Abstract

Disc displacement with reduction represent the early stage of disc derangement disorders. Disc derangementdisorders result from elongation of capsular and discal ligaments coupled with thinning of the articular disc.These changes commonly result from either macro or micro trauma. Mandibular repositioning splint can be usedto treat disc displacement with reduction. This clinical report aims to explain management of disc displacementwith reduction and myofacial pain with mandibular repositioning splint. 23-years-old female patient wascomplaining in mouth limited opening and pain around cheeks, ears, neck and shoulders since six months ago,especially after wake up in the morning but returned to normal a few hours later. On clinical examinations therewas a clicking sound while mouth opening and closing, also pain during palpation on facial muscles, neck andshoulders. Patient was suggested to use mandibular repositioning splint 24 hours a day for three to six months.Mandibular repositioning splint is effective if the patient uses it continuously. Functional and esthetic concernsare important if the mandibular repositioning splint is to be used during the day. The advantages of themandibular repositioning splint are least affected of speech function, more esthetic and the patient easier to movethe mandible to posterior. Mandibular repositioning splint is the best choice for disc displacement and myofascialpain treatment because it may be more acceptable from a functional and esthetic standpoint. Abstract

Sinusitis is an inflammation of nose and paranasal sinuses. Signs and symptoms that found in sinusitis patients are problem in sense of smell, facial pain, nasal obstruction, and discharge of mucopurulentnasal drip. Nasal obstruction tend to causes patient mouth breathing then patient having symptom ofxerostomia. In xerostomia patient which accompanied by decrease of salivary flow rate, theantimicrobial action of saliva will decrease, so the formation of plaque will increase. This study aimsto determine periodontal status of sinusitis patients. This research is a descriptive study with nonexperimental design. The method used to select the subject is purposive sampling, which the subjectsare sinusitis outpatients in Ear, Nose, Throat Polyclinic of Meuraxa Hospital. This research startedfrom August to October 2019 in Meuraxa Hospital, Banda Aceh. Research done by the examination ofOHIS, pocket periodontal depth, and clinical attachment loss. The results show that subjects with acutesinusitis are 24% and chronic sinusitis are 76%. Oral hygiene of sinusitis patients show 72% fair scoreand 28% poor score. Periodontal pocket depth under 4 mm in percentage of 100%. Clinical attachmentloss show 88% mild, 8% fair, and 4% severe. The conclusion of this study are no sinusitis patienthaving good oral hygiene, but the higher score is fair oral hygiene with percentage of 72%. Periodontalpocket depth in all subjects are under 4 mm. Abstract

Impression making is the main requirement to do dentures. Dental impressions become contaminated with the patients microorganisms from patients saliva, blood, pus and debris which are known as opportunistic patogens that can cross infect from the dental impressions to the gypsum casts pouredagainst them. This cross infection of microorganism remains, even after rinsing and desinfecting thegypsum casts by immersing the gypsum cast in desinfeting solutions, such as glutaraldehyde 2% orsodium hypoclorite 0,5% solutions, can be one of the way to prevent the potential of cross infectionduring dental procedures the purpose of this study is to evaluate the effect of immersing gypsum caststype III with the amount of Pseudomonas aeruginosa and the value of the surface hardness of gypsumcasts type III. The sample of this study is basedon the spesification ANSI/ADA No. 25, with three groupsof activity, with 9 samples each, which is, rinsing with distilled water (group A), immersion inglutaraldehyde 2% (group B) and immersion in sodium hypoclorite 0,5% (group C), so that totalsamples are 54 samples. Analisys with kruskal wallis and one way Anova. The number and standarddeviation of Pseudomonas aeruginosa in group A (222 74.11 CFU / mL), group B and C (0 CFU /mL). The value and standard deviation of surface hardness in group A (23.83 0.69 VHN) , group B(12.60 0.72 VHN) and C (17.24 1.14 VHN). There was an effect of immersion of gypsum type IIIwith glutaraldehyde 2% and sodium hypochlorite 0.5% on the amount of Pseudomonas aeruginosa andsurface hardness in the fabricating of dentures (p = 0.0001).Keyword: Glutaraldehyde 2%, Sodium hypochlorite 0,5%, Pseudomonas aeruginosa, Surfacehardness.__________________________________________________________________________________

Abstract

Disc displacement with reduction adalah gangguan temporomandibular non inflamasi yang umumditemukan. Kelainan ini dapat berhubungan dengan open bite anterior. Meskipun perawatan discdisplacement with reduction dengan gejala kliking tanpa gejala lain masih kontroversial, perawatandengan splin dapat menghilangkan kliking dan mengembalikan diskus ke posisi yang tepat. Tulisan inibertujuan untuk menjelaskan perawatan gangguan sendi temporomandibular disc displacement withreduction pada kasus maloklusi open bite anterior tanpa disertai rasa sakit dengan splin reposisianterior. Pasien laki laki usia 24 tahun datang ke Klinik Prostodonsia RSGMP FKG USU dengankeluhan terdengar bunyi saat membuka mulut lebar sejak 5 bulan lalu. Pada pemeriksaan klinisdidapati bunyi disendi rahang kanan, pembukaan mulut 45mm, deviasi kekanan dan tidak ada sakitpada daerah sendi pada saat membuka mulut. Pasien memiliki riwayat pernah jatuh dan dagu koyakkarena terbentur kelantai sekitar 12 tahun lalu. Pasien memiliki kebiasaan mengunyah lebih sering disebelah kiri. Radiografi menunjukkan posisi kondilus kanan dan kiri berbeda saat membuka mulut.Pasien merasakan berkurangnya bunyi kliking saat membuka mulut setelah 2 minggu pemasangan splinreposisi. Splin reposisi anterior digunakan pada perawatan fase simtomatik setelah perawatanpendahuluan berupa anjuran mengunyah dua sisi dan latihan buka-tutup mulut tanpa deviasi padakasus disc displacement with reduction dengan etiologi maloklusi open bite anterior. Splin reposisianterior akan memperbaiki hubungan fossa glenoid, diskus dan kondilus, sehingga mengurangi suarasendi. Dengan menghilangkan gangguan mekanis, maka diskus mampu menyesuaikan kondisi didalam sendi sehingga deformasi diskus tidak semakin parah. Splin reposisi anterior dapat mengatasigejala gangguan sendi temporomandibula, sehingga tercapai keseimbangan oklusi, otot dan sendirahang. Perawatan harus dilanjutkan dengan perawatan ortodonti untuk mengoreksi open biteanterior sehingga tercapai keseimbangan oklusi, otot dan sendi rahang secara defenitif.Keywords : Open bite, Temporomandibular joint, Disc displacement with reduction, Klicking, Splin reposisi anterior.

Abstract

Introduction: Flabby ridge compromise support, retention and stability of complete dentures. Thepresence of flabby ridge tends to create a difficulty while making complete dentures. Many impressiontechniques have been proposed to overcome these difficulties. Objectives: This paper aims to explainan impression technique on flabby ridge anterior mandibular using modified final impression tray withtwo tunnels. Case Description: An 80-years-old female patient using complete dentures. She wasconcerned that her lower complete denture was loose. On clinical examination there was anteriorflabby ridge from canine to canine of the mandibular arch. The technique described in this case wasusing clear swapolimerized acrylic resin with two tunnels for final impression tray, using polivynilsiloxane light body impression was injected around the flabby ridge using a dispensing gun from oneside of the tunnel, taking care that the flow to the opposite side was controlled by the clear custom tray,to ensure minimal distortion on flabby tissue and coverage of impression material on all edentulousarea. Discussion: Flabby ridge is a challenge in prosthodontics to achieve stability and retention ofcomplete dentures. Provision of complete dentures in patient with flabby ridge can be managed bycorrect impression method. By using this modified final tray, it will not displace the flabby tissue thatcan dislodge the denture, simple custom tray, easy to practice and less time. Abstract

One drink that is processed by the reverse osmosis method is Reverse Osmosis (RO) drinking water.Through this method, all micro particles contained in filtered water including essential minerals. Thereduction in essential minerals can reduce the activation function of salivary gland secretory cellsand it will have an impact on salivary flow, pH, and viscosity. This study aims to determine the effectof RO drinking water consumption to the flow rate, pH, and viscosity of saliva on students of FajarHarapan Senior High School Banda Aceh. The study subjects consisted of 45 subjects who consumedRO drinking water as the treatment group and 8 subjects who did not consume RO drinking water asa control group. Saliva from the treatment and control group subjects was collected by spittingmethod to measure the flow rate, pH, and viscosity of saliva. The results of the study that wereanalyzed using the unpaired T test showed that there was no effect of RO drinking water consumptionon the flow rate, pH, and viscosity of saliva in SMA Negeri 10 Fajar Harapan Banda Aceh. Based onthe results of the research, it can be concluded that the consumption of RO drinking water does notaffect the flow rate, pH and viscosity of the saliva in students of Fajar Harapan Senior High SchoolBanda Aceh. Abstract

Gingival bleeding is an early feature of gingivitis. Gingivitis is characterized by gingival redness,swelling, and bleeding on probing. Gingival bleeding may be caused by local factor and systemicfactors. Negative stress (distress) is thought to be the risk factor of gingival bleeding. Previous studiesshowed that negative stress (distress) may contribute to the severity of gingival bleeding. However, insome cases heavy gingival bleeding was found in the presence of minimal plaque retention. Theobjective of this study was to see the correlation between negative stress (distress) and gingivalbleeding. This study was an analytic cross-sectional study taking Banda Aceh mental hospital. Thesubjects of this study were 58 patients aged between 15 60 years old. There were 30 male and 28female patients. Data was collected from medical record, history taking and clinical examination ofgingival bleeding and oral hygiene index-simplified (OHI-S). Statistic analysis using chi-square testrevealed that there was no correlation between negative stress (distress) to gingival bleeding (p0.05).Conclusion: This study shows that there is no significant correlation between negative stress (distress)to gingival bleeding. Abstract

The home bleaching procedure is a tooth whitening process that can be done at home using a prostheticlikedevicecalledatrayornightguardwith16%carbamideperoxideconcentration.Chemicalssuchascarbamideperoxidecanhaveasensitiveeffectontheteeth.Greenpear(Pyruscommunis)isbelievedtohaveacontentthatcanremovestainsonthesurfaceoftheteethandwhitenteethsuchasmalicacid,citricacid, oksalat acid, sicamat acid, fumarate acid, tartaric acid and lactic acid. This study was alaboratory experimental study of the effectiveness of tooth discoloration applied with green pears(Pyrus communis) and then compared with the color of teeth applied to 16% carbamide peroxide. Toothcolor changes that occurs is measured using the shade guide. In the group of green pear juice (Pyruscommunis) it produces the final color A3.5 and A3, while in the carbamide peroxide group 16%produces the final color A1. The results of the study were analyzed using Wilcoxon and Mann Whitneyanalysis to see the comparison of the effectiveness of tooth color change in the teeth. Based on theresults of the study it can be concluded that the application of green pear (Pyrus communis) is lesseffective than the application of 16% carbamide peroxide.Keywords: Green Pear (Pyrus communis), Carbamide Peroxide, Home Bleaching

Abstract

It is important to prepare the pillars of clinical governance to achieve a quality of hospital servicesystem. The aim of implementing clinical governance is overall improvement based on improvingpatient-focused services, service satisfaction and continuous quality improvement. Clinical governanceis also paying attention to level of security both medical and non-medical related to risk management,having high performance, responsiveness and proactivity, and patient centered-care. Literature reviewregarding the quality of hospital services. Browse PubMed and the main organizational databasesabout clinical governance such as WHO and NHS Quality Improvement Scotland. A number of articlesrelated to hospital management strategies based own clinical governance have been reviewedincluding their application in several countries. The result showed clinical governance can be used todevelop a system to improve the quality of hosital services, carried out by integrating management,organizational, and clinical approaches simultaneously. The application of clinical governance builton 7 main pillars, namely Clinical Effectiveness, Risk Management, Patient Experience,Communication Effectiveness, Resources Effectiveness, Strategic Effectiveness, Learning Effectiveness.Conclusion. Development of clinical governance requires the active participation from all parties tocreate quality and professionalism in health services. Clinical governance ensuring that there was asystem to monitor of quality services that functioning properly, conduct the evaluation and the resultsof evaluations was used for improvement. Abstract

Dental caries is still a major problem in the child's oral cavity, generally at high risk of caries in the 8-10 year age range. Permanent dental caries can cause changes in tooth position, loss of teeth, disruption of absorption of food nutrients until the onset of infection. The purpose of this study was to determine the prevalence of caries in incisors and permanent molars in Baiturrahman MIN 1 students in Banda Aceh class IIIA and IVB. Descriptive research uses cross sectional research design with total sampling technique and obtained research subjects amounting to 81 students aged 8-10 years. Data obtained directly through DMF-T examination. The results showed high rates of caries of young permanent teeth in the incisors and first molars of 82%, of 81 students found 67 students had caries. On average 2 out of 12 young permanent teeth in one oral cavity each student aged 8 experienced caries. It was concluded that caries prevalence in incisors and permanent molars of Baiturrahman MIN 1 students was high for ages 8-10 years, there were no significant differences between male and female students, and had an impact on the emergence of new risks.Keywords: caries prevalence, incisors, first molars
