Abstract

This study aims to evaluate the effectiveness of supportive therapy combined with family psychoeducation in treating a 32 years old male patient diagnosed with disorganized schizophrenia (F20.1). Disorganized schizophrenia is a subtype of schizophrenia characterized by behavioral disorganization, speech incoherence, and inappropriate affect, which often triggers stigma and a heavy burden of care for families. The participant exhibited clinical symptoms such as delusions of control (controlled by power of Ratu Kidul), auditory and tactile hallucinations, inappropriate affect (giggling), and sharp relational conflict with his stepfather. The intervention was carried out using a single case study design with Supportive Psychotherapy techniques that included emotional ventilation, reassurance, and persuasion, as well as environmental modification through family and community psychoeducation. The intervention results showed a significant decrease in positive and negative symptoms, increased insight, and improved social functioning. The client was able to control his emotions, establish more organized communication, and regain acceptance from his family and social environment. These findings confirm that a supportive therapy approach that integrates social and family support is highly effective in the functional recovery of psychotic patients with a dependent personality background and double-bind parenting style.

Abstract

Background: Preoperative anxiety is a common response to perceived physical and psychological threats that may affect hemodynamic stability, increase postoperative pain, prolong hospitalization, and even lead to surgery cancellation. Limited knowledge of anesthesia procedures is considered an important contributing factor because inadequate information can increase fear and unpreparedness before surgery. Methods: This cross-sectional study involved 126 patients undergoing elective surgery at Raden Mattaher Regional General Hospital, Jambi. Respondents were selected based on the criteria of age ≥18 years, ASA physical status I–II, literacy, and willingness to participate. Data on anesthesia-related knowledge and preoperative anxiety were collected using structured questionnaires. The relationship between variables was analyzed using the Kolmogorov–Smirnov test because the assumptions for the Chi-square test were not fulfilled. Results: Most respondents were aged 51–60 years, had completed senior high school, were employed, had no previous surgical experience, presented with hypertension, and had normal pulse rates. Moderate knowledge (48.4%) and moderate anxiety (48.2%) were the most common findings. Statistical analysis demonstrated a significant association between knowledge of anesthesia procedures and preoperative anxiety (p = 0.0001). Conclusion: Improving patients' knowledge through comprehensive preoperative education may reduce anxiety, enhance readiness, and support physiological stability before elective surgery.

Abstract

Hypertension is a chronic disease with a high global prevalence and a major risk factor for cardiovascular complications. Angiotensin-Converting Enzyme Inhibitors (ACE-Is) are widely recommended as first-line antihypertensive therapy, making evaluation of their rational use essential to ensure treatment safety and effectiveness. This descriptive observational study employed a retrospective design using medical records of hypertensive outpatients at Puskesmas Simpang IV Sipin, Jambi, from January 2023 to December 2024. A total of 100 eligible patients were included. The evaluation assessed the appropriateness of indication, dosage, frequency, duration of therapy, and potential drug interactions based on World Health Organization (WHO) criteria and the JNC 8 guidelines. Most patients were aged 60 years or older and were diagnosed with essential hypertension or hypertensive heart disease. All ACE-I prescriptions were appropriate for their clinical indications (100%), while dosage, frequency, and treatment duration fully complied with guideline recommendations. Most identified drug interactions were synergistic, whereas antagonistic (11%) and potentiation (25%) interactions were generally mild to moderate and clinically manageable. Overall, ACE-I prescribing was considered rational. Continuous monitoring remains important, particularly for patients at risk of clinically significant drug interactions to optimize therapeutic outcomes and patient safety.

Abstract

Ectopic pregnancy occurs when a fertilized ovum implants outside the uterine cavity and may cause life-threatening internal bleeding. Although the intrauterine device is an effective contraceptive method, pregnancies resulting from contraceptive failure have a greater likelihood of becoming ectopic. This study aimed to determine the relationship between intrauterine device use and the incidence of ectopic pregnancy at RSUD Raden Mattaher and RSI Arafah Jambi. An analytical observational design with a retrospective approach was applied. The samples were selected using total sampling from the medical records of first-trimester pregnant women treated between 2020 and 2024 who met the inclusion and exclusion criteria, resulting in 99 respondents. Statistical analysis showed a p-value of 0.013 (p < 0.05) and an odds ratio of 0.172. These findings indicate a statistically significant association between intrauterine device use and the incidence of ectopic pregnancy, with intrauterine device use associated with a lower risk of ectopic pregnancy in the study population. Therefore, intrauterine device use was significantly related to the incidence of ectopic pregnancy among patients at RSUD Raden Mattaher and RSI Arafah Jambi.

Abstract

Chocolate (Theobroma cacao) is a natural source of bioactive compounds, particularly flavonoids such as catechin, epicatechin, and procyanidins, which exhibit potent antioxidant, anti-inflammatory, and tissue-regenerative properties. These bioactive substances have attracted increasing attention in regenerative medicine due to their potential to influence cellular functions and promote tissue repair. Recent studies suggest that cacao-derived compounds play a significant role in regulating stem cell biology by enhancing cell survival, proliferation, differentiation, and resistance to oxidative stress. The antioxidant activity of flavonoids helps reduce reactive oxygen species (ROS), thereby protecting stem cells from cellular damage and maintaining their viability during regeneration. In addition, flavonoids may modulate various signaling pathways, including Wnt/β-catenin, PI3K/Akt, and MAPK, which are essential for stem cell self-renewal and lineage commitment. These molecular mechanisms contribute to improved regenerative capacity and tissue healing. Although current evidence demonstrates promising therapeutic potential, most findings are derived from in vitro and animal studies, indicating the need for further clinical investigations to establish safety, optimal dosage, and long-term efficacy in humans. Overall, cacao bioactive compounds represent a promising natural therapeutic agent for stem cell-based regenerative medicine and may contribute to the development of innovative strategies for tissue engineering and the treatment of degenerative diseases.
