale (TIAS). The study was conducted at the Diploma Three Nursing Program of Universitas Abulyatama from April 26 to May 9, 2025. Data analysis was performed using bivariate analysis with the Chi-Square test. The results showed that the majority of respondents were in the moderate category of Instagram addiction (56 students, 71.8%), followed by the high category (22 students, 28.2%). Meanwhile, GPA distribution indicated that 35 students (44.9%) were in the very high category, 24 students (30.8%) in the high category, and 19 students (24.3%) in the moderate category. The bivariate analysis with the Chi-Square test obtained a p-value of 0.001 (≤ 0.05), indicating a significant relationship between Instagram addiction and GPA. In conclusion, there is a correlation between Instagram social media addiction and students’ academic performance. Although Instagram use is widespread, students are expected to develop good time management skills and prioritize academic responsibilities to minimize the negative impact of social media use.

Abstract

At the higher education level, students are expected to develop independence in their learning process. One crucial aspect that influences the achievement of self-directed learning is the ability to manage time effectively. This study focuses on the relationship between time management and self-regulated learning among students of the Diploma Three in Nursing Program at Abulyatama University. The research employed a quantitative approach using a correlational design and a cross-sectional method. A total of 85 participants were selected through proportional random sampling from a population of 554 students. The instruments used were the Time Management Questionnaire (TMQ) with a Cronbach's Alpha reliability of 0.910, and the Motivated Strategies for Learning Questionnaire (MSLQ) with a Cronbach's Alpha of 0.909. Data collection took place within the Diploma Three in Nursing Program at Abulyatama University from April 26 to May 9, 2025.The results showed that the majority of students had a moderate level of time management, with 63 students (74.1%), and a high level with 22 students (25.9%). Meanwhile, the level of self-regulated learning among students was moderate in 59 students (69.4%) and high in 26 students (30.6%). Analysis using the Chi-Square test revealed a significant correlation between time management and self-regulated learning, as indicated by a p-value of 0.000 (p ≤ 0.05). These findings suggest that students who are skilled in time management are more likely to possess higher self-regulated learning abilities. It is hoped that the results of this study will enhance students' understanding of the importance of time management, thereby encouraging them to adopt a more structured, focused, and sustainable self-learning pattern. Time management skills are believed to be a foundational element in strengthening self-regulated learning in students’ academic activities.

Abstract

Appendicitis is a common health problem in Indonesia. Appendicitis requires surgery because it's an acute inflammation. Appendicitis can be treated with surgery to remove the appendix, namely appendectomy, by performing surgery on the abdominal tract. Appendectomy is a clean-contaminated surgery that can cause surgical wound infection in around 5-15% of cases. Prophylactic antibiotics are included in infection prevention. It is important to evaluate the use of prophylactic antibiotics in appendicitis surgery patients to reduce drug side effects and prevent surgical site infections. Infections can be caused by microorganisms such as pathogenic bacteria, and antibiotics are the most commonly used drugs to treat such infections. SSI can cause morbidity, mortality, increased length of stay, and increased patient care costs. The researchers chose Arun Lhokseumawe Hospital as the research site because no research had ever been conducted at Arun Lhokseumawe Hospital on the use of prophylactic antibiotics in appendicitis surgery patients. Preliminary survey results based on Arun Lhokseumawe Hospital medical records from 2020 to 2022 showed that 202 patients underwent surgery. The purpose of this study was to determine the profile of prophylactic antibiotic use in appendicitis surgery patients using the Gyssens method at Arun Lhokseumawe Hospital from 2020 to 2022. This study was observational in nature, using total sampling as the sampling technique. The Results of the study showed that the most frequently used type of prophylactic antibiotic was Cefotaxime (53.5%) and the appropriateness of antibiotic use was category 0 (97%) because it was appropriate and category IVA (3%) because there were other antibiotics that were more effective. It can be concluded that Cefotaxime is the most frequently used antibiotic, and the appropriateness of prophylactic antibiotics at Arun Hospital in Lhokseumawe City is already appropriate.

Abstract

In the final phase of their studies, students often experience intense psychological pressure, especially when writing their final thesis. If this academic stress is not managed optimally, it can cause mental health problems, reduce internal motivation, and lead to despair in the context of education. Hardiness personality is one of the psychological characteristics that is believed to play a crucial role in dealing with such situations. Hardiness encompasses the dimensions of control, commitment, and the ability to see challenges as opportunities, thereby encouraging individuals to persevere in the face of external pressures. This study focuses on the relationship between hardiness personality and the level of stress experienced by final-year students of the Diploma Three Nursing Study Program at Abulyatama University while writing scientific papers. This study used a correlational quantitative approach with a cross-sectional design, involving 63 students selected through proportional random sampling. The Dispositional Resilience Scale (DRS) was used to measure hardiness, while the Student-Life Stress Inventory (SSI) was used to assess stress levels. Data collection took place from April 26 to May 8, 2025. To obtain valid results, data analysis was performed using the Chi-Square test with a significance level of 0.05. The research findings show a significant negative correlation between hardiness personality and stress levels in the process of writing scientific papers (p=0.004; p<0.05). Students who show high levels of hardiness tend to experience lower stress during the preparation of scientific papers, while those with low hardiness are more vulnerable to high academic pressure. Based on these results, it can be concluded that increasing hardiness in students is very important as a strategy for dealing with academic demands, especially in writing scientific papers. Thus, students are expected to be able to strengthen their self-control, commitment to the learning process, and positive perspective towards various challenges.

Abstract

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder whose prevalence continues to rise, making it a major challenge for health systems worldwide. This disease results from a combination of insulin resistance and impaired pancreatic β-cell function, leading to persistent hyperglycemia and increased risk of long-term complications affecting the kidneys, cardiovascular system, nervous system, and eyes. This report describes the case of a 52-year-old woman diagnosed with T2DM for approximately ten years. The patient presented with fatigue, nocturnal polyuria, nausea after meals, significant weight loss, and tingling in her extremities. Laboratory findings revealed an HbA1c level of 12%, reflecting very poor glycemic control. A family medicine approach was applied through detailed history taking, physical and laboratory examinations, home visits, and completion of a family folder to assess clinical, personal, social, and functional aspects. Interventions included counseling on balanced diet, encouragement of regular physical activity, education on diabetic foot care, and pharmacological treatment with metformin and insulin. The family received counseling about hereditary risk factors, the importance of emotional support, and the need for consistent monitoring of health status. The patient was still capable of light daily activities, supported by a highly functional family environment with an APGAR score of 10. A holistic family medicine–based approach was shown to improve treatment adherence, patient knowledge, and overall quality of life. Therefore, management of T2DM requires a comprehensive strategy that integrates promotive, preventive, curative, and rehabilitative components, emphasizing the active involvement of family and community at the primary care level to reduce complications, slow disease progression, and enhance patient well-being. Abstract

The elderly is among the most vulnerable groups in society due to progressive biological decline that weakens physical resistance, making them prone to chronic diseases such as hypertension. Hypertension is one of the most common multifactorial disorders in older adults, and its severity may be influenced by anxiety, which stimulates the sympathetic nervous system and increases blood pressure. This study aimed to examine the relationship between anxiety levels and hypertension among the elderly in the working area of Andalas Health Center, Padang City. An analytic study with a cross-sectional design was conducted involving 81 elderly participants diagnosed with hypertension, selected through accidental sampling based on inclusion and exclusion criteria. The Geriatric Anxiety Scale (GAS) was employed as the research instrument, while data analysis used the Chi-square test. Results showed that the majority of respondents experienced mild anxiety (67 respondents, 82.7%) and uncontrolled hypertension (54 respondents, 66.6%). Statistical analysis revealed a p-value of 0.534, indicating no significant association between anxiety levels and hypertension in the elderly population studied. These findings are consistent with several previous studies reporting that the relationship between anxiety and blood pressure in the elderly is often inconsistent, largely due to other risk factors including age, gender, low physical activity, comorbidities, and poor adherence to antihypertensive treatment. The implication of this study highlights the necessity of a holistic approach to hypertension management in the elderly, addressing not only psychological aspects but also lifestyle modification, treatment adherence education, and strengthening public health programs. Therefore, this research is expected to serve as input for health workers at community health centers to improve strategies for preventing and controlling hypertension in elderly populations.

Abstract

Developmental deviations in children under five years of age are estimated to affect approximately 5-10% of this population, with inappropriate stimulation, particularly from electronic devices such as gadgets, being a significant contributing factor. Early exposure to these technologies can lead to decreased physical activity, adversely impacting the overall development of children, including their gross and fine motor skills, language acquisition, socialization, and independence. The objective of this study was to evaluate the effect of gadget usage on the developmental outcomes of preschool children in the Banda Sakti District of Lhokseumawe, utilizing an observational analytic research method with a cross-sectional design to collect both primary and secondary data. The results indicated varying levels of gadget usage among respondents: 40 individuals (42%) fell into the high usage category, 31 individuals (32%) were classified as moderate users, and 25 individuals (26%) were categorized as low users. Among those with low gadget usage, 6 respondents (24%) displayed questionable developmental outcomes, while in the moderate and high usage categories, 22 respondents (71.1%) and 22 respondents (55%) respectively exhibited similar concerns. This study reveals that a significant number of children are facing developmental disorders, with 21 respondents (37.5%) experiencing challenges related to socialization and independence, and 17 respondents (30.4%) demonstrating speech and language difficulties. Notably, a substantial proportion of preschool children (52 respondents or 54.2%) were identified as having high levels of gadget usage, and the study established a significant correlation between the intensity of gadget use and the occurrence of developmental disorders in preschool children within the Banda Sakti District of Lhokseumawe.
