Abstract

Background: Diabetes mellitus is a degenerative disease commonly experienced by the elderly and is characterized by elevated blood sugar levels above normal limits. Hyperglycemia can cause various complaints, one of which is sleep disturbances. Poor sleep quality in the elderly can worsen blood glucose control and reduce quality of life. This study aims to determine the relationship between random blood sugar levels and sleep quality in elderly with diabetes mellitus at Health center of Purwosari District. Method: This study used an observational analytical design with a cross-sectional approach. The population was all elderly people with diabetes mellitus at the Elderly Health at Purwosari Health Care Center, with a sample size of 33 respondents selected using accidental sampling. Random blood sugar levels were measured using a glucometer, while sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Data were analyzed using the Spearman Rho correlation test. Result : The result of the study showed that most respondents had random blood sugar levels above the normal limit (>199 mg/dl) and poor sleep quality. The p-value was 0.000, meaning the significance value was <0.05. Meanwhile, the Correlation Coefficient value was 0.685. This means there is a positive relationship with a strong relationship. Conclucion:  High random blood sugar levels can cause various complaints such as frequent urination at night, thirst, tingling, and pain, which impact the quality of sleep in the elderly. In order to enhance the quality of life of the elderly, healthcare facilities must establish educational programs that underscore the significance of glycaemic control and sleep quality as interrelated factors.   Keyword : Random Blood Glucose, Sleep Quality, Elderly, Diabetes Mellitus

Abstract

Background: Pleural effusion involves abnormal fluid accumulation in the pleural cavity, stemming from increased production or reduced lymphatic absorption. This buildup compresses the lungs, impairing expansion and causing symptoms like dyspnea, chest pain, and cough. Non-pharmacological interventions, such as the tripod position and pursed-lip breathing, enhance lung ventilation, oxygenation, and ease breathing effort. Objective: This case report evaluates their application in nursing care for Mrs. S, a patient with pleural effusion, to boost oxygen saturation. Methods: A descriptive case study design targeted one patient diagnosed with pleural effusion. Data were gathered via medical-surgical nursing assessments, including interviews, observations, and physical exams. Findings informed a tailored nursing care plan. Interventions comprised tripod positioning where the patient leans forward with arms supported on a surface to optimize the use of diaphragmatic breathing along with pursed-lip breathing, involving inhalation through the nose and prolonged exhalation through pursed lips to reduce air trapping and improve gas exchange. These were implemented daily for three consecutive days, with ongoing monitoring and evaluation of vital signs, particularly SpO2. Result: Pre-intervention, Mrs. S exhibited SpO2 of 84%, marked dyspnea, and ineffective airway clearance. Post-intervention, SpO2 rose to 96%, with notable relief in shortness of breath and enhanced respiratory comfort. No adverse effects occurred, confirming intervention tolerability. Conclusion: The use of the tripod position and pursed lip breathing technique has been shown to improve oxygen saturation in Mrs. S with a nursing diagnosis of impaired gas exchange.   Keywords:  Pleural Efussion, Tripod Position, Pursed Lip Breathing

Abstract

  Background: The implementation of standard precautions is essential in preventing infection transmission in hospitals. Nurses, as healthcare workers who have direct contact with patients, are at high risk of exposure to blood and body fluids; therefore, their perceptions play an important role in infection prevention practices. The Health Belief Model (HBM) can be used to explain health-related behaviors based on perceived threats, benefits, and barriers. This study aimed to describe nurses’ perceptions of standard precautions at Hospital X based on the Health Belief Model. Methods: A descriptive quantitative design with a cross-sectional approach was used, involving 317 nurses selected through total sampling. Data were collected using the Indonesian version of the Basic Principles of Standard Precautions questionnaire, which measures perceived threat, perceived benefits, and perceived barriers. Results: The results showed that most nurses had good perceptions, including perceived threat (71.3%), perceived benefits (66.6%), and perceived barriers (64.0%). Most respondents had experienced occupational exposure to blood or body fluids (70.3%) and needle stick injuries (39.7%). The highest incidence of exposure to blood or body fluids was found in the surgical ward (IBS), with 21 respondents (9.4%), and similarly, the highest incidence of needle stick injuries or injuries from infectious sharp objects was also found in the surgical ward, with 21 respondents (16.7%). Conclusion: Nurses’ perceptions of standard precautions were generally good; however, some nurses still had poor perceptions, particularly regarding barriers. These findings highlight the need to strengthen training and improve occupational safety culture to support consistent implementation of standard precautions.   Keywords: health belief model; infection prevention; nurse perception; occupational exposure; standard precautions

Abstract

Background : This study focuses on attitudes regarding daily skills mastered by each gender in Gen Z. The Methods : research method in this study is a quantitative descriptive research method. Data collection using a digital questionnaire (google form). The population in this study amounted to 325 respondents using a simple random sampling technique. Results : The characteristics of the respondents above present the largest gender data is male, which is 179 (55%). The age of the largest respondents is 20 years old as many as 114 (35%). Most respondents 227 (70%) have never received gender education and most respondents' education is bachelor 247 (76%). Attitudes towards Daily Skills that must be mastered by each gender according to Gen Z on the statement of being able to prepare daily food to eat, most respondents agree 306 (94%). The ability to clean the house, most respondents agree to be mastered 318 (97%). The ability to wash dirty clothes, most respondents agree to be mastered 300 (92%). The ability to clean the toilet, most respondents agree to be mastered 290 (89%). The ability to take care of family/siblings/younger siblings, most respondents stated that they agreed to be mastered 264 (81%). Conclusion : The results of the study showed that Gen Z has an attitude that agrees that every gender must be able to master daily skills. For further research, research can be conducted by looking at the implementation of these abilities or Gen Z's behavior towards the ability to master daily skills.
