Abstract

Chronic kidney disease (CKD) is an independent risk factor for cognitive impairment in all domains, especially delayed memory and executive function. The purpose of this study was to determine the correlation between chronic kidney disease severity and cognitive function. This study used a cross-sectional design in stage III, IV, and V CKD patients in the Nephrology Polyclinic of Haji Adam Malik Central General Hospital. Cognitive function tests were performed using the Montreal Cognitive Assessment (MoCA INA), digit span, and Trail Making Test A & B. The Spearman test was used to analyze the correlation between CKD severity and cognitive function. This study involved 45 chronic kidney disease patients consisting of 28 (62.2%) males and 17 (37.8%) females with a mean age of 49.67±12.18 years. The results of statistical analysis showed that there was a significant positive correlation between CKD on the MoCA-INA examination (r=0.618, p=<0.001), FDS (r=0.414, p=0.005), there was a significant negative correlation on the TMT A time examination (r=-0.425, p=0.004), TMT A error (r=-0.497, p=0.001), TMT B time (r=-0.618, p=<0.001), TMT B error (r=-0.370, p=0.012). The results of this study prove a significant correlation between the severity of CKD and cognitive function.

Abstract

Parasagittal meningioma is a benign extra-axial tumor from the arachnoid cap cell that fills the parasagittal angle. This case report presents a case of a 21-years old young female with a history of complete visual loss, left side paresthesia, and progressive blunt headache. Brain MRI and MRV revealed a giant enhancing tumor measuring 9.2 cm x 8.41 cm x 7.5 cm on the right parietooccipital lobe with obstruction of the posterior third of the superior sagittal sinus. Gross total removal was achieved. The pathology reports confirmed a WHO grade I meningioma. The paresthesia and headache were improved, but the loss of visions did not change. Giant parasagittal meningioma may invade the superior sagittal sinus leading to intracranial hypertension. The surgical strategy should target the survival and postoperative quality of life. Occlusion of posterior third superior sagittal sinus affects deficiency venous return resulting in visual loss.

Abstract

Weight-gain is one of the antipsychotic side effects, and it can increase the risk factor of metabolic syndrome. Several studies relate it to increase leptin levels. This research was conducted to determine the profile of leptin levels in schizophrenic patients who were receiving antipsychotic therapy at Prof. DR. HB Saanin Mental Hospital. The research was conducted from November 2019 to January 2020 on schizophrenic patients who were taking antipsychotic drugs. This research was conducted on 50 samples by using consecutive sampling techniques. Data analysis using univariate are presented in geometric mean and CI 95%. Moreover, a Comparison of leptin levels between groups was performed by T-test and one-way ANOVA. The average leptin level from 50 samples of schizophrenic patients was 5.12µg/ml (CI 95%=3.32-7.90). The highest average leptin level is from the 46-55 year age group which is 11.32µg/ml (CI 95% =5.24 - 24.42), female is  13.29µg/ml (CI 95%=5.84-30.26), BMI ≥30kg/m2  is 12.84µg/ml (CI 95%=4.31-38.23), subjects with above-average waist circumference is 5.54µg/ml ( CI 95%=3.45-8.90), and the atypical group of drugs is 6.08µg/ml (IK 95%=3.41-10.84). Increasing levels of leptin occur in schizophrenic patients who were 46-55 y.o, female BMI ≥30kg/m2, above-average waist circumference, and receiving atypical antipsychotics.

Abstract

Oxidative stress is related to pregnancy complications that could increase maternal and infant mortality. This study aimed to determine the effect of propolis extract supplementation during pregnancy on oxidative stress level and pregnancy outcomes utilizing Malonedealdehyde (MDA) and 8-Oxo-2′-Deoxogunosine (8-OHdG) levels, maternal body weight, and the average number of fetuses as the parameters. The study was conducted by using a posttest only control group design on 24 pregnant Wistar rats, which were divided into four groups. Group I was control, Group II-IV were the treatment groups given propolis extract of 1.8mg, 3.6mg, and 7.2mg/200gBW/day, respectively. The standard feed given was AIN93G dose of 20g/day and distilled water ad libitum. Propolis extract was given using a gastric feeding tube every morning for 20 days. At the end of the treatment, body weight was meisured and blood collected for assessed MDA and 8-OHdG levels  by ELISA method  and then we performed abdominal surgery to count number of fetuses. The result are there were decreasing level of MDA and 8-OHDG by administration of propolis significantly (p<0.05) group: I: 2,04±0,091, II: 1,55±0,067, III: 1,05±0,176, IV: 0,73±0,075 (mmol/mL) (p=0.001); 8 OHdG level (ng/mL) group I: 10,02±0,403, II: 8,60±0,078, III: 7,89±0,051, IV: 7,53±0,063 (p=0,001). Average of maternal body weight (g) were increased: group I: 228,33±3,93, II: 237,17±4,36, III: 244,83±4,02, IV: 248,00±5,76 (p=0,001) and Average number of fetuses tend to increased as well, group I : 8,5±0,05, II: 7,8±0,41, III: 9,5±1,05, IV: 9,6±0,52 (p=0,02). The conclusion of this research are supplementation of propolis extract in pregnant rats can reduce oxidative stress and improve pregnancy outcomes.

Abstract

Excessive usage of synthetic pediculicides, such as permethrin, lindane, and malathion, can induce resistance, environmental pollution, health problems, and even death. Efforts to prevent the side-effects of synthetic pediculicides are needed, one of which is by shifting to powerful yet safe natural pediculicides, such as Citrus limon (lemon) and Allium sativum (garlic). This study aimed to observe the effective concentration of Citrus limon and Allium sativum in vitro on the mortality of Pediculus humanus capitis (P. h. capitis). This research was an experimental laboratory study conducted in July to September 2019 in the Laboratory of Parasitology at Faculty of Medicine, University of Riau, while the making of Allium sativum extract was done at the Faculty of Mathematics and Science, University of Riau. A total of 288 samples of adult P. h. capitis was taken from children in two orphanages in Pekanbaru. Citrus limon juice and Allium sativum extract of 25%, 50%, 75%, and 100% concentrations, permethrin 1% as the positive control, and distilled water as the negative control were used in this study, and repetition was done three times. P. h. capitis mortality was observed every 10 - 120 minutes, characterized by the absence of movement of the legs and antenna. Data analysis was performed after 120 minutes with the one-way ANOVA test, LSD (=0.05), and Lethal Concentration 50 test (LC50). The one-way ANOVA test results of this study showed that statistically Citrus limon juice and Allium sativum extracts showed differences in each treatment formulation of juice on P. h. capitis mortality (Sig 0.017<0.05 and Sig 0.000<0.05) and LC50 juice of Citrus limon and Allium sativum extract by 51.999% and 72.426%. In this study, Citrus limon juice and Allium sativum extract had effect as pediculicides.

Abstract

Hand hygiene is an act to prevent and control infections, one of which is nosocomial phlebitis infection. The results of a preliminary study conducted at Syuhada Haji Hospital Blitar showed the number of phlebitis cases had not reached the target of ≤1.5% (1.63% in December 2018). One of the reasons was hand hygiene non-compliance. This study intended to analyze the factors causing non-compliance with hand hygiene in Syuhada Haji Hospital Blitar and find alternative solutions. This study was a case study conducted with observations on implementing nurses in five inpatient rooms and Focus Group Discussion (FGD) on five implementing nurses and eight management staff at Syuhada Haji Hospital Blitar. The main factor influencing compliance with hand hygiene at Syuhada Haji Hospital is that no supervision policy supports hand hygiene implementation, and the solution is supervision for monitoring hand hygiene compliance. Supervision is expected to increase compliance and awareness of the importance of hand hygiene; thus, in turn, affect the service quality.

Abstract

ABSTRACTThe process of admitting patients to the inpatient wardfrom the emergency room is of the factors that influence the access block time in services in the emergency room and hospitals. This study aimed to identify the cause of transition time to inpatient services from the emergency room at a private C class hospital in Malang. This research is a descriptive exploratory study using interviews withfour general practitioners, two emergency nurses, two general inpatient nurses, two children inpatient nurses, and one administrative staff, and also observation of the stages and duration of emergency services and inpatient care. The process from arrival, registration, and inspection takes 0-15 minutes. Overall, the stages of registration until the inpatient ward transfer require 1-3.5 hours, which exceeds the hospital quality standards. Factors that influence the transfer time are the condition of the patient, the absence of a transition room, the availability of inpatient wards, and the patient discharge process. To manage ER LOS, especially block access to inpatient services, hospitals need to apply Bed Management properly.

Abstract

Urethrocutaneous fistula development is the most common complication found hypospadias surgery. Several factors associated with the incidence of fistulas are the age at the time the surgery. The type of hypospadias, the degree of chordae, the operative technique applied and catheter type. This study aimed to determine the risk factors associated with the urethro-cutaneous fistula development in hypospadias surgery. The results showed that from 310 hypospadias patients undergoing urethroplasty, urethra cutaneous fistula development occurred among 105 patients (30.6%) with a mean age of surgery 10.89+6.27 years. Hypospadias type has a significant difference with the incidence of urethro-cutaneous fistula development (p = 0.027). Age (p = 0.615), degree of chordae (P=0.805), operative technique (P=0.901) and catheter type (P=691) do not have a significant impact to urethro-cutaneous fistula development. It can be concluded that operative urethroplasty has a risk of urethra-cutaneous fistula that associated with hypospadias type.

Abstract

Medication errors, ranging from prescription to administration errors, are still problems of patient safety with an average error rate of 8% - 10% and can cause severe morbidity, prolonged length of stay (LOS) in a hospital, unnecessary diagnostic tests and care, and mortality. The objective of this study is to describe the number of medication errors and their potential causes according to the perceptions of health personnel. The study was carried out by questionnaires, interviews, and data exploration on prescriptions made before the observation period and new prescriptions made during the observation period. The priority determination of the solutions was carried out using Capability, Assessibility, Readiness, and Leverage method (CARL) and discussions with related units. The priority root factors that caused medication errors in the inpatient pharmacy at RSI Malang were high workloads and high turnover of inpatient pharmacist, incomplete prescription identity, illegible doctor's writing, and lack of training for the pharmacist. The priority outcome of the alternative solutions to overcome the medication errors in inpatient pharmacist is to regularly conduct training or knowledge refreshing for the inpatient pharmacist at RSI Malang and the implementation of e-prescription.

Abstract

Systemic Lupus Erythematosus (SLE) is a complex autoimmune disease that requires long-term corticosteroid treatment that may lead to osteoporosis characterized by low bone mass and microarchitectural deterioration of the trabecular and cortical skeletal. This study presents a severe case of thrombocytopenia in a 25-year old woman with SLE and osteoporosis who underwent routine therapy and was pregnant for the first time. The complexity of pregnancy and autoimmune conditions create therapeutic challenges that need to be considered in SLE patients with pregnancy, osteoporosis, and severe thrombocytopenia.
