Abstract

Keratinous cysts, predominantly referred to as epidermoid cysts (ECs) or epidermal inclusion cysts (EICs), represent the most prevalent subtype of cutaneous cysts. These benign, slow-growing lesions are defined by a pathological cavity situated within the dermis or subcutis, lined by stratified squamous epithelium and filled with macerated keratin and lipid-rich debris. While often asymptomatic, their clinical significance arises from a high frequency of complications, including inflammation, infection, and rupture, which prompt medical consultation. Furthermore, their presentation can mimic a wide range of other soft tissue tumors, necessitating accurate diagnosis. This paper provides a exhaustive, systematic review of keratinous cysts, delving into their historical context, precise nomenclature, and epidemiological patterns. It elaborates on the multifactorial etiologies and molecular pathways underpinning their pathogenesis. The clinical and histopathological features are described in detail, alongside advanced diagnostic imaging characteristics. A critical analysis of the differential diagnosis is presented, followed by a comprehensive discussion on management strategies, from conservative approaches to sophisticated surgical techniques aimed at minimizing recurrence and optimizing cosmetic outcomes. The paper also explores rare associations and future research directions, consolidating current evidence to serve as a definitive reference for dermatologists, surgeons, and primary care physicians. condition. Abstract

Childhood anemia is a major global health issue, impairing growth, cognitive development, and immune function. Its prevalence is particularly concerning in post-conflict regions where healthcare infrastructure, nutrition, and sanitation are compromised. In Mosul, Iraq, years of conflict and displacement have heightened children’s vulnerability to nutritional deficiencies and infections, yet localized epidemiological data remain scarce. Limited evidence exists on the combined biological and community health determinants of anemia in Iraqi primary school children, restricting effective intervention planning. This study aimed to determine the prevalence of anemia among school-aged children in Mosul and identify its key biological and socio-environmental determinants. Results: A cross-sectional study of 850 children aged 6–12 years revealed an overall anemia prevalence of 42.5%, with mild anemia as the most common form (65.1%). Logistic regression analysis identified low maternal education (OR = 2.34, p < 0.001), low household income (OR = 2.16, p < 0.001), infrequent red meat intake (<2 times/week, OR = 2.80, p < 0.001), stunting (OR = 3.50, p < 0.001), and a history of parasitic infection (OR = 2.10, p = 0.003) as significant predictors. This is among the first large-scale studies in Mosul to integrate both biological and community-level determinants of anemia, reflecting the multifactorial nature of the condition in post-conflict settings. The findings underscore the urgent need for multisectoral interventions, including nutritional supplementation, deworming, health education, and sanitation improvements, to combat childhood anemia and improve long-term child health outcomes in Mosul. Highlights: Childhood anemia prevalence in Mosul primary schools was 42.5%, classifying it as a severe public health problem. Key determinants included low maternal education, low household income, infrequent red meat consumption, stunting, and parasitic infections. Abstract

General Background: Medical records are a critical component of hospital administration, ensuring accuracy, completeness, reliability, validity, and timeliness for quality healthcare services. Specific Background: However, delays in returning medical record files remain a challenge in inpatient care, disrupting service standards and continuity of care. Knowledge Gap: Limited research has systematically analyzed delay factors using a structured framework in the Indonesian hospital setting. Aim: This study aims to analyze the factors contributing to delays in returning medical record files at Wahidin Sudiro Husodo Hospital using the 5M framework (man, method, machine, material, money). Results: Findings revealed human factors such as high school-level education, limited knowledge, and lack of training; methodological issues where standard operating procedures for inpatient care were not aligned with the 2x24 hours service standard; material shortcomings due to the absence of a tracer; machine factors showing only the use of an expedition book; and financial factors with no incentive or sanction mechanisms. Novelty: This research provides a comprehensive factor-based analysis of medical record return delays by integrating the 5M approach. Abstract

General Background: Carboxyhemoglobin (COHb) formation occurs when carbon monoxide (CO) binds with hemoglobin, showing an affinity 200–300 times stronger than oxygen, which disrupts oxygen transport and utilization in the body. Specific Background: In urban environments, CO exposure may be influenced by lifestyle factors such as daily travel distance and smoking habits, both of which are suspected contributors to elevated COHb levels. Knowledge Gap: Despite known toxicological effects of CO, limited research addresses how these lifestyle factors correlate with COHb levels in young adults, particularly male health students. Aim: This study investigated the association between distance traveled and smoking habits with COHb levels among male students of the Faculty of Health Sciences, Muhammadiyah University of Sidoarjo. Results: Findings showed that all respondents (100%) had COHb levels below the standard threshold (<3.5%) set by the Indonesian Ministry of Health Regulation No. 70 of 2016, and statistical analysis indicated no significant association between either variable and COHb levels (p>0.05). Novelty: This research highlights that common risk factors like smoking and travel distance may not always predict elevated COHb in health student populations. Implications: The results provide reassurance regarding current exposure levels and may guide preventive health education for student populations. Highlights: All respondents had COHb levels below the national threshold. No significant association was found between smoking or travel distance and COHb. Abstract

Background: Mental health is a crucial component of overall well-being, yet mental disorders remain highly prevalent and often manifest through negative emotional expressions such as anger. Specific Background: People with mental disorders (ODGJ) in social institutions frequently express anger in disruptive ways, affecting their daily functioning and care environment. Knowledge Gap: Despite the use of various therapies, limited research has focused on structured music therapy as a tool to reduce anger among ODGJ in Indonesian social facilities. Aim: This study investigates whether music therapy can reduce angry emotional behavior in ODGJ at Liponsos Sidoarjo. Results: Using a quantitative pre-experimental One Group Pretest-Posttest design with two subjects, data were collected through observation using a behavior checklist and analyzed with descriptive statistics and the Mann-Whitney U Test. Results indicated a significant reduction in anger behavior (W = 57.5, p = 0.028). Novelty: Unlike prior studies that specify certain music genres, this research employed a mix of songs based on patient preference, demonstrating flexibility in therapy application. Implications: Findings suggest that music therapy can be an effective intervention for reducing anger in ODGJ, with potential integration into routine care programs at social institutions. Abstract

Background: Self-confidence is an essential psychological aspect for the elderly, yet aging often leads to psychosocial challenges, including stress, depression, and reduced social roles. Specific Background: Elderly residents in social institutions frequently experience low confidence, affecting their well-being and social interactions. Knowledge Gap: Although various interventions exist, limited research in Indonesia has investigated horticultural therapy as a structured approach to build elderly self-confidence. Aim: This study examines whether horticultural therapy can significantly improve self-confidence among elderly individuals in Liponsos Sidoarjo. Results: Using a quantitative pre-experimental design with a One Group Pretest-Posttest method, three elderly participants aged 60 and above were observed. Data were collected using a self-confidence scale based on Lauster’s theory and analyzed with Paired Sample T-Test. Findings revealed a significant improvement between pretest and posttest scores (p = 0.048), with a large effect size (Cohen’s d = 2.547). Novelty: Unlike prior studies, this research applied structured gardening activities, from planting to harvesting, within a social institutional context. Implications: Horticultural therapy can serve as a practical and sustainable intervention to enhance psychological well-being and self-confidence in the elderly population of social care facilities. Abstract

Background: Maternal and infant mortality remain critical public health challenges worldwide and in Indonesia, requiring continuous and integrated healthcare services. Specific Background: Comprehensive midwifery care from pregnancy through postpartum and family planning is essential to reduce risks of maternal and neonatal complications. Knowledge Gap: Despite the recognition of Continuity of Care (CoC) as a standard approach, limited case-based documentation from Indonesian hospital settings exists, particularly in managing high-risk conditions such as positive HBsAg. Aim: This study aims to describe the implementation of continuity midwifery care provided to a pregnant woman at Rahman Rahim Hospital, Sidoarjo. Results: Using a descriptive case study from May to July 2024, midwifery care included pregnancy management, spontaneous delivery using 60 APN steps, newborn care with immediate immunization, postpartum monitoring, and contraceptive counseling. The client, diagnosed with positive HBsAg, underwent safe delivery, and her newborn received HBIG and Hepatitis B vaccination to prevent vertical transmission. Novelty: This study highlights the application of structured continuity care in a clinical setting while addressing infectious disease concerns. Implications: Findings confirm that continuous and comprehensive midwifery care can reduce maternal and infant risks, strengthen clinical practice, and support national health goals to lower maternal and neonatal mortality rates. Abstract

General Background: Access to safe drinking water remains a critical challenge in Indonesia, with nearly 30% of the population lacking proper access. Specific Background: Reverse Osmosis (RO) technology provides a solution for producing clean water, but conventional systems still face issues of efficiency, reliability, and safety. Knowledge Gap: Limited studies document the integration of automated control systems with RO units in real-world industrial applications. Aim: This study aims to develop and test a drinking water treatment control system using Schneider Smart Relay integrated with pH and TDS sensors. Results: Experiments on four water samples showed average pH of 7.67 (input), 7.80 (reject), and 7.86 (output), while average TDS values were 136 ppm (input), 298 ppm (reject), and 33.5 ppm (output). The system automatically activated or deactivated pumps according to preset water quality parameters, with buzzer alerts for anomalies, ensuring compliance with health standards. Novelty: The system combines automation, energy efficiency, and safety through smart relay integration, offering advantages over conventional RO setups. Implications: Findings demonstrate that this system can improve water treatment reliability and is suitable for household and industrial-scale implementation. Abstract

Background (general): The elderly often face health challenges due to biological aging, leading to decreased physical function and increased risk of chronic diseases.Specific background: An unhealthy diet among elderly populations contributes to conditions such as hypertension, diabetes, and high cholesterol, which diminish their quality of life.Knowledge gap: Limited psychoeducational interventions specifically address healthy eating patterns to improve quality of life among elderly individuals in community settings.Aims: This study aims to evaluate the effectiveness of psychoeducation in increasing knowledge of healthy eating patterns and improving quality of life among elderly participants.Results: Thirty elderly participants from Sepande Village, Sidoarjo, were involved in a one-group pretest-posttest design using a structured questionnaire. The findings demonstrated a significant increase in understanding of healthy eating patterns, with mean scores rising from 3.767 (pretest) to 7.900 (posttest). Additionally, improvements were observed in quality-of-life categories, with more participants achieving higher levels after psychoeducation.Novelty: This study provides empirical evidence that structured psychoeducation can enhance dietary knowledge and quality of life among the elderly, an area rarely explored in Indonesian community contexts.Implications: The results highlight the need for broader implementation of psychoeducation to support elderly well-being, and recommend further studies using stronger experimental designs for long-term evaluation. Abstract

Background : In Sidoarjo Regency, cases increased annually, with 65,813 cases recorded in 2021. Exclusive breastfeeding and adequate nutrition are crucial protective factors.Knowledge Gap: Previous studies identified various risk factors, yet limited evidence exists on their combined association in local community settings.Aim: This study examined the relationship between exclusive breastfeeding history and nutritional status with diarrhea incidence among toddlers in the Candi Sidoarjo Health Center area.Methods: A cross-sectional study was conducted on 45 mothers with toddlers aged 6–59 months using random sampling. Data were collected via questionnaires and anthropometric measurements, analyzed with chi-square tests.Results: Among toddlers, 60% received exclusive breastfeeding and 68.9% had normal nutritional status. Diarrhea incidence in the past three months was 55.6%. Chi-square analysis showed significant associations between exclusive breastfeeding (p=0.014) and nutritional status (p=0.037) with diarrhea occurrence.Novelty: The findings highlight how breastfeeding practices and nutritional adequacy collectively reduce diarrhea risk in community-level settings.Implications: Strengthening maternal awareness and nutrition interventions may lower diarrhea incidence and improve child health outcomes in similar populations.
