Abstract

Background: Palliative care is a healthcare approach that aims to improve the quality of life of patients with life-threatening illnesses through symptom management, psychosocial and spiritual support, and effective communication. The need for palliative care in the Intensive Care Unit (ICU) continues to increase alongside the growing complexity of critically ill patients. Nurses, as frontline healthcare providers, play a crucial role in implementing palliative care and therefore require comprehensive and standardized core competencies. Purpose: To examine research findings related to core competencies in palliative care among ICU nurses and to identify the factors influencing these competencies in clinical practice. Method: This study employed a literature review design using a narrative synthesis approach. Literature searching were conducted in PubMed, Scopus, ScienceDirect, SAGE, and Google Scholar for articles published between 2020 and 2025. Among the 312 articles identified, eight studies met the inclusion criteria and were analyzed in depth. Results: The findings indicate that core competencies in palliative care among nurses consistently encompass several key domains, including clinical symptom management, therapeutic communication, team collaboration, ethical and legal aspects, psychosocial and spiritual support, and self-reflection and professionalism. However, the level of nurses’ competencies, particularly in the ICU, remains predominantly in the low to moderate categories. Various factors influencing competency include work experience, level of education, self-directed learning ability, attitudes toward death, and moral distress. In addition, studies have revealed that palliative care education and training remain suboptimal, which contributes to variations in practice and quality of care. Conclusion: Palliative care competencies among ICU nurses still require strengthening, particularly through structured education, competency-based training, and the integration of palliative care into clinical practice. Strengthening educational systems and organizational support is essential for improving the quality of palliative care services in the ICU.

Abstract

Background: Cervical cancer is the fourth most common cancer in women worldwide, disproportionately affecting those in low- and middle-income countries (LMICs). Despite the availability of effective screening tools such as  Pap test and HPV testing, participation rates remain suboptimal due to complex behavioral barriers including low awareness, cultural stigma, and limited access to healthcare services. Purpose: To synthesize evidence on behavioral-based health promotion interventions designed to increase cervical cancer screening uptake, examining their theoretical frameworks, delivery modalities, and effectiveness. Method: A systematic search was conducted in three electronic databases—PubMed/MEDLINE, Scopus, and ScienceDirect—for studies published between January 2015 and March 2026. The PRISMA 2020 framework guided the review process. Studies evaluating behavioral interventions targeting cervical cancer screening in women eligible for or targeted by cervical screening were included. Results: A total of 760 records were identified from three databases (PubMed: n=32; Scopus: n=463; ScienceDirect: n=265). After removing 38 duplicates, 722 records were screened; 159 proceeded to full-text assessment, and 54 studies from 25 countries were ultimately included. Interventions were categorized as: face-to-face health education (n=18), digital/mHealth (n=11), reminder-based systems (n=7), motivational interviewing (n=4), peer-led education (n=4), culturally tailored programs (n=7), and multicomponent strategies (n=3). Theoretical frameworks used included the Health Belief Model, Protection Motivation Theory, and Social Cognitive Theory. Of 54 studies, 47 (87.0%) reported statistically significant improvement in at least one outcome in screening uptake or related behavioral outcomes. Conclusion: Behavioral-based health promotion interventions effectively increase cervical cancer screening uptake. Theory-driven, culturally tailored, and multicomponent approaches demonstrate the most consistently favorable findings and should be prioritized in public health programs.

Abstract

Backgroound: Helminthiasis remains a significant public health problem among children in developing countries, including Indonesia. The prevalence of helminth infections among elementary school children in Indonesia averages 28.12%, with regional prevalence ranging from 2.5% to 62%. Soil-transmitted helminth infections are still widely reported and may contribute to impaired growth, anemia, and other nutritional disorders. Cases of helminthiasis, particularly ascariasis, among children under five in Seluma, Bengkulu, in 2025 were associated with poor sanitation, poverty, and inadequate living conditions. Purpose: To analyze helminthiasis according to worm species, infection intensity, polyparasitism, and environmental sanitation, as well as its effects on children’s nutritional status. Method: This study used a systematic review design involving scientific articles published in relevant journals. Literature searches were conducted through Google Scholar, PubMed, ScienceDirect, Cochrane, and MDPI using keywords related to helminthiasis, worm species, infection intensity, polyparasitism, environmental sanitation, and children’s nutritional status. Articles that met the inclusion criteria were reviewed descriptively to determine how worm species, infection intensity, polyparasitism, and environmental sanitation were associated with the nutritional status of children. Results: A total of 30 eligible publications addressing worm species, infection intensity, polyparasitism, environmental sanitation, and children’s nutritional status were included in the review. The findings showed that helminth infections were associated with poor nutritional outcomes, including underweight, impaired growth, and an increased risk of stunting and anemia. Conclusion: Helminth infections, particularly those involving Ascaris lumbricoides, Trichuris trichiura, and hookworm, were associated with poorer nutritional outcomes among children. Infection intensity, polyparasitism, poor sanitation, and inadequate hygiene contributed to an increased risk of stunting, wasting, underweight, and anemia. Recommendations: Improving environmental sanitation, strengthening routine deworming programs, increasing hygiene education, and conducting regular screening for helminth infections and nutritional problems are recommended. Further research should also consider socioeconomic, educational, and behavioral factors.

Abstract

Background: Breakfast is an important health behavior that helps meet children’s energy needs and supports cognitive function, particularly learning concentration. School-age children who regularly skip breakfast may experience inadequate glucose intake, even though glucose is the primary energy source for the brain. This condition can reduce their ability to concentrate during classroom learning. Purpose: To determine the relationship between breakfast habits and learning concentration among students in grades IV-VI. Method: A quantitative approach with a cross-sectional design. A total of 59 students at SDN Ujung Karang, North Lampung Regency, in 2026. The students were included as respondents using a total sampling technique. Data were collected through questionnaires assessing breakfast habits and learning concentration. The data were analyzed using univariate and bivariate analyses, with the Pearson Chi-Square test used to examine the relationship between the variables. Results: 22 respondents (37.3%) had breakfast habits in the sufficient category. Regarding learning concentration, 20 respondents (33.9%) were classified as having good concentration, while another 20 respondents (33.9%) were classified as having poor concentration. The statistical analysis obtained a p-value of 0.000 (p < 0.05), indicating a significant relationship between breakfast habits and students’ learning concentration. Conclusion: Breakfast habits were significantly associated with learning concentration among elementary school students. Regular breakfast may help provide sufficient energy and support students’ ability to concentrate during the learning process.

Abstract

Background: Leprosy (Morbus Hansen) remains a significant global health problem, and delayed detection in areas with limited specialist access continues to drive permanent disability. Digital health offers a promising means of strengthening the screening, diagnosis, and management of the disease in such settings. Purpose: To summarise the most recent evidence on the implementation of digital technologies in leprosy care. Method: A systematic literature search was conducted in the Scopus, PubMed, ScienceDirect, and EBSCO databases up to 21 November 2025, using the keywords leprosy, digital health, mHealth, artificial intelligence, screening, diagnosis, and management. Eligible studies were original research that evaluated digital tools for the screening, diagnosis, or management of leprosy. Study selection followed the PRISMA 2020 guidelines, and the risk of bias was appraised using the Mixed Methods Appraisal Tool (MMAT). Results: Of 80 records identified, five studies met the inclusion criteria. These covered an artificial intelligence (AI) web-based application for classifying leprosy type, a mobile application supporting primary health care, an e-Leprosy information system with SMS reminders, a machine-learning algorithm based on the Leprosy Suspicion Questionnaire (MaLeSQs), and the AI4Leprosy diagnostic assistant based on skin images. Reported sensitivity ranged from 85.7% to 93.97% and specificity from 69.2% to 90%. Conclusion: Digital technologies such as mobile applications, health information systems, and AI can enhance leprosy screening and management. However, most studies remain at an early stage and require field validation and the evaluation of their long-term clinical impact.

Abstract

Background: Occupational accidents remain a major challenge in the mining industry because mining operations involve complex activities and high-risk working conditions. To prevent accidents and improve safety performance, PT X implemented the Safety Friendly Program, which includes safety observations, safety inspections, and management engagement activities. An evaluation of the program is needed to determine whether its implementation is accompanied by an improvement in occupational safety performance. Purpose: To analyze the effectiveness of the Safety Friendly Program in reducing the Accident Frequency Rate (AFR) at PT X. Method: A quantitative approach with a time-series comparative design. Secondary data were obtained from the Health, Safety, and Environment Department of PT X from July 2025 to March 2026. The data included safety observations, safety inspections, management engagement activities, and AFR. Data were analyzed descriptively using IBM SPSS Statistics version 25 and presented in tables and trend analyses to compare changes across the study periods. Results: The Safety Friendly Program was implemented consistently throughout the study period. Safety observations reached 513 activities in the third quarter of 2025, 512 activities in the fourth quarter of 2025, and 494 activities in the first quarter of 2026. Safety inspection findings increased from 279 in the fourth quarter of 2025 to 290 in the first quarter of 2026. The number of workers involved in management engagement activities also increased from 293 in the third quarter of 2025 to 309 in the first quarter of 2026. During the same period, AFR decreased from 10.57 in the third quarter of 2025 to 8.02 in the fourth quarter of 2025 and further declined to 3.07 in the first quarter of 2026. Overall, AFR decreased by 70.96%. Conclusion: The consistent implementation of the Safety Friendly Program was accompanied by a substantial decline in AFR during the study period. Safety observations, safety inspections, and management engagement activities may support accident prevention and strengthen safety practices in mining operations. However, this descriptive trend should not be interpreted as evidence of a causal relationship. Continued implementation and periodic evaluation are recommended to improve occupational safety performance.

Abstract

Background: Exclusive breastfeeding is a key strategy to improve infant health because breast milk provides complete nutrition, supports growth, and protects infants from infectious diseases. Although national policies and health promotion programs have been implemented, exclusive breastfeeding coverage remains below the expected target in several areas, including the working area of the Biru Biru Community Health Center. Purpose: To analyze the effectiveness of promotional program services in increasing exclusive breastfeeding coverage in the working area of the Biru Biru Community Health Center. Method: A qualitative descriptive design was employed with a program evaluation approach based on the input-process-output framework. Data were collected through in-depth interviews, observation, and documentation involving health workers and breastfeeding mothers who were directly related to the exclusive breastfeeding promotion program. The data were analyzed through data reduction, data display, and conclusion drawing. NVivo 12 was used to assist the coding and organization of qualitative data Results: The input component was generally adequate, particularly in terms of health workers, facilities, and promotional media. However, several limitations were found, including incomplete written guidelines, limited program funding, and uneven participation among mothers. In the process component, health education, counseling, and mentoring activities were routinely implemented, although they were constrained by mothers’ low participation and health workers’ workload. In the output component, the program contributed to increased maternal knowledge and a more positive attitude toward exclusive breastfeeding, but the coverage had not yet reached the expected target. Conclusion: The exclusive breastfeeding promotion program at the Biru Biru Community Health Center has supported the improvement of maternal knowledge and awareness, but its effectiveness remains suboptimal. Strengthening program planning, increasing community participation, optimizing promotional media, and improving the role of health workers are needed to achieve better exclusive breastfeeding coverage.

Abstract

Background: Spiritual needs are a critical component of holistic care for patients with life-threatening illnesses, yet their manifestation varies significantly across different disease pathways. Purpose: To identify and synthesize the multifaceted factors influencing spiritual needs in patients with cancer, cardiovascular disease (CVD), chronic kidney disease (CKD), and stroke, while mapping how different clinical trajectories contribute to variations in spiritual experiences. Method: Following PRISMA 2020 guidelines, a systematic search was conducted in Scopus and PubMed for primary empirical studies published between 2020 and 2026. A total of 398 records were identified, and after rigorous screening and quality assessment, 49 studies were included in the final synthesis. Results: Spiritual needs among patients with chronic diseases: cancer, cardiovascular disease, chronic kidney disease, and stroke are associated with interconnected clinical, demographic, physical, psychological, psychosocial, socioeconomic, and cultural factors. Disease burden, physical symptoms, psychological distress, social support, and personal beliefs collectively shape spiritual well-being, highlighting its dynamic nature throughout the illness trajectory. Conclusion: Spiritual needs among patients with chronic and life-threatening illnesses are associated with clinical, demographic, physical, psychological, psychosocial, socioeconomic, and spiritual-cultural factors. Integrating routine spiritual assessment and personalized spiritual care into healthcare practice may help improve coping, resilience, and quality of life.

Abstract

Background: Chronic kidney disease (CKD) patients undergoing hemodialysis (HD) often exhibit elevated levels of ICAM-1 and VCAM-1, key biomarkers of inflammation, and are prone to microcirculatory dysfunction. Recent evidence suggests that carbon dioxide (CO₂)-rich water immersion may enhance microcirculation, yet its impact on inflammatory biomarkers in CKD patients on HD remains unclear. Purpose: To evaluate the effect of CO₂-rich water immersion on ICAM-1 and VCAM-1 levels in CKD patients receiving HD. Method: This experimental study involved 22 CKD patients randomized into two groups: 14 received CO2-rich water immersion and 8 received freshwater immersion. CO2-rich water was generated using JESC CREABC-2000, with both immersions maintained at 37–38°C. Patients immersed both legs for 10 minutes, 2–3 times weekly for one month, prior to HD sessions. Blood samples were collected on day 1 and day 28. Data were analyzed using the Wilcoxon test, with statistical significance set at P < 0.05. Results: After one month, ICAM-1 and VCAM-1 levels decreased in the CO2-rich water group (42.895±58.833 to 39.690±41.911 ng/mL, P=0.650; 140.520±177.43 to 96.170±30.17 ng/mL, P=0.917). Conversely, levels increased in the freshwater group (ICAM-1: 38.642±40.975 to 63.978±80.659 ng/mL, P=1.000; VCAM-1: 100.430±34.89 to 131.920±59.80 ng/mL, P=0.461). Conclusion: CO2-rich water immersion tended to reduce ICAM-1 and VCAM-1 levels in CKD patients undergoing HD, although the changes were not statistically significant. These findings suggest potential anti-inflammatory effects warranting further investigation with larger cohorts.

Abstract

Background: Circumcision is one of the most commonly performed pediatric surgical procedures and is traditionally closed using absorbable sutures. However, conventional suturing may increase postoperative pain, edema, infection risk, foreign-body reactions, and cosmetic irregularities. Sutureless techniques, including cyanoacrylate-based tissue adhesives and clamp-assisted methods, have emerged as minimally invasive alternatives that may improve wound healing and reduce complications. Purpose: To systematically compare the safety, procedural efficiency, cosmetic outcomes, and postoperative pain associated with sutureless versus conventional sutured circumcision in pediatric populations. Method: This systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive literature search was performed using PubMed, Wiley, and Google Scholar databases. Study quality and risk of bias were evaluated using the Revised Tool for Risk of Bias in Randomized Trials (ROB 2). Three eligible comparative studies were included in the final analysis. Results: Sutureless circumcision demonstrated several advantages over conventional sutured techniques, including shorter operative time, fewer postoperative complications, and superior cosmetic outcomes. Most suture-related adverse events, such as local irritation and foreign-body reactions, were reported exclusively in the sutured groups. In addition, patients undergoing sutureless procedures generally experienced lower postoperative pain scores. Conclusion: Sutureless circumcision using cyanoacrylate adhesives appears to be a safe and effective alternative to traditional sutured closure, offering improved procedural efficiency and postoperative outcomes. Nevertheless, the current evidence is limited by small sample sizes, heterogeneous study populations, and non-standardized outcome measures. Further large-scale, well-designed comparative studies are needed to establish robust clinical recommendations.
