Abstract

Background: Diarrhea is a medical condition characterized by a change in the consistency of the feces, resulting in softer, thinner, or watery stools. It typically occurs more than three times per day. This condition is caused by pathogenic microorganisms and is influenced by various risk factors, including environmental conditions, community behavior, healthcare services, diet, education, and socio-economic status. Objective: To identify the risk factors that impact the occurrence of diarrhea in young children at the Mamboro Community Health Center. Research Design: This study employs a descriptive analytical method with a cross-sectional approach. The study included 74 respondents, divided into two groups: those with diarrhea and those without. Data were collected from primary sources through questionnaires and secondary data from KIA book records. The data were analyzed using the Chi-Square statistical method. Results & Findings: The study found a significant association between a history of exclusive breastfeeding (P=0.002, P<0.005) and a history of allergies (P=0.000, P<0.005) with the occurrence of diarrhea in toddlers at the Mamboro Health Center. Conclusion: The history of exclusive breastfeeding significantly impacts the occurrence of diarrhea in toddlers. Additionally, there is a strong correlation between a history of allergies and the incidence of diarrhea at the Mamboro Health Center.

Abstract

Aim and objective of this research is to establish the relating and contributory factors to the formation of breast milk among post-caesarean section (SC) mothers at Embung Fatimah Regional Hospital Batam City. Breastfeeding is known to offer lots of health benefits to the baby, and the mother as well; unfortunately, many post-SC mothers face difficulties in lactation. Sequential exploratory design was used: quantitative data was collected from 54 post-SC mothers, qualitative data from post-SC mothers, midwives and health staff. Some of the factors that were examined in the study included; frequency of feedings, breast care, rooming-in, maternal diet, birth weight, feeding techniques employed and the gestational period. Altogether, quantitative findings pointed towards the association of independent variables with smooth breast milk production as follows: regularly frequency of breastfeeding (p = 0. 000), proper care of the breasts (p = 0. 002), rooming in (p = 0. 002), adequate diet of the mothers (p = 0. 000), mothers’ gestational age /full term/b (p = 0. Other observable variables such as mode of feeding which includes the combined nursing care and birth weight do not significantly predict each other. Most dominant factors considered by the logistic regression analysis were maternal nutrition and the frequency of breastfeeding. The qualitative results that supported the significance of educating women on how to breast feed, proper care of the breast and the value of family support on breastfeeding results. There was also an indication that mothers who were closely monitored by midwives and other health care givers experienced less problems in their milk flow. These results emphasise the need to enhance the provisions of breastfeeding education, healthcare and nutrition; especially for post-SC mothers to ease successful breastfeeding and improve the health of both mother and child.

Abstract

The rising prevalence of hypertension in South Nias Regency, particularly within the UPTD working area of Lolowau Health Center, has become a critical public health concern. POSBINDU (Integrated Development Post for Non-Communicable Diseases) services are underutilized, despite being vital for early detection and management of hypertension. This study aims to identify the key factors influencing behavioral changes in hypertensive clients attending POSBINDU within this region. A cross-sectional quantitative research design was employed, gathering data from 45 hypertensive clients registered at the Lolowau Health Center. Data were collected through structured questionnaires and analyzed using multiple linear regression and chi-square tests to determine significant relationships between variables such as knowledge, attitude, employment status, family support, and health worker information. The results revealed that knowledge (p = 0.014), attitude (p = 0.033), employment status (p = 0.011), family support (p = 0.003), community leader support (p = 0.023), and health worker information (p = 0.009) were significantly associated with behavior changes among hypertensive clients. Notably, family support emerged as the most influential factor in promoting positive behavior change, with an Exp B value of 12.346. These findings underscore the importance of enhancing family involvement, improving health worker communication, and fostering community leadership to increase the engagement of hypertensive clients with POSBINDU services. Strengthening these factors can lead to more effective hypertension management and improved health outcomes within the community.

Abstract

The phenomenon of occupational health and safety (Kesehatan dan Keselamatan Kerja, or K3) implementation has not been widely adopted in the micro, small, and medium enterprise (MSME) sector, as most MSMEs still do not consider occupational health and safety as a priority. Many view its implementation as an expense, which can potentially lead to significant issues. These include challenges related to worker welfare and business conditions, fire safety, financial burdens resulting from work accidents or occupational diseases, and an uncomfortable work environment that diminishes worker motivation. This situation increases the risk of work accidents, occupational diseases, and decreased productivity. All these issues stem from the lack of awareness about occupational health and safety among MSME actors. One effort to encourage the adoption of occupational health and safety practices in MSMEs is through the Work Improvements in Small Enterprise (WISE) program developed by the International Labour Organization (ILO). This program aims to assist MSMEs in improving working conditions and productivity through participatory training. The training is designed to educate MSME actors, fostering a shift from unawareness to awareness of occupational health and safety practices. The purpose of this study is to determine the effectiveness of the Work Improvements in Small Enterprise program through participatory training, analysis, implementation, and evaluation of MSME actors. This research adopts a qualitative approach using a case study method to provide a comprehensive description of how the Work Improvements in Small Enterprise program is implemented. The study is based on observation data, surveys, and documentation of various tasks and workplaces within a tofu factory, employing a risk identification approach to analyze and identify control measures for these risks. The findings indicate that various tasks in the tofu factory pose specific hazards. Providing training to tofu factory workers through the Work Improvements in Small Enterprise program has proven effective in addressing these risks. The study concludes that participatory training is an effective solution to mitigate potential hazards, preventing work accidents and promoting safer working conditions.

Abstract

This study examines the key factors influencing patient loyalty at RSU Dr. R.M. Djoelham Binjai, focusing on patient satisfaction, hospital image, and trust. The purpose is to determine how these factors impact patients' intentions to return for future healthcare services. A quantitative research design was employed, using a cross-sectional analytical survey conducted on 93 inpatients from Super VIP, VIP, and Class 1 rooms. Data were collected through structured questionnaires and analysed using univariate, bivariate, and multivariate methods to explore the relationships between satisfaction, image, trust, and revisit intentions. The results demonstrate that higher patient satisfaction significantly increases the likelihood of returning to the hospital. A positive hospital image also plays a crucial role in fostering patient trust, which further enhances revisit intentions. The multivariate analysis confirms that patient satisfaction is the most significant predictor of loyalty, followed closely by trust and hospital image. In conclusion, the study highlights the importance of maintaining high service quality, a positive hospital image, and strong patient trust to enhance patient loyalty. Hospital management should focus on these areas to improve patient retention and ensure ongoing patronage.

Abstract

Background: Cigarettes are a major cause of avoidable deaths in society. A non-smoking area is a designated space that explicitly prohibits the manufacturing, sale, advertising, promotion, and/or use of cigarettes. The Subulussalam City Government has implemented Subulussalam Mayor Regulation Number 52 of 2016, which prohibits smoking in designated areas. The purpose of this study is to identify the factors that impact visitor compliance with the implementation of Regional Regulation Policy Number 52 of 2016, which pertains to non-smoking areas in Subulussalam City Hospital's BLUD. The study employed an analytical survey methodology, utilizing a cross-sectional approach. We selected a subset of 86 individuals using the incidental sampling approach from the overall population of 636 individuals. The data analysis included univariate, bivariate, and multivariate analysis techniques, together with logistic regression test statistics. Outcome: The logistic regression analysis revealed significant findings for the variable’s knowledge (p-value = 0.000), attitude (p-value =.009), and the role of health personnel (p-value =.001). Conclusion: The knowledge, attitudes, and duties of health workers influence the implementation of Regional Regulation Number 52 of 2016, which pertains to non-smoking areas. We anticipate that the hospital will enhance visitor awareness by disseminating information through healthcare professionals and using posters, banners, and other informational media. This will help visitors understand the potential hazards to the hospital's surroundings and adhere to the hospital's designated no-smoking zone.

Abstract

Introduction: The hospital staff exert an impact on the accessibility of potable water inside the hospital premises. The provision of uncontaminated water is crucial for the provision of essential services in hospitals. Both the quality and quantity of water must meet stringent standards to prevent the spread of infections and diseases within the hospital premises. Similarly, efficient drainage systems are necessary to ensure proper water distribution. Goal: Determine the variables that influence the accessibility of uncontaminated water at the Muhammad Ali Kasim Gayo Lues General Hospital in 2023. Method: This study employs an analytical survey research method utilizing a cross-sectional methodology. The research population consisted of 300 officers and personnel at the hospital. A sample of 75 individuals was selected using the Slovin formula approach. The data analysis included both univariate and bivariate analysis, utilizing logistic regression test statistics. The Analysis Result: The chi-square test analysis revealed that the p-values for each variable were as follows: p-value = 0.010 for method, p-value = 0.000 for machine, p-value = 0.000 for material, and p-value = 0.003 for money. Conclusion: The availability of clean water is influenced by factors such as human activity, techniques used, machinery involved, materials used, and financial resources. It is expected that the hospital will prioritize and foresee the provision of water inside the hospital premises to provide an adequate and high-quality supply for everyday activities.

Abstract

Malnutrition remains a persistent global health challenge with profound implications for individuals and communities. This study evaluates the role of nursing in addressing malnutrition and strengthening healthcare systems to improve accessibility and quality of care. Utilizing a mixed-methods approach, quantitative and qualitative data were collected from diverse populations, including children, adolescents, adults, and older individuals, to assess the prevalence of malnutrition and the effectiveness of nursing-led interventions. Findings indicate a concerning prevalence of stunting and wasting among children under five, underscoring the urgency of targeted nutrition-focused initiatives. Additionally, the study highlights the dual burden of malnutrition, where both undernutrition and overnutrition coexist across various age groups. Evidence from this research supports the effectiveness of nursing-led health promotion programs, including nutrition education, maternal and child health services, and food fortification initiatives, in reducing malnutrition-related morbidity. Nurses play a crucial role in identifying at-risk populations, implementing community-based nutrition programs, and advocating for policy changes that promote food security and sustainable healthcare interventions. Furthermore, nursing integration within interprofessional healthcare teams has been shown to enhance patient-centered care, increase vaccination rates, improve chronic disease management, and expand healthcare access through telehealth services. Despite the proven benefits of nursing-led interventions, challenges remain, including workforce shortages, healthcare disparities, and administrative constraints that hinder program implementation. Addressing these barriers requires strong policy support, workforce development, and investment in nursing education to ensure equitable access to high-quality nutrition and healthcare services. This study emphasizes the importance of evidence-based, context-specific nursing interventions in combating malnutrition and advancing public health. Future research should focus on the cost-effectiveness and long-term impact of these interventions to further optimize public health outcomes.

Abstract

Introduction: Referrals are carried out based on medical indications so that community health centers and their networks can filter referrals. The problem that occurs in patients' understanding of referral services is that socialization to the community is still very limited. The results of interviews with 10 outpatients at the UPT Puskesmas Sawit Seberang showed that 6 patients (60%) still did not understand the referral criteria, while the other 4 patients (40%) clearly understood the outpatient referral system. Objective; to determine the factors that influence BPJS patients' lack of understanding of the outpatient referral system at the Sawit Seberang Community Health Center UPT, Langkat Regency. Methods: This type of research is quantitative using an analytical survey design with a cross-sectional approach. The research population was all 781 BPJS patients with a sample of 89 people taken using accidental sampling technique. Data analysis used univariate, bivariate analysis with the chi-square statistical test and multivariate analysis with the logistic regression test. Results: The results of the research show that the p value for the knowledge variable is p = 0.020, attitude p = 0.017, information p = 0.012 and health workers p = 0.025, which means that there is an influence of knowledge, attitudes, information and health workers on BPJS patients' understanding of the outpatient referral system. Conclusion: The research conclusion is that there is an influence of knowledge, attitudes, information and health workers on BPJS patients' understanding of the outpatient referral system at the UPT Puskesmas Sawit Seberang, Langkat Regency.

Abstract

Genetically modified organisms (GMOs) have revolutionized agriculture, offering the potential for enhanced food production, improved nutritional content, and resistance to diseases. As the global demand for food increases, biotechnology and genetic engineering have emerged as essential tools to address food scarcity. The rapid development of these technologies has led to the widespread use of genetically modified crops, making it necessary to adopt efficient methods for detecting GMOs in food products. This study explores various detection techniques for GM foods, focusing on the application of Polymerase Chain Reaction (PCR), Capillary Gel Electrophoresis (CGE), Enzyme-Linked Immunosorbent Assay (ELISA), Next-Generation Sequencing (NGS), and biosensors. While PCR remains the gold standard for GMO detection, advances such as multiplex PCR, CRISPR-based methods, and NGS offer enhanced sensitivity and specificity, allowing for the detection of multiple GM traits and minimizing false positives and negatives. Biosensors, particularly DNA-based systems, provide a rapid, cost-effective, and portable option for on-site detection, while NGS offers a comprehensive approach to analyze entire genomes. This paper reviews the strengths, limitations, and applications of these methods, discusses their integration for improved accuracy, and highlights their role in regulatory compliance and ensuring food safety. Ultimately, the integration of these advanced techniques promises a robust solution for GMO detection, supporting regulatory authorities in monitoring and labeling GM foods, safeguarding public health, and enhancing transparency in the food supply chain.
