Abstract

Tracer is an important tool in controlling the use of medical record files. Usually used to replace medical records that are out of storage. These exit instructions remain in storage until the borrowed medical records are returned and stored again. This exit sign is equipped with a pocket for storage of request slips. The purpose of this study was to determine the factors that influence the non-application of a medical record tracer at the Pratama Klambir clinic in 2021. This research method used a descriptive qualitative method which was carried out from November to completion. The informants of this research were 6 informants, namely 1 key informant, 4 main informants and 1 triangular informant obtained by purposive sampling technique. The study obtained the man factor (human resources), namely the education of officers who did not graduate from medical records and had never attended training, machines (machines) that had not been applied to the use of tracers and lacked storage racks, materials (raw materials), namely the folders used were still made of plastic and had not used color codes, methods (methods/procedures), namely SPO had not been carried out properly, and money, namely there was a shortage of money. The conclusion in this study is that there are many factors (human resources), machines (machines), materials (raw materials), method (methods/procedures) and money (money) which causes the medical record tracer not to be applied at the Pratama Klambir clinic.

Abstract

Skin disease is a condition that affects the body's surface and has a number of causes. The most prevalent infectious illnesses that afflict people of all ages are skin conditions. The majority of skin infection treatments take a while to start working. The goal of this study was to ascertain how scavengers at the Medan City Waste Landfill's personal hygiene habits and contact dermatitis symptoms related to one another. Approach: This study uses a cross-sectional analytic survey design as its methodology. The population of this study consisted of 72 respondents who served as research samples. They were all scavengers in Environment VII of the Waterfall Village. The chi-square statistical test was utilized in the univariate and bivariate analyses. The Findings: Scavengers' symptoms of contact dermatitis were related to age (p = 0.019), education (p = 0.021), length of labor (p = 0.007), and personal hygiene (p = 0.000). The study's findings suggest that scavengers in the Medan City Waste Landfill may have contact dermatitis symptoms in relation to their age, education, length of employment, and personal hygiene. It is planned that TPA Falls would offer health education about skin conditions, including dermatitis, so that scavengers may avoid contracting these illnesses by practicing good personal hygiene after work, including as cleaning their hands, feet, and taking a shower.

Abstract

The World Health Organization (WHO) predicts that between 2010-2014, there would be 300-400 maternal deaths per 100,000 live births, with hemorrhage accounting for 28% of these deaths and preterm membrane rupture for 20%. Premature membrane rupture is a complication of preterm pregnancy that significantly increases the risk of perinatal death and premature birth. The purpose of this study is to ascertain the association between the prevalence of early membrane rupture at RSU Haji Medan and the age and parity of pregnant women. This study adopted a cross sectional methodology and an analytical survey research design. All pregnant women who suffered early membrane rupture and other problems made up the population of this study, and a total of 71 pregnant women were sampled. Data from medical records as a source It was done using both univariate and bivariate analysis. According to the study's findings, there were 35 pregnant women, of whom 13 were under 20 years old (18.3%), 11 were between 20-35 years old (15.5%), and 11 > 35 years old (15.5%) when their membranes ruptured prematurely. With a 95% confidence interval, the values are = 0.005 and P = 0.046. Consequently, the findings indicate that there is a correlation between age and the frequency of premature membrane rupture, with ? = 0.005 and P = 0.022. The findings thus indicate a connection between parity and the frequency of early membrane rupture at RSU Haji Medan.

Abstract

Malnutrition still poses a serious threat to world health and has far-reaching effects on both individuals and societies. The objective of this study is to objectively evaluate and assess the efficacy of various approaches used to combat hunger and promote healthy nutrition on a worldwide scale. We used a mixed-methods approach to gather quantitative and qualitative information from a variety of demographics, including kids, teens, adults, and elderly people, in order to evaluate their nutritional health and the prevalence of malnutrition. The prevalence of stunting and wasting among children under the age of five was found to be alarming, highlighting the urgent need for focused interventions to enhance child nutrition. The study also brought attention to the dual effects of malnutrition, with considerable rates of undernutrition and overnutrition seen across a range of age groups. Programs for nutrition education have been demonstrated to be helpful at raising participants' nutrition-related knowledge and encouraging healthier eating habits. Initiatives to fortify foods have shown promise in treating micronutrient deficiencies, and improvements to agriculture and food production have had a favorable influence on the variety and accessibility of food. Programs for maternal and child health made a significant contribution to the decrease in stunting and low birth weight by highlighting the value of comprehensive prenatal care in ending the cycle of malnutrition. The study emphasizes the need for context-specific and evidence-based measures to combat malnutrition and promote healthy nutrition, offering insightful information for decision-makers in healthcare and policy. We may endeavor to find lasting and fair solutions to this global health issue by utilizing effective tactics and resolving constraints. In order to enhance nutrition outcomes and general health on a larger scale, more study is needed to examine the cost-effectiveness and long-term effects of interventions.

Abstract

Objective: This study aimed to investigate the impact of midwifery-led care on maternal outcomes, maternal satisfaction, and newborn health in a hospital setting. Specifically, the study sought to compare the effects of midwifery-led care to standard care models, assess maternal preparedness for childbirth, explore rates of medical interventions, evaluate maternal satisfaction, and examine newborn health outcomes. Methods: A total of 200 expectant mothers receiving prenatal care at a tertiary hospital were recruited and randomly assigned to two groups: Group A (midwifery-led care) and Group B (standard care). Participants' demographic information, medical history, and preferences related to prenatal care were collected using a structured questionnaire. Validated surveys, including the Edinburgh Postnatal Depression Scale (EPDS), were used to assess postpartum depression. Medical records and prenatal visits were reviewed throughout the pregnancy, and postpartum follow-ups were scheduled to assess maternal outcomes. Statistical analyses were performed to compare outcomes between the two groups. Results: Participants in the midwifery-led care group reported significantly higher levels of birth preparedness (p < 0.001), and rates of medical interventions during childbirth, including cesarean sections, were significantly lower compared to the standard care group (p < 0.05). Maternal satisfaction scores were significantly higher in the midwifery-led care group (p < 0.001). Additionally, the midwifery-led care group showed lower rates of postpartum depression (p < 0.01) and higher rates of exclusive breastfeeding at six weeks postpartum (p < 0.05). Neonates born to mothers in the midwifery-led care group had significantly lower rates of neonatal complications (p < 0.01). Conclusion: The findings of this study provide strong evidence for the positive impact of midwifery-led care on maternal outcomes, maternal satisfaction, and newborn health. The personalized and woman-centered approach of midwifery-led care contributes to improved maternal preparedness, reduced medical interventions, increased maternal satisfaction, lower rates of postpartum depression, and better newborn health outcomes. These results highlight the potential benefits of integrating midwifery-led care models into maternity services, aiming to enhance the overall birthing experience and improve maternal and infant health outcomes.
