Abstract

The elderly will experience an aging process that is characterized by physical changes, intellectual changes, and a decrease in various body functions. Deterioration of intellec tual function including cognitive function is one of the functions that will decrease in the elderly. Physical activity is a factor that can inhibit cognitive decrease in the elderly. One of the preventive actions that can be done by the elderly in slowing down the decrease of cognitive function is to increase physical activity. Physical activity is thought to stimulate nerve growth which might inhibit cognitive decrease in the elderly. When doing physical activity, the brain will be stimulated by talamus so that it will increase the Brain Derived Neurotropic Factor which plays a role in keeping nerve cells fit and healthy. The purpose of this study was to identify the relationship between physical activity and cognitive func tion in the elderly. This study was a quantitative study with a categorical correlative analytic method. The study used cross sectional study design. The chi-square test and ordinal logistic regression test were used for data analysis. The results of this study stated that the overall physical activities which were exercise, mild activity, heavy activity, use of stairs and participate in social activities showed a significant relationship to cognitive function. Participating in social activities and mild activity were the most significant physical activities that showed relationship to cognitive function, with p-value = 0.001 < 0.05. In conclusion, the findings highlight that the choice of appropriate physical activity can help to maintain cognitive function in the elderly

Abstract

Moral distress is psychological disequilibrium and negative feelings that results from rec ognizing an ethically appropriate action, yet not taking it because of such barriers. Caused by increased workload of nurses in the intensive care room, resulting burnout that affects nurses, patients and health care providers. This study aimed to analyze the relationship between moral distress and the level of burnout in intensive care room nurses at the dr. Soebandi Hospital Jember. This study used a quantitative descriptive observational ana lytic with the approach cross sectional design selected 47 nurses as respondents used total sampling. Data collected by Measure of Moral Distress Scale - Healthcare Profes sionals and Maslach Burnout Inventory. The results revealed that most respondents ex perienced low moral distress (85.1%) and low burnout (61.7%). The bivariate analysis showed with spearman test that p-value was < 0.001 (alfa = 0.05; r = 0.531). There was a relationship between moral distress and the level of burnout with moderate and positive relationship strength. A healthy work environment plays an important role in the low level of moral distress and burnout, if there are staff skilled in communication, good collabora tion between people or professions, effective decision making, proper staffing, recogni tion and leadership

Abstract

Patient safety is a global issue and a top priority in hospitals. Incidence of unexpected events varies significantly in Indonesia. Nurses' compliance is an essential factor in the successful implementation of patient safety policies in hospitals. This study aimed to determine the factors influencing nurses' compliance with patient safety policies in inpa tient units of Dr. Kariadi general hospital, Semarang. This study employed a descriptive correlational design with a cross-sectional approach. The samples were 220 staff nurses recruited using a proportional random sampling technique. The data were collected using a questionnaire developed by the researchers and analyzed using the univariate analysis, Chi-square test, and multivariate multiple logistic regression test. Results of the bivariate analysis showed that there were effects of supervision (p = 0.038; OR = 1.828), the character of authority figures (p = 0.001; OR = 2.648), and non-compliant colleagues (p = 0.0001; OR = 2.899) on nurses' compliance with patient safety policies. However, gender (p = 0.691), working environment (p = 0.891), and contradictory orders (p = 0.243) were found to have no effects on nurses' compliance with patient safety policies. Results of multivariate analysis using a multiple logistic regression analysis showed that non-compliant colleagues were the most influential factor of nurses' compliance with patient safety policies in the hospital. There were effects of supervision, the character of authority figures, and non-compliant colleagues on nurses' compliance with patient safety policies. In contrast, gender, working environment, and contradictory orders showed no effects on nurses' compliance. Non-compliant colleagues had the most effect on nurses' compliance.

Abstract

The problems of people with mental health disorders (PWMHD) include physical problems, psychological problems, and social problems. Jember Regency in 2018 had 514 new cases, and Panti village was ranked first with 114 clients (22.1%) of the total casesin the Jember Regency. Community views accompanied by cultural background often prevent families with PWMHD from coming to the health center to seek care and management of health services. Negative public views on PWMHD will reduce PWMHD visits to seek treatment so that cases of drug withdrawal will increase. Nurses have an essential role in providing nursing care to clients with mental disorders. Without knowledge of the cultural viewpoints of the community, nurses will find it difficult to find strategies to reduce the occurrence of stigma and discrimination in PWMHD This study was used an ethnography study, qualitative approach, and descriptive design to explore and to understand about the Jember community view in the treatment of mental disorders with a perspective of transcultural nursing. The technique of collecting data was used in-depth interviews with a semi-structured interview guide involving seven participants, and the data were analyzed using interpretive phenomenology analysis. This research has passed the ethical test with ethical approval No.595 / UN25.8 / KEPK / DL / 2019. This research conducted five themes. First is the family efforts to seek healing; the second theme is received trials from God; the third theme is community stigma to people with mental disorders; the fourth theme is community expectations for people with mental disorders, and the fifth theme is community acceptance of people with mental disorders. Considering the impact that will arise from this condition if it does not get proper treatment, it is needed the role of health workers in providing services to the family to be able to increase visits to health services in order to improve the recovery of people with mental disorders.

Abstract

First-degree students have many experiences with academic problems, namely stress. Stress is a response to the burden received. Increased stress for students causes a depressive condition that adversely affects them. Management that can be used is coloringthe mandala pattern which can affect the endorphin hormone so that the individual is in a state of relaxation. The purpose of this study was to determine the effect of therapeutic coloring on mandala patterns on academic stress levels in first-degree students. The research method in this study used a type of pre-experimental research with the PretestPosttest One Group approach. The number of samples in this study amounted to 60 firstlevel students nursing undergraduate study program at Bhakti Kencana Bandung Institute of health science with purposive sampling technique. The instruments used were the Student-life Stress Inventory questionnaire and the coloring book of the mandala pattern with measurements of stress levels carried out pre and post-test. Therapy for coloring the mandala pattern was carried out for 60 minutes with 3 meetings for 3 days in 1 weekusing pencil drawing media and mandala drawing patterns. Analysis of the data used is a paired t-test.The results showed that before the mandala pattern coloring therapy almost all (98.3%) students experienced moderate stress, after being given mandala pattern coloring therapy almost all (78.3%) students were still experiencing moderate stress but with a decrease in percentage and a small percentage (21.7%) students experienced a decrease in stress levels to mild stress. The paired t-test results showed a sig p-value of 0,000 < alfa (0.05). So it can be concluded that there is a therapeutic effect of coloring mandala patterns on academic stress levels at first-level students. Based on the results of this study, institutions are expected to be able to apply color therapy to the mandala pattern as a complementary therapy in handling students who experience academic stress.

Abstract

Tuberculosis is one of the world's health problems, even in Indonesia. The immune response that needed to eliminate TB can be influenced by other infections, such as helminth infection. The helminth infections induce Th2 immune responses and eosinophilia to eliminate the helminths. Meanwhile, the Th2 response and eosinophilia can also suppress the Th1 immune response, which is very important to eliminate the bacterium M. tuberculosis and make a negative impact on the success of TB therapy. Thus, the eosinophil profile can describe the immune response in TB patients with helminth co-infection. This study aims to determine the prevalence of helminthiasis in TB patients and determine the differences in eosinophil amount in tuberculosis patients with and without helminth coinfection. This study was an observational analytic study with a cross-sectional design conducted at Panti local health center in August 2019-January 2020 using fecal and blood samples from 24 research subjects. Helminth co-infection status was obtained from fecal examination with sedimentation and flotation methods while the number of eosinophils was obtained from leukocyte counts. Data analysis used the Mann Whitney U test to determine differences in the number of eosinophils in the two groups. The results showed there was no difference in the number of eosinophils in TB patients with helminth coinfection and without helminth co-infection (p> 0.05). There was no typical picture of the number of eosinophils in both groups so that eosinophils cannot describe the immune response that arose in TB patients with helminth co-infections in Panti district, Jember.Further study is needed to measure IgE and interleukin (IL4 and IL10) to know the immune response in TB patients with helminth co-infection.

Abstract

Problems that occur in clients with mental disorders are not obedient to taking medica tion, where clients who have finished undergoing treatment at the Mental Hospital are returned to their families but the obstacles found are that the role of nurses who hold health programs is not optimal in carrying out mental nursing care in either com munity for the nursing diagnosis of mental, psychosocial and mental health disorders. Therefore it requires a family role related to treatment and care for clients with mental disorders. One of the programs to stimulate the role of the family towards clients with mental disorders is the training of BC-CMHN (Basic Course Community Mental Health Nursing) nurses. The research objective was to determine the effect of BC-CMHNtraining on the self-ability of nurses in implementing community mental nursing care inJambi City. This research is a quasy experimental design with a pretest-posttest one group design. Respondents in this study were 20 public health center nurses who are responsible for mental health programs. Based on the results of the bivariate analysis using the dependent t-test that cognitive abilities before and after the intervention p val ues (p =0.000). There is an effect of BC-CMHN training on the self-efficacy of nurses in implementing community mental health insurance in Jambi City, it is recommended that it be implemented at both the provincial and district or city levels to improve the quality ofcommunity mental health services.

Abstract

The flash flood that occurred in the Garut Regency three years ago brought about devas tating effects, one of which was the psychological impact in adolescents, namely PTSD. This study aimed to identify Post-Traumatic Stress Disorder (PTSD) in adolescent vic tims of the flash flood that occurred in Garut, Indonesia three years ago. PTSD was assessed using the questionnaire of PTSD Checklist (PCL) that had been reported to be reliable and valid with Cronbach's alpha was 0.91 and 0.947. The participant of this study were 102 teenagers of the State Junior High-School (SMPN) 3 Tarogong Kidul whowere victims of the flood and selected using a purposive sampling technique. This study used a quantitative descriptive method with cross-sectional approach. The results showed that three years after the Garut flash flood, a total of 80% of adolescent victims did not experience PTSD and 20 (20%) of the adolescents were identified having PTSD. Re experiencing symptom and hyper-arousal symptom were most common trauma encoun tered, while 90% participants have feeling intruded by memories of trauma and 80% participants showed symptoms of increased alertness. It is expected that nurses and experts can perform early and continuous screening for PTSD following a traumatic event, both in adolescents and other age groups to prevent another more severe impact.

Abstract

The HIV testing program's implementation among pregnant mothers is still not going well due to their intention. Their intention is needed because it can affect pregnant mothers' behavior toward HIV testing. New interventions are needed to solve this problem, such as Home-based HIV Test and Education (HOPE). This study aimed to analyze the influ ence of HOPE to improve HIV testing intention among pregnant mothers. The pre experimental design used with the one group pretest-posttest approach where the imple mentation was carried out for one month with the distribution of the pretest carried out at week 2, and posttest carried out at week 4 of the study thirty pregnant mothers in Jember following this study and involved by using a purposive sampling technique. The data were collected by using the intention questionnaire and analyzed with the Wilcoxon test. The results showed that the pretest results were 20 pregnant women who had the intention to carry out HIV tests and increased to 27 pregnant women on posttest measurements. The results indicate a significant influence of HOPE on the intention of HIV testing in pregnant women (p = 0.033). The Home-based HIV Test and Education (HOPE) interven tion can improve pregnant mothers' intention toward HIV testing. The HOPE interven tion can reach all pregnant mothers (and their husbands) to discover HIV testing information. Involving their husbands in HOPE intervention encouraged pregnant mothers' intention of HIV testing because their husbands were decision-makers in their family, andpregnant mothers will take HIV testing.
