Abstract

Tujuan utama penelitian ini adalah untuk mengeksplorasi peran apoteker dalam kolaborasi interprofesional (interprofessional collaboration/IPC) di pelayanan kesehatan dan memahami dampaknya terhadap hasil klinis, kepatuhan pasien, serta efektivitas tim lintas profesi. Penelitian ini menggunakan pendekatan kualitatif dengan metode studi pustaka, memanfaatkan literatur akademik terbaru yang relevan, termasuk artikel ilmiah dan laporan resmi. Data dikumpulkan melalui penelusuran literatur sistematis dan dianalisis secara deskriptif-induktif dengan tahapan identifikasi tema, reduksi data, kategorisasi konsep, dan penarikan kesimpulan. Hasil penelitian menunjukkan bahwa apoteker memainkan peran penting dalam lima dimensi kolaborasi: kemitraan, koordinasi, kerja sama, pengambilan keputusan, dan hasil terapeutik. Bentuk kolaborasi yang ditemukan mencakup review obat, manajemen terapi, edukasi pasien, partisipasi dalam ward round, serta integrasi di praktik primer. Faktor pendukung meliputi kejelasan peran, komunikasi efektif, dukungan sistemik, co-location, dan teknologi komunikasi, sedangkan hambatan meliputi keterbatasan waktu, ketidakjelasan deskripsi tugas, serta tantangan dalam membangun identitas kolaboratif. Implikasi penelitian ini menekankan pentingnya strategi pendidikan interprofesional, penguatan identitas profesional, dan adaptasi budaya organisasi untuk meningkatkan efektivitas IPC. Kesimpulannya, optimalisasi peran apoteker dalam kolaborasi interprofesional tidak hanya meningkatkan hasil klinis dan kepatuhan pasien, tetapi juga memberikan kontribusi signifikan terhadap pengembangan teori dan praktik kolaborasi lintas profesi di pelayanan kesehatan.

Abstract

This study aims to examine the rational pharmacotherapy of antibiotic use in hospital settings, focusing on patterns, determinants, and the role of antimicrobial stewardship (AMS) in optimizing therapeutic outcomes. Employing a qualitative descriptive design through a literature review, this research synthesized findings from scientific journals, official reports, and academic studies published between 2015 and 2025. Data collection involved systematic literature tracing and document analysis, while the data analysis process consisted of thematic identification, data reduction, categorization, and inductive interpretation. The results reveal that the rationality of antibiotic use in Indonesian hospitals remains low, ranging between 6.67% and 68.03%, with key issues involving inappropriate antibiotic selection, incorrect dosing, and inadequate therapy duration. Limited AMS implementation and minimal pharmacist involvement were identified as major contributing factors. Conversely, evidence demonstrates that multidisciplinary AMS programs, particularly those led by clinical pharmacists, significantly improve prescribing rationality, reduce resistance rates, and lower healthcare costs. The findings provide a comprehensive understanding of how institutional, behavioral, and policy factors influence rational antibiotic use, offering implications for both clinical practice and healthcare policy reform. In conclusion, the study underscores the need for sustained AMS integration, pharmacist empowerment, and standardized evaluation systems to strengthen rational pharmacotherapy and combat antimicrobial resistance.

Abstract

Penelitian ini bertujuan untuk menganalisis dan mensintesis pendekatan farmakoterapi terhadap Penyakit Refluks Gastroesofageal (GERD) dengan menelaah mekanisme aksi, landasan klinis, dan paradigma pengobatan yang berkembang. Proses penelusuran literatur mengikuti pedoman Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) melalui basis data internasional seperti PubMed, ScienceDirect, dan SpringerLink. Data dianalisis secara tematik untuk mengidentifikasi fokus farmakoterapi GERD. Hasil kajian menunjukkan bahwa Inhibitor Pompa Proton (IPP) tetap menjadi terapi utama karena efektivitasnya dalam menekan asam lambung dan mempercepat penyembuhan mukosa, namun munculnya kasus refraktori terhadap IPP mendorong pengembangan Penghambat Asam Kompetitif Kalium (PCAB) yang menawarkan onset lebih cepat, kontrol asam lebih kuat, dan stabilitas farmakodinamik yang lebih baik. Selain itu, integrasi agen prokinetik, pelindung mukosa, serta terapi herbal menunjukkan tren menuju pendekatan multimodal yang menargetkan motilitas, integritas mukosa, dan jalur inflamasi. Studi ini menegaskan pergeseran paradigma dari terapi tunggal berbasis penekanan asam menuju manajemen GERD yang personal, berbasis mekanisme, dan holistik, serta menyoroti pentingnya validasi klinis jangka panjang yang terstandarisasi.

Abstract

This study aimed to evaluate the effect of counseling on medication adherence among patients with type 2 diabetes mellitus through a narrative review of 50 national and international articles published between 2020 and 2025. The narrative review method was conducted by selecting journals from Elsevier, MDPI, PubMed, SpringerLink, Wiley, as well as nationally indexed SINTA journals using relevant keywords.The results of the review indicate that pharmacist-led interventions through counseling, including brief counseling, home pharmacy care, and digital media–based education, consistently improved patient knowledge, enhanced medication adherence, and reduced clinical parameters such as blood glucose levels, HbA1c, and HOMA-IR. These interventions also improved patients’ quality of life and disease perception.Based on these findings, it can be concluded that pharmacist-led interventions through structured counseling and pharmaceutical education have a significant positive impact on the management of type 2 diabetes mellitus, particularly in improving patient knowledge, disease perception, and adherence to therapy. Improvements in therapeutic behavior directly contributed to better clinical outcomes, including reductions in blood glucose, HbA1c, HOMA-IR, body weight, and waist circumference after 90 days of intervention. The greatest effects were observed in patients with HbA1c levels > 9%, indicating that high-risk groups derived the greatest benefit from pharmacist involvement in the management of type 2 diabetes mellitus.Future studies are recommended to evaluate the long-term effects of counseling and to standardize adherence assessment instruments. Strengthening pharmaceutical care policies is also recommended to improve the quality and consistency of counseling services and to develop sustainable intervention models in primary healthcare settings.
