Abstract

Background: Preterm infants are a group of infants with a high risk of various health problems, especially respiratory disorders that can affect their oxygenation status and cardiorespiratory function. Body positioning of preterm infants in the neonatal intensive care unit (NICU) is one of the important interventions that can affect lung function and hemodynamic stability Purpose:  To determine the effect of body position on cardiorespiratory status in preterm infants. Method: This systematic review used the PICO search strategy.  The data sources used came from 3 databases namely Pubmed, ProQuest, and ScientDirect. Based on the database, 5 articles were obtained that met the inclusion criteria. Inclusion criteria include respondents are premature infants < 37 weeks gestational age, articles used in the last 10 years (2016 to 2026) and use English. Results: The study showed that position change interventions had a significant effect on the cardiorespiratory status of preterm infants. Pronation (prone) positioning and its variations, such as quarter prone positioning help maintain cardiorespiratory stability of preterm infants. However, there are certain indications and contraindications in the application of pronation position that need to be considered. Conclusion: Pronation and semi pronation positions have been shown to increase lung expansion, oxygen saturation, and decrease respiratory frequency and pulse rate, thus helping to maintain cardiorespiratory stability of preterm infants.

Abstract

Background: The toddler period, often referred to as the golden period, is critical for optimizing child growth and development. Based on Padang Health Office data (2024), Community Health Center Pemancungan achieved the highest performance in growth and development monitoring indicators, while Community health center Pauh recorded the lowest. Purpose: To comparatively analyze the implementation of health policies in toddler growth and development monitoring between Community health center Pemancungan and Community health center Pauh in Padang City. Method: A qualitative design using an input–process–output system approach. Informants included the mayor, head of the health office, heads of Community health center, program managers, regional supervisors, and Integrated health service post cadres. Data were collected through interviews, observation, and documentation, and analyzed using the Miles and Huberman method. Results: Policy implementation refers to Minister of Health Regulation No. 6 of 2024 on Minimum Service Standards and is operationalized through Integrated health service post activities, including weighing, height measurement, vitamin A supplementation, and developmental screening. Although Community health center Pauh has more human resources and facilities, utilization remains suboptimal. Both Community health center show gaps in implementation processes, particularly in adherence to standards, recording, and reporting. Output indicators indicate that monthly monitoring is conducted but has not yet achieved program targets. Conclusion: The implementation of toddler growth and development monitoring policies in both Community health center remains suboptimal. Strengthening planning, improving implementation quality, and reinforcing continuous monitoring and evaluation are essential to enhance program performance.

Abstract

Background: Gallbladder cancer is the most common tumor of the biliary tract and is recognized as one of the most aggressive gastrointestinal malignancies with a poor prognosis. Most cases are diagnosed at an advanced stage, which limits therapeutic options. According to data from the International Agency for Research on Cancer through the Global Cancer Observatory (GLOBOCAN) on 2018, gallbladder cancer ranks 22nd in global cancer incidence and 17th among the leading causes of cancer-related mortality worldwide. Purpose: To describe the diagnostic approach and surgical management of a patient with suspected malignant gallbladder tumor. Method: Descriptive analytic research used a case study approach involving a 51-year-old woman presented with a chief complaint of right upper abdominal pain that had persisted for three months prior to hospital admission. Clinical and diagnostic evaluations were performed using abdominal ultrasonography, multislice computed tomography (MSCT), and magnetic resonance imaging with Magnetic Resonance Cholangiopancreatography (MRI-MRCP) to establish the diagnosis and determine tumor staging. Results: Radiological findings revealed a mass in the gallbladder suggestive of a malignant gallbladder tumor with a clinical stage of cT3N1M0. The patient subsequently underwent surgical intervention through an open laparotomy approach consisting of cholecystectomy combined with resection of tumor-invaded tissues or adjacent organs. Postoperatively, the patient’s condition was stable, and further oncological management with chemotherapy was planned as part of a comprehensive treatment strategy. Conclusion: Gallbladder tumor at stage cT3N1M0 represents an aggressive malignancy that is often diagnosed at an advanced stage. A multimodal diagnostic approach and adequate surgical resection are essential in the management of this condition to improve patient survival outcomes.

Abstract

Background: Dangke is a traditional fermented dairy product from South Sulawesi, Indonesia, with considerable potential to be developed as a functional food based on local wisdom. Beyond its role as a source of animal protein, dangke contains lactic acid bacteria and bioactive compounds that may confer health benefits. However, scientific evidence regarding its nutritional value, microbiological characteristics, and bioactive potential remains fragmented and has not been systematically synthesized. Purpose: To identify, synthesize, and critically evaluate various research results related to nutritional composition, microbiological safety, and potential of bioactive compounds in dangke. Method: This study employed a literature review design by analyzing national and international scientific articles published between 2015 and 2025. Literature searches were conducted using PubMed, Google Scholar, ScienceDirect, and ResearchGate databases. Eligible articles were analyzed narratively through summarizing, synthesizing, comparing, and critically appraising processes to evaluate the nutritional composition, microbiological properties, and bioactive potential of dangke. Results: A total of 10 relevant studies were included. The findings indicate that dangke contains relatively high protein levels (approximately 14–18%), fat, and essential micronutrients such as vitamin A, vitamin C, zinc, and selenium. Microbiologically, dangke is predominantly composed of lactic acid bacteria, particularly Lactobacillus plantarum and Lactobacillus fermentum, which exhibit probiotic potential. Fermentation processes produce bioactive peptides associated with antihypercholesterolemic, antihyperglycemic, anti-obesity, antimicrobial, and immunomodulatory effects, primarily supported by experimental animal studies. Conclusion: Based on available scientific evidence, dangke demonstrates a combination of nutritional value, beneficial microbiological activity, and bioactive potential that supports its role as an Indonesian functional food. Nevertheless, broader application requires standardized production methods, improved food safety practices, and further human clinical studies to strengthen the level of evidence.

Abstract

Background: One of the families at risk experiencing stunting, namely Mother which has child age toddler or those who are breastfeeding. Age toddler as stages development enough children​ prone to to various attack diseases, including diseases caused by deficiencies​ or excess intake nutrition. Purpose: To analyze the effect of home visits and video-based health counseling on mothers' roles in managing stunting among toddlers. Method: Quasi-experimental design with pre-test and post-test approach. Population study is mother who has toddlers in the Wajo Regency. Samples selected use purposive sampling technique. Data collected use questionnaire structured to measure knowledge, attitudes, and practices mother, and sheet observation to monitor growth children. The interventions provided in the form of visit home and education health video- based. Data analysis was performed using statistical tests in pairs to see difference before and after intervention. Results: Showed a significant improvement in mothers’ knowledge, attitudes, and practices following the intervention. Average scores showed an increase between the pre-test and post-test. Statistical test results showed a p-value < 0.05 for all variables, indicating that the intervention had a significant effect. In addition, there were indicators of improved child growth, such as weight and height. Conclusion: Visit home and education health proven video based effective in increase role mother in stunting management. Intervention This can recommended as a strategy in a child-based stunting prevention program public.

Abstract

Background: My Healthy Indonesia Application is a cornerstone of Indonesia's health system digital transformation, designed to streamline vaccination recording and reporting. However, its implementation at community health centers in Padang City faces challenges, marked by data discrepancies and suboptimal vaccination coverage. Purpose: To identify healthcare worker performance in implementing the healthy Indonesia application. Method: A mixed-methods study was conducted in five Community Health Center in Padang City. The quantitative phase employed a cross-sectional design with total sampling of 127 health workers. Data were collected using validated questionnaires and analyzed using chi-square and multivariate logistic regression. The qualitative phase involved in-depth interviews with 13 key informants, analyzed using thematic analysis. This study investigates factors associated with health workers' performance in using My Healthy Indonesia Application and explores underlying reasons behind these challenges. Results: Quantitative analysis revealed that motivation (p=0.014), leadership (p=0.018), and workload (p=0.009) were significantly associated with performance, with motivation as the most dominant factor (OR 0.254; p=0.011). Knowledge (p=0.602) was not significantly associated. Qualitative findings generated seven themes: complex motivational dynamics dominated by professional responsibility; significant infrastructure limitations (reliance on personal devices/internet); persistent application bugs (data loss, duplication); increased administrative workload; a digital generation gap; adaptive strategies (time management, team collaboration); and a clear recommendation for a single, integrated application. Conclusion: Health worker performance in using My Healthy Indonesia Application presents a paradox: good performance persists despite significant systemic barriers, primarily driven by strong intrinsic motivation. However, this reliance on individual resilience is unsustainable. Improving My Healthy Indonesia Application effectiveness requires holistic interventions addressing technical issues, reducing workload, bridging the digital divide, and nurturing the professional dedication of health workers.

Abstract

Background: Heart failure (HF) is a complex clinical syndrome with an increasing prevalence that contributes to decreased quality of life and increased mortality. Adherence to medication therapy and a low-sodium diet is a key component of HF management; however, patient adherence remains low. Various factors, including psychosocial aspects, influence this condition. Purpose: To identify and synthesize psychosocial factors that significantly influence adherence to medication therapy and a low-sodium diet in patients with heart failure based on the literature published between 2015 and 2025 Method: This systematic literature review was conducted in accordance with the PRISMA guidelines. A literature search was performed across six databases using the PICOT framework: PubMed, ProQuest, Scopus, EBSCOhost, SAGE, and ClinicalKey for Nursing. The quality of the articles was assessed using the Joanna Briggs Institute (JBI) critical appraisal tools. Results: A total of 10 articles with a combined sample of 5.611 participants pant were analyzed. The results indicate that depression and anxiety are negative psychological predictors that reduce patients’ motivation and self-regulation regarding adherence. Depression emerged as the dominant factor associated with decreased medication adherence. Social factors, particularly social support, act as protective factors that improve treatment adherence. As a component of social support, family support specifically contributes to adherence to a low-sodium diet through a proxy adherence mechanism. Conclusion: Psychosocial factors influence adherence to therapy in patients with heart failure. Therefore, a holistic approach integrating psychosocial interventions is needed to optimize therapeutic adherence and improve clinical outcomes.

Abstract

Background: Emotional dysregulation is a major risk factor for varios mental health issues and maladaptive behaviors in adolescents, including self-harm and sacred ideation. Self-compassion (SC) based interventions have been recognized as a promising approach to counter self-criticism and enhance emotional coping skills. Purpose: To evaluate and synthesize the evidence regarding the effectiveness of SC interventions in mental health of adolescents. Method: This systematic review followed the PRISMA 2020 guidelines. Literature searches were conducted in PubMed, ScienceDirect, ProQuest, and Sage Journals for studies published between 2016 and 2025. Eligible studies included adolescents aged 10–19 years receiving self-compassion–based interventions, using RCT or quasi-experimental designs, and reporting mental health outcomes. Screening was conducted using Rayyan after duplicate removal. Methodological quality was assessed using the Cochrane Risk of Bias 2.0 and ROBINS-I tools. Results: All eight studies consistently demonstrated that self-compassion based interventions led to a significant improvement in emotion regulation capacity, encompassing enhanced adaptive cognitive strategies (eg, positive reappraisal) and behavioral aspects of emotional regulation improvement. The interventions prove effective in both group and individual settings. However, most studies presented methodological limitations, including small sample sizes, quasi-experimental design and short follow-up periods. Conclusion: Self-compassion based intervention is an effective and clinically relevant approach for enhancing emotion regulation in adolescents, especially for those in high-risk groups. Further research, ideally large-scale RCTs, with standardized measurements and long-term follow up, is necessary to strengthen the external validity of these findings and support broader program implementation in schools and clinical services.

Abstract

Background: Rapid urbanization has increased exposure to multiple environmental stressors including air pollution, inadequate housing, and limited access to green open spaces, all of which adversely affect respiratory health. Understanding the interplay of these factors is essential for developing evidence-based urban health policies. Purpose: To synthesize evidence on the determinants of urban environments — specifically air pollution, housing conditions, and green open spaces — and their effects on respiratory health outcomes. Method: This study employed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology. Literature searches were conducted using keywords "urban environment", "air pollution", "housing", "green space", and "respiratory health" across databases. A total of 15 articles published between 2022 and 2025 met the inclusion criteria. Results: Evidence consistently demonstrates that exposure to PM2.5, NO2, and traffic-related air pollutants is significantly associated with reduced lung function and increased incidence of asthma, COPD, and acute respiratory infections. Poor housing conditions — particularly overcrowding, dampness, and inadequate ventilation — exacerbate respiratory disease burden. Access to green open spaces was associated with protective respiratory effects. Conclusion: Urban environmental determinants collectively and interactively shape respiratory health. Integrated urban planning that reduces air pollution, improves housing standards, and expands green infrastructure is critical for respiratory health promotion in rapidly growing cities.

Abstract

Background: The emergence of industry 4.0 in Indonesia has transformed various aspects of life, including employment systems and workplace dynamics. This industrial revolution integrates humans, machines, and internet-based technologies into production processes through innovative methods. Along with the rapid growth of start-up companies, new work patterns have emerged, including a culture of excessive productivity known as hustle culture, which may affect workers’ mental health. Purpose: To analyze the relationship between hustle culture and stress levels among start-up employees in the industry 4.0 era. Method: A literature review approach was employed analyzing 7 scientific journals on hustle culture, occupational stress, and employment transformation in the industry 4.0 era. Relevant articles were selected and reviewed to identify the impact of hustle culture on workers’ stress levels and mental well-being. Results: The hustle culture was associated with increased stress levels among workers. High job demands, pressure for extreme productivity, and imbalance between work and personal life were identified as major contributing factors to work-related stress, burnout, depression, and reduced psychological well-being. Conclusion: Hustle culture has a significant impact on workers’ stress levels in the industry 4.0 era. Organizations should implement effective work stress management and create a supportive work environment that promotes work–life balance and mental well-being. These efforts may reduce the negative effects of occupational stress while improving productivity, job satisfaction, and employee retention.
